1Imaging evaluation of traumatic thoracolumbar spine injuries: Radiological review显示文摘Spine fractures account for a large portion of musculoskeletal injuries worldwide. A classification of spine fractures is necessary in order to develop a common language for treatment indications and outcomes. Several classification systems have been developed based on injury anatomy or mechanisms of action, but they have demonstrated poor reliability, have yielded little prognostic information, and have not been widely used. For this reason, the Arbeitsgemeinschaftfür Osteosynthesefragen(AO) committee has classified thorocolumbar spine injuries based on the pathomorphological criteria into3 types(A: Compression; B: Distraction; C: Axial torque and rotational deformity). Each of these types is further divided into 3 groups and 3 subgroups reflecting progressive scale of morphological damage and the degree of instability. Because of its highly detailed sub classifications, the AO system has shown limited interobserver variability. It is similar to its predecessors in that it does not incorporate the patient's neurologic status.The need for a reliable, reproducible, clinically relevant, prognostic classification system with an optimal balance of ease of use and detail of injury description contributed to the development of a new classification system, the thoracolumbar injury classification and severity score(TLICS). The TLICS defines injury based on three clinical characteristics: injury morphology, integrity of the posterior ligamentous complex, and neurologic status of the patient. 2Ebola virus disease: past, present and future显示文摘Ebola virus disease is one of the most deadly ailments known to mankind due to its high mortality rate(up to 90%) accompanying with the disease. Ebola haemorrhagic fever(EHF) is an infectious disease of animal that can be transmitted to both human and non-human primates. The first epidemic of EHF occurred in 1976 in the Democratic Republic of the Congo. The incubation period of ebola is less than 21 days. Ebola virus infections are depicted by immune suppression and a systemic inflammatory response that leads to damage of the vascular, coagulation and immune systems, causing multi-organ failure and shock. Five genetically distinct members of the Filoviridae family responsible for EHF are as follows: Zaire ebolavirus, Sudan ebolavirus, C?te d'Ivoire ebolavirus, Bundibugyo ebolavirus and Reston ebolavirus. The ongoing 2014 West Africa ebola epidemic has been considered as the most serious panic in the medical field with respect to both the number of human cases and death toll. The natural host for ebola virus is unknown, thus it is not possible to carry out programs to regulate or abolish virus from transmission to people. The ebola virus infection provides little chance to develop acquired immunity causing rapid progression of the disease. It is pertinent to mention that at present, there is no antiviral therapy or vaccine that is helpful against ebola virus infection in humans. 32,3-Diaryl-3H-imidazo[4,5-b]pyridine derivatives as potential anticancer and anti-inflammatory agents显示文摘In this study we examined the suitability of the 3H-imidazo[4,5-b]pyridine ring system in developing novel anticancer and anti-inflammatory agents incorporating a diaryl pharmacophore. Eight 2,3-diaryl-3H-imidazo[4,5-b]pyridine derivatives retrieved from our in-house database were evaluated for their cytotoxic activity against nine cancer cell lines. The results indicated that the compounds showed moderate cytotoxic activity against MCF-7, MDA-MB-468, K562 and SaOS2 cells, with K562 being the most sensitive among the four cancer cell lines. The eight 2,3-diaryl-3H-imidazo[4,5-b]pyridine derivatives were also evaluated for their COX-1 and COX-2 inhibitory activity in vitro. The results showed that compound 3f exhibited 2-fold selectivity with IC_(50) values of 9.2 and 21.8 mmol/L against COX-2 and COX-1, respectively. 4Prevalence and clinical characteristics associated with left atrial thrombus detection: Apixaban显示文摘BACKGROUND The prevalence of left atrial appendage(LAA) thrombus detection by transesophageal echocardiogram(TEE) in patients with non-valvular atrial fibrillation(AF) anticoagulated with apixaban is not well defined and identification of additional risk factors may help guide the selection process for pre-procedural TEE. The purpose of our study was to retrospectively analyze the prevalence of LAA thrombus detection by TEE in patients continuously anticoagulated with apixaban for ≥ 4 wk and evaluate for any cardiac risk factors or echocardiographic characteristics which may serve as predictors of thrombus formation.AIM To retrospectively