1Recent advances in the diagnosis and treatment of acute myocardial infarction显示文摘The Third Universal Definition of Myocardial Infarction(MI) requires cardiac myocyte necrosis with an increase and/or a decrease in a patient's plasma of cardiac troponin(cT n) with at least one cT n measurement greater than the 99 th percentile of the upper normal reference limit during:(1) symptoms of myocardialischemia;(2) new significant electrocardiogram(ECG) ST-segment/T-wave changes or left bundle branch block;(3) the development of pathological ECG Q waves;(4) new loss of viable myocardium or regional wall motion abnormality identified by an imaging procedure; or(5) identification of intracoronary thrombus by angiography or autopsy.Myocardial infarction,when diagnosed,is now classified into five types.Detection of a rise and a fall of troponin are essential to the diagnosis of acute MI.However,high sensitivity troponin assays can increase the sensitivity but decrease the specificity of MI diagnosis.The ECG remains a cornerstone in the diagnosis of MI and should be frequently repeated,especially if the initial ECG is not diagnostic of MI.There have been significant advances in adjunctive pharmacotherapy,procedural techniques and stent technology in the treatment of patients with MIs.The routine use of antiplatelet agents such as clopidogrel,prasugrel or ticagrelor,in addition to aspirin,reduces patient morbidity and mortality.Percutaneous coronary intervention(PCI) in a timely manner is the primary treatment of patients with acute ST segment elevation MI.Drug eluting coronary stents are safe and beneficial with primary coronary intervention.Treatment with direct thrombin inhibitors during PCI is non-inferior to unfractionated heparin and glycoprotein Ⅱb/Ⅲa receptor antagonists and is associated with a significant reduction in bleeding.The intra-coronary use of a glycoprotein Ⅱb/Ⅲa antagonist can reduce infarct size.Pre- and post-conditioning techniques can provide additional cardioprotection.However,the incidence and mortality due to MI continues to be high despite all these recent advances.The initial ten year experience with autologous human bone marrow mononuclear cells(BMCs) in patients with MI showed modest but significant increases in left ventricular(LV) ejection fraction,decreases in LV endsystolic volume and reductions in MI size.These studies established that the intramyocardial or intracoronary administration of stem cells is safe.However,many of these studies consisted of small numbers of patients who were not randomized to BMCs or placebo.The recent LateT ime,Time,and Swiss Multicenter Trials in patientswith MI did not demonstrate significant improvement in patient LV ejection fraction with BMCs in comparison with placebo.Possible explanations include the early use of PCI in these patients,heterogeneous BMC populations which died prematurely from patients with chronic ischemic disease,red blood cell contamination which decreases BMC renewal,and heparin which decreases BMC migration.In contrast,cardiac stem cells from the right atrial appendage and ventricular septum and apex in the SCIPIO and CADUCEUS Trials appear to reduce patient MI size and increase viable myocardium.Additional clinical studies with cardiac stem cells are in progress.Koushik Reddy Asma Khaliq Robert J Henning 2015World Journal of Cardiology2015,7,5:107
