1Antihyperglycemic activity,antihyperlipedemic activity,haematological effects and histopathological analysis of Sapindus mukorossi Gaerten fruits in streptozotocin induced diabetic rats显示文摘Objective:To investigate the antihyperglycemic and antihyperlipidemie properties of hydroalcoholic extract of fruits of Sapindus mukorossi Caerlen and its beneficial effect on haeniatological parameters with histopathological analysis in streptpzotocin induced diabetic rats.Methods:Sapindus mukorossi fruits extract(250 and 500 mg/kg body weight) and standard drug glybenclamide(0.5 mg/kg body weight) were administered to diabetic rats.Effect of extract on hyperglycemia,hyperlipidemia and hematological parameters was studied in diabetic rats.Histopathological changes in diabetic rat pancreas were also observed after extract and glybenclamide treatment.Results:Daily oral administration of Sapindus mukorossi fruits extract(250 and 500 mg/kg body weight) and glybenclamide for 20 days showed beneficial effects on blood glucose level(P<0.01) and lipid level.The extract has a favorable effect on the histopathological changes of the pancreas in streptozotocin induced diabetes.Conclusions: These findings reveal that the hydroalcoholic extract of Sapindus mukorossi fruits extract possesses antihyperglycemic and antihyperlipidemie properties.In addition.the extract can prevent various complications of diabetes and improve some haeniatological parameters.Neeraj Verma G Amresh P K Sahu Neelam Mishra Anil P Singh Ch V Rao 2012Asian Pacific Journal of Tropical Medicine2012,5,7:14
2Probiotics for gastrointestinal disorders: Proposed recommendations for children of the Asia-Pacific region显示文摘Recommendations for probiotics are available in several regions. This paper proposes recommendations for probiotics in pediatric gastrointestinal diseases in the Asia-Pacific region. Epidemiology and clinical patterns of intestinal diseases in Asia-Pacific countries were discussed. Evidence-based recommendations and randomized controlled trials in the region were revised. Cultural aspects,health management issues and economic factors were also considered. Final recommendations were approved by applying the Likert scale and rated using the GRADE system. Saccharomyces boulardii CNCM I-745(Sb) and Lactobacillus rhamnosus GG(LGG) were strongly recommended as adjunct treatment to oral rehydration therapy for gastroenteritis. Lactobacillus reuteri could also be considered. Probiotics may be considered for prevention of(with the indicated strains): antibiotic-associated diarrhea(LGG or Sb); Clostridium difficile-induced diarrhea(Sb); nosocomial diarrhea(LGG); infantile colic(L reuteri) and as adjunct treatment of Helicobacter pylori(Sb and others). Specific probiotics with a history of safe use in preterm and term infants may be considered in infants for prevention of necrotizing enterocolitis. There is insufficient evidence for recommendations in other conditions. Despite a diversity of epidemiological,socioeconomical and health system conditions,similar recommendations apply well to Asia pacific countries. 3Biodiversity,current developments and potential biotechnological applications of phosphorus-solubilizing and-mobilizing microbes:A review显示文摘As one of the most important and essential macronutrients next to nitrogen,phosphorus(P)is important for plant development,but it is the least mobile nutrient element in plant and soil.Globally,P is mined from geological sediments and added to agricultural soils so as to meet the critical requirements of crop plants for agronomic productivity.Phosphorus exists in soil in both organic and inorganic forms.The various inorganic forms of the element in soil are salts with calcium,iron,and aluminum,whereas the organic forms come from decaying vegetation and microbial residue.There is a huge diversity of plant microbiomes(epiphytic,endophytic,and rhizospheric)and soil microbiomes that have the capability to solubilize the insoluble P and make it available to plant.The main mechanism for the solubilization of inorganic P is by the production of organic acids,which lowers soil pH,or by the production of acid and alkaline phosphatases,which causes the mineralization of organic P.The P-solubilizing and-mobilizing microorganisms belong to all three domains,comprising archaea,bacteria,and eukarya.The strains belonging to the genera Arthrobacter,Bacillus,Burkholderia,Natrinema,Pseudomonas,Rhizobium,and Serratia have been reported as efficient and potential P solubilizers.The use of P solubilizers,alone or in combination with other plant growth-promoting microbes as an eco-friendly microbial consortium,could increase the P uptake of crops,increasing their yields for agricultural and environmental sustainability.Divjot KOUR Kusam Lata RANA Tanvir KAUR Neelam YADAV Ajar Nath YADAV Manish KUMAR Vinod KUMAR Harcharan Singh DHALIWAL Anil Kumar SAXENA 2021Pedosphere2021,31,1:10
