| 1 | Clinical Validation of Global Coagulation Tests to Guide Blood Component Transfusions in Cirrhosis and ACLF显示文摘Background and Aims:Patients with cirrhosis and acuteon-chronic liver failure(ACLF)may have bleeding complications and need for invasive procedures.Point-of-care(POC)coagulation tests like thromboelastography(TEG)and Sonoclot may be better for guiding patient management than the standard coagulation tests(SCTs),like prothrombin time,platelet count and international normalized ratio.Methods:We prospectively compared and validated the POC tests and SCTs in 70 persons with ACLF and 72 persons with decompensated cirrhosis who had clinical bleeding and checked for episodes of re-bleeding and transfusion requirements.We assessed pre-procedure requirement of blood components when correction was done based on an SCT or POC strategy.Results:Episodes of bleeding were seen in 45%and 28%of ACLF and cirrhosis patient,respectively(p=0.036),with the major site of bleeding being gastrointestinal(31%and 16%,respectively).Platelet counts correlated with TEG-maximum amplitude in cirrhosis(p=0.045)and prothrombin time correlated positively with TEG-reaction(R)time(p=0.032),TEG-Clot kinetics(K)time(p=0.042),Son-activated clotting time(p=0.038)and negatively with clot rate(p=0.043)in ACLF,making these correctable target variables in POC transfusion algorithms.Of 223 procedures,transfusion of fresh frozen plasma and platelet concentrate was reduced by 25%(p=0.035)and 20.8%(p=0.045)by using a POC strategy in 76 patients.Correction of deranged Son-activated clotting time and TEG-reaction time was noted in 68%and 72%after 24 h of fresh frozen plasma transfusion in ACLF and 85%and 80%in cirrhosis,respectively.Conclusions:Our study clinically validates that POC tests can better detect coagulation defects and transfusion thresholds in ACLF and cirrhosis,whereas use of conventional tests appear to be less suitable in patients with clinical bleeding.Trial Registration:NCT04332484. | Madhumita Premkumar Rohit Mehtani Smita Divyaveer Kamal Kajal Anand VKulkarni Syed Ahmed Harmanpreet Kaur Harpreet Kaur Radhakrishna Dhiman Ajay Duseja Arka De | 2021 | Journal of Clinical and Translational Hepatology2021,9,2: | 7 |

| 4 | Review of materials used in laser-aided additive manufacturing processes to produce metallic products显示文摘Rapid prototyping (RP) or layered manufacturing (LM) technologies have been extensively used to manufacture prototypes composed mainly of plastics, polymers, paper, and wax due to the short product development time and low costs of these technologies. However, such technologies, with the exception of selective laser melting and sintering, are not used to fabricate metallic products because of the resulting poor life, short cycle, poor surface finish, and low structural integrity of the fabricated parts. The properties endowed by these parts do not match those of functional parts. Therefore, extensive research has been conducted to develop new additive manufacturing (AM) technologies by extending existing RP technologies. Several AM technologies have been developed for the fabrication of metallic objects. These technologies utilize materials, such as Ni-, A1-, and Ti-based alloys and stainless steel powders, to fabricate high-quality functional components. The present work reviews the type of materials used in laserbased AM processes for the manufacture of metallic products. The advantages and disadvantages of processes and different materials are summarized, and future research directions are discussed in the final section. | 6 | Enhanced photocatalytic activity of electrochemically synthesized aluminum oxide nanoparticles显示文摘In this study, aluminum oxide(Al_2O_3) nanoparticles(NPs) were synthesized via an electrochemical method. The effects of reaction parameters such as supporting electrolytes, solvent, current and electrolysis time on the shape and size of the resulting NPs were investigated. The Al_2O_3 NPs were characterized by Fourier transform infrared spectroscopy, X-ray diffraction, transmission electron microscopy, thermogravimetric analysis/differential thermal analysis, energy-dispersive X-ray analysis, and ultraviolet–visible spectroscopy. Moreover, the Al_2O_3 NPs were explored for photocatalytic degradation of malachite green(MG) dye under sunlight irradiation via two processes: adsorption followed by photocatalysis; coupled adsorption and photocatalysis. The coupled process exhibited a higher photodegradation efficiency(45%) compared to adsorption followed by photocatalysis(32%). | 7 | Representative Elementary Volume(REV) in Spatio-Temporal Domain: A Method to Find REVfor Dynamic Pores显示文摘One of the potential risks associated with subsurface storage of CO_2 is the seepage of CO_2 through existing faults and fractures. There have been a number of studies devoted to this topic. Some of these studies show that geochemistry, especially mineralization, plays an important role in rendering the faults as conduits for CO_2 movement while others show that mineralization due to CO_2 injection can result in seep migration and flow diversion. Therefore, understanding the changes in reservoir properties due to pore alterations is important to ensure safe long term CO_2 storage in the subsurface. We study the changes in the Representative Elementary Volume(REV) of a rock due to reactive kinetics over a time, using a statistical approach and pore-scale CO_2-rock interactiondata.The goal of this study is to obtain the REV of a rock property that accounts for