1Current status of laparoscopic and robotic ventral mesh rectopexy for external and internal rectal prolapse显示文摘External and internal rectal prolapse with their affiliated rectocele and enterocele, are associated with debilitating symptoms such as obstructed defecation, pelvic pain and faecal incontinence. Since perineal procedures are associated with a higher recurrence rate, an abdominal approach is commonly preferred. Despite the description of greater than three hundred different procedures, thus far no clear superiority of one surgical technique has been demonstrated. Ventral mesh rectopexy(VMR) is a relatively new and promising technique to correct rectal prolapse. In contrast to the abdominal procedures of past decades, VMR avoids posterolateral rectal mobilisation and thereby minimizes the risk of postoperative constipation. Because of a perceived acceptable recurrence rate, good functional results and low mesh-related morbidity in the short to medium term, VMR has been popularized in the past decade. Laparoscopic or robotic-assisted VMR is now being progressively performed internationally and several articles and guidelines propose the procedure as the treatment of choice for rectal prolapse. 3Value of three-dimensional reconstructions in pancreatic carcinoma using multidetector CT:Initial results显示文摘AIM:To evaluate the use of three-dimensional imaging of pancreatic carcinoma using multidetector computed tomography(CT)in a prospective study.METHODS:Ten patients with suspected pancreatic tumors were examined prospectively using multidetec-tor CT(Somatom Sensation 16,Siemens,Erlangen,Germany).The images were evaluated for the pres-ence of a pancreatic carcinoma and invasion of the peripancreatic vessels and surrounding organs.Using the isotropic CT data sets,a three-dimensional image was created with automatic vascular analysis and semi-automatic segmentation of the organs and pancreatic tumor by a radiologist.The CT examinations and the three-dimensional images were presented to the sur-geon directly before and during the patient's operation using the Medical Imaging Interaction Toolkit-based software 'ReLiver'.Immediately after surgery,the value of the two images was judged by the surgeon.The operation and the histological results served as the gold standard.RESULTS:Nine patients had a pancreatic carcinoma(all pT3),and one patient had a serous cystadenoma.One tumor inf iltrated the superior mesenteric vein.The inf iltration was correctly evaluated.All carcinomas were resectable.In comparison to the CT image with axial and coronal reconstructions,the three-dimensional image was judged by the surgeons as better for operation planning and consistently described as useful.CONCLUSION:A 3D-image of the pancreas repre-sents an invaluable aid to the surgeon.However,the 3D-software must be further developed in order to be integrated into daily clinical routine.Miriam Klauβ Max Schbinger Ivo Wolf Jens Werner Hans-Peter Meinzer Hans-Ulrich Kauczor Lars Grenacher 2009World Journal of Gastroenterology2009,15,46:16
4计算机辅助设计与制作一体化玻璃纤维桩核修复漏斗状根管的抗疲劳和抗折性能显示文摘目的:评价计算机辅助设计与制作(computer aided design and computer aided manufacturing,CAD/CAM)一体化玻璃纤维桩核修复漏斗状根管的抗疲劳性能和抗折强度。方法:收集32颗离体上颌中切牙,制作漏斗状根管模型后,利用随机数字表分为4组,每组8颗牙齿,分别用4种桩核修复,第1组:CAD/CAM一体化玻璃纤维桩核;第2组:预成石英纤维桩与树脂核;第3组:铸造金合金桩核;第4组:CAD/CAM一体化氧化锆桩核。使用化学固化的树脂粘接剂Superbond C&B将制作完成的桩核粘入到预备后的根管中,为每个牙齿制作氧化锆全冠并使用Superbond C&B粘接。使用加成型硅橡胶模拟牙周膜后把制作完成的试件固定到专用的咀嚼模拟器上,以1.67 Hz的频率在5℃和50℃每2 min交替一次的水浴中沿牙体长轴45°角,用49 N的力在牙冠舌侧加载1.2×106次,再将试件用力学测试机沿牙体长轴45°角加力将桩核折断,记录抗折强度,观察折断模式。结果:咀嚼模拟结束时,预成石英纤维桩组有4个样本的牙冠脱落,剩余样本中未观察到破坏;在抗力测试中,CAD/CAM一体化玻璃纤维桩核组和铸造金合金桩核组的抗折强度分别为(441.5±103.2)N和(462.9±170.0)N,差异无统计学意义(P>0.05),但是都高于CAD/CAM一体化氧化锆桩核组[(284.1±99.0)N]和预成石英纤维桩组[(315.4±112.3)N],其差异有统计学意义(F=4.613,P<0.05)。结论:漏斗状根管使用CAD/CAM一体化玻璃纤维桩核或铸造金合金桩核修复可能能取得更好的临床效果。李智 王新知 高承志 IVO Krejci 2013北京大学学报(医学版)2013,45,1:13