4Marine Bacterial Biofilms in Bioremediation of Polycyclic Aromatic Hydrocarbons(PAHs) Under Terrestrial Condition in a Soil Microcosm显示文摘Polycyclic aromatic hydrocarbons(PAHs) in soil retain for a quite long period due to their hydrophobicity and aggregation properties. Biofilm-forming marine bacterial consortium(named as NCPR), composed of Stenotrophomonas acidaminiphila NCW702,Alcaligenes faecalis NCW402, Pseudomonas mendocina NR802, Pseudomonas aeruginosa N6P6, and Pseudomonas pseudoalcaligenes NP103, was used for the bioremediation of PAHs in a soil microcosm. Phenanthrene and pyrene were used as reference PAHs. Parameters that can affect PAH degradation, such as chemotaxis, solubility of PAHs in extracellular polymeric substances(EPS), and catechol2,3-dioxygenase(C23O) activity, were evaluated. P. aeruginosa N6P6 and P. pseudoalcaligenes NP103 showed chemotactic movement towards both the reference PAHs. The solubility of both the PAHs was increased with an increase in EPS concentration(extracted from all the 5 selected isolates). Significantly(P < 0.001) high phenanthrene(70.29%) and pyrene(55.54%) degradation was observed in the bioaugmented soil microcosm. The C23O enzyme activity was significantly(P < 0.05) higher in the bioaugmented soil microcosm with phenanthrene added at 173.26 ± 2.06 nmol min^(-1) mg^(-1) protein than with pyrene added at 61.80 ± 2.20 nmol min^(-1) mg^(-1) protein. The C23O activity and gas chromatography-mass spectrometer analyses indicated catechol pathway of phenanthrene metabolism. 5Current testing strategies for hepatitis C virus infection in blood donors and the way forward显示文摘Screening tests for blood donations are based upon sensitivity,cost-effectiveness and their suitability for high-throughput testing.Enzyme immunoassay(EIAs)for hepatitis C virus(HCV)antibodies were the initial screening tests introduced.The'first generation'antibody EIAs detected seroconversion after unduly long infectious window period.Improved HCV antibody assays still had an infectious window period around 66 d.HCV core antigen EIAs shortened the window period considerably,but high costs did not lead to widespread acceptance.A fourth-generation HCV antigen and antibody assay(combination EIA)is more convenient as two infectious markers of HCV are detected in the same assay.Molecular testing for HCV-RNA utilizing nucleic acid amplification technology(NAT)is the most sensitive assay and shortens the window period to only4 d.Implementation of NAT in many developed countries around the world has resulted in dramatic reductions in transfusion transmissible HCV and relative risk is now<1 per million donations.However,HCV serology still continues to be retained as some donations are serology positive but NAT negative.In resource constrained countries HCV screening is highly variable,depending upon infrastructure,trained manpower and financial resource.Rapid tests which do not require instrumentation and are simple to perform are used in many small and remotely located blood centres.The sensitivity as compared to EIAs is less and wherever feasible HCV antibody EIAs are most frequently used screening assays.Efforts have been made to implement combined antigen-antibody assays and even NAT in some of these countries.Neelam Marwaha Suchet Sachdev 2014World Journal of Gastroenterology2014,20,11:9
