| 2 | A 2D DEM-LBM study on soil behaviour due to locally injected fluid显示文摘Leakage from underground pipes could result in foundations being undermined and cause damage to adjacent infrastructure. Soil particles surrounding the leaking area could be mobilised, displaced, and even washed out of the soil matrix by the leaking fluid, generating a void or cavity. A two-dimensional simulation using a coupled discrete element method and lattice Boltzmann method (DEM-LBM) has been used to investigate the behaviour of a soil bed subject to a locally injected fluid, which represents a leak in a pipe. Various values of inter-particle surface energy were also adopted to model the mechanical effects of cohesive particles. | 4 | Proton pump inhibitor use increases mortality and hepatic decompensation in liver cirrhosis显示文摘BACKGROUND Proton pump inhibitors(PPIs)are widely prescribed,often without clear indications.There are conflicting data on its association with mortality risk and hepatic decompensation in cirrhotic patients.Furthermore,PPI users and PPI exposure in some studies have been poorly defined with many confounding factors.AIM To examine if PPI use increases mortality and hepatic decompensation and the impact of cumulative PPI dose exposure.METHODS Data from patients with decompensated liver cirrhosis were extracted from a hospital database between 2013 to 2017.PPI users were defined as cumulative defined daily dose(cDDD)≥28 within a landmark period,after hospitalisation for hepatic decompensation.Cox regression analysis for comparison was done after propensity score adjustment.Further risk of hepatic decompensation was analysed by Poisson regression.RESULTS Among 295 decompensated cirrhosis patients,238 were PPI users and 57 were non-users.PPI users had higher mortality compared to non-users[adjusted HR=2.10,(1.20-3.67);P=0.009].Longer PPI use with cDDD>90 was associated with higher mortality,compared to non-users[aHR=2.27,(1.10-5.14);P=0.038].PPI users had a higher incidence of hospitalization for hepatic decompensation[aRR=1.61,(1.30-2.11);P<0.001].CONCLUSION PPI use in decompensated cirrhosis is associated with increased risk of mortality and hepatic decompensation.Longer PPI exposure with cDDD>90 increases the risk of mortality. | Marianne Anastasia De Roza Lim Kai Jia Wen Kam Yiong Huak Chan Andrew Kwek Tiing Leong Ang John Chen Hsiang | 2019 | World Journal of Gastroenterology2019,25,33: | 6 |
| 5 | GPGPU-parallelised hybrid finite-discrete element modelling of rock chipping and fragmentation process in mechanical cutting显示文摘Mechanical cutting provides one of the most flexible and environmentally friendly excavation methods.It has attracted numerous efforts to model the rock chipping and fragmentation process,especially using the explicit finite element method(FEM) and bonded particle model(BPM),in order to improve cutting efficiency.This study investigates the application of a general-purpose graphic-processing-unit parallelised hybrid finite-discrete element method(FDEM) which enjoys the advantages of both explicit FEM and BPM,in modelling the rock chipping and fragmentation process in the rock scratch test of mechanical rock cutting.The input parameters of FDEM are determined through a calibration procedure of modelling conventional Brazilian tensile and uniaxial compressive tests of limestone,A series of scratch tests with various cutting velocities,cutter rake angles and cutting depths is then modelled using FDEM with calibrated input parameters.A few cycles of cutter/rock interactions,including their engagement and detachment process,are modelled for each case,which is conducted for the first time to the best knowledge of the authors,thanks to the general purpose graphic processing units(GPGPU) parallelisation.The failure mechanism,cutting force,chipping morphology and effect of various factors on them are discussed on the basis of the modelled results.Finally,it is concluded that GPGPU-parallelised FDEM provides a powerful tool to further study rock cutting and improve cutting efficiencies since it can explicitly capture different fracture mechanisms contributing to the rock chipping as well as chip formation and the separation process in mechanical cutting.Moreover,it is concluded that chipping is mostly owed to the mix-mode Ⅰ-Ⅱ fracture in all cases although mode Ⅱ cracks and mode Ⅰ cracks are the dominant failures in rock cutting with shallow and deep cutting depths,respectively.The chip morphology is found to be a function of cutter velocdty,cutting depth and cutter rake angle. | Mojtaba Mohammadnejad Sevda Dehkhoda Daisuke Fukuda Hongyuan Liu Andrew Chan | 2020 | Journal of Rock Mechanics and Geotechnical Engineering2020,12,2: | 5 |
| 6 | Identification of Ca2+ signaling components in neural stem/progenitor cells during differentiation into neurons and glia in intact and dissociated zebrafish neurospheres显示文摘The development of the CNS in vertebrate embryos involves the generation of different sub-types of neurons and glia in a complex but highly-ordered spatio-temporal manner. Zebrafish are commonly used for exploring the development, plasticity and regeneration of the CNS, and the recent development of reliable protocols for isolating and culturing neural stem/progenitor cells(NSCs/NPCs) from the brain of adult fish now enables the exploration of mechanisms underlying the induction/specification/differentiation of these cells. Here, we refined a protocol to generate proliferating and differentiating neurospheres from the entire brain of adult zebrafish. We demonstrated via RT-qPCR that some isoforms of ip3 r, ryr and stim are upregulated/downregulated significantly in differentiating neurospheres, and via immunolabelling that 1,4,5-inositol trisphosphate receptor(IP3 R) type-1 and ryanodine receptor(RyR) type-2 are differentially expressed in cells with neuron-or radial glial-like properties. Furthermore, ATP but not caffeine(IP3 R and RyR agonists, respectively), induced