| 1 | Subcurative radiation significantly increases cell proliferation, invasion, and migration of primary glioblastoma multiforme in vivo显示文摘Tumor cell proliferation, infiltration, migration, and neovascularization are known causes of treatment resistance in glioblastoma multiforme(GBM). The purpose of this study was to determine the effect of radiation on the growth characteristics of primary human GBM developed in a nude rat. Primary GBM cells grown from explanted GBM tissues were implanted orthotopically in nude rats. Tumor growth was confirmed by magnetic resonance imaging on day 77(baseline) after implantation. The rats underwent irradiation to a dose of 50 Gy delivered subcuratively on day 84 postimplantation(n = 8), or underwent no radiation(n = 8). Brain tissues were obtained on day 112(nonirradiated) or day 133(irradiated). Immunohistochemistry was performed to determine tumor cell proliferation(Ki-67) and to assess the expression of infiltration marker(matrix metalloproteinase-2, MMP-2) and cell migration marker(CD44). Tumor neovascularization was assessed by microvessel density using von-Willebrand factor(vWF) staining. Magnetic resonance imaging showed well-developed, infiltrative tumors in 11 weeks postimplantation. The proportion of Ki-67-positive cells in tumors undergoing radiation was(71 ± 15)% compared with(25 ± 12)% in the nonirradiated group(P = 0.02). The number of MMP-2-positive areas and proportion of CD44-positive cells were also high in tumors receiving radiation, indicating great invasion and infiltration. Microvessel density analysis did not show a significant difference between nonirradiated and irradiated tumors. Taken together, we found that subcurative radiation significantly increased proliferation, invasion, and migration of primary GBM. | 2 | Palliative surgical bypass for unresectable periampullary carcinoma显示文摘BACKGROUND:Around 60%to 80%of patients with periampullary carcinoma are unresectable either due to distant metastasis or local vascular invasion.With the advancement of endoscopic interventional procedures,the role of surgical bypass has diminished.However,surgical bypass is still appropriate in patients with unresectable disease discovered at the time of surgery.This study was conducted to assess the results of palliative surgical bypass for patients with unresectable periampullary carcinoma at our hospital,a tertiary referral center of Northern India. METHOD:The study group comprised 204 patients who had undergone surgical bypass for advanced periampullary carcinoma over the last 15 years. RESULTS:Between January 1990 and December 2004,204 patients(128 males,76 females)consisting of 179 patients with carcinoma of head of the pancreas,14 patients with ampullary carcinoma,8 patients with lower end cholangiocarcinoma and 3 patients with duodenal carcinoma underwent surgical bypass.Their average age was 51 years (range 20-78 years).Both biliary and gastric bypasses were done in 158(77.45%),biliary bypass alone in 37(18.13%) and gastric bypass alone in 9(4.32%).Biliary bypass was done by Roux-en-Y hepaticojejunostomy,and gastric bypass by retrocolic gastrojejunostomy.The overall postoperative mortality and morbidity were 0.98%and 26.9%,respectively The patients who died had undergone previously endoscopic intervention.Complications included wound infection in 12.25%of the patients,bile leak in 5.12%,delayed gastric emptying in 5.38%,ascitic leak from drains in 8.8%,and upper gastrointestinal bleeding in 1.96%.The incidences of wound infection and bile leak both were significantly higher in patients who had had preoperative biliary stenting None of the patients who had undergone Roux-en-Y hepati cojejunostomy+retrocolic gastrojejunostomy required any intervention later in their life.CONCLUSIONS:Surgical bypass is a safe procedure with negligible mortality and minimal morbidity.It has not lost its relevance and is an appropriate treatment in patients deemed unresectable on peroperative assessment. | Shivendra Singh Ajay Kumar Sachdev Adarsh Chaudhary Anil Kumar Agarwal | 2008 | Hepatobiliary & Pancreatic Diseases International2008,7,3: | 3 |