pore-scale changes over time due to reactive kinetics, and we call this as spatiotemporal REV. Scale-up results suggest that the REV changes with time when CO_2-rock interaction is considered. It is hypothesized that the alteration in pore structure introduces more heterogeneity in the rock, and because of this the magnitude of REV increases. | 11 | Interconversion of two commonly used performance tools: An analysis of 5844 paired assessments in 1501 lung cancer patients显示文摘AIM To establish the Karnofsky performance status(KPS) categories which would facilitate the interconversion of the KPS scale to the Eastern Cooperative Oncology Group(ECOG) performance status(PS) scale.METHODS This was a retrospective analysis of all patients attending the lung cancer clinic at a tertiary care center over a 5-year period(September 2009 to August 2014). All patients were assessed with both KPS and ECOG PS scales at each visit. Correlation between KPS and ECOG PS was assessed using Spearman's correlation coefficient. KPS categories equivalent to ECOG PS scores were compared using hit rate and weighted kappa(κw).RESULTS A total of 1501 patients were assessed over the study period, providing 5844 paired KPS and ECOG PS assessments. The study cohort had a mean(standard deviation; SD) age of 58.4(10.8) years, with the majority being current or ex-smokers(76.9%) and males(82.3%). Non-small cell lung cancer was the most common histological type(n = 1196, 79.7%) with the majority having advanced(stage ⅢB/Ⅳ) disease(83.4%). Mean baseline KPS and ECOG PS scores were 77.6(SD = 14.4) and 1.5(SD = 1) respectively. The most frequent KPS score was 80(29%), and the most frequent ECOG PS score was 1(43%). The overall correlation between KPS and ECOG PS was good(Spearman r =-0.84, P < 0.0001) but ranged from-0.727 to-0.972 between visits. KPS categories derived from our cohort [10-40(ECOG 4), 50-60(ECOG 3), 70(ECOG 2), 80-90(ECOG 1), 100(ECOG 0)] performed better [hit rate 78.1%, κw = 0.749(0.736-0.762) P < 0.0001] than those suggested in the past literature.CONCLUSION The current study provides the largest set of paired KPS-ECOG assessments to date. | 12 | Enhanced cavitation erosion resistance of a friction stir processed high entropy alloy显示文摘Friction stir processing of an Al0.1CoCrFeNi high entropy alloy(HEA)was performed at controlled cooling conditions(ambient and liquid submerged).Microstructural and mechanical characterization of the processed and as-cast HEAs was evaluated using electron backscatter diffraction,micro-hardness testing and nanoindentation.HEA under the submerged cooling condition showed elongated grains(10μm)with fine equiaxed grains(2μm)along the boundary compared to the coarser grain(~2 mm)of as-cast HEA.The hardness showed remarkable improvements with four(submerged cooling condition)and three(ambient cooling condition)times that of as-cast HEA(HV^150).The enhanced hardness is attributed to the significant grain refinement in the processed HEAs.Cavitation erosion behavior was observed for samples using an ultrasonication method.All of the HEAs showed better cavitation erosion resistance than the stainless steel 316L.The sample processed under a submerged liquid condition showed approximately 20 and 2 times greater erosion resistance than stainless steel 316L and ascast HEA,respectively.The enhanced erosion resistances of the processed HEAs correlate to their increased hardness,resistance to plasticity,and better yield strength than the as-cast HEA.The surface of the tested samples showed nucleation and pit growth,and plastic deformation of the material followed by fatigue-controlled disintegration as the primary material removal mechanism. | Rakesh B.Nair H.S.Arora Harpreet Singh Grewal | 2020 | International Journal of Minerals,Metallurgy and Materials2020,27,10: | 2 |

| 17 | Epigenetic regulation of autophagy: A key modification in cancer cells and cancer stem cells显示文摘Aberrant epigenetic alterations play a decisive role in cancer initiation and propagation via the regulation of key tumor suppressor genes and oncogenes or by modulation of essential signaling pathways.Autophagy is a highly regulated mechanism required for the recycling and degradation of surplus and damaged cytoplasmic constituents in a lysosome dependent manner.In cancer,autophagy has a divergent role.For instance,autophagy elicits tumor promoting functions by facilitating metabolic adaption and plasticity in cancer stem cells(CSCs)and cancer cells.Moreover,autophagy exerts pro-survival mechanisms to these cancerous cells by influencing survival,dormancy,immunosurveillance,invasion,metastasis,and resistance to anti-cancer therapies.In addition,recent studies have demonstrated that various tumor suppressor genes and oncogenes involved in autophagy,are tightly regulated via different epigenetic modifications,such as DNA methylation,histone modifications and non-coding RNAs.The impact of epigenetic regulation of autophagy in cancer cells and CSCs is not wellunderstood.Therefore,uncovering the complex mechanism of epigenetic regulation of autophagy provides an opportunity to improve and discover novel cancer therapeutics.Subsequently,this would aid in improving clinical outcome for cancer patients.In this review,we provide a comprehensive overview of the existing knowledge available on epigenetic regulation of autophagy and its importance in the maintenance and homeostasis of CSCs and cancer cells. | Harpreet K Mandhair Urban Novak Ramin Radpour | 2021 | World Journal of Stem Cells2021,13,6: | 1 |
