1A gist of gastrointestinal stromal tumors: A review显示文摘Gastrointestinal stromal tumors (GISTs) have been recognized as a biologically distinctive tumor type, different from smooth muscle and neural tumors of the gastrointestinal tract (GIT). They constitute the majority of gastrointestinal mesenchymal tumors of the GIT and are known to be refractory to conventional chemotherapy or radiation. They are defined and diagnosed by the expression of a proto-oncogene protein detected by immunohistochemistry which serves as a crucial diagnostic and therapeutic target. The identification of these mutations has resulted in a better understanding of their oncogenic mechanisms. The remarkable antitumor effects of the molecular inhibitor imatinib have necessitated accurate diagnosis of GIST and their distinction from other gastrointestinal mes-enchymal tumors. Both traditional and minimally invasive surgery are used to remove these tumors with minimal morbidity and excellent perioperative outcomes. The revolutionary use of specific, molecularlytargeted therapies, such as imatinib mesylate, reduces the frequency of disease recurrence when used as an adjuvant following complete resection. Neoadjuvant treatment with these agents appears to stabilize disease in the majority of patients and may reduce the extent of surgical resection required for subsequent complete tumor removal. The important interplay between the molecular genetics of GIST and responses to targeted therapeutics serves as a model for the study of targeted therapies in other solid tumors. 2Study of Bacillus subtilis on growth performance, nutrition metabolism and intestinal microflora of 1 to 42 d broiler chickens显示文摘To study the influence of different levels of Bacillus subtilis on growth performance, nutrition metabolism and intestinal microflora of 1 to 42 d Arbor Acres(AA) broilers, a total of 800 one-day-old healthy broilers were randomly divided into 5 groups with 4 replicates per group and 40 broilers per replicate. Broilers were fed a basic diet(group 1) which acted as the control group, and 4 other groups(2 to 5) were fed the basal diet with B. subtilis added at concentrations of 100, 150, 200 and 250 mg/kg, respectively for 42 days. The results showed as follow: the average daily gain(ADG) of group 4 was significantly higher than(P < 0.05) that of group 1, and the average daily feed intake(ADFI) of group 5 was the highest but the differences between groups were not significant(P > 0.05). The feed to gain ratio(F/G) of all the experimental groups was lower than that of the control and the difference was significant in group 4(P < 0.05). In addition, supplementation of B. subtilis increased the apparent metabolism of crude protein(P > 0.05), crude fat(P > 0.05), dry matter(P > 0.05) and organic matter(P < 0.05). B. subtilis decreased the Escherichia coli and Salmonella populations in the cecum. This shows that adding B. subtilis to the broiler diet can improve the growth performance, increase feed efficiency, regulate serum index and reduce harmful bacteria in the intestinal tract. 3Delphi methodology in healthcare research:How to decide its appropriateness显示文摘The Delphi technique is a systematic process of forecasting using the collective opinion of panel members.The structured method of developing consensus among panel members using Delphi methodology has gained acceptance in diverse fields of medicine.The Delphi methods assumed a pivotal role in the last few decades to develop best practice guidance using collective intelligence where research is limited,ethically/logistically difficult or evidence is conflicting.However,the attempts to assess the quality standard of Delphi studies have reported significant variance,and details of the process followed are usually unclear.We recommend systematic quality tools for evaluation of Delphi methodology;identification of problem area of research,selection of panel,anonymity of panelists,controlled feedback,iterative Delphi rounds,consensus criteria,analysis of consensus,closing criteria,and stability of the results.Based on these nine qualitative evaluation points,we assessed the quality of Delphi studies in the medical field related to coronavirus disease 2019.There was inconsistency in reporting vital elements of Delphi methods such as identification of panel members,defining consensus,closing criteria for rounds,and presenting the results.We propose our evaluation points for researchers,medical journal editorial boards,and reviewers to evaluate the quality of the Delphi methods in healthcare research.Prashant Nasa Ravi Jain Deven Juneja 2021World Journal of Methodology2021,11,4:21
