| 6 | Evaluation of Glycosaminoglycan in the Lumbar Disc Using Chemical Exchange Saturation Transfer MR at 3.0 Tesla:Reproducibility and Correlation with Disc Degeneration显示文摘Objective This study aims to explore the clinical applicability and relevance of glycosaminoglycan Chemical Exchange Saturation Transfer(gag CEST) for intervertebral disc. Methods 25 subjects ranging in age from 24 yrs to 74 yrs were enrolled. gag CEST was acquired using a single-slice TSE sequence on a 3T. Saturation used a continuous rectangular RF pulse with B1=0.8 μT and a fixed duration time =1100 ms. Sagittal image was obtained firstly without saturation pulse, and then saturated images were acquired at 52 offsets ranging from ±0.125 to ±7 parts per million(ppm). MR T2 relaxivity map was acquired at the identical location. Six subjects were scanned twice to assess scan-rescan reproducibility. Results Gag CEST intraclass correlation coefficient(ICC) of six subjects was 0.759 for nucleus pulposus(NP) and 0.508 for annulus fibrosus(AF). Bland-Altman plots showed NP had a mean difference of 0.10%(95% limits of agreement:-3.02% to 3.22%); while that of AF was 0.34%(95% limits of agreement:-2.28% to 2.95%). For the 25 subjects, gag CEST in NP decreased as disc degeneration increased, with a similar trend to T2 relaxivity. Gag CEST of AF showed a better correlation with disc degeneration than T2 relaxivity. | 7 | Disease Progression, Response to ACEI/ATRA Therapy and Influence of ACE Gene in IgA Nephritis显示文摘Various studies have shown that angiotensin-converting enzyme (ACE) gene insertion/deletion (ID) polymorphism may play a role in the progression to end stage renal failure (ESRF) in patients with IgA nephritis (IgAN). In this randomized controlled trial, patients were followed up for 5 years to determine their long-term renal outcome to ACEI/ATRA therapy and to ascertain if their ACE gene profile could play a role in determining their response to therapy. Seventy-five patients with IgAN were enlisted. Thirty-seven were on ACEI/ATRA therapy for 62 ± 5 months and thirty-eight were untreated and served as controls. All patients had their ACE gene ID polymorphism genotyped. Compared to controls, treated patients had lower serum creatinine (p < 0.001), lower proteinuria (p < 0.002) and fewer numbers progressing to ESRF (p < 0.002). Among patients with genotype II, there were less ESRF in the treatment group when compared to the untreated control group (p < 0.02). The advantage of therapy was not seen in patients with ID or DD genotypes. ACEI/ATRA therapy was found to be effective in retarding disease progression in IgAN with years to ESRF significantly extended in patients at all levels of renal function, including patients whose outcome were ESRF. Genotyping showed better response to therapy only for those with genotype II. | 10 | A cross-sectional study on compliance with topical glaucoma medication and its associated socioeconomic burden for a Chinese population显示文摘AIM: To estimate the overall drug compliance for local Chinese glaucoma patients on long-term topical treatment.METHODS: This was a retrospective cross-sectional study.Fifty-seven primary glaucoma patients from the subspecialty clinic of a publicly-funded tertiary care hospital in Hong Kong completed a questionnaire on compliance with topical glaucoma medication,attitude towards glaucoma and therapy and vision related quality of life.Noncompliance was defined as reporting missing more than or equal to 10% of the prescribed topical glaucoma medication during the 2wk immediately prior to the consultation.Relationships between noncompliance and demographics,attitude,disease and treatment status was studied.Cost was estimated with quality of life and direct medical cost involved with noncompliance.Multivariable logistic regression on noncompliance was performed on selected factors.RESULTS: Compliance was calculated as 75%(95% CI: 64%-87%) among 57 subjects(mean age 69 y,female 51%).No statistical significant relationship was established between noncompliance and any single factors or outcomes.Age(P=0.048) and forgetfulness(P=0.064) were found to be marginally significant predictive factors on noncompliance in multivariable logistic regression.Noncompliance might be related(P=0.130) to poorer self-rated vision-associated quality of life.The societal cost of noncompliance was estimated to be over 2510 life-years and US$ 3.7 million territory-wide.CONCLUSION: The compliance of Chinese glaucoma patients in Hong Kong is comparable to other parts in the world,and carries detrimental impacts on individual and societal levels.Age and forgetfulness are two possible independent predictors for noncompliance. | Man-Hin Lui Jason Chun Ho Lam Yi Lee Kwong Tak Siu Wong Pak Long Cheung Sherri Shun Yan Lai Pak Lun Yuen Wai Yiu Lam Ho Yan Chan Yiu Fai Wong Jimmy Shiu Ming Lai Kendrick Co Shih | 2017 | International Journal of Ophthalmology(English edition)2017，10，2： | 5 |