2Role of the intestinal barrier in inflammatory bowel disease显示文摘A critical function of the intestinal mucosa is to form a barrier that separates luminal contents from the interstitium. The single layer of intestinal epithelial cells (IECs) serves as a dynamic interface between the host and its environment. Cell polarity and structural properties of the epithelium is complex and is important in the development of epithelial barrier function. Epithelial cells associate with each other via a series of intercellular junctions. The apical most intercellular junctional complex referred to as the Apical Junction Complex (AJC) is important in not only cell-cell recognition, but also in the regulation of paracellular movement of fluid and solutes. Defects in the intestinal epithelial barrier function have been observed in a number of intestinal disorders such as inflammatory bowel disease (IBD). It is now becoming evident that an aberrant epithelial barrier function plays a central role in the pathophysiology of IBD. 42型糖尿病患者抗原特异性T细胞功能研究及其临床意义显示文摘目的 研究2型糖尿病患者抗原特异性T细胞免疫功能特征及其对疾病的影响.方法 收集2016年8月至2017年5月于上海交通大学医学院附属新华医院内分泌科住院的2型糖尿病患者38例,以及健康体检者47名(对照组)的外周血样本,运用流式细胞技术检测2组研究对象CD4+和CD8+T细胞所占比例,测定佛波酯/离子霉素(PMA/Iono)刺激下外周血分泌干扰素(IFN)-γ、白细胞介素2( IL-2)和肿瘤坏死因子(TNF)-α 的 CD4+和 CD8+T 细胞百分比,以及 EB 病毒抗原肽刺激下分泌 IFN-γ、IL-2和TNF-α的抗原特异性 CD8+T 淋巴细胞百分比.结果 2 型糖尿病患者外周血 CD4+T 细胞所占比例和CD4+/CD8+均明显高于对照组(P<0.05),而CD8+T细胞比例在2组之间的差异无统计学意义;非特异性PMA/Iono刺激后,2型糖尿病患者外周血CD4+和CD8+T细胞中分泌IFN-γ、IL-2和TNF-α的细胞百分比与对照组相比均显著降低(P<0.05);而EB病毒抗原肽刺激后,2型糖尿病患者外周血分泌IFN-γ、IL-2和TNF-α的CD8+T细胞百分比则显著高于对照组(P<0.05),且分泌TNF-α的细胞比例与HbA1C呈显著正相关(P<0.05).结论 2型糖尿病患者CD4+和CD8+T细胞分泌能力降低,但抗原特异性免疫应答则处于异常激活状态,提示慢性高血糖可损伤部分T细胞功能,加重慢性炎症.张一帆 黄艳红 Asma Khanniche 陈颖盈 王树军 季萍 马小燕 王颖 董艳 2018中华内分泌代谢杂志2018,34,9:9
5Th1和Th17细胞在活动期肺结核病患者外周比例失衡的临床研究显示文摘结核病是全球性严重危害人类健康的传染性疾病之一,深入解析结核分枝杆菌(Mycobacterium tuberculosis,Mtb)感染过程中宿主免疫应答特征是研发和评价有效的结核疫苗的重要前提。本研究以Th1、Th17和Th22细胞免疫应答为切人点,检测抗原特异性和非特异性活化后的Th1、Th17和Th22型细胞因子分泌细胞的比例。结果显示,和正常人群相比,结核病患者的T细胞在非特异性刺激下的Th1型细胞免疫应答能力明显高于健康人,即分泌IFN-γ和TNF-α的细胞数量明显增多,但IL-17型应答能力则低于健康人群;经M.tb特异性抗原ESAT6-CFP10刺激后,活动期肺结核病患者分泌IFN-γ的T细胞比例高于健康人,而分泌TNF-α的T细胞的比例与健康人相比却显著下降,分析Th1型与Th17型细胞之比显示结核病患者要高于健康人,而Th22型细胞应答在活动期结核病患者中的水平和正常人群相近。上述研究结果显示活动性肺结核病患者外周Th1型和Th17型细胞和正常人相比存在应答失衡,这一比例失调有望为评价结核疫苗效果和抗结核免疫应答的免疫学特征提供新依据。Asma khanniche 王颖 沙巍 孙勤 曾维宏 赖允鑫 肖和平 沈浩 2012现代免疫学2012,32,5:6
6High power and stable P-doped yolk-shell structured Si@C anode simultaneously enhancing conductivity and Li^(+)diffusion kinetics显示文摘Silicon is a low price and high capacity ancxje material for lithium-ion batteries.The yolk-shell structure can effectively accommodate Si expansion to improve stability.However,the limited rate performance of Si anodes can't meet people's growing demand for high power density.Herein,the phosphorus-doped yolk-shell Si@C materials(P-doped Si@C)were prepared through carbon coating on P-doped Si/SiO_(x)matrix to obtain high power and stable devices.Therefore,the as-prepared P-doped Si@C electrodes delivered a rapid increase in Coulombic efficiency from 74.4%to 99.6%after only 6 cycles,high capacity retention of-95%over 800 cycles at 4 A·g^(-1),and great rate capability(510 mAh·g^(-1)at 35 A·g^(-1)).As a result,P-doped Si@C anodes paired with commercial activated carbon and LiFePO_(4)cathode to assemble lithium-ion capacitor(high power density of〜61,080 W·kg^(-1)at 20 A·g^(-1))and lithium-ion full cell(good rate performance with 68.3 mAh·g^(-1)at 5 C),respectively.This work can provide an effective way tofurther improve power density and stability for energy storage devices.Ming Chen Qinnan Zhou Jiantao Zai Asma Iqbal TsegayeTadesse Tsega Boxu Dong Xuejiao Liu Yuchi Zhang Changyu Yan Liang Zhao Ali Nazakat SharelPeisan E CheeTongJohn Low Xuefeng Qian 2021Nano Research2021,14,4:5
