Intraoperative blood loss in orthotopic liver transplantation:The predictive factors显示文摘Liver transplantation has been associated with massive blood loss and considerable transfusion requirements. Bleeding in orthotopic liver transplantation is multifactorial. Technical difficulties inherent to this complex surgical procedure and pre operative derangements of the primary and secondary coagulation system are thought to be the principal causes of perioperative hemorrhage. Intraoperative practices such as massive fluid resuscitation and resulting hypothermia and hypocalcemia secondary to citrate toxicity further aggravate the preexisting coagulopathy and worsen the perioperative bleeding. Excessive blood loss and transfusion during orthotopic liver transplant are correlated with diminished graft survival and increased septic episodes and prolonged ICU stay. With improvements in surgical skills, anesthetic technique, graft preservation, use of intraoperative cell savers and overall perioperative management, orthotopic liver transplant is now associated with decreased intra operative blood losses. The purpose of this review is to discuss the risk factors predictive of increased intra operative bleeding in patients undergoing orthotopic liver transplant.Chandra Kant Pandey Anshuman Singh Kamal Kajal Mandeep Dhankhar Manish Tandon Vijay Kant Pandey Sunaina Tejpal Karna 2015World Journal of Gastrointestinal Surgery2015,7,6:8
Experimental assessment and prediction of temporal scour depth around a spur dike显示文摘Spur dikes are river training structures that have been extensively used worldwide for towards enhancing flood control and the stability of embankments and riverbanks. However, scour around spur dikes can be a major problem affecting their stability and hydraulic performance. The precise computation of temporal scour depth at spur dikes is very important for the design of economical and safe spur dikes. This study focuses on experimentally assessing the temporal variation of scour depth around a vertical wall spur dike and identifying the parameters, which mostly influence spur dike performance for a channel bed surface comprised of sand-gravel mixtures. In the current study, the authors did physical experiments in a flume based study to obtain new data, aimed at deriving a new predictive model for spur dike scour and comparing its performance to others found in the literature. It was found that the dimensionless temporal scour depth variation increases with an increase in(i)the threshold velocity ratio,(ii)the densimetric Froude number of the bed surface sediment mixture,(iii)the flow shallowness(defined as the ratio of the approach flow depth,y,to the spur dike’s transverse length,l),and(iv)the flow depth-particle size ratio. It is also concluded that the temporal scour depth variation in the sediment mixture is influenced by the non-uniformity of sediment and decreases with an increase in the non-uniformity of the sediment mixture.A new mathematical model is derived for the estimation of temporal scour depths in sand-gravel sediment mixtures. The proposed equation has been calibrated and validated with the experimental data, demonstrating a good predictive capacity for the estimation of temporal scour depth evolution.Manish Pandey Manousos Valyrakis Meilan Qi Anurag Sharma Ajay Singh Lodhi 2021International Journal of Sediment Research2021,36,1:8
Identification of ExpIdentification of Expressed Resistance Gene Analogs from Peanut (Arachis hypogaea L.) Expressed Sequence Tags显示文摘Low genetic diversity makes peanut (Arachis hypogaea L.) very vulnerable to plant pathogens, causing severe yield loss and reduced seed quality. Several hundred partial genomic DNA sequences as nucleotide-binding-site leucine-rich repeat (NBS-LRR) resistance genes (R) have been identified, but a small portion with expressed transcripts has been found. We aimed to identify resistance gene analogs (RGAs) from peanut expressed sequence tags (ESTs) and to develop polymorphic markers. The protein sequences of 54 known R genes were used to identify homologs from peanut ESTs from public databases. A total of 1,053 ESTs corresponding to six different classes of known R genes were recovered, and assembled 156 contigs and 229 singletons as peanut-expressed RGAs. There were 69 that encoded for NBS-LRR proteins, 191 that encoded for protein kinases, 82 that encoded for LRR-PK/transmembrane