1卒中相关性肺炎的诊断卒中肺炎共识小组的推荐意见显示文摘背景和目的下呼吸道感染常并发于卒中并对转归造成不利影响。对于并发于卒中的下呼吸道感染性疾病,目前尚无公认的专业术语或诊断金标准,这导致临床诊疗和研究受到影响。本共识旨在提出急性卒中并发下呼吸道感染的标准化术语和可操作性诊断标准。方法对多个文献数据库进行系统检索,经过证据回顾和2轮讨论,最后于2014年9月在英国曼彻斯特召开共识会议,经75%以上共识小组成员达成一致意见后形成共识。结果达成的共识如下:(1)推荐使用卒中相关性肺炎(stroke-associated pneumonia, SAP )作为卒中发病后7 d内发生的下呼吸道感染性疾病的专业术语。(2)提议对 SAP 使用经过改良的美国疾病控制预防中心(Centers for Disease Control and Prevention, CDC)诊断标准:①很可能的 SAP:符合 CDC 诊断标准,但复查或动态监测胸部 X 片仍缺乏典型改变;②确诊的 SAP:符合 CDC 诊断标准,且胸部 X 片有典型改变。(3)关于白细胞计数或 C反应蛋白在 SAP 诊断中的作用证据有限。(4)其他生物学标记物(如降钙素原)的证据不足。结论提议根据 CDC 诊断标准对 SAP 的术语和诊断制定可操作性的共识标准,需要在卒中患者中进行前瞻性研究来确定其信度、效度以及对医生行为(包括抗生素选择)和临床转归的影响。Craig J. 2Virtual preoperative planning and 3D printing are valuable for the management of complex orthopaedic trauma显示文摘Purpose:The technology of 3D printing(3DP)exists for quite some time,but it is still not utilized to its full potential in the field of orthopaedics and traumatology,such as underestimating its worth in virtual preoperative planning(VPP)and designing various models,templates,and jigs.It can be a significant tool in the reduction of surgical morbidity and better surgical outcome avoiding various associated complications.Methods:An observational study was done including 91 cases of complex trauma presented in our institution requiring operative fixation.Virtual preoperative planning and 3DP were used in the management of these fractures.Surgeons managing these cases were given a set of questionnaire and responses were recorded and assessed as a quantitative data.Results:In all the 91 cases,where VPP and 3DP were used,the surgeons were satisfied with the outcome which they got intraoperatively and postoperatively.Surgical time was reduced,with a better outcome.Three dimensional models of complex fracture were helpful in understanding the anatomy and sketching out the plans for optimum reduction and fixation.The average score of the questionnaire was 4.5,out of a maximum of 6,suggesting a positive role of 3DP in orthopaedics.Conclusion:3DP is useful in complex trauma management by accurate reduction and placement of implants,reduction of surgical time and with a better outcome.Although there is an initial learning curve to understand and execute the VPP and 3DP,these become easier with practice and experience.Abhishek Mishra Tarun Verma Abhishek Vaish Riya Vaish Raju Vaishya Lalit Maini 2019Chinese Journal of Traumatology2019,22,6:7
3Human sperm immobilization effect of Carica papaya seed extracts: an in vitro study显示文摘Aim: To examine if the seed extracts of Carica papaya, which showed antispermatogenic/sperm immobilization properties in animal models, could cause human sperm immobilization in vitro. Methods: Chloroform extract, benzene chromatographic fraction of the chloroform extract, its methanol and ethyl acetate sub-fractions and the isolated compounds from the sub-fractions i.e., ECP 1 & 2 and MCP 1 & 2, of the seeds of Cadca papaya were used at concentrations of 0.1%, 0.5%, 1% and 2%. Sperm motility was assessed immediately after addition of extracts and every 5 minutes thereafter for 30 minutes. Results: There were dose-dependent spermicidal effects showing an instant fall in the sperm motility to less than 20 % at 2 % concentration. Isolated compounds ECP 1 & 2 were more effective inducing a motility of less than 10%. Many of the spermatozoa became vibratory on the spot. Total inhibition of motility was observed within 20 - 25 min at all concentrations of all products. Scanning and transmission electron microscopy revealed deleterious changes in the plasma membrane of the head and mid-piece of spermatozoa. Sperm viability test and the number of abnormal spermatozoa after completion of incubation suggested that the spermatozoa were infertile. The effects were spermicidal but not spermiostatic as revealed by the sperm revival test. 4Hepatoprotective activity of Terminalia paniculata against paracetamol induced hepatocellular damage in Wistar albino rats显示文摘Objective:To evaluate the hepatoprotective activity of Terminalia paniculata against paracetamol induced hepatic damage in rats.Methods:The plant material was shade dried, powdered and extracted with ethanol.Liv 52 and silymarin were used as standard drugs and 2%gum acacia as a control(vehicle).Alteration in the levels of biochemical markers of hepatic