7Issues and mitigations of wind energy penetrated network:Australian network case study显示文摘Longest geographically connected Australian power system is undergoing an unprecedented transition,under the effect of increased integration of renewable energy systems. This change in generation mix has implications for the whole interconnected system designs, its operational strategies and the regulatory framework. Frequency control policies about real-time balancing of demand and supply is one of the prominent and priority operational challenge requiring urgent attention. This paper reviews the Australian electricity market structure in presence of wind energy and its governance. Various issues related to increased wind generation systems integration are discussed in detail. Currently applied mitigations along with prospective mitigation methods requiring new or improved policies are also discussed. 8Upper gastrointestinal bleeding in Scotland 2000-2010: Improved outcomes but a significant weekend effect显示文摘AIM: To assess numbers and case fatality of patients with upper gastrointestinal bleeding(UGIB),effects of deprivation and whether weekend presentation affected outcomes.METHODS: Data was obtained from Information Services Division(ISD) Scotland and National Records of Scotland(NRS) death records for a ten year period between 2000-2001 and 2009-2010. We obtained data from the ISD Scottish Morbidity Records(SMR01) database which holds data on inpatient and daycase hospital discharges from non-obstetric and nonpsychiatric hospitals in Scotland. The mortality data was obtained from NRS and linked with the ISD SMR01 database to obtain 30-d case fatality. We used 23 ICD-10(International Classification of diseases) codes which identify UGIB to interrogate database. We analysed these data for trends in number of hospital admissions with UGIB,30-d mortality over time and assessed effects of social deprivation. We compared weekend and weekday admissions for differences in 30-d mortality and length of hospital stay. We determined comorbidities for each admission to establish if comorbidities contributed to patient outcome. RESULTS: A total of 60643 Scottish residents were admitted with UGIH during January,2000 and October,2009. There was no significant change in annual number of admissions over time,but there was a statistically significant reduction in 30-d case fatality from 10.3% to 8.8%(P < 0.001) over these 10 years. Number of admissions with UGIB was higher for the patients from most deprived category(P < 0.05),although case fatality was higher for the patients from the least deprived category(P < 0.05). There was no statistically significant change in this trend between 2000/01-2009/10. Patients admitted with UGIB at weekends had higher 30-d case fatality compared with those admitted on weekdays(P < 0.001). Thirty day mortality remained significantly higher for patients admitted with UGIB at weekends after adjusting for comorbidities. Length of hospital stay was also higher overall for patients admitted at the weekend when compared to weekdays,although only reached statistical significance for the last year of study 2009/10(P < 0.0005). Patients are at increased risk for venous thromboembolic events due to various mechanisms that may lead to chronic thromboembolic pulmonary hypertension(CTEPH). 13Metformin alleviates cholestasis-associated nephropathy through regulating oxidative stress and mitochondrial function显示文摘Background and aim:Cholestasis-associated renal injury or cholemic nephropathy(CN)is a serious clinical problem.Previous studies mentioned that oxidative stress and mitochondrial impairment play a role in CN.There is no specific pharmacological intervention for CN.Metformin is an anti-diabetic drug administered for decades.On the other hand,novel pharmacological properties have emerged for this drug.The effect of metformin on oxidative stress parameters has been well-recognized in different experimental models.It has also been found that metformin positively affected mitochondrial function.The current study aimed to evaluate the effects of metformin in an animal model of CN.Methods:Rats underwent bile duct ligation(BDL)and were treated with metformin(250 and 500 mg/kg)for 14 consecutive days.Two weeks after the BDL