| 2 | Effect of alternative antibiotics in treatment of cefotaxime resistant spontaneous bacterial peritonitis显示文摘AIM:To evaluate effective alternative antibiotics in treatment of cefotaxime-resistant spontaneous bacterial peritonitis.METHODS:One hundred cirrhotic patients with spontaneous bacterial peritonitis [ascitic fluid polymorphonuclear cell count(PMNLs) ≥ 250 cells/mm 3 at admission] were empirically treated with cefotaxime sodium 2 g/12 h and volume expansion by intravenous human albumin.All patients were subjected to history taking,complete examination,laboratory tests(including a complete blood cell count,prothrombin time,biochemical tests of liver and kidney function,and fresh urine sediment),chest X-ray,a diagnostic abdominal paracentesis,and the sample subjected to total and differential cell count,chemical examination,aerobic and anaerobic cultures.Patients were divided after 2 d by a second ascitic PMNL count into group Ⅰ;patients sensitive to cefotaxime(n = 81),group Ⅱ(n = 19);cases resistant to cefotaxime(less than 25% decrease in ascitic PMNL count).Patients of group Ⅱ were randomly assigned into meropenem(n = 11) or levofloxacin(n = 8) subgroups.All patients performed an end of treatment ascitic PMNL count.Patients were considered improved when:PMNLs decreased to < 250 cells/mm 3,no growth in previously positive culture cases,and improved clinical manifestations with at least 5 d of antibiotic therapy.RESULTS:Age,sex,and Child classes showed no significant difference between group Ⅰ and group Ⅱ.Fever and abdominal pain were the most frequent manifestations and were reported in 82.7% and 80.2% of patients in group Ⅰ and in 94.7% and 84.2% of patients in group Ⅱ,respectively.Patients in group Ⅱ had a more severe ascitic inflammatory response than group Ⅰ and this was demonstrated by more ascitic lactate dehydrogenase(LDH) [median:540 IU/L(range:150-1200 IU/L) vs median:240 IU/L(range:180-500 IU/L),P = 0.000] and PMNL [median:15 000 cell/mm 3(range:957-23 822 cell/mm 3) vs 3400 cell/mm 3(range:695-26 400 cell/mm 3),P = 0.000] counts.Ascitic fluid culture was positive in 32% of cases.Cefotaxime failed in 19% of patients;of these patients,11(100%) responded to meropenem and 6(75%) responded to levofloxacin.Two patients with failed levofloxacin therapy were treated according to the in vitro culture and sensitivity(one case was treated with vancomycin and one case was treated with ampicillin/sulbactam).In group Ⅱ the meropenem subgroup had higher LDH(range:108-860 IU/L vs 120-491 IU/L,P = 0.042) and PMNL counts(range:957-23 822 cell/mm 3 vs 957-15 222 cell/mm 3,P = 0.000) at initiation of the alternative antibiotic therapy;there was no significant difference in the studied parameters between patients responsive to meropenem and patients responsive to levofloxacin at the end of therapy(mean ± SD:316.01 ± 104.03PMNLs/mm 3 vs 265.63 ± 69.61 PMNLs/mm 3,P = 0.307).The isolated organisms found in group Ⅱ were;enterococci,acinetobacter,expanded-spectrum β-lactamase producing Escherichia coli,β-lactamase producing Enterobacter and Staphylococcus aureus.CONCLUSION:Empirical treatment with cefotaxime is effective in 81% of cases;meropenem is effective in cefotaxime-resistant cases. | Ahmed Abouelkhair Badawy Tarik Ibrahim Zaher Samar Mahmoud Sharaf Mohamed Hassan Emara Noha Elsaid Shaheen Talaat Fathy Aly | 2013 | World Journal of Gastroenterology2013,19,8: | 13 |

EST is either used solely for the treatment of diseases of the papilla of Vater, such as sphincter of Oddi dysfunction or to facilitate subsequent therapeutic biliary interventions, such as stone extraction, stenting, etc. It is a prerequisite for biliary interventions, thus every practitioner who performs endoscopic retrograde cholangiopancreatography needs to know different techniques and the clinical and anatomic parameters related to the efficacy and safety of the procedure. | 5 | Association of ezrin expression in intestinal and diffuse gastric carcinoma with clinicopathological parameters and tumor type显示文摘AIM: To investigate the correlation between ezrin expression and types of gastric carcinoma and clinicopathological variables. METHODS: We examined ezrin protein expression in 75 gastric carcinoma (53 intestinal types of adenocarcinoma, 22 diffuse types of carcinoma) tissues by immunohistochemistry. The results were compared with clinicopathological parameters such as tumor type, grade of tumor, clinical stage, presence of metastatic lymph node, and depth of invasion. RESULTS: Ezrin immunostaining was positive in 43 cases (81.1%) of intestinal type and in 9 (40.9%) cases of diffuse type adenocarcinomas (P < 0.001). In gastric carcinomas, the expression of ezrin protein correlated with the status of H pylori and survival. There was no correlation between expression of ezrin with TNM stage and histological grade of gastric carcinomas (P > 0.05). CONCLUSION: The low expression of ezrin implicates the loss of adhesion in diffuse carcinomas. | 7 | Effects of Organic Amendments on Soil Physical Attributes and Aggregate-Associated Phosphorus Under Long-Term Rice-Wheat Cropping显示文摘The quantification of phosphorus(P) in bulk soil and P distribution in different size fractions of water-stable aggregates(WSAs)are important for assessing potential P loss through runoff. We evaluated available and total P distribution within WSAs of a sitty clay to clay soil in a long-term fertility experiment of a rice-wheat cropping system in India. Surface soil samples were collected from seven plots amended with NPK fertilizers in combination with or without organic amendments, farmyard manure(FYM), green manure(GM), and paddy