analyze the prevalence of LAA thrombus detection by TEE in patients continuously anticoagulated with apixaban.METHODS Clinical and echocardiographic data for 820 consecutive patients with AF undergoing TEE at Augusta University Medical Center over a four-year period were retrospectively analyzed. All patients(apixaban: 226) with non-valvular AF and documented compliance with apixaban for ≥ 4 wk prior to index TEE were included.RESULTS Following ≥ 4 wk of continuous anticoagulation with apixaban, the prevalence ofLAA thrombus and LAA thrombus/dense spontaneous echocardiographic contrast was 3.1% and 6.6%, respectively. Persistent AF, left ventricular ejection fraction < 30%, severe LA dilation, and reduced LAA velocity were associated with thrombus formation. Following multivariate logistic regression, persistent AF(OR: 7.427; 95%CI: 1.02 to 53.92; P = 0.0474), and reduced LAA velocity(OR:1.086; 95%CI: 1.010 to 1.187; P = 0.0489) were identified as independent predictors of LAA thrombus. No Thrombi were detected in patients with a CHA2 DS2-VASc score ≤ 1.CONCLUSION Among patients with non-valvular AF and ≥ 4 wk of anticoagulation with apixaban, the prevalence of LAA thrombus detected by TEE was 3.1%. 5Predicting suitability of different scale-dependent dispersivities for reactive solute transport through stratified porous media显示文摘In this paper,the behavior of breakthrough curves(BTCs) for reactive solute transport through stratified porous media is investigated.A physical laboratory model for layered porous media was constructed,in which thin layer of gravel was sandwiched in between two thick layers of natural soil.Gravel layer and natural soil layers were hydraulically connected as single porous continuum.A constant source of tracer was connected through gravel layer and elucidated at different sampling points in the direction of flow.Flexible multiprocess non-equilibrium(MPNE) transport equation with scale-dependent dispersivity function was used to simulate experimental BTCs of reactive solute transport through layered porous media.The values of equilibrium sorption coefficient and other input parameters were obtained experimentally.The simulation of BTC was performed using MPNE model with scale-dependent dispersivity.The simulation of different scale-dependent dispersivities was then compared and it was found that for field scale of estimation of dispersivity,asymptotic and exponential dispersivity functions performed better.In continuation to the comparison of simulated BTCs obtained using different models,spatial moment analysis of each aforesaid scale-dependent dispersivity model was also done.Spatial moment analysis provides the information related to mean solute mass,rate of mass travel,and mean plume dispersion.Linear and constant dispersivities showed higher variance as compared to asymptotic and exponential dispersion functions.This supports the field applicability of asymptotic and exponential dispersivity functions.The BTCs were also found to elucidate a nonzero concentration with time,which was clearly affected by physical non-equilibrium.In natural condition,such information is required in effective aquifer remediation process.Deepak Swami Abhimanyu Sharma Pramod Kumar Sharma Dericks P.Shukla 2016Journal of Rock Mechanics and Geotechnical Engineering2016,8,6:2
6Endoscopic ultrasound guided interventional procedures显示文摘Endoscopic ultrasound(EUS) has emerged as an important diagnostic and therapeutic modality in the field of gastrointestinal endoscopy. EUS provides access to many organs and lesions which are in proximity to the gastrointestinal tract and thus giving an opportunity to target them for therapeutic and diagnostic purposes. This modality also provides a real time opportunityto target the required area while avoiding adjacent vascular and other structures. Therapeutic EUS has found role in management of pancreatic fluid collections, biliary and pancreatic duct drainage in cases of failed endoscopic retrograde cholangiopancreatography, drainage of gallbladder, celiac plexus neurolysis/blockage, drainage of mediastinal and intra-abdominal abscesses and collections and in targeted cancer chemotherapy and radiotherapy. Infact, therapeutic EUS has emerged as the therapy of choice for management of pancreatic pseudocysts and recent innovations like fully covered removable metallic stents have improved results in patients with organised necrosis. Similarly, EUS guided drainage of biliary tract and pancreatic duct helps drainage of these systems in patients with failed cannulation, inaccessible papilla as with duodenal/gastric obstruction or surgically altered anatomy. EUS guided gall bladder drainage is a useful emergent procedure in patients with acute cholecystitis who are not fit for surgery. EUS guided celiac plexus neurolysis and blockage is more effective and less morbid vis-à-vis the percutaneous technique. The field of interventional EUS is rapidly advancing and many more interventions are being continuously added. 