| 1 | Genetics of non-alcoholic fatty liver disease: From susceptibility and nutrient interactions to management显示文摘Genetics plays an important role in determining the susceptibility of an individual to develop a disease. Complex, multi factorial diseases of modern day(diabetes, cardiovascular disease, hypertension and obesity) are a result of disparity between the type of food consumed and genes, suggesting that food which does not match the host genes is probably one of the major reasons for developing life style diseases. Non-alcoholic fatty liver is becoming a global epidemic leading to substantial morbidity. While various genotyping approaches such as whole exome sequencing using next generation sequencers and genome wide association studies have identified susceptibility loci for non-alcoholic fatty liver disease(NAFLD) including variants in patatin-like phospholipase domain containing 3 and transmembrane 6 superfamily member 2 genes apart from others; nutrient based studies emphasized on a combination of vitamin D, E and omega-3 fatty acids to manage fatty liver disease. However majority of the studies were conducted independent of each other and very few studies explored the interactions between the genetic susceptibility and nutrient interactions. Identifying such interactions will aid in optimizing the nutrition tailor made to an individual's genetic makeup, thereby aiding in delaying the onset of the disease and its progression. | 3 | Pancreatic transplantation: Brief review of the current evidence显示文摘Kidney transplantation is the treatment of choice for management of end-stage renal disease.However,in diabetic patients,the underlying metabolic disturbance will persist and even may get worse after isolated kidney transplantation.Pancreatic transplantation in humans was first introduced in 1966.The initial outcome was disappointing.However,this was changed after the improvement of surgical techniques together with better patient selection and the availability of potent and better-tolerated immune-suppression like cyclosporine and induction antibodies.Combined kidney and pancreas transplantation will not only solve the problem of organ failure,but it will also stabilise or even reverse the metabolic complications of diabetes.Combined kidney and pancreas transplantation have the best long term outcome in diabetic cases with renal failure.Nevertheless,at the cost of an initial increase in morbidity and risk of mortality.Other transplantation options include pancreas after kidney transplantation and islet cell transplantation.We aim by this work to explore various options which can be offered to a diabetic patient with advanced chronic kidney disease.Our work will provide a simplified,yet up-to-date information regarding the different management options for those diabetic chronic kidney failure patients. | Ahmed Aref Tariq Zayan Ravi Pararajasingam Ajay Sharma Ahmed Halawa | 2019 | World Journal of Transplantation2019，9，4： | 2 |

| 5 | Arterial pH,bicarbonate levels and base deficit at presentation as markers of predicting mortality in acute pancreatitis:a single-centre prospective study显示文摘Background.Arterial blood gas(ABG)parameters such as pH form part of multi-parameter scoring systems for predicting severe acute pancreatitis;however,literature on detailed evaluation of ABG alone in this context is scarce.Methods.Patients with acute pancreatitis presenting to our unit between January 2012 and November 2013 were prospectively studied.ABG analysis was done at admission and development of organ failure,any need for intervention,and mortality were noted.The association between various parameters of ABG analysis and the development of organ failure or local complications,need for interventions(endoscopic/radiological/surgical)and mortality were analysed.Results.Two hundred and five patients(mean age:39.33-13.85 years;61.0%males)were prospectively studied.The aetiology of acute pancreatitis was alcohol in 93 patients(45.4%)and gall stone disease in 73(35.6%).Organ failure developed in 71.2%patients and 83.9%had local complications.In 18%of patients,endoscopic/radiological/surgical interventions were needed and 14.6%died.The patients(n=35)with metabolic acidosis(pH<7.35)suffered higher frequency of organ failure,need for interventions and mortality.Patients with low arterial bicarbonate levels,as well as higher base deficit,also displayed higher frequency of organ failure,need for interventions and mortality.The receiver operating characteristic(ROC)curves for pH<7.35,bicarbonate<22 meq/L and base deficit of>4 meq/L for prediction of mortality were 0.771(95%CI:0.664–0.878),0.707(95%CI:0.622–0.791)and 0.780(95%CI:0.693–0.867),respectively.Conclusion.Arterial pH,bicarbonate levels,and base deficit at presentation are useful early markers for predicting adverse outcome in acute pancreatitis. | Vishal Sharma Thingbaijam Shanti Devi Ravi Sharma Puneet Chhabra Rajesh Gupta Surinder S.Rana Deepak K.Bhasin | 2014 | Gastroenterology Report2014，2，3： | 1 |

| 11 | Ethnic differences in inflammatory bowel disease: Results from the United Kingdom inception cohort epidemiology study显示文摘BACKGROUND The current epidemiology of inflammatory bowel disease(IBD)in the multiethnic United Kingdom is unknown.The last incidence study in the United Kingdom was carried out over 20 years ago.AIM To describe the incidence and phenotype of IBD and distribution within ethnic groups.METHODS Adult patients(>16 years)with newly diagnosed IBD(fulfilling Copenhagen diagnostic criteria)were prospectively recruited over one year in 5 urban catchment areas with high South Asian population.Patient demographics,ethnic codes,disease phenotype(Montreal classification),disease activity and treatment within 3 months of diagnosis were recorded onto the Epicom database.RESULTS Across a population of 2271406 adults,339 adult patients were diagnosed with IBD over one year:218 with ulcerative colitis(UC,64.3%),115 with Crohn's disease(CD,33.9%)and 6 with IBD unclassified(1.8%).The crude incidence of IBD,UC and CD was 17.0/100000,11.3/100000 and 5.3/100000 respectively.The age adjusted incidence of IBD and UC were significantly higher in the Indian group(25.2/100000 and 20.5/100000)compared to White European(14.9/100000,P=0.009 and 8.2/100000,P<0.001)and Pakistani groups(14.9/100000,P=0.001 and 11.2/100000,P=0.007).The Indian group were significantly more likely to have extensive disease than White Europeans(52.7%vs 41.7%,P=0.031).There was no significant difference in time to diagnosis,disease activity and treatment.CONCLUSION This is the only prospective study to report the incidence of IBD in an ethnically diverse United Kingdom population.The Indian ethnic group showed the highest age-adjusted incidence of UC(20.5/100000).Further studies on dietary,microbial and metabolic factors that might explain these findings in UC are underway. | Ravi Misra Jimmy Limdi Rachel Cooney Samia Sakuma Matthew Brookes Edward Fogden Sanjeev Pattni Naveen Sharma Tariq Iqbal Pia Munkholm Johan Burisch Naila Arebi | 2019 | World Journal of Gastroenterology2019，25，40： | 1 |
