1Differentiating Crohn's disease from intestinal tuberculosis显示文摘Differentiating Crohn's disease(CD) and intestinal tuberculosis(ITB) has remained a dilemma for most of the clinicians in the developing world, which are endemic for ITB, and where the disease burden of inflammatory bowel disease is on the rise. Although, there are certain clinical(diarrhea/hematochezia/perianal disease common in CD; fever/night sweats common in ITB), endoscopic(longitudinal/aphthous ulcers common in CD; transverse ulcers/patulous ileocaecal valve common in ITB), histologic(caseating/confluent/large granuloma common in ITB; microgranuloma common in CD), microbiologic(positive stain/culture for acid fast-bacillus in ITB), radiologic(long segment involvement/comb sign/skip lesions common in CD; necrotic lymph node/contiguous ileocaecal involvement common in ITB), and serologic differences between CD and ITB, the only exclusive features are caseation necrosis on biopsy, positive smear for acid-fast bacillus(AFB) and/or AFB culture, and necrotic lymph node on cross-sectional imaging in ITB. However,these exclusive features are limited by poor sensitivity, and this has led to the development of multiple multi-parametric predictive models. These models are also limited by complex formulae, small sample size and lack of validation across other populations. Several new parameters have come up including the latest Bayesian meta-analysis, enumeration of peripheral blood T-regulatory cells, and updated computed tomography based predictive score. However, therapeutic anti-tubercular therapy(ATT) trial, and subsequent clinical and endoscopic response to ATT is still required in a significant proportion of patients to establish the diagnosis. 2Loss of disabled-2 expression is an early event in esophageal squamous tumorigenesis显示文摘AIM: Disabled-2 (DAB2) is a candidate tumor-suppressor gene identified in ovarian cancer that negatively influences mitogenic signal transduction of growth factors and blocks ras activity. In a recent study, we observed down-regulation of DAB2 transcripts in ESCCs using cDNA microarrays. In the present study, we aimed to determine the clinical significance of loss of DAB2 protein in esophageal tumorigenesis, hypothesizing that DAB2 promoter hypermethylation-mediated gene silencing may account for loss of the protein. METHODS: DAB2 expression was analyzed by immunohistochemistry in 50 primary esophageal squamous cell carcinomas (ESCCs), 30 distinct hyperplasia, 15 dysplasia and 10 non-malignant esophageal tissues. To determine whether promoter hypermethylation contributes to loss of DAB2 expression in ESCCs, methylation status of DAB2 promoter was analyzed in DAB2 immuno-negative tumors using methylation-specifi c PCR. RESULTS: Loss of DAB2 protein was observed in 5/30 (17%) hyperplasia, 10/15 (67%) dysplasia and 34/50 (68%) ESCCs. Significant loss of DAB2 protein was observed from esophageal normal mucosa to hyperplasia, dysplasia and invasive cancer (Ptrend < 0.001). Promoter hypermethylation of DAB2 was observed in 2 of 10 (20%) DAB2 immuno-negative ESCCs. CONCLUSION: Loss of DAB2 protein expression occurs in early pre-neoplastic stages of development of esophageal cancer and is sustained down the tumorigenic pathway. 3Mineralogy and carbothermal reduction behaviour of vanadium-bearing titaniferous magnetite ore in Eastern India显示文摘Vanadium-bearing titaniferous magnetite bands hosted by Precambrian gabbro-norite-anorthositic rocks or their metamorphic equivalents were discovered in some parts of Eastern Indian Shield,containing 48%-49%Fe(total),10%-25%TiO2,and 0.3%-2.20%V2O5by mass.Mineralogical and petrological study,composition,and characterization of the vanadium-bearing titaniferous magnetite ore were carried out by scanning electron microscopy-energy dispersive X-ray(SEM-EDX),wave length X-ray florescence(WDXRF),inductively coupled plasma-atomic emission spectroscopy(ICP-AES),X-ray difraction(XRD),etc.Chemical beneficiation for valuable metals,such as Fe,Ti,and V,was performed by reduction roasting.The direct and indirect reduction were investigated by mixing the lump ore with solid activated charcoal in a closed reactor and purging the reducing gas mixture in standard reducibility index apparatus at diferent temperatures and time intervals.The reduction roasting parameters were optimized.Finally,the reduced samples were crushed and upgraded by magnetic separation.The results show that,the maximum mass fractions of magnetic and nonmagnetic parts achieved are 69.36%and 30.64%,respectively,which contain 10.6%TiO2and 0.84%V2O5in the magnetic part and 36.5%TiO2and 0.22%V2O5in the nonmagnetic part.Saikat Samanta Manik Chandra Goswami Tapan Kumar Baidya Siddhartha Mukherjee Rajib Dey 2013International Journal of Minerals,Metallurgy and Materials2013,20,10:9
