1Regional lymphadenectomy for gallbladder cancer: Rational extent, technical details, and patient outcomes显示文摘AIM: To define the rational extent of regional lymphadenectomy for gallbladder cancer and to clarify its effect on long-term survival. METHODS: A total of 152 patients with gallbladder cancer who underwent a minimum of 'extended' portal lymph node dissection (defined as en bloc removal of the first-and second-echelon nodes) from 1982 to 2010 were retrospectively analyzed. Based on previous studies, regional lymph nodes of the gallbladder were divided into first-echelon nodes (cystic duct or pericholedochal nodes), second-echelon nodes (node groups posterosuperior to the head of the pancreas or around the hepatic vessels), and more distant nodes. RESULTS: Among the 152 patients (total of 3352 lymph nodes retrieved, median of 19 per patient), 79 patients (52%) had 356 positive nodes. Among nodepositive patients, the prevalence of nodal metastasis was highest in the pericholedochal (54%) and cystic duct (38%) nodes, followed by the second-echelon node groups (29% to 19%), while more distant node groups were only rarely (5% or less) involved. Disease-specific survival after R0 resection differed according to the nodal status (P < 0.001): most node-negative patients achieved long-term survival (median, not reached; 5-year survival, 80%), whereas among nodepositive patients, 22 survived for more than 5 years (median, 37 mo; 5-year survival, 43%). CONCLUSION: The rational extent of lymphadenectomy for gallbladder cancer should include the first-and second-echelon nodes. 2Catheter tract implantation metastases associated with percutaneous biliary drainage for extrahepatic cholangiocarcinoma显示文摘AIM: To estimate the incidence of catheter tract implantation metastasis among patients undergoing percutaneous transhepatic biliary drainage (PTBD) for extrahepatic cholangiocarcinoma, and to provide data regarding the management of this unusual complication of PTBD by reviewing cases reported in the literature.METHODS: A retrospective analysis of 67 consecutive patients who underwent PTBD before the resection of extrahepatic cholangiocarcinoma was conducted. The median follow-up period after PTBD was 106 mo. The English language literature (PubMed, National Library of Medicine, Bethesda, MD, USA), from January 1966through December 2004, was reviewed.RESULTS: Catheter tract implantation metastasis developed in three patients. The cumulative incidence of implantation metastasis reached a plateau (6%)at 20 mo after PTBD. All of the three patients with implantation metastasis died of tumor progression at 3, 9, and 20 mo after the detection of this complication. Among the 10 reported patients with catheter tract implantation metastasis from extrahepatic cholangiocarcinoma (including our three patients), two survived for more than 5 years after the excision of isolated catheter tract metastases.CONCLUSION: Catheter tract implantation metastasis is not a rare complication following PTBD for extrahepatic cholangiocarcinoma. 3Insulin resistance and chronic liver disease显示文摘Increased insulin resistance is frequently associated with chronic liver disease and is a pathophysiological feature of hepatogenous diabetes.Distinctive factors including hepatic parenchymal cell damage,portalsystemic shunting and hepatitis C virus are responsible for the development of hepatogenous insulin resistance/diabetes.Although it remains unclear whether insulin secretion from pancreatic beta cells is impaired as it is in type 2 diabetes,retinopathic and cardiovascular risk is low and major causes of death in cirrhotic patients with diabetes are liver failure,hepatocellular carcinoma and gastrointestinal hemorrhage.Hemoglobin A1c is an inaccurate marker for the assessment and management of hepatogenous diabetes.Moreover,exogenous insulin or sulfonylureas may be harmful because these agents may promote hepatocarcinogenesis.Thus,pathogenesis,cause of death,assessment and therapeutic strategy for hepatogenous insulin resistance/diabetes differ from those for lifestyle-related type 2 diabetes.In this article,we review features of insulin resistance in relationship to chronic liver disease.We also discuss the impact of anti-diabetic agents on interferon treatment and hepatocarcinogenesis.Takumi Kawaguchi Eitaro Taniguchi Minoru Itou Masahiro Sakata Shuji Sumie Michio Sata 2011World Journal of Hepatology2011,3,5:25
