| 1 | Management of pancreaticobiliary disease using a new intra-ductal endoscope:The Texas experience显示文摘AIM:To evaluate a new single-operator mini-endoscope, Spyglass,for its performance,feasibility and safety in the management of pancreaticobiliary disease. METHODS:In a multicenter retrospective analysis of patients undergoing intraductal endoscopy,we evaluated 128 patients(71 men,mean age 57.6 years).Indications were therapeutic(TX)in 72(56%) and diagnostic(DX)in 56(44%). RESULTS:Peroral endoscopy was performed in 121 and percutaneous in seven.TX indications included CBD stones in 41,PD stones in six,and biliary strictures in 25.DX indications included abnormal LFT’s in 15, abnormal imaging in 38 and cholangiocarcinoma staging in three.Visualization of the stone(s)was considered good in 31,fair in six,and poor in four. Advancement of the electrohydraulic lithotripsy probe was not possible in three patients and proper targeting of the lesion was partial in four patients.A holmium laser was used successfully in three patients.Ductal clearance was achieved in 37 patients after one procedure and in four patients after two procedures.Diagnosis of biliary strictures was modified in 20/29 and confirmed to be malignant in 10/23.Of the modified patients,no diagnosis was available in 17.Spyglass demonstrated malignancy in 8/17 and non-malignancy in nine.Suspected pathology by imaging studies and abnormal LFT’s was modified in 43/63(66%).Staging of cholangiocarcinoma demonstrated multicentric cholangiocarcinoma in 2/3.There was no morbidity associated with the use of Spyglass. | 2 | Hepatocellular carcinoma in non-alcoholic steatohepatitis:Current knowledge and implications for management显示文摘With the prevalence of hepatitis C virus expected to decline, the proportion of hepatocellular carcinoma(HCC) related to non-alcoholic steatohepatitis(NASH) is anticipated to increase exponentially due to the growing epidemic of obesity and diabetes. The annual incidence rate of developing HCC in patients with NASH-related cirrhosis is not clearly understood with rates ranging from 2.6%-12.8%. While multiple new mechanisms have been implicated in the development of HCC in NASH; further prospective long-term studies are needed to validate these findings. Recent evidence has shown a significant proportion of patients with non-alcoholic fatty liver disease and NASH progress to HCC in the absence of cirrhosis. Liver resection and transplantation represent curative therapeutic options in select NASHrelated HCC patients but have placed a significant burden to our healthcare resources and utilization. Currently NASH-related HCC is the fastest growing indication for liver transplant in HCC candidates. | 3 | Epidemiology of pelvic fractures in adults: Our experience at a tertiary hospital显示文摘Purpose: Pelvic fractures are severe injuries and are often associated with multiple system injuries, exacerbating the overall outcome. In India, the incidence of pelvic fractures is on a rise due to suboptimal roads and traffics but related literature regarding the overall epidemiology of these injuries is scarce and scanty. Our aim was to study the epidemiology of patients admitted with pelvic fractures at a level 1 trauma centre in India. Methods: A 16-month (between September 2015 and December 2016) prospective observational study was carried out on trauma patients with pelvic fractures at a level 1 trauma centre of a tertiary care hospital. Demography of patients, mechanism of injuries and complications were recorded prospectively. Results: We observed 75 patients who presented with pelvic fractures, where 56 were males and 19 were females. Mean age of the study populationwas 37.57 years. Road traffic accidents were the most common mode of injuries. Lateral compression injuries were the most common pattern. Associated injuries frequently encountered were lower extremities and acetabulum fractures, blunt abdominal trauma, urogenital injuries and head injuries. Out of the 75 patients, 52 were treated surgically and 23 were managed by conservative methods. Associated injuries of the extremities, head, abdomen and urogenital system indicated a longer hospital stay. Conclusion: Pelvic fractures, although belong to a relatively rare trauma subset, cause a high morbidity and mortality with considerable burden on the economy. | 4 | Performance studies on an oscillating water column employing a Savonius rotor显示文摘A new oscillating water column (OWC) design is proposed in this study to incorporate a simpler Savonius type turbine. Conventional OWC devices employ a bi-directional turbine such as a Wells or an Impulse turbine to extract energy from the air. The disadvantages of the Wells turbine include its inability to self start and stalling. The Savonius turbine is much cheaper and is an effective option at low Reynolds numbers. In the current rectangular OWC device, unlike the circular OWC, the width of entry of the capture chamber can be increased without being influenced by the diameter at the turbine section. To improve its primary capture efficiency, the front and rear walls of the OWC are inclined to minimize reflection. The Savonius rotor characteristics are studied with respect to the change in frequency of the incoming waves. The rotor rpm is sensitive to wave period and primary conversion efficiency while changes in depth only affect the rotor rpm at lower frequencies. | 17 | How do we manage post-OLT redundant bile duct?