straw(PS). The plot with no NPK fertilizers or organic amendments was set as a control. The soil samples were separated by wet sieving into four soil aggregate size fractions: large macroaggregates(> 2.0 mm), small macroaggregates(0.25–2.0 mm), fine microaggregates(0.05–0.25 mm), and a silt + clay-sized fraction(< 0.05 mm). Structural indices were higher in the soil receiving organic amendments than in the soil receiving inorganic fertilizer alone. Organically amended soil had a higher proportion of stable macroaggregates than the control and the soil receiving inorganic fertilizer alone, which were rich in microaggregates. Total and available P contents within WSAs were inversely related to the aggregate size, irrespective of treatment. The distribution of available and total P in the soil aggregate size fraction was as follows: silt + clay-size fraction > small macroaggregates > fine microaggregates> large macroaggregates. Within a size class, aggregate-associated available and total P contents in the organically amended soil were in the following order: FYM > PS ≥ GM. The available P content of the microaggregates(< 0.25 mm) was 8-to 10-times higher than that of the macroaggregates(> 0.25 mm), and the total P content of the microaggregates was 4-to 5-times higher than that of the macroaggregates. | 15 | Predictors of hypoglycemia in insulin-treated patients with type 2 diabetes mellitus in Basrah显示文摘AIM To measure the incidence and determinants(predictors) of hypoglycemia among patients with type 2 diabetes mellitus(T2DM) who were on insulin treatment for at least one year. METHODS The present study is an out-patients based inquiry about the risk and predictors of hypoglycemia among patients with T2DM seeking care at the Al-Faiha Specialized Diabetes, Endocrine, and Metabolism Center, in Basrah over a period of 7 mo(from 15^(th) of April, 2013 to 15^(th) of October, 2013). The data used in the study were based on all detailed interview and selected laboratory investigations. A total of 336 patients could be included in the study.RESULTS The incidence of overall hypoglycemia among the studied patients was 75.3% within the last 3 mo preceding the interview. The incidence of hypoglycemia subtypes were 10.2% for severe hypoglycemia requiring medical assistance in the hospital, 44.36% for severe hypoglycemia treated at home by family; this includes both confirmed severe hypoglycemia with an incidence rate of 14.6% and unconfirmed severe hypoglycemia for which incidence rate was 29.76%. Regarding mild self-treated hypoglycemia, the incidence of confirmed mild hypoglycemia was 21.42%, for unconfirmed mildhypoglycemia the incidence rate was 50.0% and for total mild hypoglycemia, the incidence rate was 71.42%. The most important predictors of hypoglycemia were a peripheral residence, increasing knowledge of hypoglycemia symptoms, in availability and increasing frequency of self-monitoring blood glucose, the presence of peripheral neuropathy, higher diastolic blood pressure, and lower Hemoglobin A1c.CONCLUSION Hypoglycemia is very common among insulin-treated patients with T2DM in Basrah. | 16 | Reaching proficiency in laparoscopic splenectomy显示文摘AIM:To investigate the proficiency level reached in laparoscopic splenectomy using the learning curve method.METHODS:All patients in need of splenectomy for benign causes in whom laparoscopic splenectomy was attempted by a single surgeon during a time period of 6 years were included in the study (n=33). Besides demographics, operation-related variables and the response to surgery were recorded. The patients were allocated to groups of five, ranked according to the date of the operation. Operation duration, complications, postoperative length of stay, conversion to laparotomy and splenic weight were then compared between these groups.RESULTS: There was a significant difference regarding operation times between the groups (P = 0.001). An improvement was observed after the first 5 cases. The learning curve was flat up to the 25th case. Following the 25th case the operation times decreased still further. There was no difference between the groups regarding the other parameters.CONCLUSION: Unlike the widely accepted 'L' shape, the learning curve for laparoscopic splenectomy is a horizontal lazy 'S' with two distinct slopes. Privileges may be granted after the first 5 cases. | 19 | Factors Influencing Post-disaster Reconstruction Project Management for Housing Provision in the Gaza Strip, Occupied Palestinian Territories显示文摘In the Occupied Palestinian Territories, the Gaza Strip has suffered regular cycles of reconstruction due to systematic destruction during Israeli military operations, as in 2006, 2008–2009, 2012, and 2014. In this context of ongoing conflict this article aims to identify, rank, and discuss the most important factors influencing post-disaster reconstruction project management(PDRPM) for housing in the Gaza Strip. A set of key factors that influence PDRPM were assembled as a result of a global literature review. A questionnaire survey was conducted, and the obtained data were analyzed using a relative importance index for each PDRPM factor. Findings are presented in six groups: housing approaches, organizational behavior, project funding, supply chain and logistics, communication and coordination, and PDRPM context. Findings indicate that the most significant factors that influence PDRPM for housing provision in the Gaza Strip are related to issues associated with financial resources. It is critical that sufficient funding should be available in order to allow organizations to undertake housing projects in an effective and efficient way.
