| 1 | Extraocular retinoblastoma in Indian children:clinical,imaging and histopathological features显示文摘AIM:To study eyes with extraocular dissemination(EORB),with the following aims:first to establish the mean lag period and to understand various reasons for delayed presentation,second to study their imaging profiles and third to analyze histopathological features of eyes enucleated after neoadjuvant chemotherapy.·METHODS:Prospective study of clinical and imaging features of EORBs(stage Ⅲ and Ⅳ International Retinoblastoma Staging System) presenting to a tertiary eye care centre.Histopathological features of eyes enucleated after receiving neoadjuvant chemotherapy were analyzed.A pictorial illustration of the varied imaging profile of EORB was also presented.·RESULTS:Over a period of one year,97 eyes were diagnosed with retinoblastoma;32 children(36 eyes)(37.1%) had EORB.Mean age 3.6±1.9 years,71.9% males,71.9% unilateral,3.1% with positive family history and40.6% with metastasis.On imaging,there was extrascleral involvement in 22.2%,involvement of orbital part of optic nerve in 33.3%,involvement of central nervous system in 27.8% and orbital wall involvement in2.9% eyes.On histopathological analysis of eyesenucleated after neoadjuvant chemotherapy,25.0% had no residual viable tumour tissue and rest all tumours were poorly differentiated.·CONCLUSION:There are very few human malignancies where definitive treatment is started without any confirmed histopathological diagnosis and imaging plays an important role in diagnosis and appropriate staging of the disease.Chemotherapy has a variable effect on EORB,75.0% of eyes with EORB had residual viable tumour tissue when enucleated after receiving neoadjuvant chemotherapy. | Sumita Sethi Neelam Pushker Seema Kashyap Sanjay Sharma Mridula Mehta Sameer Bakhshi Saurbhi Khurana Supriyo Ghose | 2013 | International Journal of Ophthalmology(English edition)2013，6，4： | 5 |
| 2 | Sleep apnoea and stroke显示文摘Sleep disorders have been known to physicians for a long time.In his famous aphorisms,Hippocrates said“Sleep or watchfulness exceeding that which is customary,augurs unfavorably”.Modern medicine has been able to disentangle some of the phenomena that disturb sleep.Among the most notable offenders is sleep apnoea that has gained prominence in the past few decades.It is being proposed as one of the potentially modifiable risk factors for vascular diseases including stroke.The pathological mechanisms linking sleep apnoea to vascular risk factors include hypoxia,cardiac arrhythmias,dysautonomia,impaired glucose tolerance,hypertension,dyslipidaemia and inflammation.In this article,we review literature linking sleep apnoea and stroke,including sleep apnoea as a risk factor for primary prevention with the potential to improve outcome after acute stroke and as a secondary risk factor,amenable to modification and hence vascular risk reduction. | Sameer Sharma Antonio Culebras | 2016 | Stroke & Vascular Neurology2016，1，4： | 4 |

| 12 | Unusual presentation of Erdheim-Chester disease in a child with acute lymphoblastic leukemia显示文摘Erdheim-Chester disease(ECD) is an uncommon, nonfamilial, non-Langerhans cell histiocytosis, which involves skeletal system and soft tissue usually in middle aged and elderly patients. The characteristic radiologic features include bilateral, symmetric cortical osteosclerosis of the diaphyseal and metaphyseal parts of the long bones, or bilateral symmetrically abnormal intense 99 mTechnetium labelling of the metaphyseal-diaphyseal region of the long bones, and computed tomography scan findings of 'coated aorta' or 'hairy kidneys'. ECD in childhood with osteolytic lesion is extremely rare. | 20 | Nonselective beta‐blockers reduce mortality in patients with acute‐on‐chronic liver failure显示文摘Aim:Nonselective beta‐blockers(NSBBs)can reduce the incidence of complications in patients with cirrhosis and prolong survival.The safety and efficacy of NSBBs in real‐world settings in patients with acute‐on‐chronic liver failure(ACLF)identified by the Asian Pacific Association for the Study of Liver criteria are unknown.This study aimed to assess the safety and efficacy of NSBBs in patients with Asian Pacific Association for the Study of Liver(APASL)‐defined ACLF Methods:In this retrospective,multicenter study,patients with ACLF with complete 30 days follow‐up from January 2019 to December 2021 were included.The primary objective was to compare 30‐day mortality among standard of care(SOC)and NSBB(+SOC)groups.The secondary objectives were to compare the incidence of infection,variceal bleed,and recompensation among both the groups.Results:A total of 346 patients were included.Only 26%(n=89)of them received NSBBs,while 74%(n=257)received only SOC.On Kaplan-Meier analysis,the incidence of mortality was 21%(95%confidence interval[CI]:16.20-26.50)in SOC group compared to only 8%(95%CI:3.22-15.53)in NSBB group at Day 30(p=0.005).Similarly,mortality in SOC group was 63%(95%CI:56.81-69.00)compared to 46%(95%CI:35.44-57.00)in NSBB group at 1 year(p=0.001).NSBB therapy could not reduce the incidence of infections or variceal bleed.Forty‐seven percent of patients in the SOC group and 73%of patients in the NSBB group(p<0.001)recompensated.Carvedilol was prescribed in 77.5%and propranolol in 22.5%of patients.Conclusions:NSBBs reduce mortality without any effect on infection or variceal bleed in patients with ACLF.However,only one in four ACLF patients are suitable for NSBB therapy. | Anand V.Kulkarni Madhumita Premkumar Karan Kumar Juned A.Khan Baqar A.Gora Sowmya Tirumalle Sameer Shaik Mithun Sharma Rajesh Gupta Nageshwar D.Reddy Padaki N.Rao | 2022 | Portal Hypertension & Cirrhosis2022，1，1： | 0 |