proteins, 28 that encoded for Toxin reductases, 11 that encoded for LRR-domain containing proteins and four that encoded for TM-domain containing proteins. Twenty-eight simple sequence repeats (SSRs) were identified from 25 peanut expressed RGAs. One SSR polymorphic marker (RGA121) was identified. Two polymerase chain reaction-based markers (Ahsw-1 and Ahsw-2) developed from RGA013 were homologous to the Tomato Spotted Wilt Virus (TSWV) resistance gene. All three markers were mapped on the same linkage group AhIV. These expressed RGAs are the source for RGA-tagged marker development and identification of peanut resistance genes.Zhanji Liu Suping Feng Manish K. Pandey Xiaoping Chen Albert K. Culbreath Rajeev K. 4在有经历非肝的外科的肝疾病的病人的 Perioperative 风险因素显示文摘 The patients with liver disease present for various surgical interventions. Surgery may lead to complications in a significant proportion of these patients. These complications may result in considerable morbidity and mortality. Preoperative assessment can predict survival to some extent in patients with liver disease undergoing surgical procedures. A review of literature suggests nature and the type of surgery in these patients determines the peri-operative morbidity and mortality. Optimization of premorbid factors may help to reduce perioperative mortality and morbidity. The purpose of this review is to discuss the effect of liver disease on perioperative outcome; to understand various risk scoring systems and their prognostic significance; to delineate different preoperative variables implicated in postoperative complications and morbidity; to establish the effect of nature and type of surgery on postoperative outcome in patients with liver disease and to discuss optimal anaesthesia strategy in patients with liver disease.Chandra Kant Pandey Sunaina Tejpal Karna Vijay Kant Pandey Manish Tandon Amit Singhal Vivek Mangla 2012World Journal of Gastrointestinal Surgery2012,4,12:4

Time course of physiological, biochemical, and gene expression changes under short-term salt stress in Brassica juncea L.显示文摘Salinity-imposed limitations on plant growth are manifested through osmotic and ionic imbalances. However, because salinity-induced responses vary considerably among crop plants, monitoring of such responses at an early stage has relevance. In this study, physiological(seed germination, seed vigor index, root length, shoot length, fresh weight, dry weight) and biochemical attributes(osmoprotectants, K^+/Na^+ ratio) were analyzed for a time-course assessment of salt responses in Indian mustard(Brassica juncea L.) with an emphasis on early monitoring. The results showed strong correlations for total soluble sugars at germination phase(24 h), proline content in the seedling establishment phase(48 h) and various physiological parameters including seed vigor index(R^2= 0.901), shoot length(R^2= 0.982), and fresh weight(R^2= 0.980) at 72 h(adaptation under stress). In addition, transcriptional changes were observed under NaCl treatment for key genes belonging to the family of selective ion transporters(NHX, HKT) and abscisic acid synthesis(AAO-3). The status of mitochondrial respiration was also examined as a probe for salinity tolerance at an early stage. The results suggested that although all the analyzed parameters showed correlations(negative or positive) with salt stress magnitude, their critical response times differed, with most of the studied biochemical, physiological, or molecular markers providing valuable information only after radicle emergence, whereas mitochondrial respiration via alternative oxidase was useful for the early detection of salt responses.Manish Pandey Suprasanna Penna 2017The Crop Journal2017,5,3:2

Molecular subtypes of colorectal cancer: An emerging therapeutic opportunity for personalized medicine显示文摘Molecular subtypes-based therapies offer new potential framework for desired and precise outcome in clinical settings.Current treatment strategies in colorectal cancer are largely‘one drug fit all’model for patients that display same pathological conditions. However,CRC is a very heterogenous set ofmalignancy that does not support for above criteria. Each subtype displays different pathological and genetic signatures.Based on these features, therapeutic stratification for individual patients may be designed, which may ultimately lead to improved therapeutic outcomes. In this comprehensive review,we have attempted to briefly outline major CRC pathways.A detailed overview of molecular subtypes and their clinical significance has been discussed.Present and futuremethods, governing CRC subtyping in the era of personalized therapywith a special emphasis on CMS subtypes of CRC has been reviewed.Together, discovery and validation of new CRC patient stratification methods, screening for novel therapeutic targets, and enhanced diagnosis of CRC may improve the treatment outcome.Manish Pratap Singh Sandhya Rai Ashutosh Pandey Nand K.Singh Sameer Srivastava 2021Genes & Diseases2021,8,2:1

