| 1 | Transdermal drug delivery systems in diabetes management: A review显示文摘Diabetes mellitus is a chronic disease in which there is an insufficient production of insulin by the pancreas, or the insulin produced is unable to be utilized effectively by the body. Diabetes affects more than 415 million people globally and is estimated to strike about 642 million people in 2040. The WHO reported that diabetes will become the seventh biggest cause of mortality in 2030. Insulin injection and oral hypoglycemic agents remain the primary treatments in diabetes management. These often present with poor patient compliance. However, over the last decade, transdermal systems in diabetes management have gained increasing attention and emerged as a potential hope in diabetes management owing to the advantages that they offer as compared to invasive injection and oral dosage forms. This review presents the recent advances and developments in transdermal research to achieve better diabetes management. Different technologies and approaches have been explored and applied to the transdermal systems to optimize diabetes management. Studies have shown that these transdermal systems demonstrate higher bioavailability compared to oral administration due to the avoidance of first-pass hepatic metabolism and a sustained drug release pattern. Besides that, transdermal systems have the advantage of reducing dosing frequency as drugs are released at a predetermined rate and control blood glucose level over a prolonged time, contributing to better patient compliance. | 2 | Systematic review of emergent laparoscopic colorectal surgery for benign and malignant disease显示文摘Laparoscopic surgery has become well established in the management of both and malignant colorectal disease.The last decade has seen increasing numbers of surgeons trained to a high standard in minimallyinvasive surgery.However there has not been the same enthusiasm for the use of laparoscopy in emergency colorectal surgery.There is a perception that emergent surgery is technically more difficult and may lead to worse outcomes.The present review aims to provide a comprehensive and critical appraisal of the available literature on the use of laparoscopic colorectal surgery(LCS)in the emergency setting.The literature is broadly divided by the underlying pathology;that is,inflammatory bowel disease,diverticulitis and malignant obstruction.There were no randomized trials and the majority of the studies were case-matched series or comparative studies.The overall trend was that LCS is associated with shorter hospital stay,par or fewer complications but an increased operating time.Emergency LCS can be safely undertaken for both benign and malignant disease providing there is appropriate patient selection,the surgeon is adequately experienced and there are sufficient resources to allow for a potentially more complex operation. | Manish Chand Muhammed RS Siddiqui Ashish Gupta Shahnawaz Rasheed Paris Tekkis Amjad Parvaiz Alex H Mirnezami Tahseen Qureshi | 2014 | World Journal of Gastroenterology2014,20,45: | 7 |

| 6 | 闪光对于减轻静脉置管痛的作用:一项前瞻性随机安慰剂对照研究显示文摘背景尽管静脉置管时的疼痛经常让人备受困扰,但通常都是在没有任何镇痛措施下进行静脉置管的。临床上曾尝试用一些药物和非药物方法来减轻静脉置管的疼痛,但结果各异。本研究拟评价一次闪光对于减轻静脉置管痛的效果。方法90例拟择期行腹腔镜胆囊切除术的ASAⅠ~Ⅱ级的成年患者(15~60岁),随机分为3组,每组30例,组1为对照组;组2为分散注意力组,即在非优势手的手背静脉进行置管前给患者照相而不使用闪光;组3为闪光组,静脉置管前照相并且使用闪光;在照相后立即使用18G的穿刺针进行静脉置管。结果每组均有2例患者首次静脉穿刺失败,在数据分析中即予以剔除,闪光组疼痛的发生率为50%(28例中14例发生),其他2组为100%(28例全部发生),通过视觉模拟评分评估的疼痛严重程度分散注意力组和闪光组显著低于对照组,而闪光组也显著低于分散注意力组。结论静脉置管前使用闪光是一种减轻疼痛的安全而简单有效的方法。 | Anil Agarwal GhanshvamYadav Devendra Gupta Manish Tandon Prabhat Kumar Singh Uttam Singh 吴志林(译) | 2009 | 麻醉与镇痛2009,,3: | 1 |

| 13 | Müllerian duct anomalies and their effect on the radiotherapeutic management of cervical cancer显示文摘Radiotherapy plays a major role in the treatment of cervical cancer. A successful radiotherapy program integrates both external beam and brachytherapy components. The principles of radiotherapy are strongly based on the anatomy of the organ and patterns of local and nodal spread. However, in patients with distorted anatomy, several practical issues arise in the delivery of optimal radiotherapy, especially with brachytherapy. Müllerian duct anomalies result in congenital malformations of the female genital tract. Though being very commonly studied for their deleterious effects on fertility and pregnancy, they have not been recognized for their potential to interfere with the delivery of radiotherapy among patients with cervical cancer.
