1Metastatic basaloid-squamous cell carcinoma of the esophagus treated by 5-fluorouracil and cisplatin显示文摘Basaloid squamous cell carcinoma (BSC) of the esophagus is a rare malignant disease. We report here a patient with recurrent esophageal BSC,who was successfully treated by systemic chemotherapy containing 5-fluorouracil (5-FU) and cisplatin (CDDP). A 57-year-old woman was diagnosed as having squamous cell carcinoma of the esophagus upon endoscopic examination. Curative esophagectomy with lymph node dissection was performed under the thoracoscope. The pathological diagnosis of the surgical specimen was BSC. Five months after operation,the patient was diagnosed as having a recurrence of the BSC with metastases to the liver and spleen,and a right paraclavicular lymph node. She was given systemic chemotherapy consisting of continuous infusion of 800 mg/d of 5-FU and 3 h infusion of 20 mg/d of CDDP for 5 consecutive days every 4 wk. The metastatic lesions in the spleen and right paraclavicular lymph node disappeared,and the liver metastasis was apparently reduced in size after 2 courses of chemotherapy. The tumor regression was seen over 6 courses,with progression afterwards. 2Nitrous oxide fluxes from upland soils in central Hokkaido,Japan显示文摘Nitrous oxide (N2O) fluxes from soils were measured using the closed chamber method during the snow-free seasons (middle April to early November),for three years,in a total of 11 upland crop fields in central Hokkaido,Japan.The annual mean N2O fluxes ranged from 2.95 to 164.17 μgN/(m2·h),with the lowest observed in a grassland and the highest in an onion field.The instantaneous N2O fluxes showed a large temporal variation with peak emissions generally occurring following fertilization and heavy rainfall events around harvesting in autumn.No clear common factor regulating instantaneous N2O fluxes was found at any of the study sites.Instead,instantaneous N2O fluxes at different sites were affected by different soil variables.The cumulative N2O emissions during the study period within each year varied from 0.15 to 7.05 kgN/hm2 for different sites,which accounted for 0.33% to 5.09% of the applied fertilizer N.No obvious relationship was observed between cumulative N2O emission and applied fertilizer N rate (P > 0.4).However,the cumulative N2O emission was significantly correlated with gross mineralized N as estimated by CO2 emissions from bare soils divided by C/N ratios of each soil,and with soil mineral N pool (i.e.,the sum of gross mineralized N and fertilizer N) (P < 0.001).Sonoko D.KIMURA Yo TOMA Ryusuke HATANO 2008Journal of Environmental Sciences2008,20,11:8
3Evaluation of the relationship between hepatocellular carcinoma location and transarterial chemoembolization efficacy显示文摘AIM To evaluate the relationship between the location of hepatocellular carcinoma(HCC) and the efficacy of transarterial chemoembolization(TACE).METHODS We evaluated 115 patients(127 nodules), excluding recurrent nodules, treated with TACE between January 2011 and June 2014. TACE efficacy was evaluated according to m RECIST. The HCC location coefficient was calculated as the distance from the central portal portion to the HCC center(mm)/liver diameter(mm) on multiplanar reconstruction images rendered(MPR) to visualize bifurcation of the right and left branches of the portal vein and HCC center. The HCC location coefficient was compared between complete response(CR) and non-CR groups in Child-Pugh grade A and B patients.RESULTS The median location coefficient of HCC among all nodules, the right lobe, and the medial segment was significantly higher in the CR group than in the non-CR group in the Child-Pugh grade A patients(0.82 vs 0.62, P < 0.001; 0.71 vs 0.59, P < 0.01; 0.81 vs 0.49, P < 0.05, respectively). However, there was no significant difference in the median location coefficient of the HCC in the lateral segment between in the CR and in the non-CR groups(0.67 vs 0.65, P > 0.05). 