| 1 | Extracorporeal shock wave lithotripsy for pancreatic and large common bile duct stones显示文摘Extraction of large pancreatic and common bile duct(CBD)calculi has always challenged the therapeutic endoscopist.Extracorporeal shockwave lithotripsy(ESWL)is an excellent tool for patients with large pancreatic and CBD calculi that are not amenable to routine endotherapy.Pancreatic calculi in the head and body are targeted by ESWL,with an aim to fragment them to<3 mm diameter so that they can be extracted by subsequent endoscopic retrograde cholangio-pancreatography(ERCP).In our experience,complete clearance of the pancreatic duct was achieved in 76% and partial clearance in 17%of 1006 patients.Short-term pain relief with reduction in the number of analgesics ingested was seen in 84%of these patients.For large CBD calculi,a nasobiliary tube is placed to help target the calculi,as well as bathe the calculi in salinea simple maneuver which helps to facilitate fragmenta-tion.The aim is to fragment calculi to<5 mm size and clear the same during ERCP.Complete clearance of the CBD was achieved in 84.4%of and partial clearance in 12.3%of 283 patients.More than 90%of the patients with pancreatic and biliary calculi needed three or fewer sessions of ESWL with 5000 shocks being de-livered at each session.The use of epidural anesthesia helped in reducing patient movement.This,together with the better focus achieved with newer third-gen-eration lithotripters,prevents collateral tissue damage and minimizes the complications.Complications in our experience with nearly 1300 patients were minimal,and no extension of hospital stay was required.Similar rates of clearance of pancreatic and biliary calculi with minimal adverse effects have been reported from the centers where ESWL is performed regularly.In view of its high efficiency,non-invasive nature and low complication rates,ESWL can be offered as the first-line therapy for selected patients with large pancreatic and CBD calculi. | Manu Tandan D Nageshwar Reddy | 2011 | World Journal of Gastroenterology2011,17,39: | 36 |

| 4 | Epidural anesthesia is effective for extracorporeal shock wave lithotripsy of pancreatic and biliary calculi显示文摘AIM: To evaluate the efficacy of thoracic epidural analgesia for extracorporeal shock wave lithotripsy (ESWL). METHODS: ESWL is an effective, non-invasive technique for the treatment of difficult pancreatic and large bile duct calculi. The procedure is often painful and requires large doses of analgesics. Many different anesthetic techniques have been used. Patients with either large bile duct calculi or pancreatic duct calculi which could not be extracted by routine endoscopic methods were selected. Thoracic epidural anesthesia (TEA) was routinely used in all the subjects unless contraindicated. Bupivacaine 0.25% with or without clonidine was used to block the segments D6 to D12. The dose was calculated depending on the age, height and weight of the patient. It was usually 1-2 mL per segment blocked.RESULTS: Ninety eight percent of the 1509 patients underwent ESWL under TEA. The subjects selected were within American Society of Anesthesiologists grade Ⅰ to Ⅲ. ESWL using EA permitted successful elimination of bile duct or pancreatic calculi with minimal morbidity. The procedure time was shorter in patients with TEA than in those who underwent ESWL under total intravenous anesthesia. | 6 | Effects of live yeast on differential genetic and functional attributes of rumen microbiota in beef cattle显示文摘Several studies have evaluated the effects of live yeast supplementation on rumen microbial population;however,its effect on differential microbial genes and their functional potential has not been described. Thus, this study applied shotgun metagenomic sequencing to evaluate the effects of live yeast supplementation on genetic and functional potential of the rumen microbiota in beef cattle. Eight rumen-cannulated Holstein steers were randomly assigned to two treatments in a cross-over design with two 25-day experimental periods and a 10-day wash-out between the two periods. The steers were housed in individual pens and fed 50% concentrate-mix and 50% red clover/orchard hay ad libitum. Treatments were(1) control(CON;basal diet without additive) and(2) yeast(YEA;basal diet plus 15 g/d of live yeast product). Rumen fluid samples were collected at 3, 6, and 9 h after feeding on the last d of each period. Sequencing was done on an Illumina Hi Seq 2500 platform. Dietary yeast supplementation increased the relative abundance of carbohydrate-fermenting bacteria(such as Ruminococcus albus, R.champanellensis, R. bromii, and R. obeum) and lactate-utilizing bacteria(such as Megasphaera elsdenii, Desulfovibrio desulfuricans, and D. vulgaris). A total of 154 differentially abundant genes(DEGs) were obtained(false discovery rate < 0.01). Kyoto Encyclopedia of Genes and Genomes(KEGG) annotation analysis of the DEGs revealed that 10 pathways, including amino sugar and nucleotide sugar metabolism, oxidative phosphorylation, lipopolysaccharide biosynthesis, pantothenate and coenzyme A biosynthesis, glutathione metabolism, beta-alanine metabolism,polyketide sugar unit biosynthesis, protein export, ribosome, and bacterial secretory system, were enriched in steers fed YEA. Annotation analysis of the DEGs in the carbohydrate-active enzymes(CAZy) database revealed that the abundance of genes coding for enzymes belonging to glycoside hydrolases, glycosyltransferases, and carbohydrate binding modules were enriched in steers fed YEA. | 7 | Efficacy of low dose peginterferon alpha-2b