| 1 | Nutritional risk index as a predictor of postoperative wound complications after gastrectomy显示文摘AIM:To investigate the correlation between the nutritional risk index (NRI) and postoperative wound complications.METHODS:From January 2008 through June 2008,669 patients who underwent curative gastrectomy for gastric cancer were included in a retrospective study.Medical records of consecutive patients were collected and analyzed to determine postoperative wound complication rates.The NRI was assessed on the fifth postoperative day and other possible risk factors for the incidence of wound complications were analyzed to identify the factors affecting postoperative wound complications.Patients with other postoperative complications were excluded from the study.RESULTS:On the 5th postoperative day,the NRI showed a malnutrition rate of 84.6% among postoperative patients.However,postoperative wound complications occurred in only 66/669 (9.86%) patients.Of the patients with wound complications,62/66 (94%) belonged to the malnourished group (NRI < 97.5),and 4/66 (6%) patients to the non-malnourished group (NRI ≥ 97.5).The only factor correlated with wound complications was the NRI on the 5th postoperative day (odds ratio of NRI ≥ 97.5 vs NRI < 97.5:0.653;95% confidence interval:0.326-0.974;P=0.014) according to univariate analysis as well as multivariate analysis.CONCLUSION:This study suggests that malnutrition immediately after surgery may play a significant role in the development of wound complications. | Cheong Ah Oh Dae Hoon Kim Seung Jong Oh Min Gew Choi Jae Hyung Noh Tae Sung Sohn Jae Moon Bae Sung Kim | 2012 | World Journal of Gastroenterology2012,18,7: | 25 |
| 2 | Real time shear wave elastography in chronic liver diseases:Accuracy for predicting liver fibrosis,in comparison with serum markers显示文摘AIM:To evaluate the correlation between liver stiffness measurement(LSM)by real-time shear wave elastography(SWE)and liver fibrosis stage and the accuracy of LSM for predicting significant and advanced fibrosis,in comparison with serum markers.METHODS:We consecutively analyzed 70 patients with various chronic liver diseases.Liver fibrosis was staged from F0 to F4 according to the Batts and Ludwig scoring system.Significant and advanced fibrosis was defined as stage F≥2 and F≥3,respectively.The accuracy of prediction for fibrosis was analyzed using receiver operating characteristic curves.RESULTS:Seventy patients,15 were belonged to F0-F1 stage,20 F2,13 F3 and 22 F4.LSM was increased with progression of fibrosis stage(F0-F1:6.77±1.72,F2:9.98±3.99,F3:15.80±7.73,and F4:22.09±10.09,P<0.001).Diagnostic accuracies of LSM for prediction of F≥2 and F≥3 were 0.915(95%CI:0.824-0.968,P<0.001)and 0.913(95%CI:0.821-0.967,P<0.001),respectively.The cut-off values of LSM for prediction of F≥2 and F≥3 were 8.6kPa with 78.2%sensitivity and 93.3%specificity and10.46 kPa with 88.6%sensitivity and 80.0%specificity,respectively.However,there were no significant differences between LSM and serum hyaluronic acid and typeⅣcollagen in diagnostic accuracy.CONCLUSION:SWE showed a significant correlation with the severity of liver fibrosis and was useful and accurate to predict significant and advanced fibrosis,comparable with serum markers. | Jae Yoon Jeong Tae Yeob Kim Joo Hyun Sohn Yongsoo Kim Woo Kyoung Jeong Young-Ha Oh Kyo-Sang Yoo | 2014 | World Journal of Gastroenterology2014,20,38: | 19 |
| 3 | Recurrence after endoscopic piecemeal mucosal resection for large sessile colorectal polyps显示文摘AIM: To evaluate the safety and outcomes of endoscopic piecemeal mucosal resection (EPMR) for large sessile colorectal polyps. METHODS: The patients enrolled in this study were 47 patients with 50 large sessile polyps (diameter, 2 cm or greater) who underwent EPMR using a submucosal saline injection technique between December 2002 and October 2005. All medical records, including characteristics of the patients and polyps, complications, and recurrences, were retrospectively reviewed. The first follow-up endoscopic examination was performed at 3-6 mo after initial endoscopic resection, and the second at 12 mo postEPMR. Subsequent surveillance colonoscopic examinations were individualized, taking risk factors into account. RESULTS: The patients were 23 men and 24 women,with a mean age of 60 years. Mean polyp size was 30.1 mm. Of 50 polyps identified, 34 (68%) were benign and 16 (32%) were malignant. There were 6 (12%) cases with EPMR-related bleeding: 5 intra-procedural and 1 early post-procedural bleeding. All bleeding episodes were managed by endoscopic clipping or argon beam coagulation. There were no perforations. Recurrence was identified in 5 cases (12.2%): 4 local recurrences detected at 3 mo post-EPMR and 1 local recurrence detected at 14 mo post-EPMR. The recurrence rate after EPMR was 3.1% for benign polyps and 33.3% for malignant polyps (P < 0.05). Median follow-up time was 37 mo. CONCLUSION: EPMR is safe, but should be applied carefully in malignant polyps. | 4 | Endoscopic ultrasound-guided biliary drainage with placement of a fully covered metal stent for malignant biliary obstruction显示文摘AIM:To determine the utility of endoscopic ultrasoundguided biliary drainage(EUS-BD)with a fully covered self-expandable metal stent for managing malignant biliary stricture. METHODS:We collected