7GC/MS-based differential metabolic profiling of human peptic ulcer disease to study Helicobacter pylori-induced metabolic perturbations显示文摘Helicobacter pylori infection has been significantly linked to Peptic Ulcer Disease and Gastric Cancer.Metabolomic fingerprinting may offer a principal way of early diagnosis and to understand the molecular mechanism of H.pylori-induced pathogenicity.The rationale of the study is to explore the underlying distinct metabolic mechanisms of H.pylori-induced PUD and to identify potential biomarkers for disease diagnosis and associated risks using Gas chromatography/mass spectrometry.GC/MS-based analytical method was used to compare metabolic profiles of healthy controls(N=20)and peptic ulcer patients(N=45).Acquired metabolomic data were analyzed by constructing a diagnostic model using principal component analysis and a non-parametric two-tailed paired Wilcoxon analysis to identify disease-specific metabolic biomarkers.A total of 75 low-molecular-weight endogenous metabolites were detected during comparative metabolomic analysis of PUD vs.healthy gut tissues,among which 16 metabolites are being proposed to be diagnostic markers of Human PUD.Perturbations related to amino acids,carbohydrates,fatty acids,organic acids,and sterol metabolism were significantly revealed during this differential metabolomic profiling.Results convincingly suggest that metabolic profiles can contribute immensely in early diagnosis of the disease and understanding molecular mechanisms of disease progression for predicting novel drug targets for prophylactic and anaphylactic measures.GAGANJOT GUPTA DEEPAK BANSAL ANSHULA SHARMA TAWSEEF AHMAD ATUL SACHDEV AJAZ AHMAD HAMED A.EL-SEREHY BALJINDER KAUR 2021BIOCELL2021,45,5:2
8Arterial pH,bicarbonate levels and base deficit at presentation as markers of predicting mortality in acute pancreatitis:a single-centre prospective study显示文摘Background.Arterial blood gas(ABG)parameters such as pH form part of multi-parameter scoring systems for predicting severe acute pancreatitis;however,literature on detailed evaluation of ABG alone in this context is scarce.Methods.Patients with acute pancreatitis presenting to our unit between January 2012 and November 2013 were prospectively studied.ABG analysis was done at admission and development of organ failure,any need for intervention,and mortality were noted.The association between various parameters of ABG analysis and the development of organ failure or local complications,need for interventions(endoscopic/radiological/surgical)and mortality were analysed.Results.Two hundred and five patients(mean age:39.33-13.85 years;61.0%males)were prospectively studied.The aetiology of acute pancreatitis was alcohol in 93 patients(45.4%)and gall stone disease in 73(35.6%).Organ failure developed in 71.2%patients and 83.9%had local complications.In 18%of patients,endoscopic/radiological/surgical interventions were needed and 14.6%died.The patients(n=35)with metabolic acidosis(pH<7.35)suffered higher frequency of organ failure,need for interventions and mortality.Patients with low arterial bicarbonate levels,as well as higher base deficit,also displayed higher frequency of organ failure,need for interventions and mortality.The receiver operating characteristic(ROC)curves for pH<7.35,bicarbonate<22 meq/L and base deficit of>4 meq/L for prediction of mortality were 0.771(95%CI:0.664–0.878),0.707(95%CI:0.622–0.791)and 0.780(95%CI:0.693–0.867),respectively.Conclusion.Arterial pH,bicarbonate levels,and base deficit at presentation are useful early markers for predicting adverse outcome in acute pancreatitis.Vishal Sharma Thingbaijam Shanti Devi Ravi Sharma Puneet Chhabra Rajesh Gupta Surinder S.Rana Deepak K.Bhasin 2014Gastroenterology Report2014,2,3:1