4A New Acylated Anthocyanin from the Red Flowers of Camellia hongkongensis and Characterization of Anthocyanins in the Section Camellia Species显示文摘Twelve anthocyanins (1-12) were isolated from the red flowers of Camellia hongkongensis Seem.by chromatography using open columns.Their structures were elucidated on the basis of spectroscopic analyses,that is,proton-nuclear magnetic resonance,carbon 13-nuclear magnetic resonance,heteronuclear multiple quantum correlation,heteronuclear multiple bond correlation,high resolution electrospray ionization mass and ultraviolet visible spectroscopies.Out of these anthocyanins,a novel acylated anthocyanin,cyanidin 3-O-(6-O-(Z)-p-coumaroyl)-β-galactopyranoside (6),two known acylated anthocyanins,cyanidin 3-O-(6-O-(E)-p-coumaroyl)-β-galactopyranoside (7) and cyanidin 3-O-(6-O-(E)-caffeoyl)-β-galactopyranoside (8),and three known delphinidin glycosides (10-12) were for the first time isolated from the genus Camellia.Furthermore,pigment components in C.japonica L.,C.chekiangoleosa Hu and C.semiserrata Chi were studied.The results indicated that the distribution of anthocyanins was differed among these species.Delphinidin glycoside was only detected in the flowers of C.hongkongensis,which is a special and important species in the section Camellia.Based on the characterization of anthocyanins in the section Camellia species,there is a close relationship among these species,and C.hongkongensis might be an important parent for creating new cultivars with bluish flower color.Fumio Hashimoto Keiichi Shimizu Yusuke Sakata 2009Journal of Integrative Plant Biology2009,51,6:14
5'Extended' radical cholecystectomy for gallbladder cancer:Long-term outcomes, indications and limitations显示文摘AIM:To delineate indications and limitations for 'ex tended' radical cholecystectomy for gallbladder cancer:a procedure which was instituted in our department in 1982. METHODS:Of 145 patients who underwent a radi cal resection for gallbladder cancer from 1982 through 2006, 52 (36%) had an extended radical cholecystec tomy, which involved en bloc resection of the gallblad der, gallbladder fossa, extrahepatic bile duct, and the regional lymph nodes (first-and second-echelon node groups). A retrospective analysis of the 52 patients was conducted including at least 5 years of follow up. Residual tumor status was judged as no residual tumor (R0) or microscopic/macroscopic residual tumor (R1 2). athological findings were documented according tothe American Joint Committee on Cancer Cancer Stag ing anual (7th edition). RESULTS:he primary t mor as classified as patho-logical T1 (pT1) in 3 patients, pT2 in 36, pT3 in 12, and pT4 in 1. Twenty three patients had lymph node metastases; 11 had a single positive node, 4 had two positive nodes, and 8 had three or more positive nodes. None of the three patients with pT1 tumors had nodal disease, whereas 23 of 49 (47%) with pT2 or more advanced tumors had nodal disease. One patient died during the hospital stay for definitive resection, giv ing an in hospital mortality rate of 2%. Overall survival (OS) after extended radical cholecystectomy was 65% at 5 years and 53% at 10 years in all 52 patients. OS differed according to the p classification ( < 0.001) and the nodal status ( = 0.010). All of 3 patients with pT1 tumors and most (29 of 36) patients with pT2 tu mors survived for more than 5 years. Of 12 patients with pT3 tumors, 8 who had an R1 2 resection, distant metastasis, or extensive extrahepatic organ involve ment died soon after resection. Of the remaining four pT3 patients who had localized hepatic spread through the gallbladder fossa and underwent an R0 resection, 2 survived for more than 5 years and another survived for 4 years and 2 mo. The only patient with pT4 tumor died of disease soon after resection. Among 23 node positive patients, 11 survived for more than 5 years, and of these, 10 had a modest degree of nodal disease (one or two positive nodes). CONCLUSION:Extended radical cholecystectomy is indicated for pT2 tumors and some pT3 tumors with localized hepatic invasion, provided that the regional nodal disease is limited to a modest degree (up to two positive nodes). 7Utility of co-transplanting mesenchymal stem cells in islet transplantation显示文摘Islet transplantation is characterized by the transplantation of isolated islets from donor pancreata into a diabetic recipient. Although it is a viable choice in the treatment of insulin dependent diabetes mellitus, most patients (approximately 90%) require insulin five years after transplantation. Recently, the co-transplantation of mesenchymal stem cells (MSCs) and islets in animal studies has revealed the effectiveness of MSCs co-transplantation for improving islet function. Themechanisms underlying the beneficial impact of MSCs include immunomodulation and the promotion of angiogenesis. 