6Anti-HBc screening in Indian blood donors:Still an unresolved issue显示文摘AIM:To study the seroprevalence of antibody to hepatitis B core antigen (anti-HBc) in healthy blood donors negative for HBsAg and to evaluate whether anti-HBc detection could be adopted in India as a screening assay for HBV in addition to HBsAg. METHODS: A total of 1700 serum samples collected from HBsAg-negative healthy blood donors were tested for the presence of anti-HBc antibody (IgM + IgG). All samples reactive for anti-HBc antibody were then investigated for presence of anti-HBs and for liver function tests (LFTs). One hundred serum samples reactive for anti-HBc were tested for HBV DNA by PCR method. RESULTS: Out of 1700 samples tested, 142 (8.4%) blood samples were found to be reactive for anti-HBc. It was signif icantly lower in voluntary (6.9%) as compared to replacement donors (10.4%, P = 0.011). Seventy- two (50.7%) anti-HBc reactive samples were also reactive for anti-HBs with levels > 10 mIU/mL and 70 (49.3%) samples were non-reactive for anti-HBs, these units were labeled as anti-HBc-only. These 142 anti-HBc reactive units were also tested for liver function test. HBV DNA was detected in only 1 of 100 samples tested. CONCLUSION: Keeping in view that 8%-18% of donor population in India is anti-HBc reactive, inclusion of anti- HBc testing will lead to high discard rate. Anti-HBs as proposed previously does not seem to predict clearance of the virus. 8Antimicrobial activity of Cannabis sativa,Thuja orientalis and Psidium guajava leaf extracts against methicillin-resistant Staphylococcus aureus显示文摘Objective: This study examined the antimicrobial activity of Cannabis sativa, Thuja orientalis and Psidium guajava against methicillin-resistant Staphylococcus aureus(MRSA) and used a standardized purification protocol to determine the presence and abundance of bioactive compounds in the leaf extracts.Methods: In vitro antimicrobial activities of the ethanolic extracts of C. sativa, T. orientalis and P. guajava were tested against MRSA. The presence of bioactive molecules in these three leaves was evaluated using biochemical assays and high-performance thin-layer chromatography(HPTLC).Results: Resistance to methicillin, penicillin, oxacillin and cefoxitin was observed in each of the clinical and nonclinical MRSA isolates. However, they were still vulnerable to vancomycin. Used individually,the 50% extract of each plant leaf inhibited MRSA growth. A profound synergism was observed when C. sativa was used in combination with T. orientalis(1:1) and when P. guajava was used in combination with T. orientalis(1:1). This was shown by larger zones of inhibition. This synergism was probably due to the combined inhibitory effect of phenolics present in the leaf extracts(i.e., quercetin and gallic acid)and catechin, as detected by HPTLC.Conclusion: The leaf extracts of C. sativa, T. orientalis and P. guajava had potential for the control of both hospital-and community-acquired MRSA. 9Pancreatic necrosis:Complications and changing trend of treatment显示文摘Incidence of acute pancreatitis seems to be increasing in the Western countries and has been associated with significantly increased morbidity. Nearly 80% of the patients with acute pancreatitis undergo resolution; some develop complications including pancreatic necrosis. Infection of pancreatic necrosis is the leading cause of death in these patients. A significant portion of these patients needs surgical interventions. Traditionally, the 'gold standard' procedure has been the open surgical necrosectomy, which is now being completed by the relatively lesser invasive interventions. Minimally invasive surgical(MIS) procedures include endoscopic drainage, percutaneous image-guided catheter drainage, and retroperitoneal drainage. 10Elizabethkingia miricola :A rare non-fermenter causing urinary tract infection显示文摘Elizabethkingia miricola(E.miricola) is a gram-negative non-fermentative bacterium which is rarely encountered.It is usually misidentified or considered as a contaminant in routine microbiology laboratories due to the limitations in conventional biochemical techniques.However,with the advent of the matrix-assisted laser desorption ionization time-of-flight mass spectrometry(MALDI-TOFMS),the identification of non-fermenters has become easy and this has led to enhanced understanding of the clinical significance of these uncommonly isolated microorganisms.The genus Elizabethkingia has only two species E.meningoseptica and E.miricola.Both of these organisms are known to be multi-drug resistant and therefore,their accurate identification and antimicrobial susceptibility testing are necessary prior to the initiation of appropriate therapy.In the world literature till date,only 3 cases of sepsis caused by E.miricola have been reported.We present the first case of E.miricola association with urinary tract infection.Parakriti Gupta Kamran Zaman Balvinder Mohan Neelam Taneja 2017World Journal of Clinical Cases2017,5,5:6