operations,urine,serum,and kidney samples were collected and analyzed.Results:Markers of oxidative stress,including reactive oxygen species(ROS)formation,lipid peroxida-tion,protein carbonylation,depleted antioxidant capacity,and decreased glutathione(GSH)levels were detected in BDL animals.Moreover,mitochondrial indices,including adenosine triphosphate(ATP)level,dehydrogenase activity,mitochondrial membrane potential,and mitochondrial permeability,were impaired in the kidney of cholestatic rats.Renal histopathological alterations in cholestatic animals included tubular degeneration and interstitial inflammation,cast formation,and fibrosis.It was found that metformin significantly alleviated oxidative stress and improved mitochondrial indices in the kidney of cholestatic rats.Tissue histopathological alterations were also mitigated in metformin-treated groups.Conclusions:Metformin could be a candidate for managing CN.The nephroprotective role of metformin is primarily associated with its effects on oxidative stress parameters and mitochondrial function.Mohammad Mehdi Ommati Hamidreza Mohammadi Khadijeh Mousavi Negar Azarpira Omid Farshad Reyhaneh Dehghani Asma Najibi Sedigheh Kamran Hossein Niknahad Reza Heidari 2021Liver Research2021,5,3:3
14Sentinel lymph node biopsy in clinically detected ductal carcinoma in situ显示文摘AIM:To study the indications for sentinel lymph node biopsy(SLNB) in clinically-detected ductal carcinoma in situ(CD-DCIS).METHODS:A retrospective analysis of 20 patients with an initial diagnosis of pure DCIS by an image-guided core needle biopsy(CNB) between June 2006 and June 2012 was conducted at King Faisal Specialist Hospital.The accuracy of performing SLNB in CD-DCIS,the rate of sentinel and non-sentinel nodal metastasis,and the histologic underestimation rate of invasive cancer at initial diagnosis were analyzed.The inclusion criteria were a preoperative diagnosis of pure DCIS with no evidence of invasion.We excluded any patient with evidence of microinvasion or invasion.There were two cases of mammographically detected DCIS and 18 cases of CDDCIS.All our patients were diagnosed by an imageguided CNB except two patients who were diagnosed by fine needle aspiration(FNA).All patients underwent breast surgery,SLNB,and axillary lymph node dissection(ALND) if the SLN was positive.RESULTS:Twenty patients with an initial diagnosis of pure DCIS underwent SLNB,2 of whom had an ALND.The mean age of the patients was 49.7 years(range,35-70).Twelve patients(60%) were premenopausal and 8(40%) were postmenopausal.CNB was the diagnostic procedure for 18 patients,and 2 who were diagnosed by FNA were excluded from the calculation of the underestimation rate.Two out of 20 had a positive SLNB and underwent an ALND and neither had additional non sentinel lymph node metastasis.Both the sentinel visualization rate and the intraoperative sentinel identification rate were 100%.The false negative rate was 0%.Only 2 patients had a positive SLNB(10%) and neither had additional metastasis following an ALND.After definitive surgery,3 patients were upstaged to invasive ductal carcinoma(3/18 = 16.6%) and 3 other patients were upstaged to DCIS with microinvasion(3/18 = 16.6%).Therefore the histologic underestimation rate of invasive disease was 33%.CONCLUSION:SLNB in CD-DCIS is technically feasible and highly accurate.We recommend limiting SLNB to patients undergoing a mastectomy.Ahmed Yahia Al-Ameer Sahar Al Nefaie Badria Al Johani Ihab Anwar Taher Al Tweigeri Asma Tulbah Mohmmed Alshabanah Osama Al Malik 2016World Journal of Clinical Oncology2016,7,2:3