4Differentiation of Crohn’s disease from intestinal tuberculosis in India in 2010显示文摘Differentiating intestinal tuberculosis from Crohn’s disease (CD) is an important clinical challenge of considerable therapeutic significance. The problem is of greatest magnitude in countries where tuberculosis continues to be highly prevalent, and where the incidence of CD is increasing. The final clinical diagnosis is based on a combination of the clinical history with endoscopic studies, culture and polymerase chain reaction for Mycobacterium tuberculosis, biopsy pathology, radiological investigations and response to therapy. In a subset of patients, surgery is required and intraoperative findings with pathological study of the resected bowel provide a definitive diagnosis. Awareness of the parameters useful in distinguishing these two disorders in each of the different diagnostic modalities is crucial to accurate decision making. Newer techniques, such as capsule endoscopy, small bowel enteroscopy and immunological assays for Mycobacterium tuberculosis, have a role to play in the differentiation of intestinal tuberculosis and CD. 7Intestinal barrier function and metabolic/liver diseases显示文摘Dietary,environmental or genetic changes contribute to the development of several metabolic and neurological diseases.Extensive research efforts have been directed towards examining how these fac-tors modulate gut bacterial composition.However,the mechanisms by which changes in gut bacterial diversity affect host function are not completely defined.Intestinal barrier dysfunction is being increasingly recognized as an early or initiating step in communicating bacterial diversity-dependent changes to the host.Here,we review the functional composition of the intestinal barrier and describe the consequences of the breach of this barrier.The functional layers of the intestinal barrier provide an initial defense and prevent the infiltration of bacteria or bacterial products from the gut lumen to the underlying lamina propria.Mesenteric lymph nodes subsequently act as the first firewall where dendritic cells from the lamina propria transport the infiltrated bacteria or bacterial products during the early stages of barrier dysfunction.When overwhelmed,the liver functions as the second firewall to detoxify bacterial endotoxins,such as lipopolysaccharide(LPS),and limit systemic involvement.Continuous translocation of LPS from the leaky gut to the liver results in liver damage or dysfunction that leads to the development of type 2 diabetes,atherosclerosis,heart disease,heart failure and also neurological dis-eases,such as Alzheimer’s and Parkinson’s disease.Thus,targeted restoration of intestinal barrier function represents a novel strategy for the attenuation of multiple diseases.Siddhartha S.Ghosh Jing Wang Paul J.Yannie Shobha Ghosh 2020Liver Research2020,4,2:4
8Plasma cathepsin L:A prognostic marker for pancreatic cancer显示文摘AIM:To assess the prognostic significance of cathepsin L,a cysteine protease that degrades the peri-tumoral tissue,in patients with pancreatic cancer.METHODS:Plasma samples from 127 pancreatic cancer patients were analyzed for cathepsin L levels by ELISA.Out of these patients,25 underwent surgeryand their paraffin-embedded tissue was analyzed for cathepsin L expression by immunohistochemistry.Survival of patients and clinicopathological parameters was correlated with cathepsin L expression in plasma and tissue using appropriate statistical analysis.RESULTS:The mean(±SD)cathepsin L in plasma samples of pancreatic cancer patients was 5.98±2.5ng/m L that was significantly higher compared to the levels in healthy controls(3.83±0.45)or chronic pancreatitis patients(3.97±1.06).Using ROC curve,a cut-off level of 5.0 ng/m L was decided for survival analysis.Elevated plasma levels of cathepsin L were found to be associated with poor prognosis(P=0.01)in multivariate analysis.The plasma levels of the protease decreased after surgery.Though no significant correlation was seen between plasma and tissue expression of this protease,a trend did emerge that high cathepsin L expression in tissue correlated with its high levels in plasma.CONCLUSION:Cathepsin L levels in plasma of pancreatic cancer patients may be used as a potential prognostic marker for the disease.Nidhi Singh Prasenjit Das Surabhi Gupta Vikas Sachdev Siddhartha Srivasatava Siddhartha Datta Gupta Ravindra Mohan Pandey Peush Sahni Shyam S Chauhan Anoop Saraya 2014World Journal of Gastroenterology2014,20,46:2

12Advances in reduction of NO_x and N_2O emission formation in an oxy-fired fluidized bed boiler显示文摘Fossil fuel combustion is one of the major means to meet the mounting global energy demand. However, the increasing NO_x and N_2 O emissions arising from fossil fuel combustion process have hazardous effects. Thus, mitigating these gases is vital to attain a sustainable environment. Interestingly, oxy-fuel combustion in fluidized bed for carbon capture and minimized NO_x emissions is strongly sustainable compare to the other approaches. It was assessed that NO_x formation and fuel-N conversion have significant limitation under oxy-fluidized bed compared to air mode and the mechanism of NO_x formation is still deficient and requires further development. In addition, this review paper discussed the potential of primary measure as low emission process with others supplementary techniques for feasible NO_x reduction. The influences of combustion mode, operating parameters, and reduction techniques such as flue gas recirculation, oxygen staging, biomass co-firing, catalyst, influence of fluidized bed design and structure, decoupling combustion and their merges are respectively evaluated.
