| 1 | 急性脑卒中综合性治疗模式的优越性显示文摘目的 探讨综合性治疗 [卒中单元 ,(strokeunit) ]模式对急性脑卒中治疗的优越性。方法 选择在澳大利亚卒中单元治疗的 2 0 7例急性脑卒中病人为A组 ,同期在我科常规治疗的 32 6例为B组。入选标准统一为经CT或MRI确诊发病 48h内的急性脑卒中 ,比较两组主要临床指标。结果 A组 1 4d内病死率为 5 3 % ,其中死于非脑内原因占 2 7 3 % ,感染发生率 1 7 9% ,而B组分别为1 0 4%、64 7%、47 9% ,差异具有显著性 (P <0 0 1 )。 2 1d神经功能评分和随访 90dRankin积分差异也有显著性 (P <0 0 1 )。结论 在综合性治疗模式下急性脑卒中患者的预后 。 | 王少石 Daniel Kam Yin CHAN David Gillies | 2002 | 中华内科杂志2002，41，11： | 47 |

| 4 | Pancreatic enzyme replacement therapy for pancreatic exocrine insufficiency in the 21^(st) century显示文摘Restitution of normal fat absorption in exocrine pancreatic insufficiency remains an elusive goal. Although many patients achieve satisfactory clinical results with enzyme therapy, few experience normalization of fat absorption, and many, if not most, will require individualized therapy. Increasing the quantity of lipase administered rarely eliminates steatorrhea but increases the cost of therapy. Enteric coated enzyme microbead formulations tend to separate from nutrients in the stomach precluding coordinated emptying of enzymes and nutrients. Unprotected enzymes mix well and empty with nutrients but are inactivated at pH 4 or below. We describe approaches for improving the results of enzyme therapy including changing to, or adding, a different product, adding non-enteric coated enzymes,(e.g., giving unprotected enzymes at the start of the mealand acid-protected formulations later), use of antisecretory drugs and/or antacids, and changing the timing of enzyme administration. Because considerable lipid is emptied in the first postprandial hour, it is prudent to start therapy with enteric coated microbead prior to the meal so that some enzymes are available during that first hour. Patients with hyperacidity may benefit from adjuvant antisecretory therapy to reduce the duodenal acid load and possibly also sodium bicarbonate to prevent duodenal acidity. Comparative studies of clinical effectiveness of different formulations as well as the characteristics of dispersion, emptying, and dissolution of enteric-coated microspheres of different diameter and density are needed; many such studies have been completed but not yet made public. Soil particles surrounding the leaking area could be mobilised, displaced, and even washed out of the soil matrix by the leaking fluid, generating a void or cavity. A two-dimensional simulation using a coupled discrete element method and lattice Boltzmann method (DEM-LBM) has been used to investigate the behaviour of a soil bed subject to a locally injected fluid, which represents a leak in a pipe. Various values of inter-particle surface energy were also adopted to model the mechanical effects of cohesive particles. | 14 | Mitochondrial neuronal uncoupling proteins:a target for potential disease-modification in Parkinson’s disease Philip显示文摘This review gives a brief insight into the role of mitochondrial dysfunction and oxidative stress in the converging pathogenic processes involved in Parkinson’s disease(PD).Mitochondria provide cellular energy in the form of ATP via oxidative phosphorylation,but as an integral part of this process,superoxides and other reactive oxygen species are also produced.Excessive free radical production contributes to oxidative stress.Cells have evolved to handle such stress via various endogenous anti-oxidant proteins.One such family of proteins is the mitochondrial uncoupling proteins(UCPs),which are anion carriers located in the mitochondrial inner membrane.There are five known homologues(UCP1 to 5),of which UCP4 and 5 are predominantly expressed in neural cells.In a series of previous publications,we have shown how these neuronal UCPs respond to 1-methyl-4-phenylpyridinium(MPP+;toxic metabolite of MPTP)and dopamine-induced toxicity to alleviate neuronal cell death by preserving ATP levels and mitochondrial membrane potential,and reducing oxidative stress.We also showed how their expression can be influenced by nuclear factor kappa-B(NF-B)signaling pathway specifically in UCP4.Furthermore,we previously reported an interesting link between PD and metabolic processes through the protective effects of leptin(hormone produced by adipocytes)acting via UCP2 against MPP+-induced toxicity.There is increasing evidence that these endogenous neuronal UCPs can play a vital role to protect neurons against various pathogenic stresses including those associated with PD.Their expression,which can be induced,may well be a potential therapeutic target for various drugs to alleviate the harmful effects of pathogenic processes in PD and hence modify the progression of this disease. | WL Ho Jessica WM Ho Hui-Fang Liu Danny HF So Zero HM Tse Koon-Ho Chan David B Ramsden Shu-Leong Ho | 2012 | Translational Neurodegeneration2012，1，1： | 3 |