9Narrow portion of the terminal choledochus is a cause of upstream biliary dilatation in patients with anomalous union of the pancreatic and biliary ducts显示文摘AIM: To clarify the pathogenesis of biliary dilatation associated with anomalous union of the pancreatic and biliary ducts (AUPBD).METHODS: Direct cholangiopancreatograms of 350 adult patients with or with suspicion of hepatobiliary or pancreatic disorders were reviewed. AUPBD was diagnosed cholangiopancreatographically, when the pancreaticobiliary ductal union was located above the narrow distal segment of the bile duct, which represents the action of the sphincter of Oddi. The narrow portion of the terminal choledochus was defined as symmetrical stricture of the common bile duct just above the pancreaticobiliary ductal union.RESULTS: AUPBD was found in 36 patients. Among cholangiopancreatographic features, the narrow portion of the terminal choledochus was the most pathognomonic for AUPBD (accuracy, 98%); it was present in 29 (81%)patients with AUPBD, but was not found in any patients without AUPBD. Among patients with AUPBD, biliary dilatation (>10 mm) was more frequent in those with the narrow portion of the terminal choledochus (23/29)than in those without (2/7; P = 0.018) AUPBD. Among the patients with both AUPBD and the narrow portion of the terminal choledochus, there was a strong negative correlation between the minimum diameter of the narrow portion and the maximum diameter of the choledochus (r = -0.78, P<0.001), suggesting that the degree of biliary narrowing at the narrow portion correlates with that of upstream biliary dilatation.CONCLUSION: The narrow portion of the terminal choledochus, a pathognomonic radiologic feature of AUPBD, may be a cause of biliary dilatation in patients with AUPBD.Tatsuya Nomura Yoshio Shirai Toshifumi Wakai Naoyuki Yokoyama Jun Sakata Katsuyoshi Hatakeyama 2005World Journal of Gastroenterology2005,11,41:5
10Insulin-like growth factor-1, IGF binding protein-3, and the risk of esophageal cancer in a nested case-control study显示文摘AIM To assess the relationship between serum levels of insulin-like growth factor-1(IGF1)/IGF-binding protein-3(IGFBP3)and the risk of esophageal carcinoma.METHODS We assessed the relationship between the serum levels of these molecules and the risk of esophageal cancer in a prospective,nested case-control study of participants from the Japan Collaborative Cohort Study.A baseline survey was conducted from 1988 to 1990.Of the 110585 enrolled participants,35%donated blood samples.Those who had been diagnosed with esophageal cancer were considered cases for nested case-control studies.A conditional logistic model was used to estimate odds ratios for the incidence of esophageal cancer associated with serum IGF1 and IGFBP3 levels.RESULTS Thirty-one cases and 86 controls were eligible for the present assessment.The molar ratio of IGF1/IGFBP3,which represents the free and active form of IGF1,was not correlated with the risk of esophageal carcinoma.A higher molar difference between IGFBP3and IGF1,which estimates the free form of IGFBP3,was associated with a decreased risk of esophageal carcinoma(P=0.0146),and people in the highest tertile had the lowest risk(OR=0.107,95%CI:0.017-0.669).After adjustment for body mass index,tobacco use,and alcohol intake,the molar difference of IGFBP3-IGF1 was inversely correlated with the risk of esophageal carcinoma(P=0.0150).CONCLUSION The free form of IGFBP3,which is estimated by this molar difference,may be inversely associated with esophageal cancer incidence.Yasushi Adachi Masanori Nojima Mitsuru Mori Kentaro Yamashita Hiro-o Yamano Hiroshi Nakase Takao Endo Kenji Wakai Kiyomi Sakata Akiko Tamakoshi 2017World Journal of Gastroenterology2017,23,19:4
11包含的小岛移植: 过去,现在和未来显示文摘 Islet transplantation could become an ideal treatment for severe diabetes to prevent hypoglycemia shock and irreversible diabetic complications, once some of the major and unresolved obstacles are overcome, including limited donor supplies and side effects caused by permanent immunosuppressant use. Approximately 30 years ago, some groups succeeded in improving the blood glucose of diabetic animals by transplanting encapsulated islets with semi-permeable membranes consisting of polymer. A semi-permeable membrane protects both the inner islets from mechanical stress and the recipient’s immune system (both cellular and humoral immunities), while allowing bidirectional diffusion of nutrients, oxygen, glucose, hormones and wastes, i.e., immune-isolation. This device, which enables immune-isolation, is called encapsulated islets or bio-artificial pancreas. Encapsulation with a semipermeable membrane can provide some advantages: (1) this device protects transplanted cells from the recipient’s immunity even if the xenogeneic islets (from large animals such as pig) or insulin-producing cells are derived from cells that have the potential for differentiation (some kinds of stem cells). In other words, the encapsulation technique can resolve the problem of limited donor supplies; and (2) encapsulation can reduce or prevent chronic administration of immunosuppressants and, therefore, important side effects otherwise induced by immunosuppressants. 