| 1 | Endoscopic ultrasound elastography for evaluation of lymph nodes and pancreatic masses:A multicenter study显示文摘AIM:To evaluate the ability of endoscopic ultrasound(EUS) elastography to distinguish benign from malignant pancreatic masses and lymph nodes.METHODS:A multicenter study was conducted and included 222 patients who underwent EUS examination with assessment of a pancreatic mass(n=121) or lymph node(n=101).The classification as benign or malignant,based on the real time elastography pattern,was compared with the classif ication based on the B-mode EUS images and with the fi nal diagnosis obtained by EUS-guided fi ne needle aspiration(EUS-FNA) and/or by surgical pathology.An interobserver study was performed.RESULTS:The sensitivity and specificity of EUS elastography to differentiate benign from malignant pancreatic lesions are 92.3% and 80.0%,respectively,compared to 92.3% and 68.9%,respectively,for the conventional B-mode images.The sensitivity and specificity of EUS elastography to differentiate benign from malignant lymph nodes was 91.8% and 82.5%,respectively,compared to 78.6% and 50.0%,respectively,for the B-mode images.The kappa coefficient was 0.785 for the pancreatic masses and 0.657 for the lymph nodes.CONCLUSION:EUS elastography is superior compared to conventional B-mode imaging and appears to be able to distinguish benign from malignant pancreatic masses and lymph nodes with a high sensitivity,specificity and accuracy.It might be reserved as a second line examination to help characterise pancreatic masses after negative EUS-FNA and might increase the yield of EUS-FNA for lymph nodes. | Marc Giovannini Botelberge Thomas Bories Erwan Pesenti Christian Caillol Fabrice Esterni Benjamin Monges Geneviève Arcidiacono Paolo Deprez Pierre Yeung Robert Schimdt Walter Schrader Hanz Szymanski Carl Dietrich Christoph Eisendrath Pierre Van Laethem Jean-Luc Devière Jacques Vilmann Peter Saftoiu Andrian | 2009 | World Journal of Gastroenterology2009,15,13: | 48 |

目的:于新近发生ST段抬高心肌梗死(ST—segment elevation myocardial infarction,STEMI)的患者中确定PCI前氯吡格雷预治疗预防严重不良心血管事件的效果是否优于PCI时开始采用氯吡格雷治疗。
设计、地点及参试者:CLARITY(PCI—Clopidogrel as Adjunctive Reperfusion Therapy)研究是一项于1863例患者中进行的前瞻性计划分析。这些患者均于CLARITY—TIMI(Thrombolysis in Myocardial Infarction)28试验中在血管造影后实施了PCI。CLARITY—TIMI28是一项在接受纤溶治疗的患者中进行的有关氯吡格雷效果的随机、双盲、安慰剂对照试验。患者于2003年2月至2004年10月入选自23个国家的319家医院。
干预:患者服用阿斯匹林并于纤溶治疗时开始随机接受氯吡格雷(300mg负荷剂量,以后75mg,每日1次)或安慰剂,直至冠状动脉造影前为止。造影在开始研究药物后2~8天进行。接受冠脉支架的患者建议于诊断性血管造影后给予开标氯吡格雷(包括负荷剂量)。
主要观测指标:一级终点为PCI至随机分组后30天心血管死亡、MI复发或脑卒中的复合发生率。二级终点包括在PCI之前发生的MI或脑卒中以及前述随机分组后30天相关终点。
结果:氯吡格雷预治疗可显著降低PCI后心血管死亡、MI或脑卒中的发生率(34[3.6%]比58[6.2%];校正优势比[odds ratio,OR],0.54[95%CI,0.35—0.85];P=0.008)。氯吡格雷预治疗还可降低PCI前MI或脑卒中的发生率(37[4.0%]比58[6.2%];OR,0.62[95%CI,0.40~0.95];P=0.03)。总体上,氯吡格雷预治疗可导致随机分组后30天心血管死亡、MI或脑卒中显著下降(70[7.5%]比112[12.0%];校正OR,0.59[95%CI,0.43~0.81];P=0.001;所需治疗例数=23)。TIMI严重或轻微出血的发生率未显著增加((18[20%]比17[1.9%];P〉0.99)。

| 18 | From azoospermia to macrozoospermia,a phenotypic continuum due to mutations in the ZMYND15 gene显示文摘Thanks to tremendous advances in sequencing technologies and in particular to whole exome sequencing(WES),many genes have now been linked to severe sperm defects.A precise genetic diagnosis is obtained for a minority of patients and only for the most severe defects like azoospermia or macrozoospermia which is very often due to defects in the aurora kinase C(AURKC)gene.Here,we studied a subject with a severe oligozoospermia and a phenotypic diagnosis of macrozoospermia.AURKC analysis did not reveal any deleterious variant.WES was then initiated which permitted to identify a homozygous loss of function variant in the zinc finger MYND-type containing 15(ZMYND15)gene.ZMYND15 has been described to serve as a switch for haploid gene expression,and mice devoid of ZMYND15 were shown to be sterile due to nonobstructive azoospermia(NOA).In man,ZMYND15 has been associated with NOA and severe oligozoospermia.We confirm here that the presence of a bi-allelic ZMYND15 variant induces a severe oligozoospermia.In addition,we show that severe oligozoospermia can be associated macrozoospermia,and that a phenotypic misdiagnosis is possible,potentially delaying the genetic diagnosis.In conclusion,genetic defects in ZMYND15 can induce complete NOA or severe oligozoospermia associated with a very severe teratozoospermia.In our experience,severe oligozoospermia is often associated with severe teratozoospermia and can sometimes be misinterpreted as macrozoospermia or globozoospermia.In these instances,specific AURKC or dpy-19 like 2(DPY19L2)diagnosis is usually negative and we recommend the direct use of a pan-genomic techniques such as WES. | Zine-Eddine Kherraf Caroline Cazin Florence Lestrade Jana Muronova Charles Coutton Christophe Arnoult Nicolas Thierry-Mieg Pierre F Ray | 2022 | Asian Journal of Andrology2022,24,3: | 2 |