damage like AST,ALT,ALP and lipid peroxides were tested,and phytochemical tests were also performed.Results:Paracetamol(2 g/kg) increased the serum levels of alanine aminotransfer (ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP) and the lipid peroxides. Treatment of Liv 52,silymarin and ethanolic extract of Terminalia paniculata(200 mg/kg) altered levels of biochemical marker and showed significant hepatoprotective activity.Ethanolic extract revealed the presence of phenolic compound and flavanoids.Our findings suggested that ethanolic bark extract of Terminalia paniculata possessed hepatoprotective activity in a dose dependent manner.Conclusions:Terminalia paniculata possesses hepatoprotective activity.It could be an effective and promising preventive agent against PCT induced hepatotoxicity.Eesha BR Mohanbabu Amberkar V Meena Kumari K Sarath babu Vijay M Lalit M Rajput R 2011Asian Pacific Journal of Tropical Medicine2011,4,6:5
5Study of locust bean gum reinforced cyst-chitosan and oxidized dextran based semi-IPN cryogel dressing for hemostatic application显示文摘Severe blood loss due to traumatic injuries remains one of the leading causes of death in emergency settings.Chitosan continues to be the candidate material for hemostatic applications due to its inherent hemostatic properties.However,available chitosan-based dressings have been reported to have an acidic odor at the wound site due to the incorporation of acid based solvents for their fabrication and deformation under compression owing to low mechanical strength limiting its usability.In the present study semi-IPN cryogel was fabricated via Schiff's base cross-linking between the polyaldehyde groups of oxidized dextran and thiolated chitosan in presence of locust bean gum(LBG)known for its hydrophilicity.Polymerization at12C yielded macroporous semi-IPN cryogels with an average pore size of 124.57±20.31 mm and 85.46%porosity.The hydrophobicity index of LBG reinforced semi-IPN cryogel was reduced 2.42 times whereas the swelling ratio was increased by 156.08%compare to control cryogel.The increased hydrophilicity and swelling ratio inflated the compressive modulus from 28.1 kPa to 33.85 for LBG reinforced semi-IPN cryogel.The structural stability and constant degradation medium pH were also recorded over a period of 12 weeks.The cryogels demonstrated lower adsorption affinity towards BSA.The cytotoxicity assays(direct,indirect)with 3T3-L1 fibroblast cells confirmed the cytocompatibility of the cryogels.The hemolysis assay showed<5%hemolysis confirming blood compatibility of the fabricated cryogel,while whole blood clotting and platelet adhesion assays confirmed the hemostatic potential of semi-IPN cryogel.Lalit Kumar Meena Pavani Raval Dhaval Kedaria Rajesh Vasita 2018Bioactive Materials2018,3,3:4
6Prospects to the formation and control of potential dimer impurity E of pantoprazole sodium sesquihydrate显示文摘Pantoprazole sodium, a substituted benzimidazole derivative, is an irreversible proton pump inhibitor which is primarily used for the treatment of duodenal ulcers, gastric ulcers, and gastroesophageal reflux disease (GERD). The monographs of European Pharmacopoeia (Ph. Eur.) and United States Pharmacopoeia (USP) specify six impurities, viz.;impurities A, B, C, D, E and F, respectively for its active pharmaceutical ingredient (API). The identification and synthesis of all impurities except impurity E are well described in the literature;however, there is no report related to impurity E. The prospects to the formation and controlling of impurity E up to -0.03% in the synthesis of pantoprazole sodium sesquihydrate (PAN) were discussed in detail for the first time. The present work described the journey towards the successful development of an optimal preparation procedure of dimer impurity E. 7Computational fluid dynamics modeling of abutment scour under steady current using the level set method显示文摘The scour and deposition pattern around an abutment under constant discharge condition is calculated using a three dimensional(3D) Computational Fluid Dynamics(CFD) model.The Reynolds-Averaged Navier Stokes(RANS) equations are solved in three dimensions using a CFD model.The Level Set Method(LSM) is used for calculation of both free surface and bed topography.The two-equation