1Reversibility and heritability of liver fibrosis:Implications for research and therapy显示文摘Liver fibrosis continues to be a major health problem worldwide due to lack of effective therapy.If the etiology cannot be eliminated,liver fibrosis progresses to cirrhosis and eventually to liver failure or malignancy;both are associated with a fatal outcome.Liver transplantation,the only curative therapy,is still mostly unavailable.Liver fibrosis was shown to be a reversible process;however,complete reversibility remains debatable.Recently,the molecular markers of liver fibrosis were shown to be transmitted across generations.Epigenetic mechanisms including DNA methylation,histone posttranslational modifications and noncoding RNA have emerged as major determinants of gene expression during liver fibrogenesis and carcinogenesis.Furthermore,epigenetic mechanisms have been shown to be transmitted through mitosis and meiosis to daughter cells and subsequent generations.However,the exact epigenetic regulation of complete liver fibrosis resolution and inheritance has not been fully elucidated.This communication will highlight the recent advances in the search for delineating the mechanisms governing resolution of liver fibrosis and the potential for multigenerational and transgenerational transmission of fibrosis markers.The fact that epigenetic changes,unlike genetic mutations,are reversible and can be modulated pharmacologically underscores the unique opportunity to develop effective therapy to completely reverse liver fibrosis,to prevent the development of malignancy and to regulate heritability of fibrosis phenotype.Hussein M Atta 2015World Journal of Gastroenterology2015,21,17:23

3基质金属蛋白酶-2/9及其组织抑制剂比例失衡在高氧所致急性肺损伤中的作用显示文摘目的探讨基质金属蛋白酶-2/9(MMP-2/9)及其组织抑制剂(TIMP-1/2)在高氧所致急性肺损伤(ALI)发病过程中的作用。方法将54只小鼠置于含体积分数大于98%氧气的密闭室中,以18只呼吸正常空气的小鼠作为对照组。分别于暴露24、48和72h活杀各组小鼠,取肺组织标本,测定肺湿/干重(W/D)比值、支气管肺泡灌洗液(BALF)中蛋白含量及胸腔积液,以评价肺损伤程度。用逆转录-聚合酶链反应(RT—PCR)测定肺组织MMP-2/9和TIMP-1/2的mRNA表达;用酶联免疫吸附法(ELLSA)测定BALF中MMP-2/9及TIMP1/2的蛋白含量。结果高氧能引起ALI,各实验组的肺W/D比值、BALF中蛋白浓度及胸腔积液均明显高于对照组(P〈0.05或P〈0.01)。RT—PCR结果显示,高氧组肺组织MMP-2/9及TIMP-1的mRNA表达均较对照组明显增高(P〈0.05或P〈0.01),TIMP-2 mRNA表达无明显变化;MMP-2/TIMP-2的mRNA比值在高氧组中均明显升高(P均〈0.01)。ELISA结果显示,高氧组BALF中MMP-2/9和TIMP-1蛋白含量较对照组明显升高(P〈0.05或P〈0.01),而TIMP-2蛋白含量同样无明显变化;MMP-2/TIMP-2的蛋白比值在高氧组中也明显升高(P均〈0.01)。结论高氧能引起ALI伴MMP-2/9和TIMP-1的表达增高,而MMP-2/TIMP-2平衡失调导致细胞外基质降解在奠发生过程中起重要作用。张向峰 朱光发 刘双 Hussein D.Foda 2008中国危重病急救医学2008,20,10:14
4Cytotoxic genes from traditional Chinese medicine inhibit tumor growth both in vitro and in vivo显示文摘OBJECTIVE: Little effort has been made to study the protein-encoding genes isolated from traditional Chinese medicine(TCM) drugs, and the delivery of these genes into malignant cells through recombinant adeno-associated virus(r AAV) vectors has not been attempted. METHODS: We synthesized the c DNAs of five known cytotoxic proteins isolated from TCM drugs and the FLAG epitope-tagged c DNAs were subcloned into a r AAV plasmid vector. The protein expression was confi rmed by Western blot assay. Various cancer cell lines were transfected with the above plasmids and cell growth was monitored both in vitro and in vivo. The best cytotoxic gene was further packaged into r AAV vectors, under the control of a liver cancer-specifi c promoter. The liver tumor growth was then monitored following intratumor administration of the r AAV vectors.RESULTS: The expression plasmids, encoding individual potential cytotoxic genes tagged with FLAG epitope, were successfully generated and sequenced. Among these genes, trichosanthin(TCS) gene yielded the most promising results for the inhibition of cancer cell growth in vitro. The over-expressed TCS functioned as a type I ribosome-inactivating protein, followed by inducing apoptosis that is associated with the Bcl-PARP signaling pathway. 5Highlights in pathogenesis of vitiligo显示文摘Vitiligo is a common pigmentary disorder. Many studies across decades and all over the world have attempted to illustrate the pathogenesis behind it; however, the pathogenesis of vitiligo remains elusive. This review article, we present the findings behind the most and updated theories behind this psychologically debilitating and disfiguring disease. The discussion begun with the role of genetic predisposition followed by neural theory first proposed in the 1950 s. Wehighlight the autoimmune hypothesis, followed by the reactive oxygen species model, zinc-α2-glycoprotein deficiency hypothesis, viral theory, intrinsic theory and biochemical, molecular and cellular alterations accounting for loss of functioning melanocytes in vitiligo. Many theories were elaborated to clarify vitiligo pathogenesis. It is a multifactorial disease involving the interplay of several factors. 