Background Delayed facial palsy ( DFP) after microvascular decompression ( MVD) in patients with hemifacial spasm ( HFS) is not uncommon, but the cause remains unknown. Objectives To assess whether intraoperative electromyography ( EMG) and brainstem auditory evoked potential ( BAEP) can predict DFP after MVD. Methods Between September 2009 and February 2011 we examined 86 patients,9 of whom ( 10. 4% ) developed DFP after MVD on the same side. All patients underwent MVD and were followed - up for a median period of 13 months ( range 6-22) . We retrospectively examined intraoperative facial EMG and BAEP findings using our MVD patients' registry. We excluded secondary HFS and immediate postoperative facial palsy after MVD in this study. We assessed the prevalence and clinical characteristics of DFP and compared EMG and BAEP findings between DFP and non-DFP groups. Results: All pa- tients recovered completely, with a mean time to recovery of 37. 8 days ( range 22-57) . There were no significant differences between DFP and non - DFP patients in terms of the amplitude and latency of intraoperative EMG and BAEP. Conclusion The usefulness of intraoperative facial EMG and BAEP is limited and cannot predict DFP after MVD for HFS. We speculate that DFP after MVD is not associated with permanent nerve damage according to the EMG findings. | Kim HJ Park YS Ryu JS Huh R Han I Shin DA Kim TG Cho KG Chung SS | 2012 | 中华神经外科疾病研究杂志2012,11,4: | 13
Ethanol extract of Chondracanthus tenellus(Harvey)Hommersand attenuates lipopolysaccharide-induced inflammatory and oxidative response by blocking the NF-κB,MAPKs,and PI3K/Akt signaling pathways显示文摘Objective:To investigate whether the ethanol extract of Chondracanthus tenellus(Harvey)Hommersand,a type of red algae, could exhibit anti-inflammatory potential in lipopolysaccharide(LPS)-stimulated macrophages. Methods:The ethanol extract of Chondracanthus tenellus was applied to 100 ng/mL LPS-stimulated RAW 264.7 cells, and cell viability, phagocytic ability, levels of pro-inflammatory factors, and the production of reactive oxygen species were measured. To identify the underlying mechanism of the ethanol extract of Chondracanthus tenellus, the expression of inflammation-regulated genes was estimated. Results:The ethanol extract of Chondracanthus tenellus had no cytotoxic effect at concentrations below 300μg/mL,and reduced the LPS-induced production of inflammatory mediators including nitric oxide(NO)and prostaglandin E2.Furthermore,the extract markedly suppressed the expression of inducible NO synthase and cyclooxygenase-2,as well as the production of reactive oxygen species. The LPS-induced up-regulation of pro-inflammatory cytokines was attenuated by treatment with the ethanol extract of Chondracanthus tenellus, reducing their extracellular secretion. The Chondracanthus tenellus extract also inhibited LPS-mediated activation of nuclear factor-kappa B(NF-κB).In addition, the phosphorylation of mitogen activated protein kinases(MAPKs)and phosphatidylinositol 3 kinase(PI3K)/Akt was markedly increased by LPS,which was significantly abolished by the Chondracanthus tenellus extract. Conclusions:Our findings indicate that the ethanol extract of Chondracanthus tenellus exhibited potential anti-inflammatory and antioxidant effects through downregulating the NF-κB,MAPKs,and PI3K/Akt signaling pathways in LPS stimulated RAW 264.7 macrophages.tenellus extract. Conclusions:Our findings indicate that the ethanol extract of Chondracanthus tenellus exhibited potential anti-inflammatory and antioxidant effects through downregulating the NF-κB,MAPKs,and PI3K/Akt signaling pathways in LPS stimulated RAW 264.7 macrophages. | Da Hye Kwon Cheol Park Hyesook Lee Su Hyun Hong Gi-Young Kim Hee-Jae Cha Suhkmann Kim Heui-Soo Kim Hye-Jin Hwang Yung Hyun Choi | 2021 | Asian Pacific Journal of Tropical Biomedicine2021,11,10: | 4