Flow turbulence presented by different vegetation spacing sizes within a submerged vegetation patch显示文摘This study presents results from a vegetation-induced flow experimental study which investigates 3-D turbulence structure profiles, including Reynolds stress, turbulence intensity and bursting analysis of open channel flow.Different vegetation densities have been built between the adjacent vegetations, and the flow measurements are taken using acoustic Doppler velocimeter(ADV)at the locations within and downstream of the vegetation panel.Three different tests are conducted, where the first test has compact vegetations, while the second and the third tests have open spaces created by one and two empty vegetation slots within the vegetated field.Observation reveals that over 10%of eddies size is generated within the vegetated zone of compact vegetations as compared with the fewer vegetations.Significant turbulence structures variation is also observed at the points in the non-vegetated row.The findings from burst-cycle analysis show that the sweep and outward interaction events are dominant, where they further increase away from the bed.The effect of vegetation on the turbulent burst cycle is mostly obvious up to approximately two-third of vegetation height where this phenomenon is also observed for most other turbulent structure.Chukwuemeka Kingsley John Jaan H.Pu Yakun Guo Prashanth R.Hanmaiahgari Manish Pandey 2023Journal of Hydrodynamics2023,35,6:0
Influence of cohesion on California bearing ratio of clay-gravel mixtures显示文摘An understanding of the behavior of cohesive sediment is required to solve various engineering problems such as scour around bridge elements, mitigation of soil erosion, pavement design, river bed degradation, stable channel design.Pavement foundation designers principally use the California bearing ratio(CBR)to describe the subgrade and subbase materials and their strength.Several laboratory experiments were done to study the variation in the CBR of cohesive mixtures comprised of clay-gravel mixtures.Nine different clay-gravel mixtures were used in which the clay content varies from 10%to 50%by weight. The variation of the CBR with clay percentage, moisture content, and undrained shear strength parameters was studied. The CBR value reduces with the increase in the moisture content and clay fraction in the mixtures and increases with an increase in the dry density of the mixture under unsoaked conditions. The CBR also increases with the increase of the angle of internal friction of clay-gravel mixtures.A functional relation has been identified to estimate the CBR of clay-gravel mixtures.Using multiple linear regression analysis(MLRA),a relation is proposed to estimate the CBR of clay-gravel mixtures under unsoaked conditions.A statistical analysis was done to judge the behavior of the pertinent variables on the CBR.The proposed relation predicts the CBR of clay-gravel mixtures very well.Artificial neural network(ANN)analysis using R programming was also done to determine the effects of the pertinent variables on the CBR.ANN methodology was applied to predict the contribution of each variable.Three different methods:Garson algorithm,Olden algorithm, and Lek's profile model are used to assess the influence of variable parameters. The Olden algorithm and Lek's profile both show positive association of cohesion with CBR in an unsoaked condition. The role of moisture content was found to be marginally negative in the Olden algorithm and Lek's profile results. It is found that both the ANN and MLRA models are accurate in predicting the CBR of clay-gravel mixtures. It was further found that the MLRA and ANN models are reliable and rapid tools for correct assessment of the CBR of cohesive soil mixtures using the basic soil properties.Rajesh Jain Ketan Timani Manish Pandey 2023International Journal of Sediment Research2023,38,3:0