6Differential control of growth,apoptotic activity and gene expression in human colon cancer cells by extracts derived from medicinal herbs,Rhazya stricta and Zingiber officinale and their combination显示文摘AIM:To investigate the effects of extracts from Rhazya stricta(R.stricta) and Zingiber officinale(Z.officinale) on human colorectal cancer cells.METHODS:Human colorectal cancer cells(HCT116) were subjected to increasing doses of crude alkaloid extracts from R.stricta(CAERS) and crude flavonoid extracts from Z.officinale(CFEZO).Cells were then harvested after 24,48 or 72 h and cell viability was examined by trypan blue exclusion dye test; clonogenicity and soft agar colony-forming assays were also carried out.Nuclear stain(Hoechst 33342),acridine orange/ethidium bromide double staining,agarose gel electrophoresis and comet assays were performed to assess pro-apoptotic potentiality of the extracts.Quantitative reverse transcription-polymerase chain reaction(qRT-PCR),using gene-specific primers and Western blot analyses were performed to assess the impact of CAERS and CFEZO on the expression levels of key regulatory proteins in HCT116 cells.RESULTS:Treatment with a combination of CAERS and CFEZO synergistically suppressed the proliferation,colony formation and anchorage-independent growth of HCT116 cells.Calculated IC50,after 24,48 and 72 h,were 70,90 and 130 μg/mL for CAERS,65,85 and 120 μg/mL for CFEZO and 20,25 and 45 μg/mL for both agents,respectively.CAERS- and CFEZO-treated cells exhibited morphologic and biochemical features of apoptotic cell death.The induction of apoptosis was associated with the release of mitochondrial cytochrome c,an increase in the Bax/Bcl-2 ratio,activation of caspases 3 and 9 and cleavage of poly ADP-ribose polymerase.CAERS and CFEZO treatments downregulated expression levels of anti-apoptotic proteins including Bcl-2,Bcl-X,Mcl-1,survivin and XIAP,and upregulated expression levels of proapoptotic proteins such as Bad and Noxa.CAERS and CFEZO treatments elevated expression levels of the oncosuppressor proteins,p53,p21 and p27,and reduced levels of the oncoproteins,cyclin D1,cyclin/cyclin-dependent kinase-4 and c-Myc.CONCLUSION:These data suggest that a combination of CAERS and CFEZO is a promising treatment for the prevention of colon cancer.Ayman I Elkady Rania Abd El Hamid Hussein Osama A Abu-Zinadah 2014World Journal of Gastroenterology2014,20,41:12

9Endoscopic retrograde cholangiopancreatography in periampullary diverticulum: The challenge of cannulation显示文摘Periampullary diverticulum(PAD) is duodenal outpunching defined as herniation of the mucosa or submucosa that occurs via a defect in the muscle layer within an area of 2 to 3 cm around the papilla. Although PAD isusually asymptomatic and discovered incidentally during endoscopic retrograde cholangiopancreatography(ERCP), it is associated with different pathological conditions such as common bile duct obstruction, pancreatitis, perforation, bleeding, and rarely carcinoma. ERCP has a low rate of success in patients with PAD,suggesting that this condition may complicate the technical application of the ERCP procedure. Moreover, cannulation of PAD can be challenging, time consuming, and require the higher level of skill of more experienced endoscopists. A large portion of the failures of cannulation in patients with PAD can be attributed to inability of the endoscopist to detect the papilla. In cases where the papilla is identified but does not point in a suitable direction for cannulation, different techniques have been described. Endoscopists must be aware of papilla identification in the presence of PAD and of different cannulation techniques, including their technical feasibility and safety, to allow for an informed decision and ensure the best outcome. 