4Impact of changing our cannulation method on the incidenceof post-endoscopic retrograde cholangiopancreatography pancreatitis after pancreatic guidewire placement显示文摘AIM: To clarify whether the incidence of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) after pancreatic guidewire placement (PGW) can be reduced by using a different cannulation method. METHODS: Between April 2001 and October 2009, PGW was performed in 142 patients with native papilla to overcome difficult biliary cannulation. Our cannulation method for ERCP was changed from contrast injection (CI) using a single-lumen catheter (April 2001-May 2008) to wire-guided cannulation (WGC) using a double-lumen catheter (June 2008-October 2009). The CI protocol was also changed during the study period: in the first period it was used for routine pancreatography for detecting small pancreatic cancer (April 2001-November 2002), whereas in the second period it was not (December 2002-May 2008). In PGW with CI using a singlelumen catheter, the contrast medium in the catheter lumen was injected into the pancreatic duct. The success rate of biliary cannulation, the incidence of PEP according to the cannulation method, and the impact of CI using a single-lumen catheter on PEP in comparison with WGC using a double-lumen catheter were investigated.RESULTS: CI with routine pancreatography, CI without routine pancreatography, and WGC were performed in 27 patients, 77 patients and 38 patients, respectively. Routine pancreatography did not contribute to the early diagnosis of pancreatic cancer in our study period. In CI without routine pancreatography and WGC, diagnostic pancreatography was performed in 17 patients and no patients, respectively. The success rate of biliary cannulation by PGW alone was 69%, and the final success rate was increased to 80.3% by the addition of consecutive maneuvers or a second ERCP. PEP occurred in 22 patients (15.5%), and the severity was mild in all cases. When analyzed according to cannulation method, the incidence of PEP was 37.0% (10/27) in the patients who underwent CI with routine pancreatography, 14.3% (11/77) in those who underwent CI without routine pancreatography, and 2.6% (1/38) in those who underwent WGC. In all patients who underwent CI using a singlelumen catheter, the incidence of PEP was 20% (21/104), which was significantly higher than that in WGC using a double-lumen catheter. In univariate and multivariate analysis, CI using a single-lumen catheter showed a high, statistically significant, odds ratio for PEP after PGW. 6小关节不对称—青少年腰椎间盘突出症的放射学特征显示文摘目的 :成人与青少年腰椎间盘突出症中小关节不对称性的比较。方法 :研究组包括青少年组 2 5名患者 ,2 9个椎间盘和成人组 3 3名患者 ,5 0个椎间盘。使用CT测量每个小关节形态、小关节角、力矩臂角和长度。小关节不对称的诊断标准为 :双侧小关节的形态不同或双侧小关节角相差 10°以上。并比较了两组间小关节不对称的发生率及其与椎间盘突出的位置、类型以及椎间盘退变之间的关系。结果 :青少年组 (12个间盘 ,占 41% )小关节不对称的发生率明显高于成人组 (4个间盘 ,占 8% ) (P <0 .0 1)。小关节不对称与椎间盘突出的位置、类型以及椎间盘退变之间无统计学差异。结论 :小关节不对称性是青少年腰椎间盘突出症的一个放射学特点。刘洪 Hirokazu Ishihara Ryusuke Osada 2003中国矫形外科杂志2003,11,24:7
7Effects of wing deformation on aerodynamic performance of a revolving insect wing显示文摘Flexible wings of insects and bio-inspired micro air vehicles generally deform remarkably during flapping flight owing to aerodynamic and inertial forces,which is of highly nonlinear fluid-structure interaction(FSI)problems.To elucidate the novel mechanisms associated with flexible wing aerodynamics in the low Reynolds number regime,we have built up a FSI model of a hawkmoth wing undergoing revolving and made an investigation on the effects of flexible wing deformation on aerodynamic performance of the revolving wing model.To take into account the characteristics of flapping wing kinematics we designed a kinematic model for the revolving wing in two-fold:acceleration and steady rotation,which are based on hovering wing kinematics of hawkmoth,Manduca sexta.Our results show that both aerodynamic and inertial forces demonstrate a pronounced increase during acceleration phase,which results in a significant wing deformation.While the aerodynamic force turns to reduce after the wing acceleration terminates due to the burst and detachment of leading-edge vortices(LEVs),the