1Endoscopic ultrasound-guided choledochoduodenostomy in patients with failed endoscopic retrograde cholangiopancreatography显示文摘Endoscopic ultrasonography (EUS)-guided biliary drainage was performed for treatment of patients who have obstructive jaundice in cases of failed endoscopic retrograde cholangiopancreatography (ERCP). In the present study,we introduced the feasibility and outcome of EUS-guided choledochoduodenostomy in four patients who failed in ERCP. We performed the procedure in 2 papilla of Vater,including one resectable case,and 2 cases of cancer of the head of pancreas. Using a curved linear array echoendoscope,a 19 G needle or a needle knife was punctured transduodenally into the bile duct under EUS visualization. Using a biliary catheter for dilation,or papillary balloon dilator,a 7-Fr plastic stent was inserted through the choledochoduodenostomy site into the extrahepatic bile duct. In 3 (75%) of 4 cases,an indwelling plastic stent was placed,and in one case in which the stent could not be advanced into the bile duct,a naso-biliary drainage tube was placed instead. In all cases,the obstructive jaundice rapidly improved after the procedure. Focal peritonitis and bleeding not requiring blood transfusion was seen in one case. In this case,pancreatoduodenectomy was performed and the surgical findings revealed severe adhesion around the choledochoduodenostomy site. 2Safety trial of high-intensity focused ultrasound therapy for pancreatic cancer显示文摘AIM:To evaluate the safety and clinical application of high-intensity focused ultrasound(HIFU)therapy for unresectable pancreatic cancer(PC).METHODS:Thirty PC patients(16 cases in stage III and 14 cases in stage IV)with visualized pancreatic tumors were admitted for HIFU therapy as an optional local therapy in addition to systemic chemotherapy or chemoradiotherapy.Informed consent was obtained.This study began at the end of 2008 and was approved by the ethics committee of our hospital[Institutional Review Board(IRB):890].The HIFU device used was the FEP-BY02(Yuande Bio-Medical Engineering,Beijing,China).RESULTS:The mean tumor size after HIFU therapy changed to 30.9±1.7 mm from 31.7±1.7 mm at pre-therapy.There were no significant changes in tumor size,mean number of treatment sessions(2.7±0.1 mm),or mean total treatment time(2.4±0.1 h).The rate of symptom relief effect was 66.7%.The effectiveness of primary lesion treatment was as follows:complete response,0;partial response,4;stable disease,22;progressive disease,4.Treatment after HIFU therapy included 2 operations,24 chemotherapy treatments,and 4 best supportive care treatments.Adverse events occurred in 10%of cases,namely pseudocyst formation in 2 cases and mild pancreatitis development in 1.However,no severe adverse events occurred in this study.CONCLUSION:We suggest that HIFU therapy is safe and has the potential to be a new method of combination therapy for PC.Atsushi Sofuni Fuminori Moriyasu Takatomo Sano Fumihide Itokawa Takayoshi Tsuchiya Toshio Kurihara Kentaro Ishii Syujiro Tsuji Nobuhito Ikeuchi Reina Tanaka Junko Umeda Ryosuke Tonozuka Mitsuyoshi Honjo Shuntaro Mukai Mitsuru Fujita Takao Itoi 2014World Journal of Gastroenterology2014,20,28:12
3Selective liquid phase oxidation of cyclohexane over Pt/CeO_(2)-ZrO_(2)-SnO_(2)/SiO_(2)catalysts with molecular oxygen显示文摘Partial oxidation of cyclohexane into cyclohexanone and cyclohexanol(KA-oil)is an industrially significant reaction for producing precursors for the synthesis ofε-caprolactam and adipic acid,which are the building blocks of nylon.However,to date,the cyclohexane conversion ratio has usually been limited to less than 6%to prevent further oxidation of the cyclohexanol and cyclohexanone targets.In this study,we report that Pt/CeO_(2)-ZrO_(2)-SnO_(2)/SiO_(2),in which CeO2-ZrO2-SnO2 provide reactive oxygen molecules from inside the bulk,can act as efficient catalysts.Optimization of the catalyst composition and reaction conditions provided a cyclohexane conversion ratio of 24.1%and a total selectivity for cyclohexanol and cyclohexanone of 83.4%at 130℃in 0.5 MPa(4.9 atm)air for 7 h over a 5wt%Pt/16wt%Ce_(0.68)Zr_(0.17)Sn_(0.15)O_(2.0)/SiO_(2)catalyst.This catalyst has significant advantages over conventional catalysts because the reaction proceeds at a lower pressure,and there is no need for toxic radical initiators or free-radical scavengers.Nobuhito IMANAKA Toshiyuki MASUI Kazuya JYOKO 2015Journal of Advanced Ceramics2015,4,2:4