10Real time endoscopic ultrasound elastography and strain ratio in the diagnosis of solid pancreatic lesions显示文摘AIM To evaluate the accuracy of the elastography score combined to the strain ratio in the diagnosis of solid pancreatic lesions(SPL). METHODS A total of 172 patients with SPL identified by endoscopic ultrasound were enrolled in the study to evaluate the efficacy of elastography and strain ratio in differentiating malignant from benign lesions. The semi quantitative score of elastography was represented by the strain ratio method. Two areas were selected, area(A) representing the region of interest and area(B) representing the normal area. Area(B) was then divided by area(A). Sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), and accuracy were calculated by comparing diagnoses made by elastography, strain ratio and final diagnoses.RESULTS SPL were shown to be benign in 49 patients and malignant in 123 patients. Elastography alone had a sensitivity of 99%, a specificity of 63%, and an accuracy of 88%, a PPV of 87% and an NPV of 96%. The best cut-off level of strain ratio to obtain the maximal area under the curve was 7.8 with a sensitivity of 92%, specificity of 77%, PPV of 91%, NPV of 80% and an accuracy of 88%. Another estimated cut off strain ratio level of 3.8 had a higher sensitivity of 99% and NPV of 96%, but with less specificity, PPV and accuracy 53%, 84% and 86%, respectively. Adding both elastography to strain ratio resulted in a sensitivity of 98%, specificity of 77%, PPV of 91%, NPV of 95% and accuracy of 92% for the diagnosis of SPL. 12Harvest and Postharvest Factors Affecting Bruise Damage of Fresh Fruits显示文摘Fresh fruits are susceptible to bruising, a common type of mechanical damage during harvest and at all stages of postharvest handling. In quest of developing and adoption of strategies to reduce bruise damage, it is of utmost importance to understand major factors influencing bruise susceptibility of fresh produce at these stages. This review presents a critical discussion of factors affecting bruising during harvest and postharvest handling of fresh fruits. Excessive compression forces during harvesting by handpicking or machines, and a series of impacts during harvesting, transport and packhouse operations can cause severe bruise damage. The review has further revealed that bruising is dependent on a number of other factors such as produce maturity, ripening, harvest time(during the day or season) and time lapse after harvest. The susceptibility to bruising is partly dependent on how these factors alter the produce physiological and biochemical properties, and the environmental conditions such as temperature, humidity and several other postharvest treatments. 13Preliminary study on the antibacterial activity of some medicinal plants of Khuzestan(Iran)显示文摘Objective:To search for antimicrobial agents among natural products.Methods:Ethanolic extracts of 4 plant species,including Beta vulgaris L.(Chenopodiaceae),Amaranthus graecizans (A.graecizans) L.(Amaranthaceae),Rumex obtusifolius(R.obtusifolius) L.and Polygonum patulum(P.patulum) M.B.(Polygonaceae),were evaluated for antibacterial activity using agar disc diffusion method against some gram-positive and gram-negative bacteria[Pseudomonas aeruginosa(P.aeruginosa),Listeria monocytogenes(L.monocytogenes).Staphylococcus epidermidis(S.epidermidis),Staphylococcus aureus(S.aureus),Klebsiella pneumoniae(K. pneumoniae),Salmonella typhi(S.typhi),Bacillus cereus(B.cereus).Bacillus anthracis(B. anthracis),Escherichia coli(E.coli) and Streptococcus pyogenes(Str.pyogenes)].These extracts were obtained from aerial parts of the used plants.Results:The majority of these extracts had inhibitory effect at different concentrations(0.05 g/mL,0.10 g/mL,0.20 g/mL and 0.40 g/mL) against above mentioned bacteria.E.coli was the most resistant strain.The highest inhibitory zone was showed by ethanolic extract of P.patulum against Str.pyogenes(28 mm) and followed by ethanolic extract of B.vulgaris against S.epidermidis(23 mm).The extract of A.graecizans didn’t show inhibitory activity except at 0.40 g/mL against B.cereus.The minimum inhibitory concentration(MIC) and minimum bactericidal concentration(MBC) values of R.obtusifolius extract that was measured against Str.pyogenes were equal(MIC=MBC=5.00mg/mL).Conclusion: The findings of this study could also be as new source for antibiotics discovery and infection treatment.Haniyeh Koochak Seyyed Mansour Seyyednejad Hussein Motamedi 2010Asian Pacific Journal of Tropical Medicine2010,3,3:10