7Platelet-rich plasma for regeneration of neural feedback pathways around dental implants: a concise review and outlook on future possibilities显示文摘Along with the development of new materials, advanced medical imaging and surgical techniques, osseointegrated dental implants are considered a successful and constantly evolving treatment modality for the replacement of missing teeth in patients with complete or partial edentulism. The importance of restoring the peripheral neural feedback pathway and thus repairing the lack of periodontal mechanoreceptors after tooth extraction has been highlighted in the literature. Nevertheless, regenerating the nerve fibers and reconstructing the neural feedback pathways around osseointegrated implants remain a challenge. Recent studies have provided evidence that platelet-rich plasma(PRP) therapy is a promising treatment for musculoskeletal injuries.Because of its high biological safety, convenience and usability, PRP therapy has gradually gained popularity in the clinical field.Although much remains to be learned, the growth factors from PRP might play key roles in peripheral nerve repair mechanisms.This review presents known growth factors contributing to the biological efficacy of PRP and illustrates basic and(pre-)clinical evidence regarding the use of PRP and its relevant products in peripheral nerve regeneration. 8Label-free sensing of ultralow-weight molecules with all-dielectric metasurfaces supporting bound states in the continuum显示文摘The realization of an efficient optical sensor based on a photonic crystal metasurface supporting bound states in the continuum is reported. Liquids with different refractive indices, ranging from 1.4000 to 1.4480, are infiltrated in a microfluidic chamber bonded to the sensing dielectric metasurface. A bulk liquid sensitivity of 178 nm/RIU is achieved, while a Q-factor of about 2000 gives a sensor figure of merit up to 445 in air at both visible and infrared excitations. Furthermore, the detection of ultralow-molecular-weight(186 Da) molecules is demonstrated with a record resonance shift of 6 nm per less than a 1 nm thick single molecular layer. 10High-fidelity multimode fibre-based endoscopy for deep brain in vivo imaging显示文摘Progress in neuroscience relies on new techniques for investigating the complex dynamics of neuronal networks.An ongoing challenge is to achieve minimally invasive and high-resolution observations of neuronal activity in vivo inside deep brain areas.Recently introduced methods for holographic control of light propagation in complex media enable the use of a hair-thin multimode optical fibre as an ultranarrow imaging tool.Compared to endoscopes based on graded-index lenses or fibre bundles,this new approach offers a footprint reduction exceeding an order of magnitude,combined with a significant enhancement in resolution.We designed a compact and high-speed system for fluorescent imaging at the tip of a fibre,achieving a resolution of 1.18±0.04μm across a 50-μm field of view,yielding 7-kilopixel images at a rate of 3.5 frames/s.Furthermore,we demonstrate in vivo observations of cell bodies and processes of inhibitory neurons within deep layers of the visual cortex and hippocampus of anaesthetised mice.This study paves the way for modern microscopy to be applied deep inside tissues of living animal models while exerting a minimal impact on their structural and functional properties.Sergey Turtaev Ivo T.Leite Tristan Altwegg-Boussac Janelle M.P.Pakan Nathalie L.Rochefort Tomáš Čižmár 2018Light(Science & Applications)2018,7,1:8
11Transjugular intrahepatic portosystemic shunt in liver transplant recipients显示文摘AIM:To evaluate the efficacy of transjugular intrahepatic portosystemic shunts (TIPSs) after liver transplantation (LT). METHODS: Between November 1996 and December 2005, 10 patients with severe recurrent hepatitis C virus infection (n=4), ductopenic rejection (n=5) or portal vein thrombosis (n=1) were included in this analysis. Eleven TIPSs (one patient underwent two TIPS procedures) were placed for management of therapy-refractory ascites (n=7), hydrothorax (n=2) or bleeding from colonic varices (n=1). The median time interval between LT and TIPS placement was 15 (4-158) mo. RESULTS: TIPS placement was successful in all patients. The mean portosystemic pressure gradient was reduced from 12.5 to 8.7 mmHg. Complete and partial remission could be achieved in 43% and 29% of patients with ascites. Both patients with hydrothorax did not respond to TIPS. No recurrent bleeding was seen in the patient with colonic varices. Nine of 10 patients died during the study period. Only one of two patients, who underwent retransplantation after the TIPS procedure, survived. The median survival period after TIPS placement was 3.3 (range 0.4-20) mo. The majority of patients died from sepsis with multiorgan failure.CONCLUSION: Indications for TIPS and technical performance in LT patients correspond to those in non-transplanted patients. At least partial control of therapy-refractory ascites and variceal bleeding could be achieved in most patients. 