| 1 | Diabetic nephropathy:Is it time yet for routine kidney biopsy?显示文摘Diabetic nephropathy(DN)is one of the most important long-term complications of diabetes.Patients with diabetes and chronic kidney disease have an increased risk of all-cause mortality,cardiovascular mortality,and kidney failure.The clinical diagnosis of DN depends on the detection of microalbuminuria.This usually occurs after the first five years from the onset of diabetes,and predictors of DN development and progression are being studied but are not yet implemented into clinical practice.Diagnostic tests are useful tools to recognize onset,progression and response to therapeutic interventions.Microalbuminuria is an indicator of DN,and it is considered the only noninvasive marker of early onset.However,up to now there is no diagnostic tool that can predict which patients will develop DN before any damage is present.Pathological renal injury is hard to predict only with clinical and laboratory findings.An accurate estimate of damage in DN can only be achieved by the histological analysis of tissue samples.At the present time,renal biopsy is indicated on patients with diabetes under the suspicion of the presence of nephropathies other than DN.Results from renal biopsies in patients with diabetes had made possible the classification of renal biopsies in three major groups associated with different prognostic features:diabetic nephropathy,non-diabetic renal disease(NDRD),and a superimposed non-diabetic condition on underlying diabetic nephropathy.In patients with type 2 diabetes with a higher degree of suspicion for NDRD,it is granted the need of a renal biopsy.It is important to identify and differentiate these pathologies at an early stage in order to prevent progression and potential complications.Therefore,a more extensive use of biopsy is advisable. | Maria L Gonzalez Suarez David B Thomas Laura Barisoni Alessia Fornoni | 2013 | World Journal of Diabetes2013,4,6: | 29 |
| 2 | H pylori and host interactions that influence pathogenesis显示文摘H pylori is probably the most prevalent human patho- gen worldwide. Since it was initially suggested in 1983 by Marshall and Warren to be implicated in gastritis and peptic ulcer disease, H pylori has also been implicated in gastric carcinoma and was classified as a class I car- cinogen. In the last two decades, a noteworthy body of research has revealed the multiple processes that this gram negative bacterium activates to cause gastroduo- denal disease in humans. Most infections are acquired early in life and may persist for the life of the individual. While infected individuals mount an inflammatory re- sponse that becomes chronic, along with a detectable adaptive immune response, these responses are ineffec- tive in clearing the infection. H pylori has unique features that allow it to reside within the harsh conditions of the gastric environment, and also to evade the host immune response. | 5 | Conversion from calcineurin inhibitors to mTOR inhibitors stabilizes diabetic and hypertensive nephropathy after liver transplant显示文摘AIM: To investigate if conversion to the mammalian target of rapamycin inhibitors(mTORi) improves renal function in diabetic and/or hypertensive liver transplant patients immunosuppressed with tacrolimus or cyclosporine.METHODS: The study included 86 liver graft recipients immunosuppressed with mTORi treatment after orthotopic liver transplantation(OLT), including all liver recipients with worsening renal function before conversion to mTORi(n = 55 patients) and recipients with normal renal function who converted to m TORi for other reasons(n = 31 patients). We identified patients with diabetes mellitus(n = 28), arterial hypertension(n = 27), proteinuria(n = 27) and all three factors(n = 8)(some patients have hypertension and diabetes and no proteinuria). The primary endpoint was evolution in renal function defined as the development in plasma creatinine as a function of diabetes mellitus(DM), hypertension(HT) or proteinuria. We required elevated serum creatinine for at least two weeks to define renal dysfunction.RESULTS: Only patients that converted because of renal failure with plasma creatinine levels > 1.5 mg/dL showed an improvement of renal function(2.14 to 1.77 mg/dL)(P = 0.02). Patients with DM showed no improvement of serum creatinine levels(1.31 mg/dL to 1.37 mg/dL) compared with non DM patients(1.31 mg/dL to 1.15 mg/dL)(P = 0.01), HT patients(1.48 mg/dL to 1.5 mg/dL) with non HT patients(1.21mg/d L to 1.08 mg/dL) and patients with proteinuria(1.44 mg/dL to 1.41 mg/dL) and no proteinuria(1.31 mg/dL to 1.11 mg/dL). CONCLUSION: In OLT recipients with diabetes or hypertensive nephropathy, conversion to m TORi does not improve renal function but stabilizes plasma levels of creatinine. Proteinuria is not a contraindication to conversion to m TORi; it also stabilizes renal function. | 7 | Interplay between microstructural evolution and tribo-chemistry during dry sliding of metals显示文摘Understanding the microstructural and tribo-chemical processes during tribological loading is of utmost importance to further improve the tribological behavior of metals. In this study, the friction, wear and tribo-chemical behavior of Ni with different initial microstructures(nanocrystalline, bi-modal, coarse-grained) is investigated under dry sliding conditions. In particular, the interplay be-tween frictional response, microstructural evolution and tribo-oxidation is considered. Friction tests are carried out using ball-on-disk experiments with alumina balls as counter-bodies, varying the load between 1 and 5 N. The microstructural evolution as well as the chemical reactions beneath the samples’ surface is investigated by means of cross-sections. The samples with finer microstructures