Tongue thickness in health vs cirrhosis of the liver:Prospective observational study显示文摘BACKGROUNDMalnutrition affects 40%-90% of patients with cirrhosis of the liver. L3 skeletalmuscle index (L3SMI) is presently accepted as the most objective and quantitativemeasure available for sarcopenia, a surrogate marker of malnutrition. L3SMIapplication is, however, limited by non-availability of computed tomographyscanning in remote areas, cost, need for extensive training, and the risk ofexposure to radiation. Therefore, an alternative dependable measure with wideravailability is needed. Malnutrition causes sarcopenia not only in skeletal musclesbut also in other muscular structures such as the psoas muscle, diaphragm andtongue. We therefore hypothesised that the tongue, being easily accessible forinspection and for measurement of thickness using ultrasonography, may be usedto document sarcopenia.AIMTo measure and compare tongue thickness in healthy individuals and in patientswith cirrhosis of the liver and to study its correlation with conventionalprognostic scores for patients with cirrhosis of the liver.METHODSTongue thickness was measured using ultrasonography. One hundred twentysubjects of either gender aged 18 to 65 years were studied, with 30 subjects in eachgroup. The tongue thickness was compared between groups based on “ChildTurcotte Pugh” (CTP) scores. The correlations between measured tonguethickness and “Model for end stage liver disease” (MELD) score and between age and measured tongue thickness were also assessed.RESULTSMean tongue thickness (mean ± SD) in patients with CTP class A, B and C was4.39 ± 0.39 cm, 4.19 ± 0.53 cm, and 3.87 ± 0.42, respectively, and was 4.33 ± 0.49 cmin normal healthy individuals. Significant differences were seen in tonguethickness between patients with CTP class C and those with CTP class A and B (P< 0.05). Patients with CTP class C also had a significantly reduced tonguethickness than normal individuals (P < 0.05). However, no significant differencewas seen in tongue thickness between patients with CTP class A and B andnormal individuals. A statistically significant, negative correlation was foundbetween MELD score and tongue thickness (r = -0.331) (P < 0.001). No correlationwas observed between L3SMI and MELD score (r = 0.074, P = 0.424). L3SMI(mean ± SD) in healthy subjects was 39.66 ± 6.8 and was 38.26 ± 8.88 in patientswith CTP class C, and the difference was not significant. No significant correlationwas found between age of the patients and tongue thickness. Intra-classcorrelation coefficient was used to determine the reliability of the tonguethickness measurements. The intra-class correlation coefficient was 0.984 (95%CI:0.979-0.989) and was indicative of good reliability.CONCLUSIONTongue thickness measured by ultrasonography, correlates significantly with theseverity of liver disease, as assessed by CTP and MELD scores. The patients with aCTP score ≥ 10 have significantly reduced tongue thickness as compared tonormal individuals and those with less severe liver disease and CTP scores of 5-9.No significant difference in tongue thickness was found between healthyindividuals and CTP class A and B patients.Manish Tandon Harshita Singh Nishant Singla Priyanka Jain Chandra Kant Pandey 2020World Journal of Gastrointestinal Pharmacology and Therapeutics2020,11,3:0
A Novel Approach for Enhancement of Geometric and Contrast Resolution Properties of Low Contrast Images显示文摘The present work encompasses a new image enhancement algorithm using newly constructed Chebyshev fractional order differentiator. We have used Chebyshev polynomials to design Chebyshev fractional order differentiator. We have generated the high pass filter corresponding to it. The designed filters are applied for decomposing the input image into four bands and low-low(L-L) sub-band is updated using correction coefficients. Reconstructed image with updated L-L sub-band provides the enhanced image. The visual results obtained are encouraging for image enhancement. The applicability of the developed algorithm is illustrated on three different test images. The effects of order of differentiation on the edges of images have also been presented and discussed.Koushlendra Kumar Singh Manish Kumar Bajpai Rajesh Kumar Pandey 2018IEEE/CAA Journal of Automatica Sinica2018,5,2:0
