1Management of recurrent rectal cancer:A population based study in greater Amsterdam显示文摘AIM: To analyze,retrospectively in a population-based study,the management and survival of patients with recurrent rectal cancer initially treated with a macroscopically radical resection obtained with total mesorectal excision (TME). METHODS: All rectal carcinomas diagnosed during 1998 to 2000 and initially treated with a macroscopically radical resection (632 patients) were selected from the Amsterdam Cancer Registry. For patients with recurrent disease,information on treatment of the recurrence was collected from the medical records. RESULTS: Local recurrence with or without clinically apparent distant dissemination occurred in 62 patients (10%). Thirty-two patients had an isolated local recurrence. Ten of these 32 patients (31%) underwent radical re-resection and experienced the highest survival (three quarters survived for at least 3 years). Eight patients (25%) underwent non-radical surgery (median survival 24 mo),seven patients (22%) were treated with radio-and/or chemotherapy without surgery (median survival 15 mo) and seven patients (22%) only received best supportive care (median survival 5 mo). Distant dissemination occurred in 124 patients (20%) of whom 30 patients also had a local recurrence. The majority (54%) of these patients were treated with radio-and/or chemotherapy without surgery (median survival 15 mo). Twenty-seven percent of these patients only received best supportive care (median survival 6 mo),while 16% underwent surgery for their recurrence. Survival was best in the latter group (median survival 32 mo). 2Current status of ultrasound-guided surgery in the treatment of breast cancer显示文摘The primary goal of breast-conserving surgery(BCS) is to obtain tumour-free resection margins. Margins positive or focally positive for tumour cells are associated with a high risk of local recurrence, and in the case of tumour-positive margins, re-excision or even mastectomy are sometimes needed to achieve definite clear margins. Unfortunately, tumour-involved margins and re-excisions after lumpectomy are still reported in up to 40% of patients and additionally, unnecessary large excision volumes are described. A secondary goal of BCS is the cosmetic outcome and one of the main determinants of worse cosmetic outcome is a large excision volume. Up to 30% of unsatisfied cosm-etic outcome is reported. Therefore, the search for better surgical techniques to improve margin status, excision volume and consequently, cosmetic outcome has continued. Nowadays, the most commonly used localization methods for BCS of non-palpable breast cancers are wire-guided localization(WGL) and radioguided localization(RGL). WGL and RGL are invasive procedures that need to be performed pre-operatively with technical and scheduling difficulties. For palpable breast cancer, tumour excision is usually guided by tactile skills of the surgeon performing 'blind' surgery. One of the surgical techniques pursuing the aims of radicality and small excision volumes includes intraoperative ultrasound(IOUS). The best evidence available demonstrates benefits of IOUS with a significantly high proportion of negative margins compared with other localization techniques in palpable and non-palpable breast cancer. Additionally, IOUS is non-invasive, easy to learn and can centralize the tumour in the excised specimen with low amount of healthy breast tissuebeing excised. This could lead to better cosmetic results of BCS. Despite the advantages of IOUS, only a small amount of surgeons are performing this technique. 13Phylogeny,ecology and taxonomy of systemic pathogens and their relatives in Ajellomycetaceae(Onygenales):Blastomyces,Emergomyces,Emmonsia,Emmonsiellopsis显示文摘The family Ajellomycetaceae(Onygenales)includes mammal-associated pathogens within the genera Blastomyces,Emmonsia,Histoplasma and Paracoccidioides,as well as the recently described genera,Emergomyces that causes disease in immunocompromised hosts,and Emmonsiellopsis,known only from soil.To further assess the phylogenetic relationships among and between members of these genera and several previously undescribed species,we sequenced and analyzed the DNA-directed RNA polymerase II(rPB2),translation elongation factor 3-a(TEF3),b-tubulin(TUB2),28S large subunit rDNA(LSU)and the internal transcribed spacer regions(ITS)in 68 strains,in addition to morphological and physiological investigations.To better understand the thermal dimorphism among these fungi,the dynamic process of transformation from mycelial to