| 1 | 亚洲地区肺炎支原体和肺炎衣原体在成人社区获得性肺炎中的流行病学研究显示文摘目的由Malaya大学医学中心牵头进行的非典型病原在亚洲社区获得性肺炎(CAP)患者中的流行病学研究。结果显示,肺炎支原体和肺炎衣原体分别占成人病原的11.4%和5.8%。本研究将对这2种病原所致感染的临床和实验室资料进一步总结分析。方法亚洲7个国家12个研究中心采用统一的检测方法(DNA检测及血清学方法)和判断标准,对16岁以上的CAP患者进行肺炎支原体和肺炎衣原体的检测。结果在急性肺部感染者中,肺炎支原体和肺炎衣原体肺炎的临床表现与其他病原所致者非常相似,并无特征性。肺炎支原体和肺炎衣原体在男性和女性中检出率相仿,但肺炎支原体感染在16至44岁患者中更为多见(22.2%),而肺炎衣原体感染者重症患者相对较多。88例在病程中血清抗体显著增高的肺炎支原体感染者中,48例(54.5%)患者急性期抗体阴性,如果未检测恢复期血清抗体,上述患者则会漏诊;51例肺炎衣原体感染者中急性期血清抗体阴性则较为少见(21.6%)。肺炎支原体和肺炎衣原体既往感染和(或)病原携带率分别为5.7%和7.9%。结论肺炎支原体肺炎和肺炎衣原体肺炎发病率高,诊断困难,因此在亚洲成人CAP抗感染经验治疗时宜选用对这2种病原亦有效的药物。 | 黄海辉 张婴元 汪复 Mediadora SANIEL Marissa ALEJANDRIA Seung-Chul PARK 刘又宁 Po-Ren HSUEH Ahmad M. | 3 | First report on Guanshan Biota(Cambrian Stage 4) at the stratotype area of Wulongqing Formation in Malong County, Eastern Yunnan, China显示文摘Exceptionally preserved fossils,such as those from Cambrian Burgess Shale-type fossil-Lagerstatten are critical because of their unique contributions to knowledge of the phylogenetic radiation and palaeoecological expansion of metazoans during the Cambrian explosion.Critically,these deposits provide information that is usually unobtainable from shelly and skeletonized fossils alone.The Guanshan Biota(Cambrian Series 2,Stage 4)in the Yunnan Province of South China,has produced abundant and diverse,exquisitely preserved fossils that often retain soft tissues and organs.To date,most fossils from the Guanshan Biota have been collected from localities such as Gaoloufang and Gangtoucun,which have become inaccessible due to new urban expansions and constructions of residential buildings.Here we present the first report of soft bodied fossils from a new section at Kanfuqing,close to the Wulongqing village in Malong County,approximately 3 km east of the Wulongqing Formation stratotype section.Fossils retain soft morphology,and include brachiopods with delicate marginal setae,priapulids with well-preserved sclerites and vetulicolians with entire sections of body.In addition,this fauna includes rare occurrences of trilobites preserved with soft tissues replicated as pyrite pseudomorphs after weathering.This discovery represents an important palaeogeographical extension of soft-bodied fossils of the Guanshan fauna to the east of the Xiaojiang Fault(related to Tsinning tectonic movements ca.700 Ma).The fauna from the new Kanfuqing section is similar to that reported from the Wulongqing Formation west of the Xiaojiang Fault,and thus has significant implications for early Cambrian palaeogeography,faunal successions and palaeoenvironments of eastern Yunnan. | Feiyang Chen Zhifei Zhang Marissa J. | 4 | NLRP3 inflammasome activation determines the fibrogenic potential of PM_(2.5) air pollution particles in the lung显示文摘Airborne fine particulate matter(PM_(2.5))is known to cause respiratory inflammation such as chronic obstructive pulmonary disease and lung fibrosis.NLRP3 inflammasome activation has been implicated in these diseases;however,due to the complexity in PM_(2.5)compositions,it is difficult to differentiate the roles of the components in triggering this pathway.We collected eight real-life PM_(2.5)samples for a comparative analysis of their effects on NLRP3 inflammasome activation and lung fibrosis.In vitro assays showed that although the PM_(2.5)particles did not induce significant cytotoxicity at the dose range of 12.5to 100μg/m L,they induced potent TNF-αand IL-1βproduction in PMA differentiated THP-1 human macrophages and TGF-β1 production in BEAS-2B human bronchial epithelial cells.At the dose of 100μg/m L,PM_(2.5)induced NLRP3 inflammasome activation by inducing lysosomal damage and cathepsin B release,leading to IL-1βproduction.This was confirmed by using NLRP3-and ASC-deficient cells as well as a cathepsin B inhibitor,ca-074 ME.Administration of PM_(2.5)via oropharyngeal aspiration at 2 mg/kg induced significant TGF-β1 production in the bronchoalveolar lavage fluid and collagen deposition in the lung at 21days post-exposure,suggesting PM_(2.5)has the potential to induce pulmonary fibrosis.The ranking of in vitro IL-1βproduction correlates well with the in vivo total cell count,TGF-β1 production,and collagen deposition.In summary,we demonstrate that the PM_(2.5)is capable of inducing NLRP3 inflammasome activation,which triggers a series of cellular responses in the lung to induce fibrosis. | Wei Cao Xiang Wang Jiulong Li Ming Yan Chong Hyun Chang Joshua Kim Jinhong Jiang Yu-Pei Liao Shannon Tseng Sydney Kusumoputro Candice Lau Marissa Huang Pengli Han Pengju Lu Tian Xia | 2022 | Journal of Environmental Sciences2022,34,1: | 3 |
