1Lesion-symptom mapping with NIHSS sub-scores in ischemic stroke patients显示文摘Background Lesion-symptom mapping(LSM)is a statistical technique to investigate the population-specific relationship between structural integrity and post-stroke clinical outcome.In clinical practice,patients are commonly evaluated using the National Institutes of Health Stroke Scale(NIHSS),an 11-domain clinical score to quantitate neurological deficits due to stroke.So far,LSM studies have mostly used the total NIHSS score for analysis,which might not uncover subtle structure–function relationships associated with the specific sub-domains of the NIHSS evaluation.Thus,the aim of this work was to investigate the feasibility to perform LSM analyses with sub-score information to reveal category-specific structure–function relationships that a total score may not reveal.Methods Employing a multivariate technique,LSM analyses were conducted using a sample of 180 patients with NIHSS assessment at 48-hour post-stroke from the ESCAPE trial.The NIHSS domains were grouped into six categories using two schemes.LSM was conducted for each category of the two groupings and the total NIHSS score.Results Sub-score LSMs not only identify most of the brain regions that are identified as critical by the total NIHSS score but also reveal additional brain regions critical to each function category of the NIHSS assessment without requiring extensive,specialised assessments.Conclusion These findings show that widely available sub-scores of clinical outcome assessments can be used to investigate more specific structure–function relationships,which may improve predictive modelling of stroke outcomes in the context of modern clinical stroke assessments and neuroimaging.Deepthi Rajashekar Matthias Wilms M Ethan MacDonald Serena Schimert Michael D Hill Andrew Demchuk Mayank Goyal Sean P Dukelow Nils Daniel Forkert 2022Stroke & Vascular Neurology2022,7,2:7
2Collagen/hyaluronan based hydrogels releasing sulfated hyaluronan improve dermal wound healing in diabetic mice via reducing inflammatory macrophage activity显示文摘Sustained inflammation associated with dysregulated macrophage activation prevents tissue formation and healing of chronic wounds.Control of inflammation and immune cell functions thus represents a promising approach in the development of advanced therapeutic strategies.Here we describe immunomodulatory hyaluronan/collagen(HA-AC/coll)-based hydrogels containing high-sulfated hyaluronan(sHA)as immunoregulatory component for the modulation of inflammatory macrophage activities in disturbed wound healing.Solute sHA downregulates inflammatory activities of bone marrow-derived and tissue-resident macrophages in vitro.This further affects macrophage-mediated pro-inflammatory activation of skin cells as shown in skin ex-vivo cultures.In a mouse model of acute skin inflammation,intradermal injection of sHA downregulates the inflammatory processes in the skin.This is associated with the promotion of an anti-inflammatory gene signature in skin macrophages indicating a shift of their activation profile.For in vivo translation,we designed HA-AC/coll hydrogels allowing delivery of sHA into wounds over a period of at least one week.Their immunoregulatory capacity was analyzed in a translational experimental approach in skin wounds of diabetic db/db mice,an established model for disturbed wound healing.The sHA-releasing hydrogels improved defective tissue repair with reduced inflammation,augmented pro-regenerative macrophage activation,increased vascularization,and accelerated new tissue formation and wound closure.Sophia Hauck Paula Zager Norbert Halfter Elke Wandel Marta Torregrossa Ainur Kakpenova Sandra Rother Michelle Ordieres Susann Räthel Albrecht Berg Stephanie Möller Matthias Schnabelrauch Jan C.Simon Vera Hintze Sandra Franz 2021Bioactive Materials2021,6,12:7
3Batch methanogenic fermentation experiments of wastewater from a brown coal low-temperature coke plant显示文摘Coke plant effluents with high contents of organic compounds are mainly treated by biological aerobic fermentation after physical pre-treatment.In this study,a brown coal condensate wastewater from a low temperature coking process was fermented under methanogenic conditions in discontinuous experiments.By this fermentation,acetate,propionate,and the main polyphenolic compounds(catechol,resorcinol and hydroquinone) were degraded to a level below the detection limit.The COD was reduced by 72% with a residual concentration of 2.1 g/L.This anaerobic fermented wastewater had a residual BOD 5 of 0.66 g/L and 2.2 L CH4 were formed per litre of wastewater.An abiotic pre-treatment for this wastewater with air had a negative effect on the COD reduction and decrease of colour on the methanogenic fermentation due to the autoxidation of polyphenolic compounds to humic-like compounds.This study showed that methanogenic fermentations in the treatment sequence of brown coal coking wastewaters could reduce energy consumption for aeration in further treatment processes and had the potential for a better effluent quality due to a less formation of recalcitrant humic-like compounds.Peter Kuschk Ulrich Stottmeister Yong-Jun Liu Arndt Wiessner Matthias Kǎistner Roland-Amo Mǔller 2010Journal of Environmental Sciences2010,22,2:7
