| 2 | Gastric intestinal metaplasia is associated with gastric dysplasia but is inversely correlated with esophageal dysplasia显示文摘AIM To determine which clinical factors might be associated with gastric intestinal metaplasia(IM) in a North American population.METHODS Pathology and endoscopy databases at an academicmedical center were reviewed to identify patients with and without gastric IM on biopsies for a retrospective cohort study. Patient demographics, insurance status, and other clinical factors were reviewed.RESULTS Four hundred and sixty-eight patients with gastric IM(mean age: 61.0 years ± 14.4 years, 55.5% female) and 171 without gastric IM(mean age: 48.8 years ± 20.8 years, 55.0% female) were compared. The endoscopic appearance of atrophic gastritis correlated with finding gastric IM on histopathology(OR = 2.05, P = 0.051). Gastric IM was associated with histologic findings of chronic gastritis(OR = 2.56, P < 0.001), gastric ulcer(OR = 6.97, P = 0.015), gastric dysplasia(OR = 6.11, P = 0.038), and gastric cancer(OR = 6.53, P = 0.027). Histologic findings of Barrett's esophagus(OR = 0.28, P = 0.003) and esophageal dysplasia(OR = 0.11, P = 0.014) were inversely associated with gastric IM. Tobacco use(OR = 1.73, P = 0.005) was associated with gastric IM.CONCLUSION Patients who smoke or have the endoscopic finding of atrophic gastritis are more likely to have gastric IM and should have screening gastric biopsies during esophagogastroduodenoscopy(EGD). | 19 | Impact of COVID-19 on presentation,management,and outcomes of acute care surgery for gallbladder disease and acute appendicitis显示文摘BACKGROUND The ongoing coronavirus disease 2019(COVID-19)pandemic has significantly disrupted both elective and acute medical care.Data from the early months suggest that acute care patient populations deferred presenting to the emergency department(ED),portending more severe disease at the time of presentation.Additionally,care for this patient population trended towards initial non-operative management.AIM To examine the presentation,management,and outcomes of patients who developed gallbladder disease or appendicitis during the pandemic.METHODS A retrospective chart review of patients diagnosed with acute cholecystitis,symptomatic cholelithiasis,or appendicitis in two EDs affiliated with a single tertiary academic medical center in Northern California between March and June,2020 and in the same months of 2019.Patients were selected through a research repository using international classification of diseases(ICD)-9 and ICD-10 codes.Across both years,313 patients were identified with either type of gallbladder disease,while 361 patients were identified with acute appendicitis.The primary outcome was overall incidence of disease.Secondary outcomes included presentation,management,complications,and 30-d re-presentation rates.Relationships between different variables were explored using Pearson’s r correlation coefficient.Variables were compared using the Welch’s t-Test,Chi-squared tests,and Fisher’s exact test as appropriate.RESULTS Patients with gallbladder disease and appendicitis both had more severe presentations in 2020.With respect to gallbladder disease,more patients in the COVID-19 cohort presented with acute cholecystitis compared to the control cohort[50%(80)vs 35%(53);P=0.01].Patients also presented with more severe cholecystitis in 2020 as indicated by higher mean Tokyo Criteria Scores[mean(SD)1.39(0.56)vs 1.16(0.44);P=0.02].With respect to appendicitis,more patients were diagnosed with a perforated appendix at presentation in 2020[20%(36)vs 16%(29);P=0.02]and a greater percentage were classified as emergent cases using the emergency severity index[63%(112)vs 13%(23);P<0.001].While a greater percentage of patients were admitted to the hospital for gallbladder disease in 2020[65%(104)vs 50%(76);P=0.02],no significant differences were observed in hospital admissions for patients with appendicitis.No significant differences were observed in length of hospital stay or operative rate for either group.However,for patients with appendicitis,30-d re-presentation rates were significantly higher in 2020[13%(23)vs 4%(8);P=0.01].CONCLUSION During the COVID-19 pandemic,patients presented with more severe gallbladder disease and appendicitis.These findings suggest that the pandemic has affected patients with acute surgical conditions. | Orly Nadell Farber Giselle I Gomez Ashley L Titan Andrea T Fisher Christopher J Puntasecca Veronica Toro Arana Arielle Kempinsky Clare E Wise Kovi E Bessoff Mary T Hawn James R Korndorffer Jr Joseph D Forrester Micaela M Esquivel | 2021 | World Journal of Gastrointestinal Surgery2021,13,8: | 0 |
| 20 | Preliminary identification of earthquake triggered multi-hazard and risk in Pleret Sub-District(Yogyakarta,Indonesia)显示文摘Yogyakarta is one of the large cities in Central Java,located on Java Island,Indonesia.The city,and the Pleret sub-district,where the study has taken place,is prone to earthquake hazards,because it is close to several seismically active zones,such as the Sunda Megathrust and the active fault known as the Opak Fault.Since a devastating earthquake of 2006,the population of the Pleret sub-district has increased significantly.Thus,the housing demand has increased,and so is the pace of low-cost housing that does not meet earthquake-safety requirements,and furthermore are often located on unstable slopes.The local alluvial material covering a jigsaw of unstable blocks and complex slope is conditions that can amplify the negative impacts of earthquakes.Within this context,this study is aiming to assess the multi-hazards and risks of earthquakes and related secondary hazards such as ground liquefaction,and coseismic landslides.To achieve this,we used geographic information systems and remote sensing methods supplemented with outcrop study and existing seismic data to derive shear-strain parameters.The results have revealed the presence of numerous uncharted active faults with movements visible from imagery and outcrops.show that the middle part of the study area has a complex geological structure,indicated by many unchartered faults in the outcrops.Using this newly mapped blocks combined with shear strain data,we reassessed the collapse probability of buildings that reach level>0.75 near the Opak River,in central Pleret sub-district.Classifying the buildings and from population distribution,we could determine that the highest risk was during nighttime as the buildings susceptible to fall are predominantly housing buildings.The secondary hazards follow a slightly different distribution with a concentration of risks in the West. | Aditya Saputra Christopher Gomez Ioannis Delikostidis Peyman Zawar-Reza Danang Sri Hadmoko Junun Sartohadi | 2021 | Geo-Spatial Information Science2021,24,2: | 0 |