4Hypoxia and fatty liver显示文摘The liver is a central organ that metabolizes excessive nutrients for storage in the form of glycogen and lipids and supplies energy-producing substrates to the peripheral tissues to maintain their function,even under starved conditions.These processes require a considerable amount of oxygen,which causes a steep oxygen gradient throughout the hepatic lobules.Alcohol consumption and/or excessive food intake can alter the hepatic metabolic balance drastically,which can precipitate fatty liver disease,a major cause of chronic liver diseases worldwide,ranging from simple steatosis,through steatohepatitis and hepatic fibrosis,to liver cirrhosis.Altered hepatic metabolism and tissue remodeling in fatty liver disease further disrupt hepatic oxygen homeostasis,resulting in severe liver hypoxia.As master regulators of adaptive responses to hypoxic stress,hypoxia-inducible factors(HIFs) modulate various cellular and organ functions,including erythropoiesis,angiogenesis,metabolic demand,and cell survival,by activating their target genes during fetal development and also in many disease conditions such as cancer,heart failure,and diabetes.In the past decade,it has become clear that HIFs serve as key factors in the regulation of lipid metabolism and fatty liver formation.This review discusses the molecular mechanisms by which hypoxia and HIFs regulate lipid metabolism in the development and progression of fatty liver disease.Tomohiro Suzuki Satoko Shinjo Takatomo Arai Mai Kanai Nobuhito Goda 2014World Journal of Gastroenterology2014,20,41:3
5Prognosis of cancer with branch duct type IPMN of the pancreas显示文摘AIM:To examine the coexistence of metachronous and synchronous cancer in branch duct intraductal papillary mucinous neoplasms of the pancreas (IPMN).METHODS: We reviewed the records of 145 patients with branch duct IPMN between January 1991 and April 2008 and assessed the relationship between IPMN and intraor extra-pancreatic carcinoma and the outcome of IPMN.RESULTS: The mean observation period was 55.9 ± 45.3 mo. Among the 145 patients, the frequency of extra-pancreatic cancer was 29.0%. The frequency of gastric cancer, colon cancer, breast cancer, and pan-creatic cancer were 25.5%, 15.7%, 13.7%, and 9.8%, respectively. Twenty (13.8%) of the patients died. 6Usefulness of contrast-enhanced ultrasonography in determining treatment efficacy and outcome after pancreatic cancer chemotherapy显示文摘AIM:To investigate if contrast-enhanced ultrasonography(CE-US) is useful for determining treatment efficacy and outcome in the early stages of pancreatic cancer chemotherapy by assessing changes in intratumor hemodynamics using CE-US with a contrast agent.METHODS:The subjects were 34 patients with unresectable advanced pancreatic cancer treated by chemotherapy.CE-US was assessed after every treatment(course) completion under the same conditions,and patients were divided into two groups according to the intratumor enhancement pattern:Vascular rich(R) group and vascular poor(P) group.RESULTS:After the second course of treatment,R group in intratumor hemodynamics had 18 patients,and P group had 16 patients.The reduction rates of serum CA19-9 level after chemotherapy which decreased to half or less of the baseline level were 2/15(0.1%) in P group,but 11/16(69%) in R group(P = 0.006).When the mean number of courses of chemotherapy and outcome were compared,P group had a mean number of courses of 4.9(R group,10.2) and mean survival time(MST) of 246 d(R group,402 d),showing that outcome was significantly better in R group(P=0.006).CONCLUSION:CE-US revealed that the change in intratumor blood flow correlated with both serum CA19-9 level and outcome.Patients with serum CA19-9 that decreased to less than half the baseline level,and patients with an abundant intratumor blood flow,had a significantly better outcome.Thus,CE-US is potentially useful for evaluating treatment efficacy and outcome in the early stages of pancreatic cancer chemotherapy.Atsushi Sofuni Takao Itoi Fumihide Itokawa Takayoshi Tsuchiya Toshio Kurihara Kentaro Ishii Syujiro Tsuji Nobuhito Ikeuchi Fuminori Moriyasu 2008World Journal of Gastroenterology2008,14,47:3

9Catalytic liquid-phase oxidation of acetaldehyde to acetic acid over a Pt/CeO_2–ZrO_2–SnO_2/γ-alumina catalyst显示文摘Pt/CeO2–ZrO2–SnO2/γ-Al2O3 catalysts were prepared by co-precipitation and wet impregnation methods for catalytic oxidation of acetaldehyde to acetic acid in water. In the present catalysts, Pt and CeO2–ZrO2–SnO2 were successfully dispersed on the γ-Al2O3 support.Dependences of platinum content and reaction time on the selective oxidation of acetaldehyde to acetic acid were investigated to optimize the reaction conditions for obtaining both high acetaldehyde conversion and highest selectivity to acetic acid. Among the catalysts, a Pt(6.4 wt.%)/Ce0.68Zr0.17Sn0.15O2.0(16 wt.%)/γ-Al2O3 catalyst showed the highest acetaldehyde oxidation activity.