| 4 | Cross-Sectional study of internalised stigma and medication adherence in patients with obsessive compulsive disorder显示文摘Background Patients suffering from psychiatric disorders tend to stigmatise themselves which had been linked to poor adherence to treatment.Aims The aim of the present study was to study internalised stigma and medication adherence and to assess the relationship between them in patients with obsessive compulsive disorder(0CD).Methods A cross-sectional study was conducted on 112 patients diagnosed with 0CD who were attending the Out-patient's department at Department of Psychiatry of a tertiary care hospital in North India.Internalised stigma and current medication adherence were assessed with Internalized Stigma of Mental Illness Scale(ISMI)and Medication Adherence Rating Scale,respectively.Yale-Brown Obsessive Compulsive Scale was used to assess the current severity of OCD symptoms.Sociodemographic and clinical details were also obtained from the patients by using a semistructured sociodemographic proforma.Results Most of the patients reported moderate level of internalised stigma with a mean ISMI score of 77.98(10.82).Most of the patients were compliant while 41.96%reported poor medication adherence.Internalised stigma was negatively correlated with the current medication adherence.Current severity of OCD symptoms also showed a significant positive correlation with internalised stigma and a significant negative correlation with medication adherence.Conclusion High levels of internalised stigma were associated with lower adherence to treatment which suggests that internalised stigma may be a very important factor influencing medication adherence in patients with OCD. | Eram Ansari Sudha Mishra Adarsh Tripathi Sujita Kumar Kar Pronob Kumar Dalai | 2020 | General Psychiatry2020,33,2: | 1 |

| 10 | Contrast-enhanced multidetector computed tomography features and histogram analysis can differentiate ameloblastomas from central giant cell granulomas显示文摘BACKGROUND No qualitative or quantitative analysis of contrast-enhanced computed tomography(CT)images has been reported for the differentiation between ameloblastomas and central giant cell granulomas(CGCGs).AIM To describe differentiating multidetector CT(MDCT)features in CGCGs and ameloblastomas and to compare differences in enhancement of these lesions qualitatively and using histogram analysis.METHODS MDCT of CGCGs and ameloblastomas was retrospectively reviewed to evaluate qualitative imaging descriptors.Histogram analysis was used to compare the extent of enhancement of the soft tissue.Fisher’s exact tests and Mann–Whitney U test were used for statistical analysis(P<0.05).RESULTS Twelve CGCGs and 33 ameloblastomas were reviewed.Ameloblastomas had a predilection for the posterior mandible with none of the CGCGs involving the angle.CGCGs were multilocular(58.3%),with a mixed lytic sclerotic appearance(75%).Soft tissue component was present in 91%of CGCGs,which showed hyperenhancement(compared to surrounding muscles)in 50%of cases,while the remaining showed isoenhancement.Matrix mineralization was present in 83.3%of cases.Ameloblastomas presented as a unilocular(66.7%),lytic(60.6%)masses with solid components present in 81.8%of cases.However,the solid component showed isoenhancement in 63%.No matrix mineralization was present in 69.7%of cases.Quantitatively,the enhancement of soft tissue in CGCG was significantly higher than in ameloblastoma on histogram analysis(P<0.05),with a minimum enhancement of>49.05 HU in the tumour providing 100%sensitivity and 85%specificity in identifying a CGCG.CONCLUSION A multilocular,lytic sclerotic lesion with significant hyperenhancement in soft tissue,which spares the angle of the mandible and has matrix mineralization,should indicate prospective diagnosis of CGCG. | Adarsh Ghosh Meyyappan Lakshmanan Smita Manchanda Ashu Seith Bhalla Prem Kumar Ongkila Bhutia Asit Ranjan Mridha | 2022 | World Journal of Radiology2022,14,9: | 0 |
