| 1 | Port site infection in laparoscopic surgery: A review of its management显示文摘Laparoscopic surgery(LS), also termed minimal access surgery, has brought a paradigm shift in the approach to modern surgical care. Early postoperative recovery, less pain, improved aesthesis and early return to work have led to its popularity both amongst surgeons and patients. Its application has progressed from cholecystectomies and appendectomies to various other fields including gastrointestinal surgery, urology, gynecology and oncosurgery. However, LS has its own package of complications. Port site infection(PSI), although infrequent, is one of the bothersome complications which undermine the benefits of minimal invasive surgery. Not only does it add to the morbidity of the patient but also spoils the reputation of the surgeon. Despite the advances in the field of antimicrobial agents, sterilization techniques, surgical techniques, operating room ventilation, PSIs still prevail. The emergence of rapid growing atypical mycobacteria with multidrug resistance, which are the causative organism in most of the cases, has further compounded the problem. PSIs are preventable if appropriate measures are taken preoperatively, intraoperatively and postoperatively. PSIs can often be treated non-surgically, with early identification and appropriate management. Macrolides, quinolones and aminoglycosides antibiotics do show promising activity against the atypical mycobacteria. This review article highlights the clinical burden, presentations and management of PSIs in LS as shared by various authors in the literature. We have given emphasis to atypical mycobacteria, which are emerging as a common etiological agent for PSIs in LS. | 2 | Late biliary complications in human alveolar echinococcosis are associated with high mortality显示文摘AIM:To evaluate the incidence of late biliary complications in non-resectable alveolar echinococcosis(AE)under long-term chemotherapy with benzimidazoles.METHODS:Retrospective analysis of AE patients with biliary complications occurring more than three years after the diagnosis of AE.We compared characteristics of patients with and without biliary complications,analyzed potential risk factor for biliary complications and performed survival analyses.RESULTS:Ninety four of 148 patients with AE in Zurich had non-resectable AE requiring long-term benzimidazole chemotherapy,of which 26(28%)patients developed late biliary complications.These patients had a median age of 55.5(35.5-65)years at diagnosis of AE and developed biliary complications after 15(8.25-19)years of chemotherapy.The most common biliary complications during long-term chemotherapy were late-onset cholangitis(n=14),sclerosing cholangitis-like lesions(n=8),hepatolithiasis(n=5),affection of the common bile duct(n=7)and secondary biliary cirrhosis(n=7).Thirteen of the 26 patients had undergone surgery(including 12 resections)before chemotherapy.Previous surgery was a risk factor for late biliary complications in linear regression analysis(P=0.012).CONCLUSION:Late biliary complications can be observed in nearly one third of patients with non-resectable AE,with previous surgery being a potential risk factor.After the occurrence of late biliary complications,the median survival is only 3 years,suggesting that late biliary complications indicate a poor prognostic outcome. | Pascal Frei Benjamin Misselwitz Meher K Prakash Alain M Schoepfer Bettina M Prinz Vavricka Beat Müllhaupt Michael Fried Kuno Lehmann Rudolf W Ammann Stephan R Vavricka | 2014 | World Journal of Gastroenterology2014,20,19: | 5 |

| 4 | Tyrosine aminotransferase: biochemical and structural properties and molecular dynamics simulations显示文摘Tyrosine aminotransferase(TAT)catalyzes the transamination of tyrosine and other aromatic amino acids.The enzyme is thought to play a role in tyrosinemia type II,hepatitis and hepatic carcinoma recovery.The objective of this study is to investigate its biochemical and structural characteristics and substrate specificity in order to provide insight regarding its involvement in these diseases.Mouse TAT(mTAT)was cloned from a mouse cDNA library,and its recombinant protein was produced using Escherichia coli cells and purified using various chromatographic techniques.The recombinant mTAT is able to catalyze the transamination of tyrosine usingα-ketoglutaric acid as an amino group acceptor at neutral pH.The enzyme also can use glutamate and phenylalanine as amino group donors and p-hydroxyphenylpyruvate,phenylpyruvate and alpha-ketocaproic acid as amino group acceptors.Through macromolecular crystallography we have determined the mTAT crystal structure at 2.9Åresolution.The crystal structure revealed the interaction between the pyridoxal-5′-phosphate cofactor and the enzyme,as well as the formation of a disulphide bond.The detection of disulphide bond provides some rational explanation regarding previously observed TAT inactivation under oxidative conditions and reactivation of the inactive TAT in the presence of a reducing agent.Molecular dynamics simulations using the crystal structures of Trypanosoma cruzi TAT and human TAT provided further insight regarding the substrate-enzyme interactions and substrate specificity.The biochemical and structural properties of TAT and the binding of its cofactor and the substrate may help in elucidation of the mechanism of TAT inhibition and activation. | Prajwalini Mehere Qian Han Justin A.Lemkul Christopher J.Vavricka Howard Robinson David R.Bevan Jianyong Li | 2010 | Protein & Cell2010,1,11: | 4 |