4RANK-ligand and osteoprotegerin as biomarkers in the differentiation between periprosthetic joint infection and aseptic prosthesis loosening显示文摘AIM To assess serum levels of RANK-ligand(RANKL) and osteoprotegerin(OPG) as biomarkers for periprosthetic joint infection(PJI) and compare their accuracy with standard tests.METHODS One hundred and twenty patients presenting with a painful total knee or hip arthroplasty with indication for surgical revision were included in this prospective clinical trial. Based on standard diagnostics(joint aspirate, microbiological, and histological samples) and Musculoskeletal Infection Society consensus classification,patients were categorized into PJI, aseptic loosening,and control groups. Implant loosening was assessed radiographically and intraoperatively. Preoperative serum samples were collected and analyzed for RANKL, OPG, calcium, phosphate, alkaline phosphatase(AP), and the bone-specific subform of AP(b AP). Statistical analysis was carried out, testing for significant differences between the three groups and between stable and loose implants. RESULTS All three groups were identical in regards to age, gender, and joint distribution. No statistically significant differences in the serum concentration of RANKL(P = 0.16) and OPG(P = 0.45) were found between aseptic loosening and PJI, with a trend towards lower RANKL concentrations and higher OPG concentrations in the PJI group. The RANKL/OPG ratio was significant for the comparison between PJI and non-PJI(P = 0.005). A ratio > 60 ruled out PJI in all cases(specificity: 100%, 95%CI: 89, 11% to 100.0%) but only 30% of non-PJI patients crossed this threshold. The positive predictive value remained poor at any cut-off. In the differentiation between stable and loose implants, none of the parameters measured(calcium, phosphate, AP, and b AP) showed a significant difference, and only AP and b AP measurements showed a tendency towards higher values in the loosened group(with P = 0.09 for AP and P = 0.19 for b AP). 6Dysfunctional stem and progenitor cells impair fracture healing with age显示文摘Successful fracture healing requires the simultaneous regeneration of both the bone and vasculature;mesenchymal stem cells (MSCs) are directed to replace the bone tissue, while endothelial progenitor cells (EPCs) form the new vasculature that supplies blood to the fracture site. In the elderly, the healing process is slowed, partly due to decreased regenerative function of these stem and progenitor cells. MSCs from older individuals are impaired with regard to cell number, proliferative capacity, ability to migrate, and osteochondrogenic differentiation potential. The proliferation, migration and function of EPCs are also compromised with advanced age. Although the reasons for cellular dysfunction with age are complex and multidimensional, reduced expression of growth factors, accumulation of oxidative damage from reactive oxygen species, and altered signaling of the Sirtuin-1 pathway are contributing factors to aging at the cellular level of both MSCs and EPCs. Because of these geriatric-specific issues, effective treatment for fracture repair may require new therapeutic techniques to restore cellular function. Some suggested directions for potential treatments include cellular therapies, pharmacological agents, treatments targeting age-related molecular mechanisms, and physical therapeutics. 8Computer planned, image-guided combined resection and ablation for bilobar colorectal liver metastases显示文摘For patients with extensive bilobar colorectal liver metastases(CRLM),initial surgery may not be feasible and a multimodal approach including microwave ablation(MWA)provides the only chance for prolonged survival.Intraoperative navigation systems may improve the accuracy of ablation and surgical resection of so-called'vanishing lesions',ultimately improving patient outcome.Clinical application of intraoperative navigated liver surgery is illustrated in a patient undergoing combined resection/MWA for multiple,synchronous,bilobar CRLM.Regular follow-up with computed tomography(CT)allowed for temporal development of the ablation zones.Of the ten lesions detected in a preoperative CT scan,the largest lesion was resected and the others were ablated using an intraoperative navigation system.Twelve months post-surgery a new lesion(SegⅣa)was detected and treated by trans-arterial embo-lization.Nineteen months post-surgery new liver and lung metastases were detected and a palliative chemotherapy started.The patient passed away four years after initial diagnosis.For patients with extensive CRLM not treatable by standard surgery,navigated MWA/resection may provide excellent tumor control,improving longer-term survival.Intraoperative navigation systems provide precise,real-time information to the surgeon,aiding the decision-making process and substantially improving the accuracy of both ablation and resection.Regular follow-ups including 3D modeling allow for early discrimination between ablation zones and recurrent tumor lesions.Vanessa M Banz Matthias Baechtold Stefan Weber Matthias Peterhans Daniel Inderbitzin Daniel Candinas 2014World Journal of Gastroenterology2014,20,40:2