15Geostatistical Assessment of the Spatial Distribution of Some Chemical Properties in Calcareous Soils显示文摘Spatial patterns of soil fertility parameters and other extrinsic factors need to be identified to develop farming practices that match agricultural inputs with local crop needs. Little is known about the spatial structure of nutrition in Iran. The present study was conducted in a 132-ha field located in central Iran. Soil samples were collected at 0-30cm depth and were then analyzed for total nitrogen (N), available phosphorus (P), available potassium (K), available copper (Cu), available zinc (Zn), available iron (Fe) and available manganese (Mn). The results showed that the contents of soil organic matter, Cu and Zn in Marvdasht’s farms were low. The spatial distribution model and spatial dependence level for soil chemical properties varied in the field. N, K, carbonate calcium equivalent (CaCO3 ) and electrical conductivity (EC) data indicated the existence of moderate spatial dependence. The variograms for other variables revealed stronger spatial structure. The results showed a longer range value for available P (480m), followed by total N (429m). The value of other chemical properties values showed a shorter range (128to 174m). Clear patchy distribution of N, P, K, Fe, Mn, Cu and Zn were found from their spatial distribution maps. This proved that sampling strategy for estimating variability should be adapted to the different soil chemical properties and field management. 16不同小鼠腹水型肝癌细胞增殖侵袭能力与膜联蛋白A7的亚细胞定位和亚型表达研究显示文摘目的探讨在小鼠淋巴道转移潜能不同的肝癌细胞中,肝癌细胞增殖侵袭能力与膜联蛋白A7的亚细胞定位和亚型的表达。方法以小鼠腹水型肝癌高、低淋巴道转移潜能不同的细胞株Hca-F细胞和Hca-P细胞为研究对象,通过CCK8法检测细胞的增殖能力,Transwell小室检测细胞侵袭能力;Western blotting方法检测膜联蛋白A7在体外增殖侵袭能力不同的肿瘤细胞中的亚细胞定位和亚型的变化。结果发现Hca-F细胞无论是增殖能力还是侵袭能力都高于Hca-P细胞。膜联蛋白A7在Hca-F细胞和Hca-P细胞主要为47 kD亚型表达,而51 kD亚型只少量表达于细胞浆。47 kD亚型在Hca-F细胞的细胞浆、细胞膜、细胞器膜以及细胞骨架中的表达均明显高于Hca-P细胞,分别为(73%±7%)v(s49%±19%)、(36%±8%)v(s24%±9%)和(53%±7%)v(s40%±12%)(均P<0.05)。结论膜联蛋白A7的47 kDa亚型在细胞浆、细胞膜、细胞器膜和细胞骨架的定位表达,可能与肝癌细胞增殖侵袭能力密切相关。黄玉红 Asma saleem qazi Mohmmed mibrahim 张军 范姝君 唐建武 2012中国现代医学杂志2012,22,35:2

18自由锻Ti-Ni形状记忆合金的显微组织和超弹性(英文)显示文摘采用放电等离子烧结(SPS)法将元素钛粉和镍粉制备成Ti-51%Ni形状记忆合金(SMAs)。研究目的是采用自由锻二次加工以提高SPS合金的性能。对自由锻前后合金的显微组织、相变温度和超弹性进行比较。结果表明,自由锻可以显著提高Ti-Ni形状记忆合金的拉伸强度和形状记忆性能,自由锻后合金的韧性从6.8%提高到了9.2%,超弹性的应变范围从5%增加到7.5%,应变恢复速率从72%提高到92%。Ti-51%Ni合金显微组织中含有立方奥氏体相(B2)基体、单斜马氏体相(B19′)、第二相(Ti_3Ni_4,Ti_2Ni和TiNi_3)和氧化物相(Ti_4Ni_2O,Ti_3O_5)。自由锻后再经500°C时效处理的最终样品的马氏体相变温度向高温方向偏移,这是由于Ti_3Ni_4相(透射电镜下可以观察到)的析出而导致基体中Ni含量的减少。总之,自由锻可以提高Ti-51%Ni形状记忆合金的超弹性和力学性能。Abdollah BAHADOR Esah HAMZAH Katsuyoshi KONDOH Tuty ASMA ABUBAKAR Farazila YUSOF Junko UMEDA Safaa N.SAUD Mustafa K.IBRAHIM 2018Transactions of Nonferrous Metals Society of China2018,28,3:2

20Investigation of structural parameters and self-aggregation of Algerian asphaltenes in organic solvents显示文摘Elemental analysis, Fourier transform infrared(FTIR),1H-NMR, fluorescence spectroscopy, and surface tension methods have been used to characterize the molecular structure and the aggregation behaviors of two asphaltenic fractions derived, respectively, from an Algerian petroleum well and a corresponding storage tank deposit. Elemental analysis, FTIR,1H-NMR, and fluorescence spectroscopy were used to investigate the chemical composition and structural parameters of asphaltenes,while the surface tension method was used to measure the critical micelle concentration(CMC) in organic solvents with different solubility parameters and polarities in order to characterize the asphaltenes' aggregation behaviors.Results show that the unstable asphaltenes fraction extracted from the storage tank deposit possesses a higher polarity(higher heteroatoms content) and a lower aromaticity than stable asphaltenes from the petroleum well.The CMC results indicate that asphaltenes with high polarity and low aromaticity have a high solubility in polar solvents such as nitrobenzene, whereas asphaltenes with low polarity and high aromaticity are more soluble in solvents with weak polarity, like toluene.
