1Does autologous blood transfusion during liver transplantation for hepatocellular carcinoma increase risk of recurrence?显示文摘AIM: To analyze outcomes in patients who underwent liver transplantation(LT) for hepatocellular carcinoma(HCC) and received autologous intraoperative blood salvage(IBS). METHODS: Consecutive HCC patients who underwent LT were studied retrospectively and analyzed according to the use of IBS or not. Demographic and surgical data were collected from a departmental prospective maintained database. Statistical analyses were performed using the Fisher's exact test and the Wilcoxon rank sum test to examine covariate differences between patients who underwent IBS and those who did not. Univariate and multivariate Cox regression models were developed to evaluate recurrence and death,and survival probabilities were estimated using the Kaplan-Meier method and compared by the log-rank test.RESULTS: Between 2002 and 2012,158 consecutive patients who underwent LT in the same medical center and by the same surgical team were identified. Among these patients,122(77.2%) were in the IBS group and 36(22.8%) in the non-IBS group. The overall survival(OS) and recurrence free survival(RFS) at 5 years were 59.7% and 83.3%,respectively. No differences in OS(P=0.51) or RFS(P=0.953) were detected between the IBS and non-IBS groups. On multivariate analysis for OS,degree of tumor differentiation remained as the only independent predictor. Regarding patients who received IBS,no differences were detected in OS or RFS(P=0.055 and P=0.512,respectively) according to the volume infused,even when outcomes at 90 d or longer were analyzed separately(P=0.518 for both outcomes).CONCLUSION: No differences in RFS or OS were detected according to IBS use. 2大型接地体性能的试验研究(英文)显示文摘Analytical and numerical simulation techniques have been developed for the calculation of earth resistance/ impedance and to estimate the potential distribution in the vicinity of earth electrodes.However,very little literature is available on experimental validation of these calculation techniques.To address this,a programme of experimental tests on various earth electrodes has been carried out at the lower water reservoir of a hydro pumped-storage power station in North Wales.In this paper,the earthing test facility at Dinorwig power station is described including the details of the experimental set up.The results from experimental tests on a 5 mX5 m earth grid,immersed in water and energized under ac,dc and impulse,are presented.The values of measured earth resistance/impedance and water surface potential distributions are compared with those obtained from analytical calculations and detailed numerical computer simulations.D. 3基于表层梯度电导调控的直流三支柱绝缘子界面电场优化方法显示文摘气-固界面和嵌件-环氧界面的电场强度集中效应被认为是导致直流三支柱绝缘子发生沿面闪络和支腿炸裂的重要原因,传统的结构优化难以同时有效调控两个界面的电场分布,迫切需要更合理的调控手段。通过电-热-流多物理场仿真,研究直流三支柱绝缘子界面电场分布特性,指出电荷积聚是造成界面处场强集中的主要原因。据此提出基于“类U型”梯度电导的绝缘子双界面电场联合调控策略,通过嵌件-环氧界面高电导涂层和气-固界面非线性电导涂层的协同使用,使得三支柱绝缘子体电导和表面电导呈“类U型”梯度分布,可降低绝缘子内部及表面电荷积聚程度,从而优化界面电场分布。进一步研究双界面涂层对直流三支柱绝缘子界面电场的调控作用,并针对电场调节与损耗控制目标,实现对涂层电导参数的优化。研究结果表明:当嵌件-环氧界面涂覆材料电导率不小于10^(-12)S/m、气-固界面非线性电导材料参数处于“临界饱和线”上时,支柱绝缘子嵌件-环氧界面最大电场强度可从4.48kV/mm降至0.04kV/mm,气-固界面最大切向电场强度从2.47kV/mm降至1.73kV/mm。参数优化后的双界面梯度电导涂层,可使界面电场集中效应得到有效抑制,同时将绝缘子最大电场与功率损耗控制在允许范围内,为高压直流三支柱绝缘子的优化设计提供了基础依据。胡琦 李庆民 刘智鹏 刘衡 AManu Haddad 2022电工技术学报2022,37,7:9
4Strength properties of soft clay treated with mixture of nano-SiO_2 and recycled polyester fiber显示文摘This paper investigates the effect of recycled polyester fiber, produced from polyethylene(PET) bottles, in combination with nano-Si O2 as a new stabilizer to improve the mechanical properties of soils. We intend to study the effect of adding nano-Si O2 and recycled polyester fiber on soil engineering properties,especially the shear strength and unconfined compressive strength(UCS), using clayey soil with low liquid limit. Three different combinations of fiber-soil ratios ranging between 0.1% and 0.5% as well as three different combinations of nano-soil ratios ranging between 0.5% and 1% are used. The shear strength and UCS of treated specimens are obtained from direct shear test and unconfined compression test, respectively. Results of this study show that the addition of recycled polyester fiber and nano-Si O2 increases the strength of soil specimens. Both the shear strength and UCS are improved by increasing the contents of recycled polyester fiber and nano-Si O2 in the soil mixture. The increase in the nano-Si O2 content leads to a reduction in failure strain, but the increase in the content of recycled polyester fiber leads to an increase in failure strain. The increase in the contents of recycled polyester fiber and nanoSi O2 leads to an increase in elastic modulus of soils. 