1Predicting the presence of adenomatous polyps during colonoscopy with National Cancer Institute Colorectal Cancer Risk-Assessment Tool显示文摘AIM To evaluate the National Cancer Institute(NCI)Colorectal Cancer(CRC)Risk Assessment Tool as a predictor for the presence of adenomatous polyps(AP) found during screening or surveillance colonoscopy.METHODS This is a retrospective single center observational study.We collected data of adenomatous polyps in each colonoscopy and then evaluated the lifetime CRC risk.We calculated the AP prevalence across risk score quintiles,odds ratios of the prevalence of AP across risk score quintiles,area under curves(AUCs)and Youden’s indexes to assess the optimal risk score cut off value for AP prevalence status.RESULTS The prevalence of AP gradually increased throughout the five risk score quintiles:i.e.,27.63%in the first and 51.35%in the fifth quintile.The odd ratios of AP prevalence in the fifth quintile compared to the first and second quintile were 2.76[confidence interval(CI):1.71-4.47]and 2.09(CI:1.32-3.30).The AUC for all patients was 0.62(CI:0.58-0.66).Youden’s Index indicated the optimal risk score cutoff value discriminating AP prevalence status was 3.60.CONCLUSION Patients with the higher NCI risk score have higher risk of AP and subsequent CRC;therefore,measures to increase the effectiveness of CRC detection in these patients include longer withdrawal time,early surveillance colonoscopy,and choosing flexible colonoscopy over other CRC screening modalities.Hassan Tariq Muhammad Umar Kamal Harish Patel Ravi Patel Muhammad Ameen Shehi Elona Maram Khalifa Sara Azam Aiyi Zhang Kishore Kumar Ahmed Baiomi Danial Shaikh Jasbir Makker 2018World Journal of Gastroenterology2018,24,34:3
2Recent advances in production,purification and applications of phycobiliproteins显示文摘An obligatory sunlight requirement for photosynthesis has exposed cyanobacteria to different quantity and quality of light. Cyanobacteria can exhibit efficient photosynthesis over broad region(450 to 650 nm) of solar spectrum with the help of brilliantly coloured pigment proteins called phycobiliproteins(PBPs). Besides light-harvesting, PBPs are found to involve in several life sustaining phenomena including photoprotection in cyanobacteria. The unique spectral features(like strong absorbance and fluorescence), proteineous nature and, some imperative properties like hepato-protective, antioxidants, anti-inflammatory and anti-aging activity of PBPs enable their use in food, cosmetics, pharmaceutical and biomedical industries. PBPs have been also noted to show beneficial effect in therapeutics of some disease like Alzheimer and cancer. Such large range of applications increases the demand of PBPs in commodity market. Therefore, the large-scale and coast effective production of PBPs is the real need of time. To fulfil this need, many researchers have been working to find the potential producer of PBPs for the production and purification of PBPs. Results of these efforts have caused the inventions of some novel techniques like mixotrophic and heterotrophic strategies for production and aqueous two phase separation for purification purpose. 3Prevalence of advanced liver fibrosis and steatosis in type-2 diabetics with normal transaminases:A prospective cohort study显示文摘BACKGROUND Nonalcoholic fatty liver disease(NAFLD)and type-2 diabetes mellitus(T2DM)have an intricate bidirectional relationship.Individuals with T2DM,not only have a higher prevalence of non-alcoholic steatosis,but also carry a higher risk of progression to nonalcoholic steatohepatitis.Experts still differ in their recommendations of screening for NAFLD among patients with T2DM.AIM To study the prevalence of NAFLD and advanced fibrosis among our patient population with T2DM.METHODS During the study period(November 2018 to January 2020),59 adult patients with T2DM and 26 non-diabetic control group individuals were recruited prospectively.Patients with known significant liver disease and alcohol use were excluded.Demographic data and lab parameters were recorded.Liver elastography was performed in all patients.RESULTS In the study group comprised of patients with T2DM and normal alanine aminotransferase levels(mean 17.8±7 U/L),81%had hepatic steatosis as diagnosed by elastography.Advanced hepatic fibrosis(stage F3 or F4)was present in 12%of patients with T2DM as compared to none in the control group.Patients with T2DM also had higher number of individuals with grade 3 steatosis[45.8%vs 11.5%,(P<0.00001)and metabolic syndrome(84.7%vs 11.5%,P<0.00001)].CONCLUSION A significant number of patients with T2DM,despite having normal transaminase levels,have NAFLD,grade 3 steatosis and advanced hepatic fibrosis as measured by liver elastography.Jasbir Makker Hassan Tariq Kishore Kumar Madhavi Ravi Danial Haris Shaikh Vivien Leung Umar Hayat Muhammad T Hassan Harish Patel Suresh Nayudu Sridhar Chilimuri 2021World Journal of Gastroenterology2021,27,6:2