yeast-like or adiaspore-like forms was also assessed over a range of temperatures(6–42C).Molecular data resolved the relationships and recognized five major well-supported lineages that correspond largely to the genus level.Emmonsia,typified by Emmonsia parva,is a synonym of Blastomyces that also accommodates Blastomyces helicus(formerly Emmonsia helica).Emmonsia crescens is phylogenetically distinct,and found closely related to a single strain from soil without known etiology.Blastomyces silverae,Emergomyces canadensis,Emergomyces europaeus and Emmonsia sola are newly described.Almost all of the taxa are associated with human and animal disease.Emmonsia crescens,B.dermatitidis and B.parvus are prevalently associated with pulmonary disease in humans or animals.Blastomyces helicus,B.percursus,Emergomyces africanus,Es.canadensis,Es.europaeus,Es.orientalis and Es.pasteurianus(formerly Emmonsia pasteuriana)are predominantly found in human hosts with immune disorders;no animal hosts are known for these species except B.helicus.Yanping Jiang Karolina Dukik Jose F.Munoz Lynne Sigler Ilan S.Schwartz Nelesh P.Govender Chris Kenyon Peiying Feng Bert Gerrits van den Ende J.Benjamin Stielow Alberto M.Stchigel Hongguang Lu Sybren de Hoog 2018Fungal Diversity2018,,3:1

20Adeno-associated virus mediated delivery of Tregitope 167 ameliorates experimental colitis显示文摘AIM:To explore the anti-inflammatory potential of adeno-associated virus-mediated delivery of Tregitope 167 in an experimental colitis model.METHODS:The trinitrobenzene sulfonate(TNBS) model of induced colitis was used in Balb/c mice.Subsequently after intravenous adeno-associated virusmediated regulatory T-cell epitopes(Tregitope) delivery,acute colitis was initiated by intra-rectal administration of 1.5 mg TNBS in 40% ethanol followed by a second treatment with TNBS(0.75 mg in 20% ethanol) 8 d later.Control groups included mice not treated with TNBS(healthy control group) and mice treated by TNBS only(diseased group).At the time of sacrifice colon weight,the disease activity index and histology damage score were determined.Immunohistochemical staining of the colonic tissues was performed to asses the cellular infiltrate and the presence of transcription factor forkhead Box-P3(Foxp3).Thymus,mesenteric lymph nodes,liver and spleen tissue were collected and the corresponding lymphocyte populations were further assessed by flow cytometry analysis for the expression of CD4+ T cell and regulatory T cell associated markers.RESULTS:The Tregitope 167 treated mice gained an average of 4% over their initial body weight at the time of sacrifice.In contrast,the mice treated with TNBS alone(no Tregitope) developed colitis,and lost 4% of their initial body weight at the time of sacrifice(P < 0.01).The body weight increase that had been observed in the mice pre-treated with Tregitope 167 was substantiated by a lower disease activity index and a decreased colon weight as compared to the diseased control group(P < 0.01 and P < 0.001,respectively).Immunohistochemical staining of the colonic tissues for CD4+ showed that inflammatory cell infiltrates were present in TNBS treated mice with or without administration with tregitope 167 and that these cellular infiltrates consisted mainly of CD4+ cells.For both TNBS treated groups CD4+ T cell infiltrates were observed in the sub-epithelial layer and the lamina propria.CD4+ T cell infiltrates were also present in the muscularis mucosa layer of the diseased control mice,but were absent in the Tregitope 167 treated group.Numerous Foxp3 positive cells were detected in the lamina propria and sub-epithelium of the colon sections from mice treated with Tregitope 167.Furthermore,the Foxp3 and glycoprotein A repetitions predominant markers were significantly increased in the CD4+ T lymphocyte population in the thymus of the mice pre-treated with adeno-associated virus serotype 5(cytomegalovirus promoter-Tregitope 167),as cytomegalovirus promoter compared to lymphocyte populations in the thymus of diseased and the healthy control mice(P < 0.05 and P < 0.001,respectively).CONCLUSION:This study identifies adeno-associated virus-mediated delivery of regulatory T-cell epitope 167 as a novel anti-inflammatory approach with the capacity to decrease intestinal inflammation and induce longterm remission in inflammatory bowel disease.Sander van der Marel Anna Majowicz Karin Kwikkers Richard van Logtenstein Anje A te Velde Anne S De Groot Sybren L Meijer Sander J van Deventer Harald Petry Daniel W Hommes Valerie Ferreira 2012World Journal of Gastroenterology2012,18,32:0