14Clinical correlates of common corneal neovascular diseases: a literature review显示文摘A large subset of corneal pathologies involves the formation of new vessels(neovascularization), leading to compromised visual acuity. This article aims to review the clinical causes and presentations of corneal neovascularization(CNV) by examining the mechanisms behind common CNV-related corneal pathologies, with a particular focus on herpes simplex stromal keratitis,contact lenses-induced keratitis and CNV secondary to keratoplasty. 15Prevention of peritoneal adhesions:A promising role for gene therapy显示文摘Adhesions are the most frequent complication of abdominopelvic surgery,yet the extent of the problem,and its serious consequences,has not been adequately recognized.Adhesions evolved as a life-saving mecha-nism to limit the spread of intraperitoneal inflammatory conditions.Three different pathophysiological mechanisms can independently trigger adhesion formation.Mesothelial cell injury and loss during operations,tissue hypoxia and inflammation each promotes adhesion formation separately,and potentiate the effect of each other.Studies have repeatedly demonstrated that interruption of a single pathway does not completely prevent adhesion formation.This review summarizes the pathogenesis of adhesion formation and the results of single gene therapy interventions.It explores the prom-ising role of combinatorial gene therapy and vector modif ications for the prevention of adhesion formation in order to stimulate new ideas and encourage rapid advancements in this field.Hussein M Atta 2011World Journal of Gastroenterology2011,17,46:10
16Dismicrobism in inflammatory bowel disease and colorectal cancer: Changes in response of colocytes显示文摘Patients with inflammatory bowel disease(IBD)have an increased risk of 10%-15%developing colorectal cancer(CRC)that is a common disease of high economic costs in developed countries.The CRC has been increasing in recent years and its mortality rates are very high.Multiple biological and biochemical factors are responsible for the onset and progression of this pathology.Moreover,it appears absolutely necessary to investigate the environmental factors favoring the onset of CRC and the promotion of colonic health.The gut microflora,or microbiota,has an extensive diversity both quantitatively and qualitatively.In utero,the intestine of the mammalian fetus is sterile.At birth,the intestinal microbiota is acquired by ingesting maternal anal or vaginal organisms,ultimately developing into a stable community,with marked variations in microbial composition between individuals.The development of IBD is often associated with qualitative and quantitative disorders of the intestinal microbial flora(dysbiosis).The healthy human gut harbours about10 different bacterial species distributed in colony forming units which colonize the gastrointestinal tract.The intestinal microbiota plays a fundamental role in health and in the progression of diseases such as IBD and CRC.In healthy subjects,the main control of intestinal bacterial colonization occurs through gastric acidity but other factors such as endoluminal temperature,competition between different bacterial strains,peristalsis and drugs can influence the intestinal microenvironment.The microbiota exerts diverse physiological functions to include:growth inhibition of pathogenic microorganisms,synthesis of compounds useful for the trophism of colonic mucosa,regulation of intestinal lymphoid tissue and synthesis of amino acids.Furthermore,mucus seems to play an important role in protecting the intestinal mucosa and maintaining its integrity.Changes in the microbiota composition are mainly influenced by diet and age,as well as genetic factors.Increasing evidence indicates that dysbiosis favors the production of genotoxins and metabolites associated with carcinogenesis and induces dysregulation of the immune response whichpromotes and sustains inflammation in IBD leading to carcinogenesis.A disequilibrium in gut microflora composition leads to the specific activation of gut associated lymphoid tissue.The associated chronic inflammatory process associated increases the risk of developing CRC.Ulcerative colitis and Crohn’s disease are the two major IBDs characterized by an early onset and extraintestinal manifestations,such as rheumatoid arthritis.The pathogenesis of both diseases is complex and not yet fully known.However,it is widely accepted that an inappropriate immune response to microbial flora can play a pivotal role in IBD pathogenesis.Giovanni Tomasello Pietro Tralongo Provvidenza Damiani Emanuele Sinagra Benedetto Di Trapani Marie Noelle Zeenny Inaya Hajj Hussein Abdo Jurjus Angelo Leone 2014World Journal of Gastroenterology2014,20,48:10