11Human sperm immobilization effect of Carica papaya seed extracts: an in vitro study显示文摘Aim: To examine if the seed extracts of Carica papaya, which showed antispermatogenic/sperm immobilization properties in animal models, could cause human sperm immobilization in vitro. Methods: Chloroform extract, benzene chromatographic fraction of the chloroform extract, its methanol and ethyl acetate sub-fractions and the isolated compounds from the sub-fractions i.e., ECP 1 & 2 and MCP 1 & 2, of the seeds of Cadca papaya were used at concentrations of 0.1%, 0.5%, 1% and 2%. Sperm motility was assessed immediately after addition of extracts and every 5 minutes thereafter for 30 minutes. Results: There were dose-dependent spermicidal effects showing an instant fall in the sperm motility to less than 20 % at 2 % concentration. Isolated compounds ECP 1 & 2 were more effective inducing a motility of less than 10%. Many of the spermatozoa became vibratory on the spot. Total inhibition of motility was observed within 20 - 25 min at all concentrations of all products. Scanning and transmission electron microscopy revealed deleterious changes in the plasma membrane of the head and mid-piece of spermatozoa. Sperm viability test and the number of abnormal spermatozoa after completion of incubation suggested that the spermatozoa were infertile. The effects were spermicidal but not spermiostatic as revealed by the sperm revival test. 12Autologous bone marrow derived stem cell therapy in patients with type 2 diabetes mellitus-defining adequate administration methods显示文摘AIM To carry out randomized trial for evaluating effects of autologous bone marrow derived stem cell therapy(ABMSCT) through different routes.METHODS Bone marrow aspirate was taken from the iliac crest of patients. Bone marrow mononuclear cells were separatedand purified using centrifugation. These cells were then infused in a total of 21 patients comprising three groups of 7 patients each. Cells were infused into the superior pancreaticoduodenal artery(Group Ⅰ), splenic artery(Group Ⅱ) and through the peripheral intravenous route(Group Ⅲ). Another group of 7 patients acted as controls and a sham procedure was carried out on them(Group Ⅳ). The cells were labelled with the PET tracer F18-FDG to see their homing and in vivo distribution. Data for clinical outcome was expressed as mean ± SE. All other data was expressed as mean ± SD. Baseline and post treatment data was compared at the end of six months, using paired t-test. Cases and controls data were analyzed using independent t-test. A probability(P) value of < 0.05 was regarded as statistically significant. Measures of clinical outcome were taken as the change or improvement in the following parameters:(1) C-peptide assay;(2) HOMA-IR and HOMA-B;(3) reduction in Insulin dose; subjects who showed reduction of insulin requirement of more than 50% from baseline requirement were regarded as responders; and(4) reduction in HbA 1c. RESULTS All the patients, after being advised for healthy lifestyle changes, were evaluated at periodical intervals and at the end of 6 mo. The changes in body weight, body mass index, waist circumference and percentage of body fat in all groups were not significantly different at the end of this period. The results of intra-group comparison before and after ABMSCT at the end of six months duration was as follows:(1) the area under C-peptide response curve was increased at the end of 6 mo however the difference remained statistically non-significant(P values for fasting C-peptide were 0.973, 0.103, 0.263 and 0.287 respectively and the P values for stimulated C-peptide were 0.989, 0.395, 0.325 and 0.408 respectively for groups Ⅰ?to Ⅳ);(2) the Insulin sensitivity indices of HOMA IR and HOMA B also did not show any significant differences(P values for HOMA IR were 0.368, 0.223, 0.918 and 0.895 respectively and P values for HOMA B were 0.183, 0.664, 0.206 and 0.618 respectively for groups Ⅰto Ⅳ);(3) Group Ⅰshowed a significant reduction in Insulin dose requirement(P < 0.01). Group Ⅱ patients also achieved a significant reduction in Insulin dosages(P = 0.01). The Group Ⅰand Group Ⅱ patients together constituted the targeted group wherein the feeding arteries to pancreas were used for infusing stem cells. Group Ⅲ, which was the intravenous group, showed a non-significant reduction in Insulin dose requirement(P = 0.137). Group Ⅳ patients which comprised the control arm also showed a significant reduction in Insulin dosages at the end of six months(P < 0.05); and(4) there was a non-significant change in the Hb A1 c levels at the end of 6 mo across all groups(P = 0.355, P = 0.351, P = 0.999 and P = 0.408 respectively for groups Ⅰto Ⅳ). 