4Gallbladder perforation:A single-center experience in north India and a step-up approach for management显示文摘Background: Spontaneous gallbladder perforation(GBP) is an uncommon diagnosis. This study presented the experience of managing spontaneous GBP over nine years at a large, tertiary care university hospital in north India and investigated the outcomes and treatment strategies. Methods: A retrospective review of prospectively maintained digital database of consecutive patients was performed. All patients received medical and/or surgical treatment for spontaneous GBP in our department between January 2010 and June 2018. Results: We identifed 151 patients(81 females and 70 males) with mean age of 53 years. Most common presenting features were pain(96.7%), fever(54.3%) and jaundice(31.1%). Most common cause was gallbladder stones(84.8%) followed by common bile duct stones(30.5%), xanthogranulomatous cholecystitis(17.9%) and malignancy(11.9%). As per Niemeier classifcation, 8.6% had type 1 GBP(free perforation in peritoneal cavity), 76.2% had type 2 GBP(localized perforation) and 13.2% had type 3 GBP(cholecystoenteric fstula). About 60% of the perforations were diagnosed preoperatively. Type 1 was more common in patients with diabetes and also had the worst prognosis. Surgery was performed in 109 patients(72.2%). Seven patients(4.6%) had a postoperative morbidity of Clavien-Dindo Ⅲ or higher. There were three mortalities in patients who underwent surgery. Conclusions: High index of suspicion is required for preoperative diagnosis of GBP, especially in types 2 and 3. Laparoscopic cholecystectomy can be difficult in these patients and patients may require open or partial cholecystectomy. 11Statistical optimization and in vivo evaluation of colon specific delivery system of herbal rhubarb extract显示文摘The present research effort was aimed to develop colon targeted drug delivery system(CTDDS)of rhubarb,herbal drug using a mixed film of pectin and ethyl cellulose(EC).Pectin and ethyl cellulose were mixed in various proportions to coat the core tablet to target colon.The methanolic extract of rhubarb was used and the dose of the extract in each formulation was finalized by estimating the emodin content in it by high performace thin layer chromatography(HPTLC).In vitro drug release,erosion study in presence and absence of pectinase enzyme and release constant(K)of zero order was measured for each formulation.The formulation was optimized by using 32full factorial design.Formulation having 50%pectin as a coating polymer with12%coat weight was selected as an optimized formulation(OF)on the basis of%similarity with maximum desirability but this formulation was not able to retard the release of drug in stomach and upper intestine fully.So it was further coated with Eudragit S100(ES)(3%coat weight).The optimized formulation,coated with ES indicated significant laxative activity on loperamide induced constipation in rats.The results revealed that CTDDS of rhubarb using two combined approaches of biodegrable microflora-activated system and pH-sensitive system exhibited a promising colon targeting performance.Biraju Patel Dhaval Patel Jayant Chavda Ravi Manek Dhaval Mori 2013Journal of Chinese Pharmaceutical Sciences2013,22,6:1

13Gut microbiota interactions with anti-diabetic medications and pathogenesis of type 2 diabetes mellitus显示文摘Microorganisms including bacteria,viruses,protozoa,and fungi living in the gastrointestinal tract are collectively known as the gut microbiota.Dysbiosis is the imbalance in microbial composition on or inside the body relative to healthy state.Altered Firmicutes to Bacteroidetes ratio and decreased abundance of Akkermansia muciniphila are the predominant gut dysbiosis associated with the pathogenesis of type 2 diabetes mellitus(T2DM)and metabolic syndrome.Pathophysiological mechanisms linking gut dysbiosis,and metabolic diseases and their complications include altered metabolism of short-chain fatty acids and bile acids,interaction with gut hormones,increased gut microbial metabolite trimethylamine-N-oxide,bacterial translocation/Leaky gut syndrome,and endotoxin production such as lipopolysaccharides.The association between the gut microbiota and glycemic agents,however,is much less understood and is the growing focus of research and conversation.Recent studies suggest that the gut microbiota and anti-diabetic medications are interdependent on each other,meaning that while anti-diabetic medications alter the gut microbiota,the gut microbiota also alters the efficacy of anti-diabetic medications.With increasing evidence regarding the significance of gut microbiota,it is imperative to review the role of gut microbiota in the pathogenesis of T2DM.This review also discusses the interaction between gut microbiota and the various medications used in the treatment of T2DM.Ravi Kant Lakshya Chandra Vipin Verma Priyanshu Nain Diego Bello Siddharth Patel Subash Ala Rashmi Chandra Mc Anto Antony 2022World Journal of Methodology2022,12,4:1