| 5 | Potential predictive factors for pathologic complete response after the neoadjuvant treatment of rectal adenocarcinoma:a single center experience显示文摘Objective:To assess the response rate of patients with rectal adenocarcinoma to neoadjuvant therapy and to identify the predictors of histological regression after neoadjuvant radiotherapy(RT)or concurrent chemoradiotherapy(CCRT).Methods:This study recruited 64 patients.The patients had resectable cancer of the lower and the middle rectum(T3/T4 and/or N+)without distant metastasis and received neoadjuvant RT or CCRT followed by radical surgery with total mesorectal excision(TME)between January 2006 and December 2011.The patients were classified into non-response(NR),partial response(PR),and pathologic complete response(p CR)based on the Dworak tumor regression grading system.Results:The median age of patients was 57 years(ranging from 22 to 85).A total of 24 patients were treated with neoadjuvant CCRT,whereas 40 patients were treated with RT alone.Abdominoperineal resection(APR)was performed on 29 patients(45%).Anterior resection with TME was performed on 34 patients(53%).One patient had local resection.Histologically,12(19%),24(73%),and 28(44%)patients exhibited p CR,PR,and NR,respectively.Univariate analysis revealed that the predictors of tumor regression were as follows:the absence of lymph node involvement from initial imaging(c N0)(P=0.021);normal initial carcinoembryonic antigen(CEA)level(P=0.01);hemoglobin level≥12 g/dl(P=0.009);CCRT(P=0.021);and tumor downstaging in imaging(P=0.001).Multivariate analysis showed that the main predictors of p CR were CT combined with neoadjuvant RT,c N0stage,and tumor regression on imaging.Conclusions:Identifying the predictors of p CR following neoadjuvant therapy aids the selection of responsive patients for nonaggressive surgical treatment and possible surveillance. | Feryel Letaief Meher Nasri Mouna Ayadi Khedija Meddeb Amina Mokrani Yosra Yahyaoui Nesrine Chraiet Henda Raies Amel Mezlini | 2017 | Cancer Biology & Medicine2017,14,3: | 3 |
| 6 | Aggressive primary hepatic epithelioid hemangioendothelioma:a case report and literature review显示文摘A new case of epithelioid hemangioendothelioma is reported to have occurred to a 67-year-old patient who consulted for rightsided chest pain. The work-up showed multiple right pulmonary lesions associated with bilateral moderate pleural effusion and left-sided pleural thickening and three hypodense nodules in the right lobe of the liver, peritoneal thickening, ascites, and multiple vertebral lytic lesions. The diagnosis of an epithelioid hemangioendothelioma was concluded through a histological examination of a computed tomography scan guided biopsy of the liver. The patient received a primary mono-chemotherapy with Adriamycin(75 mg/m^2 every three weeks) and intravenous bisphosphonates without response and general status impairment. | 9 | Time-series analysis of monthly rainfall data for the Mahanadi River Basin, India显示文摘Time series analysis has two goals, modeling random mechanisms and predicting future series using historical data. In the present work, a uni-variate time series autoregressive integrated moving average (ARIMA) model has been developed for (a) simulating and forecasting mean rainfall, obtained using Theissen weights; over the Mahanadi River Basin in India, and (b) simulating and forecasting mean rainfall at 38 rain-gauge stations in district towns across the basin. For the analysis, monthly rainfall data of each district town for the years 1901-2002 (102 years) were used. Theissen weights were obtained over the basin and mean monthly rainfall was estimated. The trend and seasonality observed in ACF and PACF plots of rainfall data were removed using power transformation (α=0.5) and first order seasonal differencing prior to the development of the ARIMA model. Interestingly, the ARIMA model (1,0,0)(0,1,1) 12 developed here was found to be most suitable for simulating and forecasting mean rainfall over the Mahanadi River Basin and for all 38 district town rain-gauge stations, separately. The Akaike Information Criterion (AIC), goodness of fit (Chi-square), R 2 (coefficient of determination), MSE (mean square error) and MAE (mea absolute error) were used to test the validity and applicability of the developed ARIMA model at different stages. | 10 | NARX neural network approach for the monthly prediction of groundwater levels in Sylhet Sadar, Bangladesh显示文摘Groundwater is important for managing the water supply in agricultural countries like Bangladesh. Therefore, the ability to predict the changes of groundwater level is necessary for jointly planning the uses of groundwater resources. In this study, a new nonlinear autoregressive with exogenous inputs(NARX) network has been applied to simulate monthly groundwater levels in a well of Sylhet Sadar at a local scale. The Levenberg-Marquardt(LM) and Bayesian Regularization(BR) algorithms were used to train the NARX network, and the results were compared to determine the best architecture for predicting monthly groundwater levels over time. The comparison between LM and BR showed that NARX-BR has advantages over predicting monthly levels based on the Mean Squared Error(MSE), coefficient of determination(R^2), and Nash-Sutcliffe coefficient of efficiency(NSE). The results show that BR is the most accurate method for predicting groundwater levels with an error of ± 0.35 m.
