| 1 | Passive expansion of sub-maximally dilated transjugular intrahepatic portosystemic shunts and assessment of clinical outcomes显示文摘AIM To assess for passive expansion of sub-maximally dilated transjugular intrahepatic portosystemic shunts(TIPS) and compare outcomes with maximally dilated TIPS.METHODS Polytetrafluoroethylene covered TIPS(Viatorr) from July 2002 to December 2013 were retrospectively reviewed at two hospitals in a single institution. Two hundred and thirty patients had TIPS maximally dilated to 10 mm(m TIPS), while 43 patients who were at increased risk for hepatic encephalopathy(HE), based on clinical evaluation or low pre-TIPS portosystemic gradient(PSG), had 10 mm TIPS sub-maximally dilated to 8 mm(sm TIPS). Group characteristics(age, gender, Model for End-Stage Liver Disease score, post-TIPS PSG and clinical outcomes were compared between groups, including clinical success(ascites or varices), primary patency,primary assisted patency, and severe post-TIPS HE. A subset of fourteen patients with sm TIPS underwent follow-up computed tomography imaging after TIPS creation, and were grouped based on time of imaging(< 6 mo and > 6 mo). Change in diameter and crosssectional area were measured with 3D imaging software to evaluate for passive expansion.RESULTS Patient characteristics were similar between the sm TIPS and m TIPS groups, except for pre-TIPS portosystemic gradient, which was lower in the sm TIPS group(19.4 mm Hg ± 6.8 vs 22.4 mm Hg ± 7.1, P = 0.01). Primary patency and primary assisted patency between sm TIPS and m TIPS was not significantly different(P = 0.64 and 0.55, respectively). Four of the 55 patients(7%) with sm TIPS required TIPS reduction for severe refractory HE, while this occurred in 6 of the 218 patients(3%) with m TIPS(P = 0.12). For the 14 patients with follow-up computed tomography(CT) imaging, the median imaging follow-up was 373 d. There was an increase in median TIPS diameter, median percent diameter change, median area, and median percent area change in patients with CT follow-up greater than 6 mo after TIPS placement compared to follow-up within 6 mo(8.45 mm, 5.58%, 56.04 mm^2, and 11.48%, respectively, P = 0.01).CONCLUSION Passive expansion of sm TIPS does occur but clinical outcomes of sm TIPS and m TIPS were similar. | 2 | Notch signaling controls chondrocyte hypertrophy via indirect regulation of Sox9显示文摘RBPjk-dependent Notch signaling regulates both the onset of chondrocyte hypertrophy and the progression to terminal chondrocyte maturation during endochondral ossification. It has been suggested that Notch signaling can regulate Sox9 transcription, although how this occurs at the molecular level in chondrocytes and whether this transcriptional regulation mediates Notch control of chondrocyte hypertrophy and cartilage development is unknown or controversial. Here we have provided conclusive genetic evidence linking RBPjk-dependent Notch signaling to the regulation of Sox9 expression and chondrocyte hypertrophy by examining tissuespecific Rbpjk mutant(Prx1Cre;Rbpjkf/f), Rbpjk mutant/Sox9 haploinsufficient(Prx1Cre;Rbpjkf/f;Sox9f/1),and control embryos for alterations in SOX9 expression and chondrocyte hypertrophy during cartilage development. These studies demonstrate that Notch signaling regulates the onset of chondrocyte maturation in a SOX9-dependent manner, while Notch-mediated regulation of terminal chondrocyte maturation likely functions independently of SOX9. | 6 | Urogenital schistosomiasis and soiltransmitted helminthiasis(STH)in Cameroon:An epidemiological update at Barombi Mbo and Barombi Kotto crater lakes assessing prospects for intensified control interventions显示文摘Background:The crater lakes of Barombi Mbo and Barombi Kotto are well-known transmission foci of schistosomiasis and soil-transmitted helminthiasis having had several important control initiatives previously.To collect contemporary epidemiological information,a cross-sectional survey was undertaken inclusive of:signs and symptoms of disease,individual treatment histories,local water,sanitation and hygiene(WASH)-related factors and malacological surveillance,with molecular characterisation of specimens.Methods:At each lake,a community cross-sectional survey was undertaken using a combination of stool and urine parasitological sampling,and interview with pro-forma questionnaires.A total of 338 children and adults participated.Material from snail and parasite species were characterised by DNA methods.Results:Egg-patent prevalence of urogenital schistosomiasis was 8.7%at Barombi Mbo(all light-intensity infections)and 40.1%at Barombi Kotto(21.2%heavy-intensity infections).Intestinal schistosomiasis was absent.At Barombi Kotto,significantly more women reported signs and symptoms associated with female genital schistosomiasis.While there had been extensive recent improvement in WASH-related infrastructure at Barombi Mbo,water contact risk scores were higher among schistosomiasis-infected participants(P<0.001)and at Barombi Kotto in general(P<0.001).Across both lakes,mean prevalence of STH was very low(6.3%)evidencing an impressive decrease of 79.0%over the last decade;neither Strongyloides stercoralis nor Ascaris lumbricoides were found.A total of 29 freshwater sampling sites were inspected for snails,13 in Barombi Mbo and 16 in Barombi Kotto;water chemistry differed significantly(P<0.0001)between lakes for both mean pH(7.9 v.9.6)and mean conductivity(64.3μS v.202.1μS)respectively.Only two Bulinus camerunensis found on the central island of Barombi Kotto were observed to shed schistosome cercariae,but schistosome DNA was later detected in Bulinus sampled from both lakes as well as in Indoplanorbis exustus,an invasive species from Asia.Conclusions:STH is currently at very low levels while urogenital schistosomiasis is of greatest concern at Barombi Kotto.This assessment highlights a unique opportunity for further study of the epidemiological dynamics at these crater lakes,to evaluate future intensified interventions both in terms of gaining and sustaining control at Barombi Kotto or in moving towards local interruption of transmission of both diseases at Barombi Mbo. | Suzy J.Campbell J.Russell Stothard Faye O’Halloran Deborah Sankey Timothy Durant Dieudonne Eloundou Ombede Gwladys Djomkam Chuinteu Bonnie LWebster Lucas Cunningham EJames LaCourse Louis-Albert Tchuem-Tchuente | 2017 | Infectious Diseases of Poverty2017,6,1: | 4 |