17Open globe injury in Hospital Universiti Sains Malaysia- a 10-year review显示文摘AIM:To identify the aetiology of open globe injuries at Hospital Universiti Sains Malaysia over a period of 10y and the prognostic factors for visual outcome.METHODS:Retrospective review of medical records of open globe injury cases that presented from January2000 to December 2009.Classification of open globe injury was based on the Birmingham Eye Trauma Terminology(BETT).Records were obtained with hospital permission via the in-house electronic patient management system,and the case notes of all patients with a diagnosis of open globe injury were scrutinised.Patients with prior ocular trauma,pre-existing ocular conditions affecting the visual acuity,contrast sensitivity,central vision or corneal thickness,as well as those with a history of previous intraocular or refractive surgery were excluded.Analysis of data was with SPSS version20.0.Ordinal logistic regression analysis was used to examine the association between prognostic factors and visual outcome.RESULTS:This study involved 220 patients(n=222eyes).The most common place of injury was the home(51.8%),followed by the workplace(23.4%).Among children aged less than 16y of age,domestic-related injury was the predominant cause(54.6%),while in those aged 16y and above,occupational injuries were the most common cause(40.0%).Most eyes(76.5%)had an initial visual acuity worse than 3/60,and in half of these,the visual acuity improved.The visual outcome was found to be significantly associated with the initial visual acuity(P<0.005),posterior extent of wound(P<0.001),length of wound(P<0.001),presence of hyphaema(P<0.001)and presence of vitreous prolapse(P<0.005).CONCLUSION:The most common causes of open globe injury are domestic accidents and occupational injuries.Significant prognostic factors for final visual outcome in patients with open globe injury are initial visual acuity,posterior extent and length of wound,presence of hyphaema and presence of vitreous prolapse.Awareness of the factors predicting a poor visual outcome may be helpful during counselling of patients with open globe injuries.Madhusudhan A/L Paramananda Evelyn-Tai Li Min Zamri Noordin Adil Hussein Wan-Hazabbah Wan Hitam 2014International Journal of Ophthalmology(English edition)2014,7,3:9

19Circulating tumor and cancer stem cells in hepatitis C virusassociated liver disease显示文摘AIM:To assess the role of circulating tumor cells(CTCs)and cancer stem cells(CSCs)in hepatitis C virus(HCV)-associated liver disease.METHODS:Blood and/or tissue samples were obtained from HCV(genotype 4)-associated hepatocellularcarcinoma patients(HCC;n=120),chronic hepatitis C patients(CH;n=30)and 33 normal control subjects(n=33).Serum levels of alpha-fetoprotein(AFP),alkaline phosphatase,and alanine and aspartate aminotransferases were measured.Cytokeratin 19(CK19)monoclonal antibody was used to enumerate CTCs,and CD133 and CD90 were used to enumerate CSCs by flow cytometry.The expression levels of the CSCs markers(CD133 and CD90)as well as telomerase,melanoma antigen encoding gene 1(MAGE1)and MAGE3 were assessed by RTPCR and quantitative real-time polymerase chain reactions.The number of CTCs and/or the expression levels of CK19,CD133,telomerase,MAGE1 and MAGE3 were correlated to the standard clinicopathologic prognostic factors and disease progression.RESULTS:Levels of AFP,alkaline phosphatase and aspartate aminotransferase were significantly different among the HCC,CH and control groups(P<0.001),whereas alanine aminotransferase differed significantly between patient(HCC and CH)and control groups(P<0.001).At the specified cutoff values determine by flow cytometry,CK19(49.8),CD90(400)and CD133(73)were significantly higher in the blood of HCC patients compared to those in the CH and control groups(P<0.001).On the other hand,CD133 at a 69.5 cutoff was significantly higher in the CH compared to the control group(P≤0.001).Telomerase,MAGE1 and MAGE3RNA were expressed in 55.71%,60.00%and 62.86%of the HCC patients,respectively,but were not detected in patients in the CH or control groups,which were statistically significant(Ps<0.001).The expression levels of telomerase,CD90,MAGE3,CD133 and CK19 were all significantly associated with high tumor grade and advanced stage in HCC patients(all P s<0.05).CONCLUSION:CTC counts and AFP,CK19,telomerase,and MAGE1/MAGE3 expression predict disease progression in patients with HCV,whereas telomerase,MAGE3,CD90,CD133 and CK19 are prognostic mark-Abeer A Bahnassy Abdel-Rahman N Zekri Ahmed El-Bastawisy Amal Fawzy Marwa Shetta Nehal Hussein Dalia Omran Abdallah A S Ahmed Samir S El-Labbody 2014World Journal of Gastroenterology2014,20,48:9
