| 3 | Magnetic resonance imaging in assessment of stress urinary incontinence in women: Parameters differentiating urethral hypermobility and intrinsic sphincter deficiency显示文摘AIM: To define the magnetic resonance imaging(MRI) parameters differentiating urethral hypermobility(UH) and intrinsic sphincter deficiency(ISD) in women with stress urinary incontinence(SUI).METHODS: The static and dynamic MR images of 21 patients with SUI were correlated to urodynamic(UD) findings and compared to those of 10 continent controls. For the assessment of the urethra and integrity of the urethral support structures, we applied the highresolution endocavitary MRI, such as intraurethral MRI, endovaginal or endorectal MRI. For the functional imaging of the urethral support, we performed dynamic MRI with the pelvic phased array coil. We assessed the following MRI parameters in both the patient and thevolunteer groups:(1) urethral angle;(2) bladder neck descent;(3) status of the periurethral ligaments,(4) vaginal shape;(5) urethral sphincter integrity, length and muscle thickness at mid urethra;(6) bladder neck funneling;(7) status of the puborectalis muscle;(8) pubo-vaginal distance. UDs parameters were assessed in the patient study group as follows:(1) urethral mobility angle on Q-tip test;(2) Valsalva leak point pressure(VLPP) measured at 250 cc bladder volume; and(3) maximum urethral closure pressure(MUCP). The UH type of SUI was defined with the Q-tip test angle over 30 degrees, and VLPP pressure over 60 cm H2 O. The ISD incontinence was defined with MUCP pressure below 20 cm H2 O, and VLPP pressure less or equal to 60 cm H2 O. We considered the associations between the MRI and clinical data and UDs using a variety of statistical tools to include linear regression, multivariate logistic regression and receiver operating characteristic(ROC) analysis. All statistical analyses were performed using STATA version 9.0(Stata Corp LP, College Station, TX).RESULTS: In the incontinent group, 52% have history of vaginal delivery trauma as compared to none in control group(P < 0.001). There was no difference between the continent volunteers and incontinent patients in body habitus as assessed by the body mass index. Pubovaginal distance and periurethral ligament disruption are significantly associated with incontinence; periurethral ligament symmetricity reduces the odds of incontinence by 87%. Bladder neck funneling and length of the suprapubic urethral sphincter are significantly associated with the type of incontinence on UDs; funneling reduced the odds of pure UH by almost 95%; increasing suprapubic urethral sphincter length at rest is highly associated with UH. Both MRI variables result in a predictive model for UDs diagnosis(area under the ROC = 0.944). | 4 | Effect of cavity flow control on high-speed train pantograph and roof aerodynamic noise显示文摘The pantograph and its recess on the train roof are major aerodynamic noise sources on high-speed trains.Reducing this noise is particularly important because conventional noise barriers usually do not shield the pantograph.However,less attention has been paid to the pantograph recess compared with the pantograph.In this paper,the flow features and noise contribution of two types of noise reduction treatments rounded and chamfered edges are studied for a simplified high-speed train pantograph recess,which is represented as a rectangular cavity and numerically investigated at 1/10 scale.Improved delayed detached-eddy simulations are performed for the near-field turbulent flow simulation,and the Ffowcs Williams and Hawkings aeroacoustic analogy is used for far-field noise prediction.The highly unsteady flow over the cavity is significantly reduced by the cavity edge modifications,and consequently,the noise radiated from the cavity is reduced.Furthermore,effects of the rounded cavity edges on the flow and noise of the pantographs(one raised and one folded)are investigated by comparing the flow features and noise contributions from the cases with and without rounding of the cavity edges.Different train running directions are also considered.Flow analysis shows that the highly unsteady flow within the cavity is reduced by rounding the cavity edges and a slightly lower flow speed occurs around the upper parts of the raised pantograph,whereas the flow velocity in the cavity is slightly increased by the rounding.Higher pressure fluctuations occur on the folded pantograph and the lower parts of the raised pantograph,whereas weaker fluctuations are found on the panhead of the raised pantograph.This study shows that by rounding the cavity edges,a reduction in radiated noise at the side and the top receiver positions can be achieved.Noise reductions in the other directions can also be found. | Hogun Kim Zhiwei Hu David Thompson | 2020 | Railway Engineering Science2020,28,1: | 10 |
