1Indocyanine green kinetics to assess liver function: Ready for a clinical dynamic assessment in major liver surgery?显示文摘Indocyanine green(ICG) kinetics(PDR/R15) used to quantitatively assess hepatic function in the perioperative period of major resective surgery and liver transplantation have been the object of an extensive, updated and critical review. New, non invasive bedside monitors(pulse dye densitometry technology) make this opportunity widely available in clinical practice. After having reviewed basic concepts of hepatic clearance, we analysed the most common indications ICG kinetic parameters have nowadays in clinical practice, focusing in particular on the diagnostic and prognostic role of PDR and R15 in the perioperative period of major liver surgery and liver transplantation. 2circadian clock circuitry in colorectal cancer显示文摘Colorectal cancer is the most prevalent among digestive system cancers.Carcinogenesis relies on disrupted control of cellular processes,such as metabolism,proliferation,DNA damage recognition and repair,and apoptosis.Cell,tissue,organ and body physiology is characterized by periodic fluctuations driven by biological clocks operating through the clock gene machinery.Dysfunction of molecular clockworks and cellular oscillators is involved in tumorigenesis,and altered expression of clock genes has been found in cancer patients.Epidemiological studies have shown that circadian disruption,that is,alteration of bodily temporal organization,is a cancer risk factor,and an increased incidence of colorectal neoplastic disease is reported in shift workers.In this review we describe the involvement of the circadian clock circuitry in colorectal carcinogenesis and the therapeutic strategies addressing temporal deregulation in colorectal cancer.Gianluigi Mazzoccoli Manlio Vinciguerra Gennaro Papa Ada Piepoli 2014World Journal of Gastroenterology2014,20,15:4
3Aeromonas veronii biovar veronii and sepsis-infrequent complication of biliary drainage placement: A case report显示文摘BACKGROUND Aeromonas species are uncommon pathogens in biliary sepsis and cause substantial mortality in patients with impaired hepatobiliary function. Asia has the highest incidence of infection from Aeromonas,whereas cases in the west are rare.CASE SUMMARY We report the case of a 64-year-old woman with advanced pancreatic cancer and jaundice who manifested fever,abdominal pain,severe thrombocytopenia,anemia and kidney failure following the insertion of a percutaneous transhepatic biliary drainage. Blood culture results revealed the presence of Aeromonas veronii biovar veronii(A. veronii biovar veronii). After antibiotic therapy and transfusions,the life-threatening clinical conditions of the patient improved and she was discharged.CONCLUSION This was a rare case of infection,probably the first to be reported in West countries,caused by A. veronii biovar veronii following biliary drainage. 4Intelligent windows for electricity generation: A technologies review显示文摘Buildings are responsible for over 40% of total primary energy consumption in the US and EU and therefore improving building energy efficiency has significant potential for obtaining net-zero energy buildings reducing energy consumption. The concurrent demands of environmental comfort and the need to improve energy efficiency for both new and existing buildings have motivated research into finding solutions for the regulation of incoming solar radiation, as well as ensuring occupant thermal and visual comfort whilst generating energy onsite. Windows as building components offer the opportunity of addressing these issues in buildings. Building integration of photovoltaics permits building components such as semi-transparent façade, skylights and shading devices to be replaced with PV. Much progress has been made in photovoltaic material science, where smart window development has evolved in areas such as semi-transparent PV, electrochromic and thermochromic materials, luminescent solar concentrator and the integration of each of the latter technologies to buildings, specifically windows. This paper presents a review on intelligent window technologies that integrate renewable energy technologies with energy-saving strategies contributing potential solutions towards sustainable zero-energy buildings. This review is a comprehensive evaluation of intelligent windows focusing on state-of-the-art development in windows that can generate electricity and their electrical, thermal and optical characteristics. 