4Risk assessment of heavy metal contaminated soil in the vicinity of a lead/zinc mine显示文摘Heavy metal contamination of soils through anthropogenic activities is a widespread and serious problem confronting scientists and regulators throughout the world. In this study we investigated the distribution, chemical species and availability of lead, zinc, cadmium and copper in nine surface(0 to 20 cm) soils from near an abandoned lead/zinc mine tailings located in Shaoxing, Zhejiang, China. Total heavy metal contents ranged from 5271 to 16369 mg/kg for Pb, 387 to 1221 mg/kg for Zn, 3.0 to 9.3 mg/kg for Cd and 65 to 206 mg/kg for Cu. In general, all heavy metals exceeded China National Standards for Soil Environmental Quality of Heavy Metals by a factor of 3—65 times. Comparison of the heavy metal concentrations(Pb, Zn, Cd and Cu) with clay content revealed a strongly significant relationship while significant relationship(P<0.001) was also obtained between Cd+Zn and Pb+Cu. Solid phase speciation of the soils using Tessier procedure showed that the heavy metals were distributed in the order: residual  organically complexed-Fe-Mn oxides occluded>carbonate bound>exchangeable > water soluble. In the organic matter fraction, the ratio of Pb(29.1%) to its total concentration in the soils was higher than those of Zn(4.70%), Cd(3.16%) and Cu(9.50%). The percentages of the water soluble and the exchangeable fractions of Pb(1.80%) and Cd(2.74%) were markedly greater than those of Zn(0.10%) and Cu(0.15%), suggesting that Pb and Cd are relatively more mobile and hence more toxic in the contaminated soils. Strongly significant relationships between H2O-Pb, H2O-Zn and H2O-Cu, strong positive correlations between H2O-Pb, H2O-Zn, H2O-Cu and organic matter in soil were found. The content of H2O-Pb, H2O-Zn, H2O-Cu was negatively correlated with pH values. The similar negative relationships between pH values and exchangeable heavy metals were also recorded. 5Nutritional support in chronic liver disease and cirrhotics显示文摘The liver is a major organ and an essential componentin maintaining an appropriate nutritional status in healthy individuals through metabolism of protein, carbohydrates, and fat. In individuals with chronic liver disease(CLD), along with a number of other essen-tial functions that the liver serves, its role in nutrition maintenance is severely impaired. Common causes of CLD include hepatitis C, alcoholic liver disease, and non-alcoholic liver disease. Amongst this population, the most common manifestation of impaired nutritional maintenance is protein-calorie malnutrition. Aside from inherent abnormalities in metabolism, such as malab-sorption and maldigestion, CLD can be associated with anorexia as well as increased metabolic requirements, all of which contribute to a state of malnutrition. Given the systemic implications and impact on prognosis of malnutrition, proper nutritional assessment is essential and can be achieved through a thorough history and physical, as well as biochemical investigations and anthropometry as needed. Following an appropriate assessment of a patient's nutritional status, an approach to management can be decided upon and is based on the extent of malnutrition which directly reflects the severity of disease. Management options can be grossly separated into enteral and parenteral nutrition. The former is usually sufficient in the form of oral supplements in less severe cases of malnutrition, but as the CLD worsens, parenteral nutrition becomes necessary. 7Approach to pseudomyxoma peritonei显示文摘Pseudomyxoma peritonei(PMP) is a mucinous tumour of the appendix that spreads into the peritoneal cavity in the form of gelatinous deposits. The incidence of PMP is believed to be approximately 1-3 out of a million per year. Nonetheless, due to its indolent nature, it is usually discovered at an advanced stage and severely impacts quality of life. Curative treatment for PMP is complete cytoreductive surgery(CRS) with hyperthermic intraperitoneal chemotherapy(HIPEC). An extensive literature review was conducted searching EMBASE, MEDLINE, Pub Med, and Google Scholar databases for PMP in aims to delineate a clinical approach to diagnosis and treatment. Literature was limited to the years 2007-2018. We found the 5-year overall survival with CRS and HIPEC estimated to be between 23%-82% and rates of major complications as high as 24%. Therefore, it is important to appropriately stage and select patients that should undergo CRS with HIPEC. Modalities like MDCT radiological scores have been shown to have sensitivity and specificity of 94% and 81%, respectively, in being able to predict resectability and survival. Despite treatment, the disease often recurs. 