Risk factors of electrocoagulation syndrome after esophageal endoscopic submucosal dissection显示文摘AIM To investigate post endoscopic submucosal dissection electrocoagulation syndrome(PEECS) of the esophagus.METHODS We analyzed 55 consecutive cases with esophageal endoscopic submucosal dissection for superficial esophageal squamous neoplasms at a tertiary referral hospital in South Korea. Esophageal PEECS was defined as 'mild' meeting one of the following criteria without any obvious perforation: fever(≥ 37.8 ℃), leukocytosis(> 10800 cells/μl), or regional chest pain more than 5/10 points as rated on a numeric pain intensity scale. The grade of PEECS was determined as 'severe' when meet two or more of above criteria.RESULTS We included 51 cases without obvious complications in the analysis. The incidence of mild and severe esophageal PEECS was 47.1% and 17.6%, respectively. Risk factor analysis revealed that resected area, procedure time, and muscle layer exposure were significantly associated with PEECS. In multivariate analysis, a resected area larger than 6.0 cm^2(OR = 4.995, 95%CI: 1.110-22.489, P = 0.036) and muscle layer exposure(OR = 5.661, 95%CI: 1.422-22.534, P = 0.014) were independent predictors of esophageal PEECS. All patients with PEECS had favorable outcomes with conservative management approaches, such as intravenous hydration or antibiotics.CONCLUSION Clinicians should consider the possibility of esophageal PEECS when the resected area exceeds 6.0 cm^2 or when the muscle layer exposure is noted. | Dae Won Ma Young Hoon Youn Da Hyun Jung Jae Jun Park Jie-Hyun Kim Hyojin Park | 2018 | World Journal of Gastroenterology2018,24,10: | 3

Effect of esmolol and lidocaine on agitation in awake phase of anesthesia among children:a double-blind,randomized clinical study显示文摘Background:Sevoflurane is widely used to anesthetize children because of its rapid action with minimal irritation of the airways. However, there is a high risk of agitation after emergence from anesthesia.Strabismus surgery,in particular, can trigger agitation because patients have their eyes covered in the postoperative period. The aim of this study was to determine whether or not esmolol and lidocaine could decrease emergence agitation in children.Methods:Eighty-four patients aged 3 to 9 years undergoing strabismus surgery were randomly assigned to a control group(saline only),a group that received intravenous lidocaine 1.5 mg/kg,and a group that received intravenous esmolol 0.5 mg/kg and lidocaine 1.5 mg/kg.Agitation was measured using the objective pain score,Cole 5-point score, and Richmond Agitation Sedation Scale score at the end of surgery,on arrival in the recovery room, and 10 and 30 min after arrival. Results:The group that received the combination of esmolol and lidocaine showed lower OPS and RASS scores than the other two groups when patients awoke from anesthesia(OPS=0(0-4),RASS=-4[(-5)-1])and were transferred to the recovery room(OPS=0(0-8),RASS=-1[(-5)-3])(P<0.05).There was no significant difference in the severity of agitation among the three groups at other time points(P>0.05).Conclusions:When pediatric strabismus surgery is accompanied by sevoflurane anesthesia,an intravenous injection of esmolol and lidocaine could alleviate agitation until arrival in the recovery room. | Jae Young Ji Jin Soo Park Ji Eun Kim Da Hyung Kim Jin Hun Chung Hea Rim Chun Ho Soon Jung Sie Hyeon Yoo | 2019 | Chinese Medical Journal2019,,7: | 3
Secondary endoscopic submucosal dissection for locally recurrent or incompletely resected gastric neoplasms显示文摘AIM To investigate the feasibility and safety of secondary endoscopic submucosal dissection(ESD) for residual or locally recurrent gastric tumors. METHODS Between 2010 and 2017, 1623 consecutive patients underwent ESD for gastric neoplasms at a single tertiary referral center. Among these, 28 patients underwent secondary ESD for a residual or locally recurrent tumor. Our analysis compared clinicopathologic factors between primary ESD and secondary ESD groups. RESULTS The en bloc resection and curative rate of resection of secondary ESD were 92.9% and 89.3%, respectively. The average procedure time of secondary ESD was significantly longer than primary ESD(78.2 min vs 55.1 min, P = 0.004), and the adverse events rate was not significantly different but trended slightly higher in the secondary ESD group compared to the primary ESD group(10.7% vs 3.8%, P = 0.095). Patients who received secondary ESD had favorable outcomes without severe adverse events. During a mean follow-up period, no local recurrence occurred in patients who received secondary ESD. CONCLUSION Secondary ESD of residual or locally recurrent gastric tumors appears to be a feasible and curative treatment though it requires greater technical efficiency and longer procedure time. | Da Hyun Jung Young Hoon Youn Jie-Hyun Kim Jae Jun Park Hyojin Park | 2018 | World Journal of Gastroenterology2018,24,33: | 3

