3美国临床肿瘤学会IV期非小细胞肺癌化疗的临床实践指南更新显示文摘本文旨在为IV期非小细胞肺癌患者的治疗提供更新版推荐。本文资料检索源自2002年以来公布的相关随机试验文献。此指南范围限于化疗与生物治疗。更新委员会对这些文献进行了总结并提供了推荐更新。162篇文献符合标准被纳入参考。本推荐基于可改善总生存期的治疗方法。仅改善无进展生存期的治疗方法推动了对毒性及生存质量的监测。对于体力状态评分为0分或1分患者的一线治疗,可推荐以铂类为基础的细胞毒性药物的两药联用。对铂类治疗有禁忌的患者,可采用非铂类细胞毒性两药联合。对于体力状态评分为2分的患者,单一细胞毒性药物即可。对于疾病进展或经过4个周期的治疗仍对治疗无反应的患者,应停止一线细胞毒性化疗。即使在6个周期后患者对治疗仍有反应,亦应停止两药细胞毒性化疗。对于伴有明确的表皮生长因子受体(epidermal growth factor receptor,EGFR)突变的患者,可推荐一线采用吉非替尼治疗;对于EGFR突变为阴性或不明确的患者,细胞毒性化疗更佳。除具有特定临床特征的患者外,可推荐贝伐单抗与卡铂-紫杉醇联用。对于通过免疫组化证实EGFR阳性的肿瘤患者,可推荐西妥昔单抗与顺铂-长春瑞滨联用。多西紫杉醇、厄洛替尼、吉非替尼或培美曲塞被推荐作为二线治疗。对于未曾接受过厄洛替尼或吉非替尼治疗的患者,可推荐厄洛替尼作为三线治疗。现有数据不足以推荐常规三线采用细胞毒性药物。已有的证据也不足以推荐常规应用分子标记物选择化疗。Christopher G. 5Identification of meta-instable stress state based on experimental study of evolution of the temperature field during stick-slip instability on a 5° bending fault显示文摘Identification of the meta-instable stress state and study of its mechanism and evolution of relevant physical fields would be of great significance for determination of potential seismic risks and estimation of critical times. In laboratory experiments, that the specimen enters the meta-instable is marked by accelerated stress release. Could we use the experimental result to identify the earthquake in natural conditions? Because the observational data from one station can only reflect the stress state beneath this station, the key problem for identification of the meta-instability is how to recognize regional stress state through observational data from many stations. In this work, we choose the evolution of the temperature field over varied deformation stages during a stick-slip event on a 5 bending fault as an example, and attempt to find the response features of the physical quantity when the fault enters the meta-instable state. We discuss the characteristics of stages for the stress build-up, stress-time process deviating from linearity before instability, meta-instability, instability, and post-instability, respectively. The result shows that the fault instability slide is a conversion process from independent activities of each fault segment to synergism activity. The instability implies completion of the synergism. The stage deviating from linearity is the onset of stress release, and it is also the onset of the synergism. At the meta-instability stages, stress release becomes dominant, while the synergism tends to finish. 6Energy-Theft Detection Issues for Advanced Metering Infrastructure in Smart Grid显示文摘With the proliferation of smart grid research,the Advanced Metering Infrastructure(AMI) has become the first ubiquitous and fixed computing platform. However,due to the unique characteristics of AMI,such as complex network structure,resource-constrained smart meter,and privacy-sensitive data,it is an especially challenging issue to make AMI secure. Energy theft is one of the most important concerns related to the smart grid implementation. It is estimated that utility companies lose more than $25 billion every year due to energy theft around the world. To address this challenge,in this paper,we discuss the background of AMI and identify major security requirements that AMI should meet. Specifically,an attack tree based threat model is first presented to illustrate the energy-theft behaviors in AMI. Then,we summarize the current AMI energy-theft detection schemes into three categories,i.e.,classification-based,state estimation-based,and game theory-based ones,and make extensive comparisons and discussions on them. 7Spatial patterns of plant diversity and communities in Alpine ecosystems of the Hengduan Mountains, northwest Yunnan, China显示文摘Aims Conduct a quantitative,but rapid,regional-level assessment of the alpine flora across northwest Yunnan(NWY)to provide a broadbased understanding of local and regional patterns of the composition,diversity and health of alpine ecosystems across NWY.Methods A stratified random sampling design was employed to select sites across the different mountain ranges of NWY.Vegetation was sampled by stratifying each site by the three major alpine vegetation community types:meadow,dwarf shrub and scree.Two 50-m transects were randomly located within each community type at each sampling site with 101-m^(2) subplots systematically placed along each transect.Environmental variables were recorded at each transect.Multivariate analyses were used to classify the major plant community assemblages and link community patterns to environmental and habitat variables.Important Findings Forb species richness varied from 19 to 105 species per site(21 sites total)with an average of 59 species per site(60 m^(2) sampled per site).Most species were patchily distributed with narrow distributions and/or small population sizes;over half the species occurred at only one or two sites.Distinct species assemblages were identified in the meadow vegetation that was strongly aggregated by geographic location suggesting the presence of distinct phytogeographic zones of the meadow alpine flora.Elevation and geographic location were the dominant environmental gradients underlying the variations in species composition.Jaccard’s coefficient of similarity averaged only 10%among sites indicating there was little similarity in the alpine flora across the region.The alpine vegetation is highly heterogeneous across the complex landscape of the Hengduan Mountains of NWY.Conservation strategies need to take into account the large geographic differences in the flora to maximize protection of biodiversity.Ruth Sherman Renee Mullen Li Haomin Fang Zhendong Wang Yi 2008Journal of Plant Ecology2008,1,2:20
