| 1 | Transjugular intrahepatic portosystemic shunt in cirrhosis: An exhaustive critical update显示文摘More than five decades after it was originally conceptualized as rescue therapy for patients with intractable variceal bleeding,the transjugular intrahepatic portosystemic shunt(TIPS)procedure continues to remain a focus of intense clinical and biomedical research.By the impressive reduction in portal pressure achieved by this intervention,coupled with its minimally invasive nature,TIPS has gained increasing acceptance in the treatment of complications of portal hypertension.The early years of TIPS were plagued by poor long-term patency of the stents and increased incidence of hepatic encephalopathy.Moreover,the diversion of portal flow after placement of TIPS often resulted in derangement of hepatic functions,which was occasionally severe.While the incidence of shunt dysfunction has markedly reduced with the advent of covered stents,hepatic encephalopathy and instances of early liver failure continue to remain a significant issue after TIPS.It has emerged over the years that careful selection of patients and diligent post-procedural care is of paramount importance to optimize the outcome after TIPS.The past twenty years have seen multiple studies redefining the role of TIPS in the management of variceal bleeding and refractory ascites while exploring its application in other complications of cirrhosis like hepatic hydrothorax,portal hypertensive gastropathy,ectopic varices,hepatorenal and hepatopulmonary syndromes,non-tumoral portal vein thrombosis and chylous ascites.It has also been utilized to good effect before extrahepatic abdominal surgery to reduce perioperative morbidity and mortality.The current article aims to review the updated literature on the status of TIPS in the management of patients with liver cirrhosis. | Sasidharan Rajesh Tom George Cyriac Abby Philips Rizwan Ahamed Sandeep Kumbar Narain Mohan Meera Mohanan Philip Augustine | 2020 | World Journal of Gastroenterology2020,26,37: | 26 |
| 2 | Epidural anesthesia is effective for extracorporeal shock wave lithotripsy of pancreatic and biliary calculi显示文摘AIM: To evaluate the efficacy of thoracic epidural analgesia for extracorporeal shock wave lithotripsy (ESWL). METHODS: ESWL is an effective, non-invasive technique for the treatment of difficult pancreatic and large bile duct calculi. The procedure is often painful and requires large doses of analgesics. Many different anesthetic techniques have been used. Patients with either large bile duct calculi or pancreatic duct calculi which could not be extracted by routine endoscopic methods were selected. Thoracic epidural anesthesia (TEA) was routinely used in all the subjects unless contraindicated. Bupivacaine 0.25% with or without clonidine was used to block the segments D6 to D12. The dose was calculated depending on the age, height and weight of the patient. It was usually 1-2 mL per segment blocked.RESULTS: Ninety eight percent of the 1509 patients underwent ESWL under TEA. The subjects selected were within American Society of Anesthesiologists grade Ⅰ to Ⅲ. ESWL using EA permitted successful elimination of bile duct or pancreatic calculi with minimal morbidity. The procedure time was shorter in patients with TEA than in those who underwent ESWL under total intravenous anesthesia. | 3 | All ileo-cecal ulcers are not Crohn's:Changing perspectives of symptomatic ileocecal ulcers显示文摘AIM To investigated clinical,endoscopic and histopathological parameters of the patients presenting with ileocecal ulcers on colonoscopy.METHODS Consecutive symptomatic patients undergoing colonoscopy,and diagnosed to have ulcerations in the ileocecal(I/C) region,were enrolled.Biopsy was obtained and theirclinical presentation and outcome were recorded.RESULTS Out of 1632 colonoscopies,104 patients had ulcerations in the I/C region and were included in the study.Their median age was 44.5 years and 59% were males.The predominant presentation was lower GI bleed(55,53%),pain abdomen ± diarrhea(36,35%),fever(32,31%),and diarrhea alone(9,9%).On colonoscopy,terminal ileum was entered in 96(92%) cases.The distribution of ulcers was as follows:Ileum alone 40%(38/96),cecum alone 33%(32/96),and both ileum plus cecum 27%(26/96).The ulcers were multiple in 98% and in 34% there were additional ulcers elsewhere in colon.Based on clinical presentation and investigations,the etiology of ulcers was classified into infective causes(43%) and noninfective causes(57%).Fourteen patients(13%) were diagnosed to have Crohn's disease(CD).CONCLUSION Non-specific ileocecal ulcers are most common ulcers seen in ileo-cecal region.And if all infections are clubbed together then infection is the most common(> 40%) cause of ulcerations of the I/C region.Cecal involvement and fever are important clues to infective cause.On the contrary CD account for only 13% cases as a cause of ileo-cecalulcers.So all symptomatic patients with I/C ulcers on colonoscopy are not Crohn's. | Jay Toshniwal Romesh Chawlani Amit Thawrani Rajesh Sharma Anil Arora Hardik L Kotecha Mohan Goyal Vijendra Kirnake Pankaj Jain Pankaj Tyagi Naresh Bansal Munish Sachdeva Piyush Ranjan Mandhir Kumar Praveen Sharma Vikas Singla Rinkesh Bansal Vineet Shah Sunita Bhalla Ashish Kumar | 2017 | World Journal of Gastrointestinal Endoscopy2017,9,7: | 5 |

| 19 | Retrospective analysis of anti-inflammatory therapies during the first wave of COVID-19 at a community hospital显示文摘BACKGROUND Our understanding of the severe acute respiratory syndrome coronavirus 2 has evolved since the first reported cases in December 2019,and a greater emphasis has been placed on the hyper-inflammatory response in severely ill patients.The purpose of this study was to determine risk factors for mortality and the impact of anti-inflammatory therapies on survival.AIM To determine the impact of various therapies on outcomes in severe coronavirus disease 2019 patients with a focus on anti-inflammatory and immune-modulating agents.METHODS A retrospective analysis was conducted on 261 patients admitted or transferred to the intensive care unit in two community hospitals between March 12,2020 and June 17,2020.Totally 167 patients received glucocorticoid(GC)therapy.Seventythree patients received GC alone,94 received GC and tocilizumab,28 received tocilizumab monotherapy,and 66 received no anti-inflammatory therapy.RESULTS Patient survival was associated with GC use,either alone or with tocilizumab,and decreased vasopressor requirements.Delayed administration of GC was found to decrease the survival benefit of GC therapy.No difference in survival was found with varying anticoagulant doses,convalescent plasma,tocilizumab monotherapy;prone ventilation,hydroxychloroquine,azithromycin,or intravenous ascorbic acid use.CONCLUSION This analysis demonstrated the survival benefit associated with anti-inflammatory therapy of GC,with or without tocilizumab,with the combination providing the most benefit.More studies are needed to assess the optimal timing of anti-inflammatory therapy initiation. | Jose I Iglesias Andrew V Vassallo Jesse B Sullivan Yasmine Elbaga Vishal V Patel Nikunjkumar Patel Lydia Ayad Payam Benson Marina Pittiglio Emad Gobran Alexander Clark Wajahat Khan Kaliope Damalas Rajesh Mohan Satyendra P Singh | 2021 | World Journal of Critical Care Medicine2021,10,5: | 0 |
