| 1 | Postoperative ileus: Impact of pharmacological treatment,laparoscopic surgery and enhanced recovery pathways显示文摘Almost all patients develop postoperative ileus (POI) after abdominal surgery.POI represents the single largest factor influencing length of stay (LOS) after bowel resection,and has great implications for patients and resource utilization in health care.New methods to treat and decrease the length of POI are therefore of great importance.During the past decade,a substantial amount of research has been performed evaluating POI,and great progress has been made in our understanding and treatment of POI.Laparoscopic procedures,enhanced recovery pathways and pharmacologic treatment have been introduced.Each factor has substantially contributed to decreasing the length of POI and thus LOS after bowel resection.This editorial outlines resource utilization of POI,normal physiology of gut motility and pathogenesis of POI.Pharmacological treatment,fast track protocols and laparoscopic surgery can each have significant impact on pathways causing POI.The optimal integration of these treatment options continues to be assessed in prospective studies. | Knut Magne Augestad Conor P Delaney | 2010 | World Journal of Gastroenterology2010，16，17： | 33 |

| 3 | Pharmacogenomics of EGFR-targeted therapies in non-small cell lung cancer:EGFR and beyond显示文摘Commonly observed aberrations in epidermal growth factor receptor(EGFR) signaling have led to the development of EGFR-targeted therapies for various cancers,including non-small cell lung cancer(NSCLC).EGFR mutations and overexpression have further been shown to modulate sensitivity to these EGFR-targeted therapies in NSCLC and several other types of cancers.However,it is clear that mutations and/or genetic variations in EGFR alone cannot explain all of the variability in the responses of patients with NSCLC to EGFR-targeted therapies.For instance,in addition to EGFR genotype,genetic variations in other members of the signaling pathway downstream of EGFR or variations in parallel receptor tyrosine kinase(RTK) pathways are now recognized to have a significant impact on the efficacy of certain EGFR-targeted therapies.In this review,we highlight the mutations and genetic variations in such genes downstream of EGFR and in parallel RTK pathways.Specifically,the directional effects of these pharmacogenetic factors are discussed with a focus on two commonly prescribed EGFR inhibitors:cetuximab and erlotinib.The results of this comprehensive review can be used to optimize the treatment of NSCLC with EGFR inhibitors.Furthermore,they may provide the rationale for the design of subsequent combination therapies that involve the inhibition of EGFR. | Christopher Delaney Samuel Frank R Stephanie Huang | 2015 | Chinese Journal of Cancer2015，34，4： | 6 |

| 8 | Feasibility of confocal endomicroscopy in the diagnosis of pediatric gastrointestinal disorders显示文摘AIM:To evaluate the feasibility and utility of confocal laser endomicroscopy(CLE) in the description of normal gastrointestinal(GI) mucosa and in the diagnosis of GI disorders in children,in comparison to histology.METHODS:Forty-four patients(19 female) median age 10.9 years(range 0.7-16.6 years) with suspected or known GI pathology underwent esophago-gastro-duodenoscopy(OGD)(n = 36) and/or ileocolonoscopy(IC)(n = 31) with CLE using sodium fluorescein and acriflavine as contrast agents.Histological sections were compared with same site confocal images by two experienced pediatric and GI histopathologists and endoscopists,respectively.RESULTS:Duodenum and ileum were intubated in all but one patient undergoing OGD and IC.The median procedure time was 16.4 min(range 7-25 min) for OGD and 27.9 min(range 15-45 min) for IC.A total of 4798 confocal images were compared with 153 biopsies from the upper GI tract from 36 procedures,and 4661 confocal images were compared with 188 biopsies from the ileocolon from 31 procedures.Confocal images were comparable to conventional histology both in normal and in pathological conditions such as esophagitis,Helicobacter pylori gastritis,celiac disease,inflammatory bowel disease,colonic heterotopia,and graft versus host disease.CONCLUSION:CLE offers the prospect of targeting biopsies to abnormal mucosa,thereby increasing diagnostic yield,reducing the number of biopsies,decreasing the burden on the histopathological services,and reducing costs. | Krishnappa Venkatesh Marta Cohen Clair Evans Peter Delaney Steven Thomas Christopher Taylor Ashraf Abou-Taleb Ralf Kiesslich Mike Thomson | 2009 | World Journal of Gastroenterology2009，15，18： | 2 |