13Extraocular retinoblastoma in Indian children:clinical,imaging and histopathological features显示文摘AIM:To study eyes with extraocular dissemination(EORB),with the following aims:first to establish the mean lag period and to understand various reasons for delayed presentation,second to study their imaging profiles and third to analyze histopathological features of eyes enucleated after neoadjuvant chemotherapy.·METHODS:Prospective study of clinical and imaging features of EORBs(stage Ⅲ and Ⅳ International Retinoblastoma Staging System) presenting to a tertiary eye care centre.Histopathological features of eyes enucleated after receiving neoadjuvant chemotherapy were analyzed.A pictorial illustration of the varied imaging profile of EORB was also presented.·RESULTS:Over a period of one year,97 eyes were diagnosed with retinoblastoma;32 children(36 eyes)(37.1%) had EORB.Mean age 3.6±1.9 years,71.9% males,71.9% unilateral,3.1% with positive family history and40.6% with metastasis.On imaging,there was extrascleral involvement in 22.2%,involvement of orbital part of optic nerve in 33.3%,involvement of central nervous system in 27.8% and orbital wall involvement in2.9% eyes.On histopathological analysis of eyesenucleated after neoadjuvant chemotherapy,25.0% had no residual viable tumour tissue and rest all tumours were poorly differentiated.·CONCLUSION:There are very few human malignancies where definitive treatment is started without any confirmed histopathological diagnosis and imaging plays an important role in diagnosis and appropriate staging of the disease.Chemotherapy has a variable effect on EORB,75.0% of eyes with EORB had residual viable tumour tissue when enucleated after receiving neoadjuvant chemotherapy.Sumita Sethi Neelam Pushker Seema Kashyap Sanjay Sharma Mridula Mehta Sameer Bakhshi Saurbhi Khurana Supriyo Ghose 2013International Journal of Ophthalmology(English edition)2013,6,4:5
14Donors for Resistance to Brown Planthopper Nilaparvata lugens(St?l) from Wild Rice Species显示文摘Out of 1 989 wild accessions sown in seed boxes for screening, only 1 003 wild accessions with good germination were screened against brown planthopper(BPH), Nilaparvata lugens(St?l) under greenhouse conditions. The collection comprised of accessions from 11 wild species and African cultivated rice. The germplasm was screened for BPH following standard seed box screening technique in the greenhouse. As many as 159 accessions were identified as resistant during the year 2012 based on one year screening. A selected set of BPH resistant accessions were screened again during 2013. Based on the two years screening, seven accessions of O. nivara(AA), one accession of O. officinalis(CC), seven accessions of O. australiensis(EE), five accessions of O. punctata(BB and BBCC) and nine accessions of O. latifolia(CCDD) were confirmed to be resistant to BPH. So far no BPH resistance genes have been identified and designated from O. nivara and O. punctata, hence these may act as new sources of resistance.Preetinder S. SARAO Gurpreet K. 15Pathogenesis and clinical spectrum of primary sclerosing cholangitis显示文摘Primary sclerosing cholangitis(PSC) is a disease of the biliary tract, which has been documented in the literature since 1867. This disease has a strong predilection for affecting men and can be seen in individuals as young as 2 years of age. PSC has a strong associated with inflammatory bowel disease, more commonly with ulcerative colitis, and is also part of the clinical spectrum of Ig G4-related diseases. Smallduct PSC, a variant of PSC, also has an association with inflammatory bowel disease. The exact pathogenesis of PSC is not well understood at present, however, is likely a combination of a genetic predisposition with alteration of the molecular structure of the gut. Abnormal serum liver chemistry and presence of certain autoimmune markers are usually the first indicators leading to a diagnosis of PCS, however, these may often be normal in early stages of this disease. The diagnosis is made by cholangiography, which is now considered the gold standard. PSC is a known pre-malignant condition. Such patients have an increased risk of developing