12Secular change in lifetime of granitic crust and the continental growth: A new view from detrital zircon ages of sandstones显示文摘U-Pb ages of detrital zircons were newly dated for 4 Archean sandstones from the Pilbara craton in Australia, Wyoming craton in North America, and Kaapvaal craton in Africa. By using the present results with previously published data, we compiled the age spectra of detrital zircons for 2.9, 2.6, 2.3,1.0, and0.6 Ga sandstones and modern river sands in order to document the secular change in age structure of continental crusts through time. The results demonstrated the following episodes in the history of continental crust:(1) low growth rate of the continents due to the short cycle in production/destruction of granitic crust during the Neoarchean to Paleoproterozoic(2.9-23 Ga),(2) net increase in volume of the continents during Paleo-to Mesoproterozoic(2.3-1.0 Ga), and(3) net decrease in volume of the continents during the Neoproterozoic and Phanerozoic(after 1.0 Ga). In the Archean and Paleoproterozoic, the embryonic continents were smaller than the modern continents, probably owing to the relatively rapid production and destruction of continental crust. This is indeed reflected in the heterogeneous crustal age structure of modern continents that usually have relatively small amount of Archean crusts with respect to the post-Archean ones. During the Mesoproterozoic, plural continents amalgamated into larger ones comparable to modern continental blocks in size. Relatively older crusts were preserved in continental interiors, whereas younger crusts were accreted along continental peripheries.In addition to continental arc magmatism, the direct accretion of intra-oceanic island arc around continental peripheries also became important for net continental growth. Since 1.0 Ga, total volume of continents has decreased, and this appears consistent with on-going phenomena along modern active arc-trench system with dominant tectonic erosion and/or arc subduction. 13Prophylactic lateral pelvic lymph node dissection in stage Ⅳ low rectal cancer显示文摘AIM To assess the clinical significance of prophylactic lateral pelvic lymph node dissection (LPLND) in stage Ⅳ low rectal cancer.METHODS We selected 71 consecutive stage Ⅳ low rectal cancer patients who underwent primary tumor resection,and enrolled 50 of these 71 patients without clinical LPLN metastasis.The patients had distant metastasis such as liver,lung,peritoneum,and paraaortic LN.Clinical LPLN metastasis was defined as LN with a maximum diameter of 10 mm or more on preoperative pelvic computed tomography scan.All patients underwent primary tumor resection,27 patients underwent total mesorectal excision(TME) with LPLND(LPLND group),and 23 patients underwent only TME(TME group).Bilateral LPLND was performed simultaneously with primary tumor resection in LPLND group.R0 resection of both primary and metastatic sites was achieved in 20 of 50 patients.We evaluated possible prognostic factors for 5-year overall survival (OS),and compared 5-year cumulative local recurrence between the LPLND and TME groups.RESULTS For OS,univariate analyses revealed no significant benefit in the LPLND compared with the TME group (28.7% vs 17.0%,P = 0.523); multivariate analysis revealed that R0 resection was an independent prognostic factor.Regarding cumulative local recurrence,the LPLND group showed no significant benefit compared with TME group (21.4% vs 14.8%,P = 0.833).CONCLUSION Prophylactic LPLND shows no oncological benefits in patients with Stage Ⅳ low rectal cancer without clinical LPLN metastasis.Hiroshi Tamura Yoshifumi Shimada Hitoshi Kameyama Ryoma Yagi Yosuke Tajima Takuma Okamura Mae Nakano Masato Nakano Masayuki Nagahashi Jun Sakata Takashi Kobayashi Shin-ichi Kosugi Hitoshi Nogami Satoshi Maruyama Yasumasa Takii Toshifumi Wakai 2017World Journal of Clinical Oncology2017,8,5:4
14Identification of sources of lead in the atmosphere by chemical speciation using X-ray absorption near-edge structure(XANES) spectroscopy显示文摘Sources of Pb pollution in the local atmosphere together with Pb species, major ions, and heavy metal concentrations in a size-fractionated aerosol sample from Higashi-Hiroshima(Japan) have been determined by X-ray absorption near-edge structure(XANES) spectroscopy, ion chromatography, and ICP-MS/AES, respectively. About 80% of total Pb was concentrated in fine aerosol particles. Lead species in the coarse aerosol particles were PbC2O4, 2PbCO3·Pb(OH)2, and Pb(NO3)2, whereas Pb species in the fine aerosol particles were PbC2O4, PbSO4, and Pb(NO3)2. Chemical speciation and abundance data suggested that the source of Pb in the fine aerosol particles was different from that of the coarse ones. The dominant sources of Pb in the fine aerosol particles were judged to be fly ash from a municipal solid waste incinerator and heavy oil combustion. For the coarse aerosol particles, road dust was considered to be the main Pb source. In addition to Pb species, elemental concentrations in the aerosols were also determined. 