with ribavirin on chronic hepatitis C显示文摘AIM: To assess the effi cacy of peginterferon alpha 2b at doses of 50 μg weekly and 80 μg weekly (based on body weight) plus ribavirin in HCV genotype 2 and genotype 3 chronic hepatitis C patients. METHODS: During the study period of Jan 2002 to Dec 2003, all patients diagnosed as chronic hepatitis C or HCV related compensated cirrhosis were treated with peginterferon alpha 2b 50 μg S/C weekly (body weight < 60 kg) or 80 μg S/C weekly (body weight > 60 kg) plus ribavirin 800 mg/d for 24 wk. RESULTS: Overall 28 patients, 14 patients in each group (based on body weight) were treated during the period. Out of 28 patients, 75% were genotype 3, 18% were genotype 2 and 7% were genotype 1. The mean dose of peginterferon alpha 2b was 0.91 μg/kg in group 1 and 1.23 μg/kg in group 2 respectively. The end of treatment and sustained virologic response rates were 82% and 78% respectively. Serious adverse effects were seen in 3.5% patients. | 8 | Per oral cholangiopancreatoscopy in pancreatico biliary diseases- Expert consensus statements显示文摘AIM: To provide consensus statements on the use of per-oral cholangiopancreatoscopy(POCPS).METHODS: A workgroup of experts in endoscopic retrograde cholangiopancreatography(ERCP), endosonography, and POCPS generated consensus statements summarizing the utility of POCPS in pancreaticobiliary disease. Recommendation grades used validated evidence ratings of publications from an extensive literature review.RESULTS: Six consensus statements were generated:(1) POCPS is now an important additional tool during ERCP;(2) in patients with indeterminate biliary strictures, POCS and POCS-guided targeted biopsy are useful for establishing a definitive diagnosis;(3) POCS and POCS-guided lithotripsy are recommended for treatment of difficult common bile duct stones when standard techniques fail;(4) in patients with main duct intraductal papillary mucinous neoplasms(IPMN) POPS may be used to assess extent of tumor to assist surgicalresection;(5) in difficult pancreatic ductal stones,POPS-guided lithotripsy may be useful in fragmentation and extraction of stones; and(6) additional indications for POCPS include selective guidewire placement,unexplained hemobilia, assessing intraductal biliary ablation therapy, and extracting migrated stents. | 11 | Parametric study of turbine NGV blade lean and vortex design显示文摘The effects of blade lean and vortex design on the aerodynamics of a turbine entry nozzle guide vane(NGV) are considered using computational fluid dynamics. The aim of the work is to address some of the uncertainties which have arisen from previous studies where conflicting results have been reported for the effect on the NGV. The configuration was initially based on the energy efficient engine turbine which also served as the validation case for the computational method. A total of 17 NGV configurations were evaluated to study the effects of lean and vortex design on row efficiency and secondary kinetic energy. The distribution of mass flow ratio is introduced as an additional factor in the assessment of blade lean effects. The results show that in the turbine entry NGV, the secondary flow strength is not a dominant factor that determines NGV losses and therefore the changes of loading distribution due to blade lean and the associated loss mechanisms should be regarded as a key factor. | 13 | 直流电压下玻璃纤维的荷电运动过程及桥接放电现象显示文摘气体绝缘全封闭组合电器/气体绝缘金属封闭输电线路(gas insulated switchgear/gas insulated metal-enclosed transmission line,GIS/GIL)在制造、运输、装配过程中容易引入绝缘纤维,是造成设备放电故障的原因之一。该文通过搭建封闭式平行电极板实验平台,获得玻璃纤维的启举与运动特性;进一步通过理论计算建立纤维的荷电与受力模型,研究直流电压下玻璃纤维的运动与放电特性。结果表明,玻璃纤维的临界启举电压与纤维直径的1/2次方近似呈正比关系,与纤维长度基本无关。玻璃纤维荷电启举后,在电极间往复运动中会分散成若干纤维簇,而带有不同极性电荷的纤维簇首尾相接,贯通电极,引发“桥接”现象。玻璃纤维的跳动会严重畸变纤维尖端电场,降低气隙的击穿电压,较无纤维时下降约7%~20%。当玻璃纤维出现桥接现象时,放电会沿着纤维小桥表面发生,气隙击穿电压下降约34%。所得结果阐释了直流应力下玻璃纤维的运动行为及放电规律,可为GIS/GIL设备的异物防护提供基础依据。 | 胡琦 李庆民 刘智鹏 刘衡 梁瑞雪 MANU Haddad | 2022 | 中国电机工程学报2022,42,6: | 4 |

| 20 | Morphological Characterization and Hydrodynamic Behavior of Shortfin Mako Shark(Isurus oxyrinchus)Dorsal Fin Denticles显示文摘The shortfin mako shark(Isurus oxyrinchus)is one of the fastest marine fishes,reaching speeds of up to 70 km·h^-1.Their speed is related to the skin surface design composed of dermal denticles.Denticles vary in size and shape according to placement on the body and minimize turbulence around the body.The objective of this study is to analyze the interaction between seawater flow and denticles on the dorsal fin.High-resolution microscopy(scanning electron microscopy and confocal microscopy)were used to measure defined parts of the dermal denticles.These measurements,along with ratios based on length-to-width define three morphologies(rounded,semi-rounded,long)that were 3D reconstructed.Computational fluid dynamics simulated fluid passage over reconstructed denticles and describe hy-drodynamic efficiency under different conditions.An increase in angle of inclination produced a relevant increase in the drag coefficient,especially for high velocity inlets.The lowest drag coefficient values were found in long and semi-rounded,followed by rounded mor-phologies.The hydrodynamic behavior of shark skin demonstrates a relation to the morphological characteristics of dermal denticles on the dorsal fin.It is concluded that the best hydroefficiency relies on the rounded morphology and may serve to design hydrodynamically efficient surfaces or manmade assemblies. | Fernandez-Waid Patricia Diez Guzman Bidaguren Inigo Izagirre Urtzi Blanco Jesus Maria Soto Manu | 2019 | Journal of Bionic Engineering2019,16,4: | 3 |