cholangiocarcinoma, gallbladder neoplasia, and colon cancer. Many new treatment modalities have emerged in the recent past, including anti-tumor necrosis factor-α and anti-integrins; however, liver transplantation is the only known cure for PSC. 16Exploiting DNA repair pathways for tumor sensitization,mitigation of resistance,and normal tissue protection in radiotherapy显示文摘More than half of cancer patients are treated with radiotherapy,which kills tumor cells by directly and indirectly inducing DNA damage,including cytotoxic DNA double-strand breaks(DSBs).Tumor cells respond to these threats by activating a complex signaling network termed the DNA damage response(DDR).The DDR arrests the cell cycle,upregulates DNA repair,and triggers apoptosis when damage is excessive.The DDR signaling and DNA repair pathways are fertile terrain for therapeutic intervention.This review highlights strategies to improve therapeutic gain by targeting DDR and DNA repair pathways to radiosensitize tumor cells,overcome intrinsic and acquired tumor radioresistance,and protect normal tissue.Many biological and environmental factors determine tumor and normal cell responses to ionizing radiation and genotoxic chemotherapeutics.These include cell type and cell cycle phase distribution;tissue/tumor microenvironment and oxygen levels;DNA damage load and quality;DNA repair capacity;and susceptibility to apoptosis or other active or passive cell death pathways.We provide an overview of radiobiological parameters associated with X-ray,proton,and carbon ion radiotherapy;DNA repair and DNA damage signaling pathways;and other factors that regulate tumor and normal cell responses to radiation.We then focus on recent studies exploiting DSB repair pathways to enhance radiotherapy therapeutic gain.Jac A.Nickoloff Lynn Taylor Neelam Sharma Takamitsu A.Kato 2021Cancer Drug Resistance2021,4,2:4
17Characteristics of life-form and growth-form of plant species in an alpine ecosystem of North-West Himalaya显示文摘The present study was conducted in the alpine pastures of Tungnath (30° 14' N and 79° 13' E) to observe life-form and growth-form patterns of alpine plant species under grazed and ungrazed conditions and to work out the plant life form spectrum. Species were categorized as plant habit, height and length of growth-cycle and life-form classes according to Raunkiaer's system. The results show that in total of 68 species at grazed site, hemicryptophytes (He) accounted for 50.00% species, followed by cryptophytes (26.47%), chamaephytes (16.18%), phanerophytes (4.41%) and therophytes (2.94%). At the ungrazed site in 65 plant species, hemicryptophytes (He) accounted for 49.23% species, cryptophytes (26.15%), chamaephytes (15.38%), phanerophytes (6.15%) and therophytes (3.08 % species). In general, hemicrptophyte are dominant in both sites i.e. graged and ungrazed. Growth form categories were classified as forbs, shrubs, grasses and sedges and undershrubs, according to plant habit and height. On the basis of length of the growth cycle, species were categorized as plant species of short growth cycle, intermediate growth cycle and long growth cycle. The short forbs of plant habit and height, had the highest emergence, and grasses and sedges had the lowest emergence in representative species. 18Recent Developments in Integrated Solar Combined Cycle Power Plants显示文摘Global concern for depleting fossil fuel reserves have been compelling for evolving power generation options using renewable energy sources. The solar energy happens to be a potential source for running the power plants among renewable energy sources. Integrated Solar Combined Cycle(ISCC) power plants have gained popularity among the thermal power plants. Traditional ISCC power plants use Direct Steam Generation(DSG) approach. However, with the DSG method, the ISCC plant’s overall thermal efficiency does not increase significantly due to variations in the availability of solar energy. Thermal Energy Storage(TES) systems when integrated into the solar cycle can address such issues related to energy efficiency, process flexibility, reducing intermittency during non-solar hours. This review work focuses and discusses the developments in various components of the ISCC system including its major cycles and related parameters. The main focus is on CSP technologies, Heat Transfer Fluid(HTF), and Phase Change Material(PCM) used for thermal energy storage. Further, study includes heat enhancement methods with HTF and latent heat storage system. 