16Stochastic analysis of laminated composite plate considering stochastic homogenization problem显示文摘This paper discusses a multiscale stochastic analysis of a laminated composite plate consisting of unidirectional fiber reinforced composite laminae. In particular, influence of a microscopic random variation of the elastic properties of component materials on mechanical properties of the laminated plate is investigated. Laminated composites are widely used in civil engineering, and therefore multiscale stochastic analysis of laminated composites should be performed for reliability evaluation of a composite civil structure. This study deals with the stochastic response of a laminated composite plate against the microscopic random variation in addition to a random variation of fiber orientation in each lamina, and stochastic properties of the mechanical responses of the laminated plate is investigated. Halpin-Tsai formula and the homogenization theory-based finite element analysis are employed for estimation of effective elastic properties of lamina, and the classical laminate theory is employed for analysis of a laminated plate. The Monte-Carlo simulation and the first-order second moment method with sensitivity analysis are employed for the stochastic analysis. From the numerical results, importance of the multiscale stochastic analysis for reliability evaluation of a laminated composite structure and applicability of the sensitivity-based approach are discussed.S. 19Perimuscular connective tissue contains more and larger lymphatic vessels than the shallower layers in human gallbladders显示文摘AIM: To clarify whether perimuscular connective tissuecontains more lymphatic vessels than the shallowerlayers in human gallbladders.METHODS: Lymphatic vessels were stainedimmunohistochemically with monoclonal antibody D2-40,which is a specific marker of lymphatic endothelium, inrepresentative sections of 12 normal human gallbladdersobtained at the time of resection for colorectal carcinomaliver metastases. In individual gallbladder specimens,nine high-power (× 200) fields with the highestlymphatic vessel density (LVD), termed 'hot spots', wereidentified for each layer (mucosa, muscle layer, andperimuscular connective tissue). In individual hot spots,the LVD and relative lymphatic vessel area (LVA) weremeasured microscopically using a computer-aided imageanalysis system. The mean LVD and LVA values for thenine hot spots in each layer were used for statisticalanalyses.RESULTS: In the mucosa, muscle layer, andperimuscular connective tissue, the LVD was 16.1 ± 9.2,35.4 ± 15.7, and 65.5 ± 12.2, respectively, and the LVAwas 0.4 ± 0.4, 2.1 ± 1.1, and 9.4 ± 2.6, respectively.Thus, both the LVD and LVA differed significantly (P <0.001 and P < 0.001, respectively; Kruskal-Wallis test)among the individual layers of the wall of the gallbladder,with the highest LVD and LVA values in the perimuscularconnective tissue. Most (98 of 108) of the hot spotswithin the perimuscular connective tissue were locatedwithin 500 μm of the lower border of the muscle layer.CONCLUSION: The perimuscular connective tissuecontains more and larger lymphatic vessels than the shallower layers in the human gallbladder. 20Coexisting State of Surge and Rotating Stall in a Two-Stage Axial Flow Com-pressor using a Double-Phase-Locked Averaging Technique显示文摘The interaction between surge and rotating stall in an axial flow compressor was investigated from the viewpoint of an unsteady inner flow structure. The aim of this study was to identify the key factor that determines the switching phenomenon of a surge cycle. The main feature of the tested compressor is a shock tube connected in series to the compressor outlet through a diaphragm, slits, and a concentric duplex pipe: this system allows surge and rotating stall to be generated by connecting the shock tube with the compressor, or enables the compression plane wave injection. The unsteady characteristics and the internal flow velocity fluctuations were measured in detail, and the stall cell structure was averaged and visualized along the movement of the operation point under a coexisting state of surge. A coefficient of the cell scale fluctuation was calculated using the result of the averaging, and it confirmed that the processes of inner flow structure change differed from each other according to the next cycle of the surge. The result suggests that the key factor that determines the next cycle is the transformation of the internal flow structure, particularly between the stall cell and the entire circumferential stall, in both the recovering and stalling processes.Yuu SAKATA Yutaka OHTA 2017Journal of Thermal Science2017,26,1:3