显示文摘AIM: To address endoscopic outcomes of post-Orthotopic liver transplantation (OLT) patients diagnosed with a 'redundant bile duct' (RBD). METHODS: Medical records of patients who underwent OLT at the Liver Transplant Center, University Texas Health Science Center at San Antonio Texas were retrospectively analyzed. Patients with suspected biliary tract complications (BTC) underwent endoscopic retrograde cholangiopancreatography (ERCP). All ERCP were performed by experienced biliary endoscopist. RBD was defined as a looped, sigmoid-shaped bile duct on cholangiogram with associated cholestatic liver biomarkers. Patients with biliary T-tube placement, biliary anastomotic strictures, bile leaks, bile-duct stonessludge and suspected sphincter of oddi dysfunction were excluded. Therapy included single or multiple biliary stents with or without sphincterotomy. The incidence of RBD, the number of ERCP corrective sessions, and the type of endoscopic interventions were recorded. Successful response to endoscopic therapy was defined as resolution of RBD with normalization of associated cholestasis. Laboratory data and pertinent radiographic imaging noted included the pre-ERCP period and a follow up period of 6-12 mo after the last ERCP intervention. RESULTS: One thousand two hundred and eighty-two patient records who received OLT from 1992 through 2011 were reviewed. Two hundred and twenty-four patients underwent ERCP for suspected BTC. RBD was reported in each of the initial cholangiograms. Twentyone out of 1282 (1.6%) were identified as having RBD. There were 12 men and 9 women, average age of 59.6 years. Primary indication for ERCP was cholestatic pattern of liver associated biomarkers. Nineteen out of 21 patients underwent endoscopic therapy and 2/21 required immediate surgical intervention. In the endoscopically managed group: 65 ERCP procedures were performed with an average of 3.4 per patient and 1.1 stent per session. Fifteen out of 19 (78.9%) patients were successfully managed with biliary stenting. All stents were plastic. Selection of stent size and length were based upon endoscopist preference. Stent size ranged from 7 to 11.5 Fr (average stent size 10 Fr); Stent length ranged from 6 to 15 cm (average length 9 cm). Concurrent biliary sphincterotomy was performed in 10/19 patients. Single ERCP session was sufficient in 6/15 (40.0%) patients, whereas 4/15 (26.7%) patients needed two ERCP sessions and 5/15 (33.3%) patients required more than two (average of 5.4 ERCP procedures). Single biliary stent was sufficient in 5 patients; the remaining patients required an average of 4.9 stents. Four out of 19 (21.1%) patients failed endotherapy (lack of resolution of RBD and recurrent cholestasis in the absence of biliary stent) and required either choledocojejunostomy (2/4) or percutaneous biliary drainage (2/4). Endoscopic complications included: 2/65 (3%) post-ERCP pancreatitis and 2/10 (20%)non-complicated post-sphincterotomy bleeding. No endoscopic related mortality was found. The medical records of the 15 successful endoscopically managed patients were reviewed for a period of one year after removal of all biliary stents. Eleven patients had continued resolution of cholestatic biomarkers (73%). One patient had recurrent hepatitis C, 2 patients suffered septic shock which was not associated with ERCP and 1 patient was transferred care to an outside provider and records were not available for our review. CONCLUSION: Although surgical biliary reconstruction techniques have improved, RBD represents a postOLT complication. | 20 | Synergistic effect of hybrid Himalayan Nettle/Bauhinia-vahlii fibers on physico-mechanical and sliding wear properties of epoxy composites显示文摘The cellulosic bast fibers are recognized as a justifiable and biodegradable substitute for producing moderate strength polymer composite materials because of their characteristics of renewability,ecofriendliness,and higher specific strength.Hence the aim of this research work is to fabricate Himalayan bast fibers(Nettle fiber(NF)/bauhinia vahlii fiber(BF)) based mono/hybrid epoxy composites at varying weight percentage of 2-6 wt% and evaluate the physical(void fraction and water absorption),mechanical(tensile strength,flexural strength,hardness) and sliding wear properties of as-fabricated composites.The 6 wt% NBF reinforced composites exhibited higher mechanical properties as compared to NF and BF composites with tensile strength of 34.04 MPa,flexural strength of 42.45 MPa,and hardness of 37.01 Hv respectively.The influence of various control factors(sliding velocity,NF/BF/NBF contents,normal load and sliding distance) on specific sliding wear rate of composites was evaluated by Taguchi(three factors at three levels) experimental design and the percentage contribution of these selected parameters on sliding wear performance was examined by Analysis of variance(ANOVA).The sliding wear property of as-developed composites was found to be greatly influenced by sliding velocity and the wear resistance was observed to be improved with the NF/BF/NBF contents.The wear mechanism of the as-fabricated composites has been elucidated by scanning electron microscopy analysis.The research outcomes demonstrated that the hybridization of Bauhinia vahlii fiber with Nettle fiber led to improve the mechanical and wear properties of epoxy composites. | Sandeep Kumar KKSMer Brijesh Gangil Vinay Kumar Patel | 2020 | Defence Technology(防务技术)2020,16,4: | 0 |