the generation of Ca^(2+) transients in cells exhibiting neuron-or glial-like morphology. These results indicate the differential expression of components of the Ca^(2+) -signaling toolkit in proliferating and differentiating cells. | 9 | Comparative effectiveness of i-SCAN^(TM) and high-definition white light characterizing small colonic polyps显示文摘AIM:To evaluate accuracy of in vivo diagnosis of adenomatous vs non-adenomatous polyps using i-SCAN digital chromoendoscopy compared with high-definition white light.METHODS:This is a single-center comparative effectiveness pilot study.Polyps(n = 103) from 75 averagerisk adult outpatients undergoing screening or surveillance colonoscopy between December 1,2010 and April 1,2011 were evaluated by two participating endoscopists in an academic outpatient endoscopy center.Polyps were evaluated both with high-definition white light and with i-SCAN to make an in vivo prediction of adenomatous vs non-adenomatous pathology.We determined diagnostic characteristics of i-SCAN and highdefinition white light,including sensitivity,specificity,and accuracy,with regards to identifying adenomatous vs non-adenomatous polyps.Histopathologic diagnosis was the gold standard comparison.RESULTS:One hundred and three small polyps,detected from forty-three patients,were included in the analysis.The average size of the polyps evaluated in the analysis was 3.7 mm(SD 1.3 mm,range 2 mm to 8 mm).Formal histopathology revealed that 54/103(52.4%) were adenomas,26/103(25.2%) were hyperplastic,and 23/103(22.3%) were other diagnoses include 'lymphoid aggregates','non-specific colitis,' and 'no pathologic diagnosis.' Overall,the combined accuracy of endoscopists for predicting adenomas was identical between i-SCAN(71.8%,95%CI:62.1%-80.3%) and high-definition white light(71.8%,95%CI:62.1%-80.3%).However,the accuracy of each endoscopist differed substantially,where endoscopist A demonstrated 63.0% overall accuracy(95%CI:50.9%-74.0%) as compared with endoscopist B demonstrating 93.3% overall accuracy(95%CI:77.9%-99.2%),irrespective of imaging modality.Neither endoscopist demonstrated a significant learning effect with i-SCAN during the study.Though endoscopist A increased accuracy using i-SCAN from 59%(95%CI:42.1%-74.4%) in the first half to 67.6%(95%CI:49.5%-82.6%) in the second half,and endoscopist B decreased accuracy using i-SCAN from 100%(95%CI:80.5%-100.0%) in the first half to 84.6%(95%CI:54.6%-98.1%) in the second half,neither of these differences were statistically significant.CONCLUSION:i-SCAN and high-definition white light had similar efficacy predicting polyp histology.Endoscopist training likely plays a critical role in diagnostic test characteristics and deserves further study. | Johanna L Chan Li Lin Michael Feiler Andrew I Wolf Diana M Cardona Ziad F Gellad | 2012 | World Journal of Gastroenterology2012,18,41: | 3 |

| 17 | Cilostazol for secondary stroke prevention:systematic review and meta-analysis显示文摘Background Stroke is one of the leading causes of death worldwide.Cilostazol,an antiplatelet and phosphodiesterase 3 inhibitor,has not been clearly established for ischaemic stroke use.We aim to determine the efficacy and safety of cilostazol for secondary stroke prevention.Methods MEDLINE,EMBASE,Cochrane Library,Web of Science and ClinicalTrials.gov were searched from inception to 25 September 2020,for randomised trials comparing the efficacy and safety of cilostazol monotherapy or dual therapy with another antiplatelet regimen or placebo,in patients with ischaemic stroke.Version 2 of the Cochrane risk-of-bias tool for randomised trials(RoB 2)was used to assess study quality.This meta-analysis was reported in line with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)statement.Results Eighteen randomised trials comprising 11429 participants were included in this meta-analysis.Most trials possessed low risk of bias and were of low heterogeneity.Cilostazol significantly reduced the rate of ischaemic stroke recurrence(risk ratio,RR=0.69,95%CI 0.58 to 0.81),any stroke recurrence(RR=0.64,95%CI 0.54 to 0.74)and major adverse cardiovascular events(RR=0.67,95%CI 0.56 to 0.81).Cilostazol did not significantly decrease mortality(RR=0.90,95%CI 0.64 to 1.25)or increase the rate of good functional outcome(Modified Rankin Scale score of 0–1;RR=1.07,95%CI 0.95 to 1.19).Cilostazol demonstrated favourable safety profile,significantly reducing the risk of intracranial haemorrhage(RR=0.46,95%CI 0.31 to 0.68)and major haemorrhagic events(RR=0.49,95%CI 0.34 to 0.70).Conclusions Cilostazol demonstrated superior efficacy and safety profiles compared with traditional antiplatelet regimens such as aspirin and clopidogrel for secondary stroke prevention but does not appear to affect functional outcomes.Future randomised trials can be conducted outside East Asia,or compare cilostazol with a wider range of antiplatelet agents. | Choon Han Tan Andrew GR Wu Ching-Hui Sia Aloysius ST Leow Bernard PL Chan Vijay Kumar Sharma Leonard LL Yeo Benjamin YQ Tan | 2021 | Stroke & Vascular Neurology2021,6,3: | 2 |
| 18 | Modelling the impact of sea-level rise on urban flood probability in SE China显示文摘Urban drainage systems in coastal cities in SE China are characterized by often complex canal and sluicegate systems that are designed to safely drain pluvial flooding whilst preventing tidal inundation.However, the risk of coastal flooding in the region is expected to increase over the next 50-100 years, as urban areas continue to expand and sea-levels are expected to rise. To assess the impact of projected sealevel rise on this type of urban drainage system, a one-dimensional model and decision support tool was developed. The model indicated that although sea-level rise represents a significant challenge, flood probability will continue to be most influenced by rainfall. Events that are significant enough to cause flooding will most likely be minimally impacted by changes to the tidal frame.