15Penoscrotal defect:a functional,esthetic,and psychological challenge显示文摘Aim:Penoscrotal defects may be caused by a variety of events.Reconstruction of the penoscrotal region is required not only for aesthetic appearance but also for functional and psychological reasons.Numerous techniques have been described for penoscrotal reconstruction reflecting the challenge and complexity of the region involved.This suggests that no single method is satisfactory for all types and degrees of tissue defects.This prospective study was conducted in a tertiary care hospital in India,over a period of 5 years.Methods:Eighteen patients with penoscrotal defects of varying etiology were included in the study and underwent different surgical techniques.Age of the patients ranged from 20 to 60 years.The etiology of penoscrotal defect was Fournier’s gangrene in 12 cases,trauma in 4 cases,and burn in 2 cases.The patients with Fournier’s gangrene were initially treated by debridement,drainage,and antibiotics.The penoscrotal defects were treated with local flap advancement with skin grafting(n=7),pedicled anterolateral thigh flap(n=4),gracilis muscle flap with split skin grafting(n=4),and medial thigh flap(n=3).Results:There was complete healing in 16 patients with minor complications in the form of partial skin graft loss(n=1)and wound dehiscence(n=1).Results were highly satisfactory in 6 patients,satisfactory in 8 patients,and not satisfactory in 4 patients.Scarring at the donor site was limited and acceptable.Conclusion:The vast arsenal of options for penoscrotal defect coverage ranges from skin grafting to flaps,and every case needs a customized approach with regard to its feasibility,outcome,and complication rate.Flaps should be the preferred choice over the skin grafts because of the superior functional and aesthetic results and better compliance.Durga Karki Pankaj Kumar Patel Ravi Prakash Narayan 2016Plastic and Aesthetic Research2016,3,1:0
16Deep Learning-Based Model for Detection of Brinjal Weed in the Era of Precision Agriculture显示文摘The overgrowth of weeds growing along with the primary crop in the fields reduces crop production.Conventional solutions like hand weeding are labor-intensive,costly,and time-consuming;farmers have used herbicides.The application of herbicide is effective but causes environmental and health concerns.Hence,Precision Agriculture(PA)suggests the variable spraying of herbicides so that herbicide chemicals do not affect the primary plants.Motivated by the gap above,we proposed a Deep Learning(DL)based model for detecting Eggplant(Brinjal)weed in this paper.The key objective of this study is to detect plant and non-plant(weed)parts from crop images.With the help of object detection,the precise location of weeds from images can be achieved.The dataset is collected manually from a private farm in Gandhinagar,Gujarat,India.The combined approach of classification and object detection is applied in the proposed model.The Convolutional Neural Network(CNN)model is used to classify weed and non-weed images;further DL models are applied for object detection.We have compared DL models based on accuracy,memory usage,and Intersection over Union(IoU).ResNet-18,YOLOv3,CenterNet,and Faster RCNN are used in the proposed work.CenterNet outperforms all other models in terms of accuracy,i.e.,88%.Compared to other models,YOLOv3 is the least memory-intensive,utilizing 4.78 GB to evaluate the data.Jigna Patel Anand Ruparelia Sudeep Tanwar Fayez Alqahtani Amr Tolba Ravi Sharma Maria Simona Raboaca Bogdan Constantin Neagu 2023Computers, Materials & Continua2023,77,10:0
17Filtration of sodium chloride from seawater using carbon hollow tube composed of carbon nanotubes显示文摘The present article deals with filtration of seawater to remove sodium chloride(NaCl)using filter made from organized structures of carbon nanotubes(CNTs).The filter consists of hollow carbon cylinder(length~10 cm,diameter~1 cm),which is composed of radially aligned CNTs.This carbon hollow cylinder has been synthesized by continuous spray pyrolysis of ferrocene–benzene solution in argon atmosphere.The hollow cylinder has been turned into a water filter by closing one end and keeping a small funnel at the other.Filtration of seawater(Marina Beach,Chennai,India)has been obtained both under the self pressure of seawater column in the hollow cylinder and under the difference of pressure created by enclosing the filter in a vacuum tight container.It has been found that the efficiency of filtration is about two times higher under partial vacuum(~10–2 torr)created on the filtrate(water)side.After filtration of seawater,a deposit in the inner surface of hollow cylinder has been found.This deposit has been characterized by X-ray diffraction,transmission electron microscopy and energy dispersive X-ray analysis,and it has been found that the deposit was NaCl.The filtration leads to almost complete removal of NaCl from the seawater.Chaudhary Ravi Prakash Patel Prashant Tripathi O.N.Srivastava T.P.Yadav 2014International Journal of Smart and Nano Materials2014,5,3:0
