| 4 | 瑞舒伐他汀对存在亚临床动脉粥样硬化的低危人群颈动脉内膜中层厚度进展的影响——METEOR试验显示文摘背景:动脉粥样硬化往往在机体出现症状之前就已有所进展。目前,我们尚不清楚Framingham风险评分(Framinghaln risk score,FRS)较低的轻中度亚临床动脉粥样硬化中年人群是否能从治疗中获益。
目的:评价为期2年的他汀类药物治疗能否减缓颈动脉内膜中层厚度(carotid intimamedia thickness,CIMT)继续增厚或使粥样斑块消退。
设计、地点及参试者:“瑞舒伐他汀对内膜中层厚度影响的测评研究(Measuring Effects on Intima-Media Thickness:an Evaluation of Rosuvastatin,METEOR)”是2002年8月至2006年5月在美国及欧洲61个基层医疗中心开展的一项随机、双盲、安慰剂对照研究,共纳入984名参试者,有的参试者仅以年龄(平均年龄,57岁)为冠心病的危险因素,有的参试者10年内FRS低于10%、CIMT重庞增厚(1.2~3.5mm)、LDL胆固醇水平有所升高(平均值,154mg/dL)。
干预:参试者服用40mg瑞舒伐他汀或安慰剂。
主要观测指标:12个颈动脉位点最大CIMT的变化率(经B超检测);颈总动脉位点、颈动脉窦位点及颈内动脉位点最大CIMT的变化,颈总动脉位点平均CIMT的变化。仅在瑞舒伐他汀组中评估CIMT的消退情况。
结果:瑞舒伐他汀组参试者LDL胆固醇平均(SD)水平由基线时的155(24.1)mg/dL降至78(27.5)mg/dL,平均降低49%(与安慰剂组相比P〈0.001)。瑞舒伐他汀组12个颈动脉位点最大CIMT的变化值为-0.0014(95%CI,-0.0041~0.0014)mm/y,安慰剂组12个颈动脉位点最大CIMT的变化值为Q0131(95%CI,Q0087~Q0174)mm/y(P〈0.001)。瑞舒伐他汀组颈总动脉位点最大CIMT的变化值为-0.0038(95%CI,-0.0064~-0.0013)mm/y(P〈0.001),颈动脉窦位点最大CIMT的变化值为-0.0040(95%CI,-0.0090~0.0010)mm/y(P〈0.001),颈内动脉位点最大CIMT的变化值为0.0039(95%CI,-0.0009~0.0088)mm/y(P=0.02)。瑞舒伐他汀组颈总动脉位点平均CIMT的变化值为0.0004(95%CI,-0.0011~0.0019)mm/y(P〈0.001)。P值均为与安慰剂组相比。总的来说,瑞舒伐他汀具有良好的耐受性,很少引发严重的心血管不良事件(在2年内有6名参试者[0.86%]出现8起不良事件[1.1%])。

| 6 | 胸腔镜肺叶切除术显示文摘背景与目的:尽管14年前我们就已报道在胸腔镜下可完成解剖意义上彻底的肺叶切除和淋巴结清扫,但关于这一术式的争论以及胸外科医师们提出的如何完成这种手术的疑问,使这种术式的被接受程度受到了限制。随着对这一术式持支持态度的人不断增多,这一术式的开展也越来越广泛。本文将讨论该术式的手术技术,临床疗效,以及关于这种术式的各种争论。方法:1992—2005年间我们共进行胸腔镜肺叶切除术1315例,其中女性700例(53.2%),男性615例(46.8%),患者平均年龄71.2岁。患者术后病理诊断:良性病变53例,肺部转移性肿瘤28例,淋巴瘤5例,肺癌1015例。腺癌在原发性肺癌中占63.1%。解剖意义上彻底的肺门切除和淋巴结活检或清扫均通过观察监视器屏幕完成,切口长约5cm,不需撑开肋骨。结果:9例(0.8%)死亡(无术中死亡及出血死亡患者)。932例(84.7%)术后没有出现并发症。1100例患者中有45例(4.1%)需输血。中位住院时间3d,平均住院时间4.78d。180例(20%)患者于术后第1天或第2天出院。28例(2.5%)患者术中转为开胸手术。5例(0.57%)患者术后切口出现复发。其中2003年内,在224例肺叶切除中89%通过胸腔镜完成。结论:在胸腔镜下可完成解剖意义上彻底的肺叶切除,且并发症率和死亡率较低。术中出血及切口复发的风险很小。 | Robert J McKenna Ward Houck 罗晓阳 | 2006 | 中国癌症杂志2006，16，8： | 20 |
| 7 | Mechanisms underlying the impaired contractility of diabetic cardiomyopathy显示文摘Cardiac dysfunction is a well-known consequence of diabetes,with sustained hyperglycaemia leading to the development of a cardiomyopathy that is independent of cardiovascular disease or hypertension.Animal models of diabetes are commonly used to study the pathophysiology of diabetic cardiomyopathy,with the hope that increased knowledge will lead ultimately to better therapeutic strategies being developed.At physiological temperature,left ventricular trabeculae isolated from the streptozotocin rat model of type 1 diabetes showed decreased stress and prolonged relaxation,but with no evidence that decreased contractility was a result of altered myocardial Ca2+handling.Although sarcoplasmic reticulum(SR)Ca2+reuptake appeared slower in diabetic trabeculae,it was offset by an increase in actionpotential duration,thereby maintaining SR Ca2+content and favouring increased contraction force.Frequency analysis of t-tubule distribution by confocal imaging of ventricular tissue labeled with wheat germ agglutinin or ryanodine receptor antibodies showed a reduced T-power for diabetic tissue,but the differences were minor in comparison to other models of heart failure.The contractile dysfunction appeared to be the result of disrupted F-actin in conjunction with the increased typeⅠcollagen,with decreased myofilament Ca2+sensitivity contributing to the slowed relaxation. | Marie-Louise Ward David J Crossman | 2014 | World Journal of Cardiology2014，6，7： | 13 |