5Review of evolution of tunnel position in anterior cruciate ligament reconstruction显示文摘Anterior cruciate ligament(ACL) rupture is one of the commonest knee sport injuries. The annual incidence of the ACL injury is between 100000-200000 in the United States. Worldwide around 400000 ACL reconstructions are performed in a year. The goal of ACL reconstruction is to restore the normal knee anatomy and kinesiology. The tibial and femoral tunnel placements are of primordial importance in achieving this outcome. Otherfactors that influence successful reconstruction are types of grafts, surgical techniques and rehabilitation programmes. A comprehensive understanding of ACL anatomy has led to the development of newer techniques supplemented by more robust biological and mechanical concepts. In this review we are mainly focussing on the evolution of tunnel placement in ACL reconstruction, focusing on three main categories, i.e., anatomical, biological and clinical outcomes. The importance of tunnel placement in the success of ACL reconstruction is well researched. Definite clinical and functional data is lacking to establish the superiority of the single or double bundle reconstruction technique. 6Chronic ischemic mitral valve regurgitation and surgical perspectives显示文摘Chronic ischemic mitral valve regurgitation is a result of disturbed left ventricular geometry secondary to myocardial ischemia in the absence of intrinsic mitral valve pathology. It is a common complication after myocardial infarction, and patients who have ischemic mitral regurgitation(IMR) have a worse prognosis compared to patients who have ischemic heart disease alone, and this is directly related to the severity of IMR. Medical therapy has limited efficacy, and surgical options including various repair techniques and valve replacement had been tried with variable success. 7Chronic permanent hypoxemia predisposes to mild elevation of liver stiffness显示文摘AIM:To evaluate the impact of long term permanent hypoxemia noticed in patients with non operated congenital cyanogenic cyanotic cardiopathy on liver stiffness.METHODS:We included ten adult patients with non operated inoperate cyanotic cardiopathy and ten matched patients for age and gender admitted to the gastroenterology department for proctologic diseases;Clinical and laboratory data were collected[age,gender,body mass index,oxygen saturation,glutamate oxaloacetate transaminase(GOT),glutamate pyruvate transaminase(GPT),glycemia and cholesterol].Measurement of hepatic stiffness by transient elastography was carried out in all patients using the Fibroscan device.All patients underwent an echocardiography to eliminate congestive heart failure.RESULTS:Among the patients with cyanotic cardiopathy,median liver stiffness 5.9±1.3 kPa was greater than control group(4.7±0.4 kPa)(P=0.008).Median levels of GOT,GPT,gamma-glutamyltransferase,glycemia and cholesterol were comparable in cardiopathy and control group.In regression analysis including age,gender,body mass index,oxygen saturation,GOT,GPT,glycemia,cholesterol showed that only oxygen saturation was related to liver stiffness(r=-0.63 P=0.002).CONCLUSION:Chronic permanent hypoxemia can induce mild increase of liver stiffness,but further studies are needed to explore the histological aspects of liver injury induced by chronic permanent hypoxemia.Mohamed Tahiri Abdenasser Drighil Yasmine Jalal Dounia Ghellab Wafaa Hliwa Haddad Fouad Wafaa Badre Ahmad Bellabah Rachida Habbal Rhimou Alaoui 2014World Journal of Gastroenterology2014,20,30:5