turbulence model(k-ε and k-ω) is used to calculate the eddy viscosity in the RANS equations.The pressure term in the RANS equations on a staggered grid is modeled using the Chorin’s projection method.The 5th order Weighted Essentially Non-Oscillatory(WENO) scheme discretizes the convective term of the RANS equations.The Kovacs and Parker and Dey formulations are used for the reduction in bed shear stress on the sloping bed.The model also used the sandslide algorithm which limits bed shear stress reduction during the erosion process.The numerical model solution is validated against experimental results collected at the Politecnico di Milano,Milan,Italy.Furthe r,the numerical model is tested for perfo rmance by varying the grid sizes and key parameters like the space and time discretization schemes.The effect of varying bed porosity has been evaluated.Overall,the free surface is well represented in a realistic manner and bed topography is well predicted using the Level Set Method(LSM).Mohammad Saud Afzal Hans Bihs Lalit Kumar 2020International Journal of Sediment Research2020,35,4:4
8Design and development of novel bioadhesive niosomal formulation for the transcorneal delivery of anti-infective agent:In-vitro and ex-vivo investigations显示文摘Gatifloxacin eye drops are frequently used in eye infections.However such formulations have a major drawback i.e.short duration of action and usually require 4e6 times installations daily.A chitosan coated niosomal formulation of gatifloxain was purposed to show a longer retention time on eyes and subsequent reduction in dosing frequency.Vesicles were prepared by solvent injection method using cholesterol and Span-60.An extensive optimization of formulation was done using different ratios of cholesterol,Span-60 and drug,revealed NS60-5(cholesterol:span-6050:50 and drug content of 20 mg)to be the optimized niosome formulation.NS60-5 had shown a highest entrapment efficiency of 64.9±0.66%with particle size 213.2±1.5 nm and zeta potential34.7±2.2 mV.Optimized niosomes were also coated with different concentrations of chitosan and evaluated.Permeation studies had revealed that optimized niosomes(86.77±1.31%)had increased the transcorneal permeation of Gatifloxacin more than two fold than simple drug solution(37.19±1.1%).Longer retention potential of the coated niosomes was further verified by fluorescence microscopy.Study revealed that simple dye solution got easily washed out with in 6 h.The uncoated niosomes(NS60-5)showed a longer retention(more than 6 h),which was further enhanced in case of coated niosomes i.e.CNS60-1(more than 12 h).Antimicrobial studies had shown the better efficacy of CNS60-1(zone of inhibition)when compared to marketed formulation.The final chitosan formulation was found to have shown better ocular tolerability as demonstrated by corneal hydration test histopathology investigations.Yahaya Zubairu Lalit Mohan Negi Zeenat Iqbal Sushama Talegaonkar 2015Asian Journal of Pharmaceutical Sciences2015,10,4:4

11复方草药制剂减轻高草酸尿导致的氧化应激及预防草酸钙沉积和大鼠肾细胞损伤(英文)显示文摘INTRODUCTION:Cystone is an approved Ayurvedic polyherbal proprietary medicine used in India for various urinary disorders,including urolithiasis.AIM:To evaluate the protective effect of Cystone against hyperoxaluria-induced oxidative stress and calcium oxalate crystal deposition in urolithiasis.METHODS:Ethylene glycol(EG)(0.75%,V/V)in drinking water was given to rats for 28 days to induce urolithiasis with simultaneous treatment of Cystone(500 and 750 mg/kg body weight),and various urinary risk factors of urolithiasis and antioxidant markers were assessed.RESULTS:EG treatment lead to increased urine volume and lowered urinary pH,along with increased urinary excretion of oxalate,calcium and phosphate in untreated animals.These changes caused extensive calcium oxalate crystal deposition,increased lipid peroxidation and decreased activity of antioxidant enzymes(SOD,catalase and GPx)in the kidney of untreated rats.Cystone prevented these hyperoxaluric manifestations and inhibited calcium oxalate crystal deposition in treated rats at both doses.CONCLUSIONS:Cystone therapy provides protection against hyperoxaluria-induced oxidative stress and calcium oxalate crystal deposition by improving renal tissue antioxidant status and diuresis.Kiran S.