| 8 | Increased hepcidin expression in colorectal carcinogenesis显示文摘AIM:To investigate whether the iron stores regulator hepcidin is implicated in colon cancer-associated anae- mia and whether it might have a role in colorectal car- cinogenesis. METHODS: Mass spectrometry (MALDI-TOF MS and SELDI-TOF MS) was employed to measure hepcidin in urine collected from 56 patients with colorectal cancer. Quantitative Real Time RT-PCR was utilised to determine hepcidin mRNA expression in colorectal cancer tissue. Hepcidin cellular localisation was determined using im- munohistochemistry. RESULTS: We demonstrate that whilst urinary hepcidin expression was not correlated with anaemia it was posi- tively associated with increasing T-stage of colorectal cancer (P < 0.05). Furthermore, we report that hepcidin mRNA is expressed in 34% of colorectal cancer tissue specimens and was correlated with ferroportin repres- sion. This was supported by hepcidin immunoreactivity in colorectal cancer tissue. CONCLUSION: We demonstrate that systemic hepcidin expression is unlikely to be the cause of the systemic anaemia associated with colorectal cancer. | 15 | Liver pathology of hepatitis C,beyond grading and stagingof the disease显示文摘Liver biopsy evaluation plays a critical role in management of patients with viral hepatitis C. In patients with acute viral hepatitis, a liver biopsy, though uncommonly performed, helps to rule out other nonviral causes of deranged liver function. In chronic viral hepatitis C, it is considered the gold standard in assessment of the degree of necroinflammation and the stage of fibrosis, to help guide treatment and determine prognosis. It also helps rule out any concomitant diseases such as steatohepatitis, hemochromatosis or others. In patients with chronic progressive liver disease with cirrhosis and dominant nodules, a targeted liver biopsy is helpful in differentiating a regenerative nodule from dysplastic nodule or hepatocellular carcinoma. In the setting of transplantation, the liver biopsy helps distinguish recurrent hepatitis C from acute rejection and also is invaluable in the diagnosis of fibrosing cholestatic hepatitis, a rare variant of recurrent hepatitis C. | 17 | Plasma microRNA profiles distinguish lethal injury in acetaminophen toxicity: A research study显示文摘AIM: To investigate plasma microRNA (miRNA) profiles indicative of hepatotoxicity in the setting of lethal acetaminophen (APAP) toxicity in mice. METHODS: Using plasma from APAP poisoned mice, either lethally (500 mg/kg) or sublethally (150 mg/kg) dosed, we screened commercially available murine microRNA libraries (SABiosciences, Qiagen Sciences, MD) to evaluate for unique miRNA profiles between these two dosing parameters. RESULTS: We distinguished numerous, unique plasma miRNAs both up- and downregulated in lethally compared to sublethally dosed mice. Of note, many of the greatest up- and downregulated miRNAs, namely 574-5p, 466g, 466f-3p, 375, 29c, and 148a, have been shown to be associated with asthma in prior studies. Interestingly, a relationship between APAP and asthma has been previously well described in the literature, with an as yet unknown mechanism of pathology. There was a statistically significant increase in alanine aminotransferase levels in the lethal compared to sublethal APAP dosing groups at the 12 h time point (P < 0.001). There was 90% mortality in the lethally compared to sublethally dosed mice at the 48 h time point (P = 0.011).
