| 1 | 应用中国商环包皮环切手术标准化方案对328例成年男性包皮环切的临床报告显示文摘目的:成年男性包皮环切手术标准化和培训可显著降低因手术引起的并发症发生率。由于目前国内外尚无可供参考的成年男性包皮环切手术标准化方案和培训制度,使得临床医生开展成年男性包皮环切手术的质量无法保证。为了建立成年男性包皮环切手术标准化和培训制度,便于临床医生和相关医护人员学习和掌握包皮环切器的临床应用技巧,以中国商环(简称'商环,ShangRing')包皮环切技术为例,根据商环的技术特点和临床实践经验,特别制订了商环包皮环切手术标准化方案,并将其标准化操作运用于临床。方法:采用商环包皮环切手术标准化方案对328例受术者(年龄18~58岁,平均27.8岁;已婚134例,未婚194例;包茎25例,包皮过长303例)进行包皮环切手术。对手术时间、疼痛评分(视觉模拟评分法,VAS)、术后并发症、包皮创口愈合时间、术后外观满意情况进行观察和随访。结果:手术时间为(4.70±1.31)min。手术时受术者疼痛(VAS)评分为(0.24±0.59)分,术后24h疼痛评分为(1.59±0.95)分,去环前24h内疼痛评分(1.72±1.11)分,去环时疼痛评分为(2.72±1.37)分。主要并发症发生率为术后感染0.6%(2/328)、出血0.6%(2/328)、伤口裂开0.6%(2/328)和水肿4.89%(16/328)。在发生出血和伤口裂开的受术者中无1例需要缝合。术后包皮创口完全愈合时间为(20.30±6.69)d。受术者对术后外观满意率99.7%(327/328)。结论:商环包皮环切术具有手术方式易于标准化、手术时间短、术中疼痛轻、术后无须特殊护理、容易完整保留系带,并完全暴露冠状沟、术后外观满意度高、受术者易于接受等优点。初步证明了中国商环包皮环切手术标准化方案的可接受性,严格按照此方案实施手术和随访护理,能最大化地实现商环包皮环切技术的诸多临床优势,将并发症发生率控制在可接受的范围内。 | 程跃 彭弋峰 刘毅东 田龙 吕年青 苏新军 严泽军 胡嘉盛 Richard LEE Howard KIM David C. | 5 | Effect of cavity flow control on high-speed train pantograph and roof aerodynamic noise显示文摘The pantograph and its recess on the train roof are major aerodynamic noise sources on high-speed trains.Reducing this noise is particularly important because conventional noise barriers usually do not shield the pantograph.However,less attention has been paid to the pantograph recess compared with the pantograph.In this paper,the flow features and noise contribution of two types of noise reduction treatments rounded and chamfered edges are studied for a simplified high-speed train pantograph recess,which is represented as a rectangular cavity and numerically investigated at 1/10 scale.Improved delayed detached-eddy simulations are performed for the near-field turbulent flow simulation,and the Ffowcs Williams and Hawkings aeroacoustic analogy is used for far-field noise prediction.The highly unsteady flow over the cavity is significantly reduced by the cavity edge modifications,and consequently,the noise radiated from the cavity is reduced.Furthermore,effects of the rounded cavity edges on the flow and noise of the pantographs(one raised and one folded)are investigated by comparing the flow features and noise contributions from the cases with and without rounding of the cavity edges.Different train running directions are also considered.Flow analysis shows that the highly unsteady flow within the cavity is reduced by rounding the cavity edges and a slightly lower flow speed occurs around the upper parts of the raised pantograph,whereas the flow velocity in the cavity is slightly increased by the rounding.Higher pressure fluctuations occur on the folded pantograph and the lower parts of the raised pantograph,whereas weaker fluctuations are found on the panhead of the raised pantograph.This study shows that by rounding the cavity edges,a reduction in radiated noise at the side and the top receiver positions can be achieved.Noise reductions in the other directions can also be found. | Hogun Kim Zhiwei Hu David Thompson | 2020 | Railway Engineering Science2020,28,1: | 10 |

| 8 | Hepatobiliary scintigraphy for detecting biliary strictures after living donor liver transplantation显示文摘AIM:To investigate the diagnostic accuracy of hepatobiliary scintigraphy(HBS) in detecting biliary strictures in living donor liver transplantation(LDLT) patients.METHODS:We retrospectively reviewed 104 adult LDLT recipients of the right hepatic lobe with duct-toduct anastomosis,who underwent HBS and cholangiography.The HBS results were categorized as normal,parenchymal dysfunction,biliary obstruction,or bile leakage without re-interpretation.The presence of biliary strictures was determined by percutaneous cholangiography or endoscopic retrograde cholangiopancreatography(ERCP).RESULTS:In 89 patients with biliary strictures,HBS showed biliary obstruction in 50 and no obstruction in 39,for a sensitivity of 56.2%.Of 15 patients with no biliary strictures,HBS showed no obstruction in 11,for a specificity of 73.3%.The positive predictive value(PPV) was 92.6%(50/54) and the negative predictive value(NPV) was 22%(11/50).We also analyzed the diagnostic accuracy of the change in bile duct size.The sensitivity,NPV,specificity,and PPV were 65.2%,27.9%,80% and 95%,respectively.CONCLUSION:The absence of biliary obstruction on HBS is not reliable.Thus,when post-LDLT biliary strictures are suspected,early ERCP may be considered. | Yu Jin Kim Kyu Taek Lee Young Cheol Jo Kwang Hyuck Lee Jong Kyun Lee Jae-Won Joh Choon Hyuck David Kwon | 2011 | World Journal of Gastroenterology2011,17,21: | 6 |

| 13 | Macrophages are the primary effector cells in IL-7-induced arthritis显示文摘Synovial macrophages are crucial in the development of joint inflammation and bone damage;however, the pathways that controlmacrophage remodeling in inflammatory M1 cells or bone-eroding osteoclasts are not fully understood. We determined thatelevated IL-7R/CD127 expression is the hallmark of rheumatoid arthritis (RA) M1 macrophages and that these cells are highlyresponsive to interleukin-7 (IL-7)-driven osteoclastogenesis. We established that lipopolysaccharide (LPS), interferon-γ (IFNγ), andtumor necrosis factor-α (TNFα), the classic M1 macrophage mediators, enhance IL-7R expression in RA and murine macrophages.The local expression of IL-7 provokes arthritis, predominantly through escalating the number of F480^(+)iNOS^(+) cells rather than CD3^(+)T cells. Ectopic LPS injection stabilizes IL-7-induced arthritis by increasing myeloid IL-7R expression, in part via IFNγ induction.Hence, in RAG−/− mice, IL-7-mediated arthritis is suppressed because of the reduction in myeloid IL-7R expression due to the lackof IFNγ. Moreover, the amelioration of IL-7-induced arthritis by anti-TNF therapy is due to a decrease in the number of cells in theunique F480^(+)iNOS^(+)IL-7R^(+)CCL5^(+) subset, with no impact on the F480^(+)Arginase^(+) cell or CD3^(+) T cell frequency. Consistent with thepreclinical findings, the findings of a phase 4 study performed with RA patients following 6 months of anti-TNF therapy revealedthat IL-7R expression was reduced without affecting the levels of IL-7.