9Influence of tool rotation speeds on mechanical and morphological properties of friction stir processed nano hybrid composite of MWCNT-Graphene-AZ31 magnesium显示文摘The ever-increasing demand for light weighted hard materials for transportation industries encouraged researchers to develop composites with excellent mechanical properties which can transform it into more economical and eco-friendly.Reinforcing the metals with carbonaceous nanomaterials are progressively in focus due to their excellent capability to inculcate and tailor the properties of MMCs.In the present research,a hybrid nanocomposite of MWCNT-Graphene-AZ31 Mg alloy has been developed by using variable tool rotation speeds with friction stir processing(FSP).Optimized reinforcement ratio of 1.6%vol.MWCNT and 0.3%vol.of graphene have been used with variable tool rotation speeds,whereas other processing parameters are kept constant.The developed specimens were investigated using standard testing equipment for evaluating and comparing the mechanical properties on the basis of the microstructure of the processing regions and their morphological analysis,according to the ASTM standards.The obtained results revealed an improvement of 19.72%in microhardness and 77.5% of compressive strength in comparison with the base metal AZ 31 Magnesium alloy,with a tool rotational speed of 1400rpm.The values of tensile stress and percentage area reduction were recorded as less than that of the base metal matrix,but an increasing trend has been observed in the values of both with the improvement on rotational speeds of the tool.The effectual strengthening mechanisms are analyzed on the bases of SEM images and observed that discussed and found that grain refinement strengthening is the major contributor to the strength of the nanocomposite.Sanjay Sharma Amit Handa Sahib Sartaj Singh Deepak Verma 2019Journal of Magnesium and Alloys2019,7,3:1
10Small bowel bleeding:a comprehensive review显示文摘The small intestine is an uncommon site of gastro-intestinal(GI)bleeding;however it is the commonest cause of obscure GI bleeding.It may require multiple blood transfusions,diagnostic procedures and repeated hospitalizations.Angiodysplasia is the commonest cause of obscure GI bleeding,particularly in the elderly.Inflammatory lesions and tumours are the usual causes of small intestinal bleeding in younger patients.Capsule endoscopy and deep enteroscopy have improved our ability to investigate small bowel bleeds.Deep enteroscopy has also an added advantage of therapeutic potential.Computed tomography is helpful in identifying extra-intestinal lesions.In cases of difficult diagnosis,surgery and intra-operative enteroscopy can help with diagnosis and management.The treatment is dependent upon the aetiology of the bleed.An overt bleed requires aggressive resuscitation and immediate localisation of the lesion for institution of appropriate therapy.Small bowel bleeding can be managed by conservative,radiological,pharmacological,endoscopic and surgical methods,depending upon indications,expertise and availability.Some patients,especially those with multiple vascular lesions,can re-bleed even after appropriate treatment and pose difficult challenge to the treating physician.Deepak Gunjan Vishal Sharma Surinder S Rana Deepak K Bhasin 2014Gastroenterology Report2014,2,4:1

18Blood cellular mutant LXR-α protein stability governs initiation of coronary heart disease显示文摘AIM: To investigate the role of [breast and ovarian cancer susceptibility 1(BRCA1)-associated RING domain 1(BARD1)]/BRCA1 E3-ubiquitin ligase complex in governing the stability of mutant liver X receptor-(LXR-α) protein in coronary heart disease(CHD) subjects.METHODS: The expression analysis of various genes was carried out by quantitative real time polymerase chain reaction and western blotting within blood mononuclear cells of human CHD subjects at various stages of coronary occlusion and their corresponding normal healthy counterparts.Immunoprecipitation experiments were performed to establish protein interactions between LXR-αand BARD1.Peripheral blood mononuclear cells were cultured and exposed to Vitamin D_3 and Cisplatin to validate the degradation of mutant LXR-αprotein in CHD subjects by BARD1/BRCA1 complex.RESULTS: The expression of mutant LXR-αprotein in CHD subjects was found to decrease gradually with the severity of coronary occlusion exhibiting a strong negative correlation,r =-0.975 at P < 0.001.Further,the expression of BARD1 and BRCA1 also increased with the disease severity,r = 0.895 and 0.873 respectively(P < 0.001).Immunoprecipitation studies established that BARD1/BRCA1 complex degrades mutant LXR-αvia ubiquitination.The absence of functional LXR-αprotein resulted in increased expression of inflammatory cytokines such as interleukin(IL)-6,IL-8 and interferon-and decreased expression of ABCA1(ATP-binding cassette A1)(r = 0.932,0.949,0.918 and-0.902 with respect to Gensini score;P < 0.001).Additionally,cell culture experiments proved that Vitamin D_3 could prevent the degradation of mutant LXR-αand restore its functional activity to some extent.CONCLUSION: Mutant LXR-αprotein in CHD subjects is degraded by BARD1/BRCA1 complex and Vitamin D_3 can rescue and restore its function.Mansi Arora Deepak Kaul Yash Paul Sharma 2013World Journal of Cardiology2013,5,8:1