8Internet of Vehicles in Big Data Era显示文摘As the rapid development of automotive telematics,modern vehicles are expected to be connected through heterogeneous radio access technologies and are able to exchange massive information with their surrounding environment. By significantly expanding the network scale and conducting both real-time and long-term information processing, the traditional Vehicular AdHoc Networks(VANETs) are evolving to the Internet of Vehicles(Io V), which promises efficient and intelligent prospect for the future transportation system. On the other hand, vehicles are not only consuming but also generating a huge amount and enormous types of data, which is referred to as Big Data. In this article, we first investigate the relationship between Io V and big data in vehicular environment, mainly on how Io V supports the transmission, storage, computing of the big data, and how Io V benefits from big data in terms of Io V characterization,performance evaluation and big data assisted communication protocol design. We then investigate the application of Io V big data in autonomous vehicles. 9Role of Sonic Hedgehog signaling during progression from inflammation to cancer in the stomach显示文摘Despite advances in treatment and the declining incidence,gastric cancer remains the second leading cause of cancer-related deaths in the world.Understanding the progression from inflammation to cancer in the stomach is crucial in the development of novel therapies and strategies for treating this disease.Chronic inflammation of the stomach is typically caused by Helicobacter pylori(H.pylori)and resulting lesions may lead to gastric cancer.During the progression from inflammation to cancer,the stomach epithelium changes with evidence of the disruption of normal epithelial cell differentiation and infiltrating inflammatory cells.Coincident with the development of atrophic gastritis and metaplasia,is the loss of the gastric morphogen Sonic Hedgehog(Shh).Given its critical role as a regulator of gastric tissue homeostasis,the disruption of Shh expression during inflammation correlates with the loss of normal epithelial cell differentiation,but this has only recently been rigorously tested in vivo using a unique mouse model of targeted gastric Shh deletion.While pre-neoplastic lesions such as atrophic gastritis and in-testinal metaplasia are associated with the loss of Shh within the acid-secreting glands of the stomach,there is a clear link between elevated Shh and signaling to gastric cancers.The current review focuses on the effects of aberrant Shh expression and its role in the development of gastric cancer,specifically in response to H.pylori infection.Alexander E Sherman Yana Zavros 2011World Journal of Gastrointestinal Pathophysiology2011,2,6:14
10Cooperative Vehicular Content Distribution in Edge Computing Assisted 5G-VANET显示文摘By leveraging the 5G enabled vehicular ad hoc network(5G-VANET), it is widely recognized that connected vehicles have the potentials to improve road safety, transportation intelligence and provide in-vehicle entertainment experience. However, many enabling applications in 5G-VANET rely on the efficient content sharing among mobile vehicles, which is a very challenging issue due to the extremely large data volume, rapid topology change, and unbalanced traffic. In this paper, we investigate content prefetching and distribution in 5G-VANET. We first introduce an edge computing based hierarchical architecture for efficient distribution of large-volume vehicular data. We then propose a multi-place multi-factor prefetching scheme to meet the rapid topology change and unbalanced traffic. The content requests of vehicles can be served by neighbors, which can improve the sharing efficiency and alleviate the burden of networks. Furthermore, we use a graph theory based approach to solve the content distribution by transforming it into a maximum weighted independent set problem. 