1Vascular complications following liver transplantation:Aliterature review of advances in 2015显示文摘Although vascular complications(VCs) following orthotopic liver transplantation(OLT) seldom occur, they are the most feared complications with a high incidence of both graft loss and mortality, as they compromise the blood flow of the transplant(either inflow or outflow). Diagnosis and therapeutic management of VCs constitute a major challenge in terms of increasing the success rate of liver transplantation. While surgical treatment used to be considered the first choice for management, advances in endovascular intervention have increased to make this a viable therapeutic option. Considering VC as a rare but a major and dreadful issue in OLT history, and in view of the continuing and rapid progress in recent years, an update on these uncommon conditions seemed necessary. 3Comparison of inhaled milrinone, nitric oxide and prostacyclin in acute respiratory distress syndrome显示文摘AIM To evaluate the safety and efficacy of inhaled milrinone in acute respiratory distress syndrome(ARDS).METHODS Open-label prospective cross-over pilot study where fifteen adult patients with hypoxemic failure meeting standard ARDS criteria and monitored with a pulmonary artery catheter were recruited in an academic 24-bed medico-surgical intensive care unit. Random sequential administration of i NO(20 ppm) or nebulized epoprostenol(10 μg/mL) was done in all patients. Thereafter, inhaled milrinone(1 mg/mL) alone followed by inhaled milrinone in association with inhaled nitric oxide(iN O) was administered. A jet nebulization device synchronized with the mechanical ventilation was use to administrate the epoprostenol and the milrinone. Hemodynamic measurements and partial pressure of arterial oxygen(PaO_2) were recorded before and after each inhaled therapyadministration.RESULTS The majority of ARDS were of pulmonary cause(n = 13) and pneumonia(n = 7) was the leading underlying initial disease. Other pulmonary causes of ARDS were: Post cardiopulmonary bypass(n = 2), smoke inhalation injury(n = 1), thoracic trauma and pulmonary contusions(n = 2) and aspiration(n = 1). Two patients had an extra pulmonary cause of ARDS: A polytrauma patient and an intra-abdominal abscess Inhaled nitric oxide, epoprostenol, inhaled milrinone and the combination of inhaled milrinone and i NO had no impact on systemic hemodynamics. No significant adverse events related to study medications were observed. The median increase of PaO 2 from baseline was 8.8 mmH g [interquartile range(IQR) = 16.3], 6.0 mm Hg(IQR = 18.4), 6 mm Hg(IQR = 15.8) and 9.2 mm Hg(IQR = 20.2) respectively with i NO, epoprostenol, inhaled milrinone, and i NO added to milrinone. Only i NO and the combination of inhaled milrinone and i NO had a statistically significant effect on PaO 2. CONCLUSION When comparing the effects of inhaled NO, milrinone and epoprostenol, only NO significantly improved oxygenation. 5GOBO projection for 3D measurements at highest frame rates: a performance analysis显示文摘Aperiodic sinusoidal patterns that are cast by a GOBO(GOes Before Optics)projector are a powerful tool for optically measuring the surface topography of moving or deforming objects with very high speed and accuracy.We optimised the first experimental setup that we were able to measure inflating car airbags at frame rates of more than 50 kHz while achieving a 3D point standard deviation of~500μm.Here,we theoretically investigate the method of GOBO projection of aperiodic sinusoidal fringes.In a simulation-based performance analysis,we examine the parameters that influence the accuracy of the measurement result and identify an optimal pattern design that yields the highest measurement accuracy.We compare the results with those that were obtained via GOBO projection of phase-shifted sinusoidal fringes.Finally,we experimentally verify the theoretical findings.We show that the proposed technique has several advantages over conventional fringe projection techniques,as the easy-to-build and cost-effective GOBO projector can provide a high radiant flux,allows high frame rates,and can be used over a wide spectral range.Stefan Heist Patrick Dietrich Martin Landmann Peter Kühmstedt Gunther Notni Andreas Tünnermann 2018Light(Science & Applications)2018,7,1:4

9Evaluation of Weather Research and Forecasting Model Parameterizations under Sea-Breeze Conditions in a North Sea Coastal Environment显示文摘Three atmospheric boundary layer(ABL) schemes and two land surface models that are used in the Weather Research and Forecasting(WRF) model, version 3.4.1, were evaluated with numerical simulations by using data from the north coast of France(Dunkerque). The ABL schemes YSU(Yonsei University),ACM2(Asymmetric Convective Model version 2), and MYJ(Mellor–Yamada–Janjic) were combined with two land surface models, Noah and RUC(Rapid Update Cycle), in order to determine the performances under sea-breeze conditions. Particular attention is given in the determination of the thermal internal boundary layer(TIBL), which is very important in air pollution scenarios. The other physics parameterizations used in the model were consistent for all simulations. The predictions of the sea-breeze dynamics output from the WRF model were compared with observations taken from sonic detection and ranging, light detection and ranging systems and a meteorological surface station to verify that the model had reasonable accuracy in predicting the behavior of local circulations. The temporal comparisons of the vertical and horizontal wind speeds and wind directions predicted by the WRF model showed that all runs detected the passage of the sea-breeze front. However, except for the combination of MYJ and Noah, all runs had a time delay compared with the frontal passage measured by the instruments. The proposed study shows that the synoptic wind attenuated the intensity and penetration of the sea breeze. This provided changes in the vertical mixing in a short period of time and on soil temperature that could not be detected by the WRF model simulations with the computational grid used. 10Immune profiling and cancer post transplantation显示文摘Half of all long-term(> 10 year) australian kidney transplant recipients(KTR) will develop squamous cell carcinoma(SCC) or solid organ cancer(SOC), making cancer the leading cause of death with a functioning graft. At least 30% of KTR with a history of SCC or SOC will develop a subsequent SCC orSOC lesion. Pharmacological immunosuppression is a major contributor of the increased risk of cancer for KTR, with the cancer lesions themselves further adding to systemic immunosuppression and could explain, in part, these phenomena. Immune profiling includes; measuring immunosuppressive drug levels and pharmacokinetics, enumerating leucocytes and leucocyte subsets as well as testing leucocyte function in either an antigen specific or non-specific manner. Outputs can vary from assay to assay according to methods used. In this review we define the rationale behind post-transplant immune monitoring assays and focus on assays that associate and/or have the ability to predict cancer and rejection in the KTR. We find that immune monitoring can identify those KTR of developing multiple SCC lesions and provide evidence they may benefit from pharmacological immunosuppressive drug dose reductions. 16Architecting Fault Tolerance with Exception Handling: Verification and Validation显示文摘When building dependable systems by integrating untrusted software components that were not originally designed to interact with each other, it is likely the occurrence of architectural mismatches related to assumptions in their failure behaviour. These mismatches, if not prevented during system design, have to be tolerated during runtime. This paper presents an architectural abstraction based on exception handling for structuring fault-tolerant software systems. This abstraction comprises several components and connectors that promote an existing untrusted software element into an idealised fault-tolerant architectural element. Moreover, it is considered in the context of a rigorous software development approach based on formal methods for representing the structure and behaviour of the software architecture. The proposed approach relies on a formal specification and verification for analysing exception propagation, and verifying important dependability properties, such as deadlock freedom, and scenarios of architectural reconfiguration. The formal models are automatically generated using model transformation from UML diagrams: component diagram representing the system structure, and sequence diagrams representing the system behaviour. Finally, the formal models are also used for generating unit and integration test cases that are used for assessing the correctness of the source code.
