| 1 | Novel 3-dimensional virtual hepatectomy simulation combined with real-time deformation显示文摘AIM:To develop a novel 3-dimensional(3D) virtual hepatectomy simulation software,Liversim,to visualize the real-time deformation of the liver.METHODS:We developed a novel real-time virtual hepatectomy simulation software program called Liversim. The software provides 4 basic functions:viewing 3D models from arbitrary directions,changing the colors and opacities of the models,deforming the models based on user interaction,and incising the liver parenchyma and intrahepatic vessels based on user operations. From April 2010 through 2013,99 patients underwent virtual hepatectomies that used the conventional software program SYNAPSE VINCENT preoperatively. Between April 2012 and October 2013,11 patients received virtual hepatectomies using the novel software program Liversim; these hepatectomies were performed both preoperatively and at the same that the actual hepatectomy was performed in an operating room. The perioperative outcomes were analyzed between the patients for whom SYNAPSE VINCENT was used and those for whom Liversim wasused. Furthermore,medical students and surgical residents were asked to complete questionnaires regarding the new software.RESULTS:There were no obvious discrepancies(i.e.,the emergence of branches in the portal vein or hepatic vein or the depth and direction of the resection line) between our simulation and the actual surgery during the resection process. The median operating time was 304 min(range,110 to 846) in the VINCENT group and 397 min(range,232 to 497) in the Liversim group(P = 0.30). The median amount of intraoperative bleeding was 510 m L(range,18 to 5120) in the VINCENT group and 470 m L(range,130 to 1600) in the Liversim group(P = 0.44). The median postoperative stay was 12 d(range,6 to 100) in the VINCENT group and 13 d(range,9 to 21) in the Liversim group(P = 0.36). There were no significant differences in the preoperative outcomes between the two groups. Liversim was not found to be clinically inferior to SYNAPSE VINCENT. | 2 | Transferring pose and augmenting background for deep humanimage parsing and its applications显示文摘Parsing of human images is a fundamental task for determining semantic parts such as the face,arms, and legs, as well as a hat or a dress. Recent deep-learning-based methods have achieved significant improvements, but collecting training datasets with pixel-wise annotations is labor-intensive. In this paper,we propose two solutions to cope with limited datasets.Firstly, to handle various poses, we incorporate a pose estimation network into an end-to-end humanimage parsing network, in order to transfer common features across the domains. The pose estimation network can be trained using rich datasets and can feed valuable features to the human-image parsing network. Secondly, to handle complicated backgrounds,we increase the variation in image backgrounds automatically by replacing the original backgrounds of human images with others obtained from large-scale scenery image datasets. Individually, each solution is versatile and beneficial to human-image parsing, while their combination yields further improvement. | 4 | Transarterial chemoembolization with miriplatin-lipiodol emulsion for neuroendocrine metastases of the liver显示文摘Miriplatin,a cisplatin derivative with a high affinity for iodized oil,is a novel chemotherapeutic agent designed for use in the transarterial treatment of hepatocellular carcinoma.This case report describes our experience with transarterial chemoembolization(TACE) using miriplatin in 2 patients with neuroendocrine liver metastases.A 38-year-old man with multiple neuroendocrine liver metastases was treated by whole liver chemoembolization,and a 35-year-old woman with a single hepatic lesion was treated by superselective chemoembolization.No serious adverse events were noted during the interventional procedures,or during the observation period of 3 mo in either patient.Sufficient iodized oil uptake was observed in the hypervascular lesions on the unenhanced computed tomography(CT) at 7 d after the procedure.Contrast-enhanced CT obtained at 3 mo after chemoembolization revealed that all hepatic lesions were substantially reduced in size irrespective of tumor vascularity or degree of cystic degeneration,although iodized oil accumulation was only marginal for lesions with cystic degeneration.Thus,TACE with miriplatin can be a safe and effective therapeutic option for the treatment of neuroendocrine metastases of the liver. | Jin Iwazawa Shoichi Ohue Keigo Yasumasa Takashi Mitani | 2010 | World Journal of Radiology2010,2,12: | 2 |

| 10 | 对有静脉内的提高对比的 C 手臂的肝肿瘤的脱离边缘评价计算了断层摄影术显示文摘 AIM:To evaluate the feasibility of intravenous contrastenhanced C-arm computed tomography(CT) for assessing ablative areas and margins of liver tumors.METHODS:Twelve patients(5 men,7 women;mean age,69.5 years) who had liver tumors(8 hepatocellular carcinomas,4 metastatic liver tumors;mean size,16.3 mm;size range,8-20 mm) and who underwent percutaneous radiofrequency ablations(RFAs) with a flatdetector C-arm system were retrospectively reviewed.Intravenously enhanced C-arm CT and multidetector computed tomography(MDCT) images were obtained at the end of the RFA sessions and 3-7 d after RFA to evaluate the ablative areas and margins.The ablated areas and margins were measured using axial plane images acquired by both imaging techniques,with prior contrast-enhanced MDCT images as the reference.The sensitivity,specificity,and positive and negative predictive values of C-arm CT for detecting insufficient ablative margins(< 5 mm) were calculated.Statistical differences in the ablative areas and margins evaluated with both imaging techniques were compared using a paired t-test.RESULTS:All RFA procedures were technically successful.Of 48 total ablative margins,19(39.6%) and 20(41.6%) margins were found to be insufficient with C-arm CT and MDCT,respectively.Moreover,there were no significant differences between these 2 imaging techniques in the detection of these insufficient ablative margins.The sensitivity,specificity,and positive and negative predictive values for detecting insufficient margins by C-arm CT were 90.0%,96.4%,94.7% and 93.1%,respectively.The mean estimated ablative areas calculated from C-arm CT(462.5 ± 202.1 mm 2) and from MDCT(441.2 ± 212.5 mm 2) were not significantly different.The mean ablative margins evaluated by C-arm CT(6.4 ± 2.2 mm) and by MDCT(6.0 ± 2.4 mm) were also not significantly different.CONCLUSION:The efficacy of intravenous contrastenhanced C-arm CT in assessing the ablative areas and margins after RFA of liver tumors is nearly equivalent to that of MDCT. | Jin Iwazawa Shoichi Ohue Naoko Hashimoto Takashi Mitani | 2012 | World Journal of Radiology2012,4,3: | 2 |