| 11 | Overview of the Shenzhen Emergency Medical Service Call Pattern显示文摘BACKGROUND:In Shenzhen,the Emergency Medical Service(EMS) system has been in service since 1997.This study aims to examine the operation of Shenzhen 120 EMS center and to identify the reasons of calling EMS.METHODS:In this retrospective quantitative descriptive study,the data from the Shenzhen 120 EMS registry in 2011 were analyzed.RESULTS:Shenzhen 120 EMS center is a communication command center.When the number of 120 are dialed,it is forwarded to the closest appropriate hospital for ambulance dispatch.In2011,the Shenzhen 120 EMS center received 153 160 ambulance calls,with an average of 420 calls per day.Calling emergency services was mainly due to traffic accidents.Trauma and other acute diseases constituted a majority of ambulance transports.The adult patients aged 15-60 years are the principal users of EMS.There are no recognized 'paramedic' doctors and nurses.The pre-hospital emergency service is under the operation of emergency departments of hospitals.Shenzhen at present does not have specialized pre-hospital training for doctors and nurses in posttrauma management.Moreover,specialized pre-hospital training,financial support,and public health education on proper use of EMS should be emphasized.CONCLUSION:The Shenzhen 120 EMS center has its own epidemiology characteristics.Traumatic injury and traffic accident are the main reasons for calling ambulance service.In-depth study emphasizing the distribution and characteristics of trauma patients is crucial to the future development of EMS. | Shuk Man Lo Yi Min Yu Lap Yip Larry Lee Mi Ling Eliza Wong Sek Ying Chair Edward J Kalinowski Tak Shing Jimmy Chan | 2012 | World Journal of Emergency Medicine2012，3，4： | 4 |

| 14 | Cystic meconium peritonitis with jejunoileal atresia:Is it associated with unfavorable outcome?显示文摘AIM To compare the outcome between patients with jejunoileal atresia(JIA) associated with cystic meconium peritonitis(CMP) and patients with isolated JIA(JIA without CMP).METHODS A retrospective study was conducted for all neonates with JIA operated in our institute from January 2005 to January 2016.Demographics including the gestation age,sex,birth weight,age at operation,the presence of associated syndrome was recorded.Clinical outcome including the type of operation performed,operative time,the need for reoperation and mortality were studied.The demographics and the outcome between the 2 groups were compared.RESULTS During the study period,53 neonates had JIA underwent operation in our institute.Seventeen neonates(32%) were associated with CMP.There was no statistical difference on the demographics in the two groups.Patients with CMP had earlier operation than patients with isolated JIA(mean 1.4 d vs 3 d,P = 0.038).Primaryanastomosis was performed in 16 patients(94%) with CMP and 30 patients(83%) with isolated JIA(P = 0.269).Patients with CMP had longer operation(mean 190 min vs 154 min,P = 0.004).There were no statistical difference the need for reoperation(3 vs 6,P = 0.606) and mortality(2 vs 1,P = 0.269) between the two groups.CONCLUSION Primary intestinal anastomosis can be performed in94% of patients with JIA associated with CMP.Although patients with CMP had longer operative time,the mortality and reoperation rates were low and were comparable to patients with isolated JIA. | Kin Wai Edwin Chan Kim Hung Lee Hei Yi Vicky Wong Siu Yan Bess Tsui Yuen Shan Wong Kit Yi Kristine Pang Jennifer Wai Cheung Mou Yuk Him Tam | 2017 | World Journal of Clinical Pediatrics2017，6，1： | 3 |

| 19 | FTIR Spectroscopy in Cultural Heritage Studies: Non-destructive Analysis of Chinese Handmade Papers显示文摘Chinese handmade papers have been the carriers of paper-based documents, paintings and calligraphies, which are of great importance for historical and cultural heritage studies. This research developed the attenuated total reflectance-Fourier transform infrared(ATR-FTIR) spectroscopic technique for the analysis of Chinese handmade papers. The test was applied to Chinese handmade papers taken from bark, hemp and bamboo groups, followed by papers from ancient books and arts collected in library and museum. Our study indicated tliat FTIR was an efficient analytical method to identify fiber types of library/museum papers of unknown origin. Moreover, information about degree of crystallinity and state of deterioration of cellulosic fibers was learned from FTIR spectra.