8直流电压下玻璃纤维的荷电运动过程及桥接放电现象显示文摘气体绝缘全封闭组合电器/气体绝缘金属封闭输电线路(gas insulated switchgear/gas insulated metal-enclosed transmission line,GIS/GIL)在制造、运输、装配过程中容易引入绝缘纤维,是造成设备放电故障的原因之一。该文通过搭建封闭式平行电极板实验平台,获得玻璃纤维的启举与运动特性;进一步通过理论计算建立纤维的荷电与受力模型,研究直流电压下玻璃纤维的运动与放电特性。结果表明,玻璃纤维的临界启举电压与纤维直径的1/2次方近似呈正比关系,与纤维长度基本无关。玻璃纤维荷电启举后,在电极间往复运动中会分散成若干纤维簇,而带有不同极性电荷的纤维簇首尾相接,贯通电极,引发“桥接”现象。玻璃纤维的跳动会严重畸变纤维尖端电场,降低气隙的击穿电压,较无纤维时下降约7%~20%。当玻璃纤维出现桥接现象时,放电会沿着纤维小桥表面发生,气隙击穿电压下降约34%。所得结果阐释了直流应力下玻璃纤维的运动行为及放电规律,可为GIS/GIL设备的异物防护提供基础依据。胡琦 李庆民 刘智鹏 刘衡 梁瑞雪 MANU Haddad 2022中国电机工程学报2022,42,6:4
9界面电荷注入与积聚行为对直流GIS/GIL绝缘子沿面电场的影响分析显示文摘气体绝缘组合电器(gas insulated switchgear,GIS)和气体绝缘金属封闭输电线路(gas insulated metalenclosed transmission line,GIL)运行电流大,设备内部存在明显的温度梯度分布,造成高压电极附近电荷的注入与迁移加剧,导致绝缘子内空间及表面电荷的积聚,畸变电场,容易诱发沿面闪络故障。为此文中建立了电—热耦合应力下直流盆式绝缘子内的电荷注入与积聚模型,研究了考虑电荷注入和迁移特性的绝缘子空间电荷及表面电荷积聚情况,并分析了不同负载电流下绝缘子沿面电场分布规律。研究结果表明:温度梯度下,导体—绝缘界面电荷注入会造成绝缘子内部同极性空间电荷的积聚,并且空间电荷积聚密度会随着负载电流的增大而增大;空间电荷的积聚会减弱高压电极附近电场强度,增强绝缘子凸侧表面法向电场强度,加剧表面电荷的积聚;空间及表面电荷的积聚会使绝缘子表面电势上升,导致接地电极附近电势差增大,电场强度显著增大,地电极三结合点处的场强由空载条件下的1.71 kV/mm增长为额定电流作用下的3.47 kV/mm,增加了103%。该研究结果有助于理解温度梯度下直流绝缘子表面电场畸变机理,并对直流绝缘子的优化设计和安全运行有一定的参考价值。胡琦 刘智鹏 刘衡 李庆民 AManu Haddad 2021高压电器2021,57,10:4
10Randomised controlled trial of triclosan coated vs uncoated sutures in primary hip and knee arthroplasty显示文摘BACKGROUND Triclosan-coated vicryl plus suture(Ethicon, Inc.) was developed to reduce microbial colonisation during surgical procedures. However, its effect on wound healing and surgical site infections remain unclear after hip and knee arthroplasty surgery.AIM To determine the effect of triclosan-coated sutures(TCS) vs non-coated sutures on wound healing, following primary hip and knee arthroplasties.METHODS A single-centred, double-blind randomised controlled trial(RCT) was undertaken. We randomly allocated patients to receive either the triclosan-coated sutures(TCS vicryl plus) or non-coated sutures(NCS vicryl) during the closure of unilateral primary hip and knee arthroplasties. We utilised the ASEPSIS wound scoring system to evaluate wound healing for the first 6 weeks post-operatively.RESULTS One hundred and fifty patients undergoing primary total hip or knee arthroplasty over a one-year period were included. Eighty-one were randomised to the TCS group and 69 to the NCS group. Despite no statistically significant difference in the ASEPSIS scores among the study groups(P = 0.75), sensitivity analysis using the Mann Whitney test(P = 0.036) as well as assessment of the wound complications at 6 weeks follow up, demonstrated significantly higher wound complication rates in the TCS group(8 vs 1, P = 0.03).CONCLUSION No clear advantage was demonstrated for using the TCS. 11A randomized open-label trial of on-demand rabeprazole vs ranitidine for patients with non-erosive reflux disease显示文摘AIM:To compare the efficacy of the proton-pump inhibitor,rabeprazole,with that of the H 2-receptor antagonist,ranitidine,as on-demand therapy for relieving symptoms associated with non-erosive reflux disease(NERD).METHODS:This is a single center,prospective,randomized,open-label trial of on-demand therapy with rabeprazole(group A) vs ranitidine(group B) for 4 wk.Eighty-three patients who presented to the American University of Beirut Medical Center with persistent gastroesophageal reflux disease(GERD) symptoms and a normal upper gastrointestinal endoscopy were eligible for the study.Patients in group A(n = 44) were allowed a maximum rabeprazole dose of 20 mg twice daily,while those in group B(n = 39) were allowed a maximum ranitidine dose of 300 mg twice daily.Efficacy was assessed by patient evaluation of global symptom relief,scores of the SF-36 quality of life(QoL) questionnaires,total number of pills used,and number of medication-free days.RESULTS:Among the 83 patients who were enrolled in the study,76 patients(40 in the rabeprazole group and 36 in the ranitidine group) completed the 4-wk trial.Baseline characteristics were comparable between both groups.After 4 wk,there was no significant difference in the subjective global symptom relief between the rabeprazole and the ranitidine groups(71.4% vs 65.4%,respectively;P = 0.9).There were no statistically significant differences between mean cumulative scores of the SF-36 QoL questionnaire for the two study groups(rabeprazole 22.40 ± 27.53 vs ranitidine 17.28 ± 37.06;P = 0.582).There was no significant difference in the mean number of pills used(rabeprazole 35.70 ± 29.75 vs ranitidine 32.86 ± 26.98;P = 0.66).There was also no statistically significant difference in the mean number of medication-free days between both groups.CONCLUSION:Rabeprazole has a comparable efficacy compared to ranitidine when given on-demand for the treatment of NERD.Both medications were associated with improved quality of life.Abdallah A Kobeissy Jana G Hashash Faek R Jamali Assaad M Skoury Reham Haddad Sarah El-Samad Rami Ladki Rola Aswad Assaad M Soweid 2012World Journal of Gastroenterology2012,18,19:4