| 11 | Phenomenology,disability and sexual functioning in female Dhat syndrome:a study of tertiary care gynaecology outpatients显示文摘Background Dhat syndrome,a clinical condition related to semen loss in urine often found among males in India,has rarely been described as a separate clinical condition in females.Women with the syndrome complain of passing vaginal discharge and can be excessively concerned and preoccupied with it,often attributing various physical symptoms to the loss of vaginal fluids.Aims This study aimed to assess the sociodemographic and clinical profiles of female patients with Dhat syndrome and their perceived stress,disability and sexual functioning.Methods Sociodemographic details of 70 females with non-pathological vaginal discharge were evaluated with a semistructured sociodemographic assessment.The phenomenology of the vaginal discharge was assessed with the Scale for Assessment of Female Dhat Syndrome Questionnaire.Anxiety and depressive symptoms were measured with the Hospital Anxiety and Depression Scale.Perceived stress in the past month and disability caused by the illness were assessed with the Perceived Stress Scale and the World Health Organization Disability Assessment Scale.In addition,the Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition,Level 2 Somatic Symptoms Scale was administered to rate the severity of somatic symptoms,and sexual functioning was evaluated using the Female Sexual Function Index(FSFI)scale for women who had had sexual intercourse in the past month.Results The mean age of onset vaginal discharge was 23.0(6.6)years.Biological factors,such as urinary tract infection,were the commonly attributed cause of the loss of vaginal fluids.Psychiatric comorbidity and perceived moderate stress in the past month were found in 38.6%and 68.6%of female patients with Dhat syndrome,respectively.Disability scores tended to be low.Among the females having had sexual intercourse in the past month,48.3%had FSFI scores indicative of a female sexual disorder.Conclusions The clinical presentation of women with non-pathological vaginal discharge is similar to that of males with Dhat syndrome.It requires comprehensive assessment and management that targets the biological,social and psychological factors and cultural issues. | Shubha Joshi Adarsh Tripathi Smriti Agarwal Nisha Singh Bandna Gupta Anil Nischal Sujita Kumar Kar | 2022 | General Psychiatry2022,35,5: | 0 |

| 13 | A comparative study of Epipactis atrorubens in two different forest communities of the Middle Urals, Russia显示文摘The objective of this study was to compare ecophysiological and morphological parameters of a regionally endangered orchid species,Epipactis atrorubens(Hoffm.ex Bernh.)Bess.,growing in two forest communities(on serpentine and granite outcrops)of the Middle Urals,Russia.Biodiversity,dominance,and phytocoenosis studies showed the colonization of a wide range of plant species on both sites.The physicochemical properties of the soil,chemical composition and morphological features of E.atrorubens,growing under technogenic conditions(asbestos deposits),on serpentine outcrops and in the natural environment of the granite massif were studied for the first time.The serpentine substrate differed from the granite one by its greater stoniness,circumneutral pH and lower contents of available nitrogen and phosphorus.Extremely high concentrations of magnesium were found in the serpentine soil,some 79 times higher than in the granite substrate.High concentrations of nickel(94 times),chromium(59 times),cobalt(17 times),and iron(4 times)were found in the serpentine substrate,higher than in the granite substrate.The differences between the sites for available metal contents and for root and shoot metal contents were significantly less.Concentrations of most of the metals in the roots were higher than in the shoots.Despite higher metal concentrations and lower nitrogen and phosphorus levels in serpentine soils,E.atrorubens had a larger population and greater viability compared to those growing on granite.Plants on serpentine outcrops were characterized by the formation of a larger number of fruits,greater root lengths and thicker leaf blades,compared to plants on granites.The well-developed orchid mycorrhizae contributed to the survival of this species under unfavorable serpentine conditions.Hence,serpentine outcrops formed due to the mining of asbestos could be a suitable substrate for the light-demanding E.atrorubens due to its capacity to adapt to dry,rocky,nutrient-depleted soils and limited competition from other plants. | Elena Filimonova Natalia Lukina Margarita Glazyrina Galina Borisova Tripti Adarsh Kumar Maria Maleva | 2020 | Journal of Forestry Research2020,31,6: | 0 |