| 5 | Efficacy and safety of tenofovir in chronic hepatitis B: Australian real world experience显示文摘AIM To evaluate the long-term treatment outcomes of tenofovir therapy in patients in a real world Australian tertiary care setting.METHODS We performed a retrospective analysis of treatment outcomes among treatment-na?ve and treatment-experienced patients receiving a minimum 3 mo tenofovir therapy through St Vincent's Hospital Melbourne, Australia. We included patients receiving tenofovir [tenofovir disoproxil fumarate(TDF)] monotherapy, as well as patients treated with TDF in combination with a second antiviral agent. Patients were excluded if they demonstrated human immune-deficiency virus/hepatitis C virus/hepatitis delta virus coinfection or were less than 18 years of age. We considered virological and biochemicalresponse, as well as safety outcomes. Virological response was determined by measurement of hepatitis B virus(HBV) DNA using sensitive assays; biochemical response was determined via serum liver function tests; histological response was determined from liver biopsy and fibroscan; safety analysis focused on glomerular renal function and bone mineral density. The primary efficacy endpoint was complete virological suppression over time, defined by HBV DNA < 20 IU/m L. Secondary efficacy endpoints included rates of biochemical response, and HB e antigen(HBe Ag)/HB surface antigen loss and seroconversion over time.RESULTS Ninety-two patients were identified who fulfilled the enrolment criteria. Median follow-up was 26 mo(range 3-114). Mean age was 46(24-78) years, 64(70%) were male and 77(84%) were of Asian origin. 55(60%) patients were treatment-na?ve and 62 patients(67%) were HBe Ag-negative. Complete virological suppression was achieved by 45/65(71%) patients at 12 mo, 37/46(80%) at 24 mo and 25/28(89%) at 36 mo. Partial virological response(HBV DNA 20-2000 IU/m L) was achieved by 89/92(96.7%) of patients. Multivariate analysis showed a significant relationship between virological suppression at end of follow-up and baseline HBV DNA level(OR = 0.897, 95%CI: 0.833-0.967, P = 0.0046) and HBe Ag positive status(OR = 0.373, 95%CI: 0.183-0.762, P = 0.0069). There was no difference in response comparing treatment-na?ve and treatment-experienced patients. Three episodes of virological breakthrough occurred in the setting of noncompliance. Tenofovir therapy was well tolerated.CONCLUSION Tenofovir is an efficacious, safe and well-tolerated treatment in an Australian real-world tertiary care setting. | 6 | Under the surface:Pressure-induced planetary-scale waves,volcanic lightning,and gaseous clouds caused by the submarine eruption of Hunga Tonga-Hunga Ha'apai volcano显示文摘We present a narrative of the eruptive events culminating in the cataclysmic January 15, 2022 eruption of Hunga Tonga-Hunga Ha’apai Volcano by synthesizing diverse preliminary seismic, volcanological, sound wave, and lightning data available within the first few weeks after the eruption occurred. The first hour of eruptive activity produced fast-propagating tsunami waves, long-period seismic waves, loud audible sound waves, infrasonic waves, exceptionally intense volcanic lightning and an unsteady volcanic plume that transiently reached-at 58km-the Earth’s mesosphere. Energetic seismic signals were recorded worldwide and the globally stacked seismogram showed episodic seismic events within the most intense periods of phreatoplinian activity, and they correlated well with the infrasound pressure waveform recorded in Fiji. Gravity wave signals were strong enough to be observed over the entire planet in just the first few hours, with some circling the Earth multiple times subsequently. These large-amplitude, long-wavelength atmospheric disturbances come from the Earth’s atmosphere being forced by the magmatic mixture of tephra, melt and gasses emitted by the unsteady but quasicontinuous eruption from 0402±1–1800 UTC on January 15, 2022. Atmospheric forcing