9Role of Helicobacter pylori in gastric cancer:Updates显示文摘Helicobacter pylori(H. pylori) infection is highly prevalent in human,affecting nearly half of the world's population; however,infection remains asymptomatic in majority of population. During its co-existence with humans,H. pylori has evolved various strategies to maintain a mild gastritis and limit the immune response of host. On the other side,presence of H.pylori is also associated with increased risk for the development of various gastric pathologies including gastric cancer(GC). A complex combination of host genetics,environmental agents,and bacterial virulence factors are considered to determine the susceptibility as well as the severity of outcome in a subset of individuals. GC is one of the most common cancers and considered as the third most common cause of cancer related death worldwide. Many studies had proved H. pylori as an important risk factor in the development of non-cardia GC. Although both H. pylori infection and GC are showing decreasing trends in the developed world,they still remain a major threat to human population in the developing countries. The current review attempts to highlight recent progress in the field of research on H. pylori induced GC and aims to provide brief insight into H. pylori pathogenesis,the role of major virulence factors of H. pylori that modulates the host environment and transform the normal gastric epithelium to neoplastic one. 10Role of Akt signaling in resistance to DNA-targeted therapy显示文摘The Akt signal transduction pathway controls most hallmarks of cancer. Activation of the Akt cascade promotes a malignant phenotype and is also widely implicated in drug resistance. Therefore, the modulation of Akt activity is regarded as an attractive strategy to enhance the efficacy of cancer therapy and irradiation. This pathway consists of phosphatidylinositol 3 kinase(PI3K), mammalian target of rapamycin, and the transforming serine-threonine kinase Akt protein isoforms, also known as protein kinase B. DNA-targeted agents, such as platinum agents, taxanes, and antimetabolites, as well as radiation have had a significant impact on cancer treatment by affecting DNA replication, which is aberrantly activated in malignancies. However, the caveat is that they may also trigger the activation of repairing mechanisms, such as upstream and downstream cascade of Akt survival pathway. Thus, each target can theoretically be inhibited in view of improving the potency of conventional treatment. Akt inhibitors, e.g., MK-2206 and perifosine, or PI3K modulators, e.g., LY294002 and Wortmannin, have shown some promising results in favor of sensitizing the cancer cells to the therapy in vitro and in vivo, which have provided the rationale for incorporation of these novel agents into multimodality treatment of different malignancies. Nevertheless, despite the acceptable safety profile of some of these agents in the clinical studies, with regard to the efficacy, the results are still too preliminary. 12Frozen shoulder-A prospective randomized clinical trial显示文摘AIM To compare the results of arthroscopic capsular release with intra-articular steroid injections in patients of frozen shoulder.METHODS Fifty-six patients with frozen shoulder were randomised to one of two treatment groups: Group 1, complete 360 degree arthroscopic capsular release and group 2, intra-articular corticosteroid injection(40 mg methyl prednisolone acetate). Both groups were put on active and passive range of motion exercises following the intervention. The outcome parameters were visual analogue scale(VAS) score for pain, range of motion and Constant score which were measured at baseline, 4, 8, 12, 16 and 20 wk after intervention.RESULTS All the parameters improved in both the groups. The mean VAS score improved significantly more in the group 1 as compared to group 2 at 8 wk. This greater improvement was maintained at 20 wk with P value of 0.007 at 8 wk, 0.006 at 12 wk, 0.006 at 16 wk and 0.019 at 20 wk. The Constant score showed a more significant improvement in group 1 compared to group 2 at 4 wk, which was again maintained at 20 wk with P value of 0.01 at 4, 8, 12 and 16 wk. The gain in abduction movement was statistically significantly more in arthroscopy group with P value of 0.001 at 4, 8, 12, 16 wk and 0.005 at 20 wk. The gain in external rotation was statistically significantly more in arthroscopy group with P value of 0.007 at 4 wk, 0.001 at 8, 12, and 16 wk and 0.003 at 20 wk. There was no statistically significant difference in extension and internal rotation between the two groups at any time. 13Mercury remediation potential of a mercury resistant strain Sphingopyxis sp.SE2 isolated from contaminated soil显示文摘A mercury resistant bacterial strain SE2 was isolated from contaminated soil.The 16 s rRNA gene sequencing confirms the strain as Sphingopyxis belongs to the Sphingomonadaceae family of the α-Proteobacteria group.The isolate showed high resistance to mercury with estimated concentrations of Hg that caused 50%reduction in growth(EC_(50)) of 5.97 and 6.22 mg/L and