dynamic wing deformation seem to delay the burst of LEVs and hence to augment the aerodynamic force during and even after the acceleration.During the phase of steady rotation,the flexible wing model generates more ver-tical force at higher angles of attack(40°–60°)but less horizontal force than those of a rigid wing model.This is because the wing twist in spanwise owing to aerodynamic forces results in a reduction in the effective angle of attack at wing tip,which leads to enhancing the aerodynamics performance by increasing the vertical force while reducing the horizontal force.Moreover,our results point out the importance of the fluid-structure interaction in evaluating flexible wing aerodynamics:the wing deformation does play a significant role in enhancing the aerodynamic performances but works differently during acceleration and steady rotation,which is mainly induced by inertial force in acceleration but by aerodynamic forces in steady rotation.Ryusuke Noda Toshiyuki Nakata Hao Liu 2014Acta Mechanica Sinica2014,30,6:5

10Hepatic reconstruction from fetal porcine liver cells using a radial flow bioreactor显示文摘AIM: To examine the efficacy of the radial flow bioreactor (RFB) as an extracorporeal bioartificial liver (BAL) and the reconstruction of liver organoids using embryonic pig liver cells. METHODS: We reconstructed the liver organoids using embryonic porcine liver cells in the RFB. We also determined the gestational time window for the optimum growth of embryonic porcine liver cells. Five weeks of gestation was designated as embryonic day (E) 35 and 8 wk of gestation was designated as E56. These cells were cultured for one week before morphological and functional examinations. Moreover, the efficacy of pulsed administration of a high concentration hepatocyte growth factor (HGF) was examined. RESULTS: Both cell growth and function were excellent after harvesting on E35. The pulsed administration of a high concentration of HGF promoted the differentiation and maturation of these fetal hepatic cells. Microscopic examination of organoids in the RFB revealed palisading and showed that bile duct-like structures were well developed, indicating that the organoids were mini livers. Transmission electron microscopy revealed microvilli on the luminal surfaces of bile duct-like structures and junctional complexes, which form the basis of the cytoskeleton of epithelial tissues. Furthermore, strong expression of connexin (Cx) 32, which is the main protein of hepatocyte gap junctions, was observed. With respect to liver function, ammonia detoxification and urea synthesis were shown to be performed effectively. 16Relationship between clinicopathological features and mucin phenotypes of advanced gastric adenocarcinoma显示文摘AIM: To investigate a relationship between the clinicopathological features and mucin phenotypes in advanced gastric adenocarcinoma (AGA). METHODS: Immunohistochemical staining was performed to determine the mucin phenotypes in 38 patients with differentiated adenocarcinomas (DACs), 9 with signet-ring cell carcinomas (SIGs), and 48 with other diffuse-type adenocarcinomas (non-SIGs) of AGA. The mucin phenotypes were classified into 4 types: gastric (G), gastrointestinal (GI), intestinal, and unclassified. RESULTS: The G-related mucin phenotypes were highly expressed in all the histological subtypes of AGA. The expression of the GI phenotype in SIG patients was lower than that in DAC patients (P = 0.02), and this phenotype was observed in 56% of the non-SIG patients in the intramucosal layer. Among non-SIG cases, the expression of the GI phenotype was significantly higherin patients with extended adenocarcinomas and those with positive rates of lymph node metastasis. There was no difference between the expressions of the G and other GI phenotypes factors. Among DAC and non-SIG patients, there were no differences between the survival rates of the corresponding patient groups. CONCLUSION: The GI phenotype might possess more invasive characteristics than the G phenotype in nonSIG.