12Choice of dialysis modality prior to kidney transplantation:Does it matter?显示文摘The population of patients with end stage renal disease(ESRD) is increasing,lengthening waiting lists for kidney transplantation.Majority of the patients are not able to receive a kidney transplant in timely manner even though it is well established that patient survival and quality of life after kidney transplantation is far better when compared to being on dialysis.A large number of patients who desire a kidney transplant ultimately end up needing some form of dialysis therapy.Most of incident ESRD patients choose hemodialysis(HD) over peritoneal dialysis(PD) as the modality of choice in the United States,even though studies have favored PD as a better choice of pre-transplant dialysis modality than HD.PD is largely underutilized in the United States due to variety of reasons.As a part of the decision making process,patients are often educated how the choice regarding modality of dialysis would fit into their life but it is not clear and not usually discussed,how it can affect eventual kidney transplantation in the future.In this article we would like to discuss ESRD demographics and outcomes,modality of dialysis and kidney transplant related events.We have summarized the data comparing PD and HD as the modality of dialysis and its impact on allograft and recipient outcomes after kidney transplantation.Deepika Jain Danny B Haddad Narender Goel 2019World Journal of Nephrology2019,8,1:3

15Predicting lower limb periprosthetic joint infections: A review of risk factors and their classification显示文摘AIM To undertook a systematic review to determine factors that increase a patient's risk of developing lower limb periprosthetic joint infections(PJI).METHODS This systematic review included full-text studies that reviewed risk factors of developing either a hip or knee PJI following a primary arthroplasty published from January 1998 to November 2016. A variety of keywords were used to identify studies through international databases referencing hip arthroplasty, knee arthroplasty, infection, and risk factors. Studies were only included if they included greater than 20 patients in their study cohort, and there was clear documentation of the statistical parameter used; specifically P-value, hazard ratio, relative risk, or/and odds ratio(OR). Furthermore a quality assessment criteria for the individual studies was undertaken to evaluate the presence of record and reporting bias. RESULTS Twenty-seven original studies reviewing risk factors relating to primary total hip and knee arthroplasty infections were included. Four studies(14.8%) reviewed PJI of the hip, 3(11.21%) of the knee, and 20(74.1%) reviewed both joints. Nineteen studies(70.4%) were retrospective and 8(29.6%) prospective. Record bias was identified in the majority of studies(66.7%). The definition of PJI varied amongst the studies but there was a general consensus to define infection by previously validated methods. The most significant risks were the use of preoperative high dose steroids(OR = 21.0, 95%CI: 3.5-127.2, P < 0.001), a BMI above 50(OR = 18.3, P < 0.001), tobacco use(OR = 12.76, 95%CI: 2.47-66.16, P= 0.017), body mass index below 20(OR = 6.00, 95%CI: 1.2-30.9, P = 0.033), diabetes(OR = 5.47, 95%CI: 1.77-16.97, P = 0.003), and coronary artery disease(OR = 5.10, 95%CI: 1.3-19.8, P = 0.017).CONCLUSION We have highlighted the need for the provider to optimise modifiable risk factors, and develop strategies to limit the impact of non-modifiable factors.David A George Lorenzo Drago Sara Scarponi Enrico Gallazzi Fares S Haddad Carlo L Romano 2017World Journal of Orthopedics2017,8,5:3
16Where does chemotherapy stands in the treatment of ampullary carcinoma?