minimum inhibitory concentrations(MICs) of 32.19 and 34.95 mg/L in minimal and rich media,respectively.The qualitative detection of volatilized mercury and the presence of mercuric reductase enzyme proved that the strain SE2 can potentially remediate mercury.ICP-QQQ-MS analysis of the remaining mercury in experimental broths indicated that a maximum of 44%mercury was volatilized within 6 hr by live SE2 culture.Furthermore a small quantity(23%) of mercury was accumulated in live cell pellets.While no volatilization was caused by dead cells,sorption of mercury was confirmed.The mercuric reductase gene merA was amplified and sequenced.Homology was observed among the amino acid sequences of mercuric reductase enzyme of different organisms from a-Proteobacteria and ascomycota groups,Khandaker Rayhan Mahbub Kannan Krishnan Ravi Naidu Mallavarapu Megharaj 2017Journal of Environmental Sciences2017,29,1:10

16Computer aided design and analysis of a tunable muzzle brake显示文摘This research work deals with the design of a tunable muzzle brake [10] for a rifle chambered in 5.56 x 45 NATO ammunition. It proposes to solve the problem of handling differences from shooter to shooter by incorporating the feature of tunability. Beside this, it also solves the problem of requirement of optimum recoil in short recoil weapons. This innovation gives this design an edge over its already existing counterparts in the market. The product is designed using the internal ballistics calculations and the investigations been performed using solidworks flow simulation tool and ANSYS static structural to check the parameters like velocity distribution, pressure growth, and muzzle brake force along the series of ports and comparison of the so found results with those devised by the authors of the documents mentioned in references. This assures the market adaptability of the product for satisfactory performance, when brought among its already existing counterpart, though with a slight edge over them due to tunability. 17Simultaneous determination of telmisartan and amlodipine in human plasma by LC-MS/MS and its application in a human pharmacokinetic study显示文摘A rapid and sensitive liquid chromatography-tandem mass spectrometric(LC-MS/MS) assay method has been developed and fully validated for the simultaneous quantification of telmisartan and amlodipine in human plasma.Carbamazepine was used as an internal standard.Analytes and the internal standard were extracted from human plasma by solid-phase extraction technique using Waters Oasis  HLB 1 cm 3(30 mg) extraction cartridge.The reconstituted samples were chromatographed on a Hypurity advance C 18 column(50 mm×4.6 mm,5 μm) using a mixture of acetonitrile-5 mM ammonium acetate buffer(pH-4.0)(50:50,v/v) as the mobile phase at a flow rate of 0.8 mL/min.The calibration curve obtained was linear(r≥0.99) over the concentration range of 2.01-400.06 ng/mL for telmisartan and 0.05-10.01 ng/mL for amlodipine.Method validation was performed as per FDA guidelines and the results met the acceptance criteria.A run time of 2.5 min for each sample made it possible to analyze more than 400 human plasma samples per day.The proposed method was found to be applicable to clinical studies.Vasu Babu Ravi Jaswanth Kumar Inamadugu Nageswara Rao Pilli Vudagandla Sreenivasulu Venkateswarlu Ponneri 2012Journal of Pharmaceutical Analysis2012,2,5:8
18Genetics of acute and chronic pancreatitis: An update显示文摘Progress made in identifying the genetic susceptibility underlying acute and chronic pancreatitis has benefitted the clinicians in understanding the pathogenesis of the disease in a better way. The identification of mutations in cationic trypsinogen gene(PRSS1 gene; functional gain mutations) and serine protease inhibitor kazal type 1(SPINK1 gene; functional loss mutations) and other potential susceptibility factors in genes that play an important role in the pancreatic secretory functions or response to inflammation during pancreatic injury has changed the current concepts and understanding of a complex multifactorial disease like pancreatitis. An indi-vidual's susceptibility to the disease is governed by ge-netic factors in combination with environmental factors. Candidate gene and genetic linkage studies have iden-tified polymorphisms in cationic trypsinogen(PRSS1), SPINK1, cystic fibrosis trans-membrane conductance regulator(CFTR), Chymotrypsinogen C(CTRC), Ca-thepsin B(CTSB) and calcium sensing receptor(CASR). Individuals with polymorphisms in the mentioned genes and other as yet identified genes are at an enhanced risk for the disease. Recently, polymorphisms in genes other than those involved in 'intra-pancreatic trypsin regulatory mechanism' namely Claudin-2(CLDN2) andCarboxypeptidase A1(CPA1) gene have also been iden-tified for their association with pancreatitis.