show a faster run-in and lower maximum values of the coefficient of friction(COF) which can be attributed to higher oxidation kinetics and a higher hardness. It is observed that with increasing sliding cycles, a stable oxide layer is formed. Furthermore, initially coarse-grained samples show grain refinement, whereas initially finer microstructures undergo grain coarsening converging towards the same superficial grain size after 2,000 sliding cycles. | 9 | Intraoperative laparoscopic complications for urological cancer procedures显示文摘AIM: To structure the rate of intraoperative complications that requires an intraoperative or perioperative resolution. METHODS: We perform a literature review of Medline database. The research was focused on intraoperative laparoscopic procedures inside the field of urological oncology. General rate of perioperative complications in laparoscopic urologic surgery is described to be around 12.4%. Most of the manuscripts published do not make differences between pure intraoperative, intraoperative with postoperative consequences and postoperative complications. RESULTS: We expose a narrative statement of complications, possible solutions and possible preventions for most frequent retroperitoneal and pelvic laparoscopic surgery. We expose the results with the following order: retroperitoneal laparoscopic surgery(radical nephrectomy, partial nephrectomy, nephroureterectomy and adrenalectomy) and pelvic laparoscopic surgery(radical prostatectomy and radical cystectomy).CONCLUSION: Intraoperative complications vary from different series. | 12 | Development of a 100 J,10 Hz laser for compression experiments at the High Energy Density instrument at the European XFEL显示文摘In this paper we review the design and development of a 100 J, 10 Hz nanosecond pulsed laser, codenamed DiPOLE100 X,being built at the Central Laser Facility(CLF). This 1 kW average power diode-pumped solid-state laser(DPSSL) is based on a master oscillator power amplifier(MOPA) design, which includes two cryogenic gas cooled amplifier stages based on DiPOLE multi-slab ceramic Yb:YAG amplifier technology developed at the CLF. The laser will produce pulses between 2 and 15 ns in duration with precise, arbitrarily selectable shapes, at pulse repetition rates up to 10 Hz, allowing real-time shape optimization for compression experiments. | 14 | The effect of total carbon on microscopic soil properties and implications for crop production显示文摘Soil structure is a dynamic property affected by physical, chemical, and microbiological processes. Addition of organic matter to soils and the use of different management practices have been reported to impact soil structure and crop production. Moderation in soil temperature and increases in microbial activity and soil water retention are often suggested as reasons for the rise in crop yield when organic matter is added to the soil. Less is known about the direct effect of changes in soil structure on crop production. A field experiment was conducted to study the effect of summer cover crop and in-season management system on soil structure. The experiment was a nested design with summer cover crop as the main plot and management system as the subplot. Summer cover crop treatments included cowpea (Vigna unguiculata L. Walp.) incorporated into the soil in the fall (CI), cowpea used as mulch in the fall (CM), sudangrass (Sorghum vulgare) incorporated into the soil in the fall (S), and dry fallow or bare ground (B). Management systems were organic (ORG) and conventional (CNV) systems. Lettuce (Lactuca sativa L.) and cantaloupes (Cucumis melo L.) were cultivated in rotation in the plots for three consecutive years using the same cover crops and management systems for each plot. Disturbed and undisturbed soil cores were collected at the end of the third year and used for laboratory experiments to measure physical, chemical, and hydraulic properties. Image analysis was used to quantify soil structure properties using a scanning electron microscope on thin sections prepared from the undisturbed soil cores. We found that total soil carbon was correlated with porosity, saturation percentage, and pore roughness. Pore roughness was correlated with crop production in general and with marketable production in particular. | 17 | H pylori receptor MHC classⅡcontributes to the dynamic gastric epithelial apoptotic response显示文摘AIM: To investigate the role of MHC classⅡin the modulation of gastric epithelial cell apoptosis induced by H pylon infection. METHODS: After stimulating a human gastric epithelial cell line with bacteria or agonist antibodies specific for MHC classⅡand CD95, the quantitation of apoptotic and anti-apoptotic events, including caspase activation, BCL-2 activation, and FADD recruitment, was performed with a fluorometric assay, a cytometric bead array, and confocal microscopy, respectively. RESULTS: Pretreatment of N87 cells with the anti-MHC classⅡIgM antibody RFD1 resulted in a reduction in global caspase activation at 24 h of H pylori infection. When caspase 3 activation was specifically measured, crosslinking of MHC class n resulted in markedly reduced caspase activation, while simple ligation of MHC classⅡdid not. Crosslinking of MHC class n also resulted in an increased activation of the anti-apoptosis molecule BCL-2 compared to simple ligation. Confocal microscope analysis demonstrated that the pretreatment of gastric epithelial cells with a crosslinking anti-MHC classⅡIgM blocked the recruitment of FADD to the cell surface. CONCLUSION: The ability of MHC class n to modulate gastric epithelial apoptosis is at least partially dependent on its crosslinking. The crosslinking of this molecule has anti-apoptotic effects during the earlier time points of H pylori infection.