13Sensory innervation around immediately vs. delayed loaded implants: a pilot study显示文摘Although neurophysiological and psychophysical proof of osseoperception is accumulating, histomorphometric evidence for the neural mechanisms of functional compensation following immediate and delayed implant loading is still lacking. For this randomized split-mouth study, six mongrel dogs randomly received one of four treatment protocols at 36 implant-recipient sites over 16 weeks(third maxillary incisor, third and fourth mandibular premolar): immediate implant placement and immediate loading(IIP1IL);delayed implant placement and delayed loading(DIP1DL); delayed implant placement and immediate loading(DIP1IL); and natural extraction socket healing(control). Histomorphometry was performed in the peri-implant bone and soft tissues within 300 mm around the implants. Immunocytochemistry and transmission electron microscopy were used to confirm the presence of neural structures and to reveal their ultrastructural characteristics, respectively. Myelinated nerve fibres densely populated the peri-implant crestal gingival and apical regions, although they were also identified in the woven bone and in the osteons near the implant threads. Compared with the control group in the mandible, the group that received IIP1 IL showed a higher innervation(in Nmm22, 5.9461.12 vs. 3.1560.63,P,0.001) and smaller fibre diameter(in mm, 1.3760.05 vs. 1.6460.13, P50.016), smaller axon diameter(in mm, 0.8960.05 vs.1.2460.10, P50.009) and g-ratio(0.6460.04 vs. 0.7660.05, P,0.001) in the middle region around the implants. Compared with DIP1 IL in the mandible, IIP1 IL had a higher nerve density(in Nmm22, 13.2362.54 vs. 9.6461.86, P50.027), greater fibre diameter(in mm, 1.3260.02 vs. 1.2060.04, P50.021), greater axon diameter(in mm, 0.9260.01 vs. 0.8960.03, P50.035) and lower g-ratio(0.6960.01 vs. 0.7460.01, P50.033) in the apical region around the implants. 14Validation of a novel imaging approach using multi-slice CT and cone-beam CT to follow-up on condylar remodeling after bimaxillary surgery显示文摘The main goal of this study was to introduce a novel three-dimensional procedure to objectively quantify both inner and outer condylar remodelling on preoperative multi-slice computed tomography(MSCT) and postoperative cone-beam computed tomography(CBCT) images. Second, the reliability and accuracy of this condylar volume quantification method was assessed. The mandibles of 20 patients(11 female and 9 male) who underwent bimaxillary surgery were semi-automatically extracted from MSCT/CBCT scans and rendered in 3D. The resulting condyles were spatially matched by using an anatomical landmark-based registration procedure.A standardized sphere was created around each condyle, and the condylar bone volume within this selected region of interest was automatically calculated. To investigate the reproducibility of the method, inter-and intra-observer reliability was calculated for assessments made by two experienced radiologists twice five months apart in a set of ten randomly selected patients. To test the accuracy of the bone segmentation, the inner and outer bone structures of one dry mandible, scanned according to the clinical set-up, were compared with the gold standard, micro-CT. Thirty-eight condyles showed a significant(P<0.05) mean bone volume decrease of 26.4% ± 11.4%(502.9 mm^3± 268.1 mm^3). No significant effects of side, sex or age were found. Good to excellent(ICC>0.6) intra-and inter-observer reliability was observed for both MSCT and CBCT. 17Identifying more reliable parameters for the detection of change during the follow-up of mild to moderate keratoconus patients显示文摘Background:Reaching a consensus on which parameters are most reliable at detecting progressive keratoconus patients with serial topography imaging is not evident.The aim of the study was to isolate the parameters best positioned to