11Chinese herbal medicine for COVID-19: Current evidence with systematic review and meta-analysis显示文摘Background: There is currently no drug or therapy that cures COVID-19, a highly contagious and life-threatening disease.Objective: This systematic review and meta-analysis summarized contemporary studies that report the use of Chinese herbal medicine(CHM) to treat COVID-19.Search strategy: Six electronic databases(PubMed/MEDLINE, Cochrane Library, ScienceDirect, Google Scholar, Wanfang Data and China National Knowledge Infrastructure) were searched from their beginning to May 15, 2020 with the following search terms: traditional Chinese medicine, Chinese medicine,Chinese herbal medicine, COVID-19, new coronavirus pneumonia, SARS-CoV-2, and randomized controlled trial.Inclusion criteria: Randomized controlled trials(RCTs) from peer-reviewed journals and non-reviewed publications were included. Further, included RCTs had a control group that was given standard care(SC;such as conventional Western medicine treatments or routine medical care), and a treatment group that was given SC plus CHM.Data extraction and analysis: Two evaluators screened and collected literature independently;information on participants, study design, interventions, follow-up and adverse events were extracted, and risk of bias was assessed. The primary outcomes included scores that represented changes in symptoms and signs over the course of treatment. Secondary outcomes included the level of inflammatory markers, improvement of pneumonia confirmed by computed tomography(CT), and adverse events. Dichotomous data were expressed as risk ratio or hazard ratio with 95% confidence interval(CI);where time-to-event analysis was used, outcomes were expressed as odds ratio with 95% CI. Continuous data were expressed as difference in means(MD) with 95% CI, and standardized mean difference(SMD) was used when different outcome scales were pooled.Results: Seven original studies, comprising a total of 732 adults, were included in this meta-analysis.Compared to SC alone, CHM plus SC had a superior effect on the change of symptom and sign score(-1.30 by SMD, 95% CI [-2.43, -0.16];3 studies;n = 261, P = 0.03), on inflammatory marker C-reactive protein(CRP, mg/L;-11.82 by MD, 95% CI [-17.95, -5.69];5 studies;n = 325, P = 0.0002),on number of patients with improved lung CT scans(1.34 by risk ratio, 95% CI [1.19, 1.51];4 studies;n = 489, P < 0.00001). 18Lubiprostone used with polyethylene glycol in diabetic patients enhances colonoscopy preparation quality显示文摘AIM: To assess the additive effect of lubiprostone on the quality of colon preparation in diabetics given single-dosed polyethylene glycol electrolyte(PEG)for colonoscopy.METHODS:This was an investigator-initiated,singlecenter,single-blinded prospective trial comparing the efficacy of L+PEG to PEG alone on colon preparation quality in diabetics undergoing screening colonoscopy.The study was approved by our institution’s IRB.The PEG was given as a single-dose to address patientcompliance concerns voiced by our IRB with splitdosing.All patients received only clear liquids the day prior to colonoscopy.Experimental group(Grp L)received PEG+1 dose L 2 h prior to and 2 h after PEG completion.Control group(Grp C)received only PEG the evening prior to the colonoscopy.Patients were randomly assigned to one of the 2 groups.The endoscopist was blinded to which colon prep was given and all colonoscopies were complete.Upon colonoscopy completion,the endoscopist rated the colon prep-quality by a validated 5-point Likert scale(1-excellent to 5-inadequate).RESULTS:Sixty patients were enrolled in the study;30 Grp L and 30 Grp C.Overall,patients were excluded due to study non-completion in 12(41%)Grp L and 5(17%) Grp C,P=0.04.Average colon preparation score Grp L =2.47 and Grp C=3.00,P=0.09.Although this was not statistically significant,there was a trend towards improved colon prep in Grp L.Statistical significance may have been achieved if completion rates had been similar between both study groups.CONCLUSION:Use of 2-L capsules with PEG resulted in a trend towards improved colon prep over PEG alone in diabetic patients when given as a single-dose regimen.Erika Grigg Moonkyung C Schubert Joshua Hall Fadi Rahhal Dimple Raina Subbaramiah Sridhar Sherman M Chamberlain 2010World Journal of Gastrointestinal Endoscopy2010,2,7:5
19Mechanical responses, texture and microstructural evolution of high purity aluminum deformed by equal channel angular pressing显示文摘Ultrafine-grained(UFG) high purity aluminum exhibits a variety of attractive mechanical properties and special deformation behavior. Equal channel angular pressing(ECAP) process can be used to easily and effectively refine metals. The microstructure and microtexture evolutions and grain boundary characteristics of the high purity aluminum(99.998%) processed by ECAP at room temperature are investigated by means of TEM and EBSD. The results indicate that the shear deformation resistance increases with repeated EACP passes, and equiaxed grains with an average size of 0.9 μm in diameter are formed after five passes. Although the orientations distribution of grains tends to evolve toward random orientations, and microtextures(80°, 35°, 0°),(40°, 75°, 45°) and(0°, 85°, 45°) peak in the sample after five passes. The grain boundaries in UFG aluminum are high-angle geometrically necessary boundaries. It is suggested that the continuous dynamic recrystallization is responsible for the formation of ultrafine grains in high purity aluminum.