19Aerobic bacterial flora of the normal conjunctiva at high altitude area of Shimla Hills in India:a hospital based study显示文摘AIM:To study the normal aerobic conjunctival flora in lower to mid Himalayan region of Shimla Hills.METHODS:Samples from normal conjunctiva of 200 individuals above the age of 13 years who visited Indira Gandhi Medical College Hospital Shimla for refraction or cataract surgery were taken by anaesthetising the conjunctival sac with sterile 4% Xylocaine solution.Samples were obtained by gently rubbing lower fornix with a sterile cotton wool swab moistened with normal saline and keeping the eye lids wide apart to avoid contamination from lid margins.It was immediately inoculated in Brain Heart infusion and subjected to standard aerobic culture and identification techniques.RESULTS:Totally,72 eyes(36%) conjunctival sacs were sterile.Predominant aerobes isolated were Staphylococcus spp.in 120(60%) followed by Haemophilus in 16(8%),diphtheroids in 10(5%),and Escherichia coli(E.coli) in 4(2%).A single aerobe was isolated from 98 eyes(49%) while 30 eyes(15%) yielded more than 1 aerobe.Staphylococcus epidermidis(S.epidermidis) was the most common bacterium,found alone in 58 eyes(29%) and in combination with another aerobe in 30 eyes(15%),followed by Staphylococcus aureus(S.aureus) in 18(9%) and 10 eyes(5%) respectively.CONCLUSION:S.epidermidis is the most common commensal organism followed by Haemophilus species.diphtheroids occupied the third position which otherwise are found more abundantly in literature.Pathogens like S.aureus,Staphylococcus citreus and E.coli were also found.Therefore,preoperative administration of topical broad spectrum antibiotics is extremely important in prophylaxis against ocular infection.The variation in microflora of normal conjunctiva in this part of world can be attributed to geographical,climatic and ethnic characteristics of the population under study.Sharma Parmeshri Dass Sharma Neelam Gupta Ravinder Kumar Singh Parul 2013International Journal of Ophthalmology(English edition)2013,6,5:4
20Colon mucosal neoplasia referred for endoscopic mucosal resection:Recurrence of adenomas and prediction of submucosal invasion显示文摘BACKGROUND Endoscopic mucosal resection(EMR)is an effective and minimally invasive alternative to surgery for large polyps and laterally spreading lesions.Gross morphology and surface characteristics may help predict submucosal invasion of the lesion(SMIL)during endoscopic evaluation.This is one of the largest singlecenter studies reporting endoscopic mucosal resection for larger(≥20 mm)colorectal lesions in the United States.AIM To determine the recurrence rate of adenomas and endoscopic features that may predict submucosal invasion of colonic mucosal neoplasia.METHODS This is a retrospective cohort study of all the patients referred for endoscopic mucosal resection for lesions≥20 mm,spanning a period from January 2013 to February 2017.The main outcome measure was identifying features that may predict submucosal invasion of mucosal lesions and predict recurrence of adenomas on follow-up surveillance colonoscopy performed at 4-6 mo.RESULTS A total of 480 patients with 500 lesions were included in the study.The median age was 68(Inter quantile range:14)with 52%males.The most common lesion location was ascending colon(161;32%).Paris classification 0-IIa(Flat elevation of mucosa-316;63.2%);Kudo Pit Pattern IIIs(192;38%)and Granular surface morphology(260;52%)were most prevalent.Submucosal invasion was present in 23(4.6%)out of 500 lesions.The independent risk factors for SMIL were Kudo Pit Pattern IIIL+IV and V(Odds ratio:4.5;P value<0.004)and Paris classification 0-IIc(Odds ratio:18.2;P value<0.01).Out of 500,354 post-endoscopic mucosal resection scars were examined at surveillance colonoscopy.Recurrence was noted in 21.8%(77 cases).CONCLUSION There was overall low prevalence of SMIL in our study.Kudo pit pattern(IIIL+IV and V)and Paris classification 0-IIc were the only factors identified as an independent risk factor for submucosal invasion.The independent risk factor for recurrence was adenoma size(>40 mm).Almost all recurrences(98.8%)were treated endoscopically.Mamoon Ur Rashid Neelam Khetpal Hammad Zafar Saeed Ali Evgeny Idrisov Yuan Du Assaf Stein Deepanshu Jain Muhammad Khalid Hasan 2020World Journal of Gastrointestinal Endoscopy2020,12,7:3