lasted much longer than rupturing from large earthquakes recorded on modern instruments, producing a type of shock wave that originated from the interaction between compressed air and ambient(wavy) sea surface. This scenario differs from conventional ideas of earthquake slip, landslides, or caldera collapse-generated tsunami waves because of the enormous(~1000x) volumetric change due to the supercritical nature of volatiles associated with the hot,volatile-rich phreatoplinian plume. The time series of plume altitude can be translated to volumetric discharge and mass flow rate. For an eruption duration of ~12 h, the eruptive volume and mass are estimated at 1.9 km^(3) and~2 900 Tg, respectively, corresponding to a VEI of 5–6 for this event. The high frequency and intensity of lightning was enhanced by the production of fine ash due to magma-seawater interaction with concomitant high charge per unit mass and the high pre-eruptive concentration of dissolved volatiles. Analysis of lightning flash frequencies provides a rapid metric for plume activity and eruption magnitude. Many aspects of this eruption await further investigation by multidisciplinary teams. | 9 | Past,present and future of cyanide antagonism research:From the early remedies to the current therapies显示文摘This paper reviews milestones in antidotal therapies for cyanide(CN) spanning early remedies,current antidotal systems and research towards next generation therapies.CN has been a part of plant defense mechanisms for millions of years.It became industrially important in the nineteenth century with the advent of CN assisted gold mining and the use of CN as a pest control agent.The biochemical basis of CN poisoning was actively studied and key mechanisms were understood as early as 1929.These fundamental studies led to a variety of antidotes,including indirect CN binders that generate methemoglobin,direct CN binders such as hydroxocobalamin,and sulfur donors that convert CN to the less toxic thiocyanate.Research on blood gases at the end of the twentieth century shed new light on the role of nitric oxide(NO) in the body.The discovery of NO's ability to compete with CN for enzymatic binding sites provided a previously missed explanation for the rapid efficacy of NO generating antidotes such as the nitrites.Presently used CN therapies include:methemoglobin/NO generators(e.g.,sodium nitrite,amyl nitrite,and dimethyl aminophenol),sulfur donors(e.g.,sodium thiosulfate and glutathione),and direct binding agents [(e.g.,hydroxocobalamin and dicobalt salt of ethylenediaminetetraacetic acid(dicobalt edetate)].A strong effort is being made to explore novel antidotal systems and to formulate them for rapid administration at the point of intoxication in mass casualty scenarios.New antidotes,formulations,and delivery systems are enhancing bioavailability and efficacy and hold promise for a new generation of improved CN countermeasures. | Ilona Petrikovics Marianna Budai Kristof Kovacs David E Thompson | 2015 | World Journal of Methodology2015,5,2: | 3 |

| 18 | Mutifactorial analysis of risk factors for reduced bone mineral density in patients with Crohn’s disease显示文摘AIM: To determine the prevalence of osteoporosis in a cohort of patients with Crohn’s disease (CD) and to identify the relative significance of risk factors for osteoporosis. METHODS: Two hundred and fifty-eight unselected patients (92 M, 166 F) with CD were studied. Bone mineral density (BMD) was measured at the lumbar spine and hip by dual X-ray absorptiometry. Bone formation was assessed by measuring bone specific alkaline phosphatase (BSAP) and bone resorption by measuring urinary excretion of deoxypyridinoline (DPD) and N-telopeptide (NTX). RESULTS: Between 11.6%-13.6% patients were osteoporotic (T score < -2.5) at the lumbar spine and/or hip. NTX levels were significantly higher in the patients with osteoporosis (P < 0.05) but BSAP and DPD levels were not significantly different. Independent risk factors for osteoporosis at either the lumbar spine or hip were a low body mass index (P < 0.001), increasing corticosteroid use (P < 0.005), and male sex (P < 0.01). These factors combined accounted for 23% and 37% of the reduction in BMD at the lumbar spine and hip respectively. CONCLUSION: Our results confirm that osteoporosis is common in patients with CD and suggest that increased bone resorption is the mechanism responsible for thebone loss.