| 5 | Endoscopic ultrasound and paracentesis in the evaluation of small volume ascites in patients with intra-abdominal malignancies显示文摘The evaluation of ascites in patients with known or suspected malignancy is a critical aspect of preoperative staging.Endoscopic evaluation by ultrasound of low volume ascites and sampling of the ascitic fluid by endoscopic ultrasound guided paracentesis(EUS-P)is both a sensitive and specific modality for the determination of peritoneal implants,which is not only an important prognostic indicator but a crucial factor in determining treatment strategy.It is common practice to utilize EUS for gastrointestinal malignancies such as pancreatic or gastric masses,with the performance of paracentesis during the same procedure for the purpose of imaging the abnormality and possibly performing fine needle aspiration for biopsy of the neoplasm itself.However,given the ability of EUS-P to adequately sample even minimal ascites,detecting much smaller volumes than traditional computed tomography or magnetic resonance imaging,EUS-P may be a useful modality for the standard metastatic workup of any newly diagnosed or suspected malignancy.In this'Field of Vision'commentary,we discuss the role of EUS-P,including the article by Suzuki et al reporting their experience with EUS-P using an automated spring-loaded needle device.We also review the utility of EUS-P for non-gastrointestinal malignancies,such as ovarian cancer,which has a high incidence of malignant ascites. | Marissa M Montgomery I Michael Leitman | 2014 | World Journal of Gastroenterology2014,20,30: | 3 |

| 7 | Neoadjuvant chemotherapy for colorectal liver metastases:A contemporary review of the literature显示文摘Colorectal carcinoma(CRC)is one of the leading causes of cancer-related deaths worldwide,and up to 50%of patients with CRC develop colorectal liver metastases(CRLM).For these patients,surgical resection remains the only opportunity for cure and long-term survival.Over the past few decades,outcomes of patients with metastatic CRC have improved significantly due to advances in systemic therapy,as well as improvements in operative technique and perioperative care.Chemotherapy in the modern era of oxaliplatin-and irinotecancontaining regimens has been augmented by the introduction of targeted biologics and immunotherapeutic agents.The increasing efficacy of contemporary systemic therapies has led to an expansion in the proportion of patients eligible for curative-intent surgery.Consequently,the use of neoadjuvant strategies is becoming progressively more established.For patients with CRLM,the primary advantage of neoadjuvant chemotherapy(NCT)is the potential to down-stage metastatic disease in order to facilitate hepatic resection.On the other hand,the routine use of NCT for patients with resectable metastases remains controversial,especially given the potential risk of inducing chemotherapy-associated liver injury prior to hepatectomy.Current guidelines recommend upfront surgery in patients with initially resectable disease and low operative risk,reserving NCT for patients with borderline resectable or unresectable disease and high operative risk.Patients undergoing NCT require close monitoring for tumor response and conversion of CRLM to resectability.In light of the growing number of treatment options available to patients with metastatic CRC,it is generally agreed that these patients are best served at tertiary centers with an expert multidisciplinary team. | Marissa Guo Ning Jin Timothy Pawlik Jordan M Cloyd | 2021 | World Journal of Gastrointestinal Oncology2021,13,9: | 2 |