detect keratoconus progression using the Pentacam HR^(®)measures based on the respective limits of repeatability and range of measurement.Method:Using the Pentacam HR^(®),a tolerance index was calculated on anterior segment parameters in healthy and keratoconic eyes.The tolerance index provides a scale from least to most affected parameters in terms of measurement noise relative to that observed in healthy eyes.Then,based on the“number of increments”from no disease to advanced disease,a relative utility(RU)score was also calculated.RU values close to 1 indicate parameters best positioned to detect a change in keratoconic eyes.Results:The tolerance index values indicated that 36%of ocular parameters for keratoconic eyes had repeatability limits which were wider than normative limits(worse),but 28%of the ocular parameters were narrower than normative limits(better).Considering only those parameters with a RU greater than 0.95,a small number of parameters were within this range,such as corneal curvature and asphericity indices.Conclusions:This study demonstrates that measurement error in keratoconic eyes is significantly greater than healthy eyes.Indices implemented here provide guidance on the levels of expected precision in keratoconic eyes relative to healthy eyes to aid clinicians in distinguishing real change from noise.Importantly maximal keratometry(Kmax),central corneal thickness(CCT)and thinnest corneal thickness(TCT)were highlighted as problematic indices for the follow-up of keratoconus in terms of repeatability.Ivo Guber Colm McAlinden François Majo Ciara Bergin 2017Eye and Vision2017,4,1:6
18Photoluminescent Tb^(3+)-based metal-organic framework as a sensor for detection of methanol in ethanol fuel显示文摘The sensing capability of a Tb^(3+)-metal-organic framework, based on its photoluminescence, was used for detection of methanol in ethanol fuel. It was synthesized using terbium and trimesic acid as a metal ion center and ligand, respectively. Powder X-ray diffraction, infrared spectroscopy, thermogravimetric analysis and scanning electron microscopy were employed to characterize the synthesized MOF-76 structural features. According to the results, MOF-76 was successfully obtained with minor synthetic modification and its activated form(named TbTMA) was tested as a sensor for methanol. Tb^(3+) luminescence intensity increases as the methanol concentration in ethanol fuel also increases and the water content does not affect this response. 20Six-year outcomes in neovascular age-related macular degeneration with ranibizumab显示文摘● AIM: To evaluate the outcomes of ≥6y ranibizumab therapy in neovascular age-related macular degeneration(AMD).● METHODS: HELIX was a retrospective, observational effeciveness study using medical records of patients treated in three clinics in Belgium. Patients had neovascular AMD and were initially treated with intravitreal ranibizumab(0.5 mg) between November 1, 2007 and October 31, 2008, had ≥6y of data available, and were treated on an ongoing, as-needed basis. Outcomes included best-corrected visual acuity(BCVA) and central retinal thickness(CRT).● RESULTS: The sample consisted of 88 eyes from 69 patients. Mean age was 76.4±6.5y, most patients were female(62.3%). Most eyes(62.5%) were treatment-naive, 33 previously treated eyes had received predominantly other anti-vascular endothelial growth factor agents and verteporfin. Mean baseline BCVA was 57.4±12.7 ETDRS letters and CRT was 292±86 μm. On average, patients received 20.6±11.9 ranibizumab injections over the ≥6y. Intervals between injections were on average 12.7±16.1wk. Mean change in BCVA from baseline to last observation for the sample was less than one letter(-0.9±17.3 letters), with an average loss of-3.2±15.6 letters in previously treated eyes versus a gain of 0.6±18.4 letters in treatment-na?ve eyes. When considering a loss of <15 letters over 6y as stabilization of disease, 75.9% of all eyes showed a positive(improvement or stabilization) outcome. Mean change in CRT from baseline to last observation for the sample was-26.9±148.4 μm with the greatest reduction observed in treatment-naive eyes.● CONCLUSION: This retrospective study of 69 neovascular AMD patients treated for ≥6y with ranibizumab demonstrates long-term visual stabilization.