A modular microfluidic system based on a multilayered configuration to generate large-scale perfusable microvascular networks显示文摘The vascular network of the circulatory system plays a vital role in maintaining homeostasis in the human body.In this paper,a novel modular microfluidic system with a vertical two-layered configuration is developed to generate largescale perfused microvascular networks in vitro. The two-layer polydimethylsiloxane(PDMS)configuration allows the tissue chambers and medium channels not only to be designed and fabricated independently but also to be aligned and bonded accordingly. This method can produce a modular microfluidic system that has high flexibility and scalability to design an integrated platform with multiple perfused vascularized tissues with high densities. The medium channel was designed with a rhombic shape and fabricated to be semiclosed to form a capillary burst valve in the vertical direction, serving as the interface between the medium channels and tissue chambers.Angiogenesis and anastomosis at the vertical interface were successfully achieved by using different combinations of tissue chambers and medium channels.Various large-scale microvascular networks were generated and quantified in terms of vessel length and density.Minimal leakage of the perfused 70-kDa FITC-dextran confirmed the lumenization of the microvascular networks and the formation of tight vertical interconnections between the microvascular networks and medium channels in different structural layers. This platform enables the culturing of interconnected,large-scale perfused vascularized tissue networks with high density and scalability for a wide range of multiorgan-on-a-chip applications, including basic biological studies and drug screening. | Tao Yue Da Zhao Duc T.T.Phan Xiaolin Wang Joshua Jonghyun Park Zayn Biviji Christopher C.W.Hughes Abraham P.Lee | 2021 | Microsystems & Nanoengineering2021,7,1: | 2

Anti-osteoarthritis effect of a combination treatment with human adipose tissue-derived mesenchymal stem cells and thrombospondin 2 in rabbits显示文摘BACKGROUND Osteoarthritis(OA),a chronic age-related disease characterized by the slowly progressive destruction of articular cartilage,is one of the leading causes of disability. As a new strategy for treatment of OA,mesenchymal stem cells(MSCs)have the potential for articular cartilage regeneration.Meanwhile, thrombospondin 2(TSP2)promotes the chondrogenic differentiation of MSCs.AIM To investigate whether TSP2 induces chondrogenic differentiation of human adipose-derived MSCs(hADMSCs)and potentiates the therapeutic effects of hADMSCs in OA rabbits.METHODS We investigated the chondrogenic potential of TSP2 in hADMSCs by analyzing the expression of chondrogenic markers as well as NOTCH signaling genes in normal and TSP2 small interfering RNA(siRNA)-treated stem cells.Anterior cruciate ligament transection surgery was performed in male New Zealand white rabbits, and 8 wk later,hADMSCs(1.7×10^6 or 1.7×10^7 cells)were injected into the injured knees alone or in combination with intra-articular injection of TSP2(100 ng/knee)at 2-d intervals.OA progression was monitored by gross, radiological, and histological examinations.RESULTS In hADMSC culture, treatment with TSP2 increased the expression of chondrogenic markers(SOX9 and collagen Ⅱ)as well as NOTCH signaling genes(JAGGED1 and NOTCH3),which were inhibited by TSP2 siRNA treatment. In vivo,OA rabbits treated with hADMSCs or TSP2 alone exhibited lower degree of cartilage degeneration, osteophyte formation, and extracellular matrix loss 8 wk after cell transplantation.Notably, such cartilage damage was further alleviated by the combination of hADMSCs and TSP2.In addition, synovial inflammatory cytokines, especially tumor-necrosis factor-α,markedly decreased following the combination treatment.CONCLUSION The results indicate that TSP2 enhances chondrogenic differentiation of hADMSCs via JAGGED1/NOTCH3 signaling, and that combination therapy with hADMSCs and TSP2 exerts synergistic effects in the cartilage regeneration of OA joints. | Kyungha Shin Yeseul Cha Young-Hwan Ban Da Woom Seo Ehn-Kyoung Choi Dongsun Park Sung Keun Kang Jeong Chan Ra Yun-Bae Kim | 2019 | World Journal of Stem Cells2019,11,12: | 2