data from 13 patients who presented with malignant biliary obstruction and underwent EUS-BD with a nitinol fully covered selfexpandable metal stent when endoscopic retrograde cholangiopancreatography(ERCP)fails.EUS-guided choledochoduodenostomy(EUS-CD)and EUS-guided hepaticogastrostomy(EUS-HG)was performed in 9 patients and 4 patients,respectively. RESULTS:The technical and functional success rate was 92.3%(12/13)and 91.7%(11/12),respectively. Using an intrahepatic approach(EUS-HG,n=4),there was mild peritonitis(n=1)and migration of the metal stent to the stomach(n=1).With an extrahepatic approach(EUS-CD,n=10),there was pneumoperitoneum(n=2),migration(n=2),and mild peritonitis (n=1).All patients were managed conservatively with antibiotics.During follow-up(range,1-12 mo),there was re-intervention(4/13 cases,30.7%)necessitated by stent migration(n=2)and stent occlusion(n=2). | 8 | Efficacy of Saam acupuncture treatment on improvement of immune cell numbers in cancer patients:a pilot study显示文摘OBJECTIVE: To collect preliminary data on the effects of Saam acupuncture with regard to the immunity in cancer patients.METHODS: Ten cancer patients were analyzed for improvements in immunity. Acupuncture was applied at the 5 acupuncture points, Jingqu(LU 8), Zutonggu(BL 66), Yanggu(SI 5), Yangchi(TE 4), and Zhongwan(CV 12) for 2 weeks with 4 sessions. We assessed the effect of Korean Saam acupuncture on the immune system in cancer patients by measuring particular blood cell subsets, including CD3 +,CD4+, CD8+, CD19+, and CD56+ cells, as well as total white blood cell count, absolute neutrophil count, and fatigue score. The measurement was performed before and after acupuncture and at a 2-week follow-up.RESULTS: There was a statistically significant increase in the number of CD3+(P=0.023) and CD8+ cells(P<0.001) and T-cell subsets, as well as a decrease in the fatigue severity scale(FSS) score(P=0.001) after Saam acupuncture using the 5 acupoints.CONCLUSION: Acupuncture may improve the immune system by increasing the counts of a few immune cells and relieve fatigue in cancer patients by decreasing FSS scores. | 11 | Association between Helicobacter pylori status and metachronous gastric cancer after endoscopic resection显示文摘AIM To investigate the effect of Helicobacter pylori(H. pylori) status test and H. pylori eradication on the occurrence of metachronous gastric cancer(MGC) after endoscopic submucosal dissection(ESD) of early gastric cancer(EGC) and risk factors of MGC. METHODS The authors retrospectively reviewed the medical records of 433 patients(441 lesions) who underwent ESD for EGC from January 2005 to January 2015 in Yeungnam University Hospital. Patients were categorized into two groups; the H. pylori tested group(n = 257) and the H. pylori non-tested group(n = 176) based on performance of H. pylori status test after ESD of EGC. The H. pylori tested group was further categorized into three subgroups based on H. pylori status; the H. pylori-eradicated subgroup(n = 120), the H. pylori-persistent subgroup(n = 42), and the H. pylori-negative subgroup(n = 95). Incidences of MGC and risk factors of MGC were identified.RESULTS Median follow-up duration after ESD was 30.00 mo(range, 6-107 mo). Total 15 patients developed MGC during follow-up. MGC developed in 11 patients of the H. pylori tested group(7 in the H. pylori-negative subgroup, 3 in the H. pylori-eradicated subgroup, and 1 in the H. pylori-persistent subgroup) and 4 patients of the H. pylori non-tested group(P > 0.05). The risk factors of MGC were endoscopic mucosal atrophy in the H. pylori tested group and intestinal metaplasia in all patients. CONCLUSION H. pylori eradication and H. pylori status test seems to have no preventive effect on the development of MGC after ESD for EGC. | 14 | Pre-operative predictive factors for gallbladder cholesterol polyps using conventional diagnostic imaging显示文摘AIM:To determine the clinical data that might be useful for differentiating benign from malignant gallbladder (GB) polyps by comparing radiological methods,including abdominal ultrasonography (US) and computed tomography (CT) scanning,with postoperative pathology fi ndings. METHODS:Fifty-nine patients underwent laparoscopic cholecystectomy for a GB polyp of around 10 mm. They were divided into two groups,one with cholesterol polyps and the other with non-cholesterol polyps. Clinical features such as gender,age,symptoms,size and number of polyps,the presence of a GB stone,the radiologically measured maximum diameter of the polyp by US and CT scanning,and the measurements of diameter from postoperative pathology were recorded for comparative analysis. RESULTS:Fifteen of the 41 cases with cholesterol polyps (36.6%) were detected with US but not CT scanning,whereas all 18 non-cholesterol polyps were observed using both methods. In the cholesterol polyp group,the maximum measured diameter of the polyp was smaller by CT scan than by US.Consequently,the discrepancy between those two scanning measurements was greater than for the non-cholesterol polyp group. CONCLUSION:The clinical signs indicative of a cholesterol polyp include:(1) a polyp observed by US but not observable by CT scanning,(2) a smaller diameter on the CT scan compared to US,and (3) a discrepancy in its maximum diameter between US and CT measurements.