5Low doses of intravenous epinephrine for refractory sustained monomorphic ventricular tachycardia显示文摘We report three cases of sustained monomorphic ventricular tachycardia(VT) in the setting of coronary artery disease,resistant to beta-blockers in two patients and to amiodarone in all,successfully terminated by low doses of intravenous(IV) epinephrine.VT was the first manifestation of coronary artery disease in one patient,whereas the other two patients had a previous history of myocardial infarction and were recipients of an implantable cardioverter-defibrillator(ICD).One of these two patients experienced an arrhythmic storm.All had hemodynamic instability at the time of epinephrine administration.A single slow administration of IV epinephrine(0.5 to 1 mg administered over 30 to 60 s) restored sinus rhythm after 30-90 s with only minor side effects.In the ICD patient with recurrent VT and several cardioversions due to transformation of VT to ventricular fibrillation,epinephrine injection led to the avoidance of further shocks.Although potentially harmful,low doses of IV epinephrine used alone or in combination with beta-blocker treatment and electrical cardioversion may be an alternative effective therapy for sustained monomorphic VT refractory to amiodarone.The role of epinephrine in the termination of VT should be studied further,especially in patients pre-treated with amiodarone in combination with beta-blockers.Aimé Bonny Antonio De Sisti Manlio F Márquez Richard Megbemado Franoise Hidden-Lucet Guy Fontaine 2012World Journal of Cardiology2012,4,10:3
6Bile duct hamartomas-the von Meyenburg complex显示文摘ABSTRACT: Hamartomas of the bile duct (von Meyenburg complex) are benign neoplasms of the liver, constituted histologically cystic dilatations of the bile duct, encompassed by fibrous stroma. We report a 42-year-old female patient with symptomatic cholecystitis, whose gross and ultrasonic appearance suggestive of multiple liver metastases. 7Correlation between thoracic aorta 18F-natrium fluoride uptake and cardiovascular risk显示文摘AIM: To investigating the relationship between thoracic and cardiac 18F-Natrium-Fluoride(18F-Na F) uptake,as a marker of ongoing calcification and cardiovascular risk factors.METHODS: Seventy-eight patients(44 females,mean age 63,range 44-83) underwent whole body 18F-Na F positron emission tomography/computed tomography. Cardiovascular risk(CVR) was used to divide these patients in three categories: Low(LR),medium(MR) and high risk(HR). 18F-Na F uptake was measured by manually drawing volumes of interest on the ascendingaorta,on the aortic arch,on the descending aorta and on the myocardium; average standardized uptake value was normalized for blood-pool,to obtain target-tobackground ratio(TBR). Values from the three aortic segments were then averaged to obtain an index of the whole thoracic aorta.RESULTS: A significant difference in whole thoracic aorta TBR was detected between HR and LR(1.84 ± 0.76 vs 1.07 ± 0.3,P < 0.001),but also between MR and HR-LR(1.4 ± 0.4,P < 0.02 and P < 0.01,respectively). Significance of this TBR stratification strongly varied among thoracic aorta subsegments and the lowest P values were reached in the descending aorta(P < 0.01). Myocardial uptake provided an effective CVR classes stratification(P < 0.001).Correlation between TBR and CVR was appreciable when the whole thoracic aorta was considered(R = 0.67),but it peaked when correlating the descending thoracic segment(R = 0.75),in comparison with the aortic arch and the ascending segment(R = 0.55 and 0.53,respectively). CONCLUSION: Fluoride uptake within the thoracic aorta wall effectively depicts patients' risk class and correlates with cardiovascular risk. 9Esophagogastric anastomosis with invagination into stomach:New technique to reduce fistula formation显示文摘AIM: To present a new technique of cervical esophagogastric anastomosis to reduce the frequency of fistula formation. METHODS: A group of 31 patients with thoracic and abdominal esophageal cancer underwent cervical esophagogastric anastomosis with invagination of the proximal esophageal stump into the stomach tube. In the region elected for anastomosis, a transverse myotomy of the esophagus was carried out around the entire circumference of the esophagus. Afterwards, a 4-cm long segment of esophagus was invaginated into the stomach and anastomosed to the anterior and the posterior walls. RESULTS: Postoperative minor complications occurred in 22 (70.9%) patients. Four (12.9%) patients had serious complications that led to death. The discharge of saliva was at a lower region, while attempting to leave the anastomosis site out of the alimentary transit. Three (9.7%) patients had fistula at the esophagogastric anastomosis, with minimal leakage of air or saliva and with mild clinical repercussions. No patients had esophago- gastric fistula with intense saliva leakage from either the cervical incision or the thoracic drain. Fibrotic stenosis of anastomoses occurred in seven (22.6%) patients.
