| 1 | Depression and anxiety levels in therapy-nave patients with inflammatory bowel disease and cancer of the colon显示文摘AIM: To assess whether depression and anxiety are more expressed in patients with the first episode of inflammatory bowel disease (IBD) than in individuals with newly discovered cancer of the colon (CCa). METHODS: A total of 32 patients with IBD including 13 males and 19 females, aged 27 to 74, and 30 patients with CCa including 20 males and 10 females, aged 39-78, underwent a structured interview, which comprised Hamilton’s Depression Rating Inventory, Hamilton’s Anxiety Rating Inventory and Paykel’s Stressful Events Rating Scale. RESULTS: Patients of the IBD group expressed both depression and anxiety. Depressive mood, sense of guilt, psychomotor retardation and somatic anxiety were also more pronounced in IBD patients. The discriminant function analysis revealed the total depressive score was of high importance for the classification of a newly diagnosed patient into one of the groups. CONCLUSION: Newly diagnosed patients with IBD have higher levels of depression and anxiety. | 2 | Psychiatric comorbidity in the treatment of patients with inflammatory bowel disease显示文摘Ulcerative colitis and Crohn’s disease,commonly known as inflammatory bowel disease(IBD),draw attention from specialists of various disorders,including gastroenterology,psychiatry,and radiology.The involvement of a cortical influence in the brain-gut axis as well as the interaction of the hypothalamic-pituitary-adrenal axis and the peripheral nervous system provide an initial explanation of the psychological symptoms associated with IBD.The involvement of structures the limbic system,such as the anterior cingulate cortex,the prefrontal cortex,and the amygdala,paves the way for the discovery of the mechanisms underlying depression depression,anxiety,alexithymia,personality traits,and other psychological impairments following the onset of IBD.Psychiatric therapy in IBD patients is almost as important as the gastroenterological approach and consists of pharmacological treatment and psychotherapy.Neither of the available psychiatric treatment methods is considered the golden standard because both methods have side effects,and psychotropic medication can provoke the worsening of IBD symptoms.Thus,both approaches must be applied with awareness of the possibility of side effects.We suggest that psychiatrists and gastroenterologists work together to reach a consensus on IBD therapy to ensure success and to reduce side effects and relapse to the lowest possible rates. | Branislav R Filipovic Branka F Filipovic | 2014 | World Journal of Gastroenterology2014，20，13： | 9 |
| 3 | Comparison of two nutritional assessment methods in gastroenterology patients显示文摘AIM:To investigate and compare efficacy and differences in the nutritional status evaluation of gastroenterology patients by application of two methods:subjective global assessment(SGA) and nutritional risk index(NRI).METHODS:The investigation was performed on 299 hospitalized patients,aged 18-84 years(average life span 55.57 ± 12.84),with different gastrointe-stinal pathology,admitted to the Department of Gastroenterohepatology,Clinical and Hospital Center 'Bezanijska Kosa' during a period of 180 d.All the patients,after being informed in detail about the study and signing a written consent,underwent nutritional status analysis,which included two different nutritional indices:SGA and NRI,anthropometric parameters,bioelectrical impedance analysis,and biochemical markers,within 24 h of admission.RESULTS:In our sample of 299 hospitalized patients,global malnutrition prevalence upon admission varied from 45.7% as assessed by the SGA to 63.9% by NRI.Two applied methods required different parameters for an adequate approach:glucose level(5.68 ± 1.06 mmol/L vs 4.83 ± 1.14 mmol/L,F = 10.63,P = 0.001);body mass index(26.03 ± 4.53 kg/m2 vs 18.17 ± 1.52 kg/m2,F = 58.36,P < 0.001);total body water(42.62 ± 7.98 kg vs 36.22 ± 9.32 kg,F = 7.95,P = 0.005);basal metabolic rate(1625.14 ± 304.91 kcal vs 1344.62 ± 219.08 kcal,F = 9.06,P = 0.003) were very important for SGA,and lymphocyte count was relevant for NRI:25.56% ± 8.94% vs 21.77% ± 10.08%,F = 11.55,P = 0.001.The number of malnourished patients rose with the length of hospital stay according to both nutritional indices.The discriminative function analysis(DFA) delineated the following parameters as important for prediction of nutritional status according to SGA assessment:concentration of albumins,level of proteins,SGA score and body weight.The DFA extracted MAMC,glucose level and NRI scores were variables of importance for the prediction of whether admitted patients would be classif ied as well or malnourished.CONCLUSION:SGA showed higher sensitivity to predictor factors.Assessment of nutritional status requires a multidimensional approach,which includes different clinical indices and various nutritional param eters. | Branka F Filipovi Milan Gaji Nikola Milini Branislav Milovanovi Branislav R Filipovi Mirjana Cvetkovi Nela ibali | 2010 | World Journal of Gastroenterology2010，16，16： | 9 |

| 5 | Pre-and intraoperative predictors of acute kidney injury after liver transplantation显示文摘BACKGROUND Acute kidney injury(AKI)after liver transplantation(LT)is a frequent and multifactorial event related to increased morbidity and mortality.Risk factors for AKI after LT still need to be clarified.AIM To identify the predictors of acute kidney injury after liver transplantation.METHODS The frequency and pre-and intraoperative predictors of AKI within the first 7 d after LT were evaluated in adult liver transplant candidates in a single LT center in Croatia.AKI was defined according to the Kidney Disease:Improving Global Outcomes criteria.RESULTS Out of 205 patients(mean age 57±10 years;73.7%males,52.7%with alcoholrelated liver disease)93(45.36%)developed AKI,and the majority of them(58.06%)had stage 1.Only 5.38%of patients required renal replacement therapy after LT.The majority of patients(82.8%)developed AKI within the first two days after the procedure.Multivariate logistic regression identified pre-LT body mass index(OR=1.1,95%CI:1.05-1.24)and red blood cell transfusion(OR=1.66,95%CI:1.09-2.53)as independent predictors of early post-LT AKI occurrence.30-d survival after LT was significantly better for patients without AKI(P=0.01).CONCLUSION Early AKI after LT is a frequent event that negatively impacts short-term survival.The pathogenesis of AKI is multifactorial,but pre-LT BMI and intraoperative volume shifts are major contributors. | Anna Mrzljak Lucija Franusic Jadranka Pavicic-Saric Tomislav Kelava Zeljka Jurekovic Branislav Kocman Danko Mikulic Ivan Budimir-Bekan Mladen Knotek | 2020 | World Journal of Clinical Cases2020，8，18： | 4 |
| 6 | Cardiac autonomic dysfunction in patients with gastroesophageal reflux disease显示文摘AIM: To investigate autonomic nervous function in patients with a diagnosis of gastroesophageal reflux disease(GERD).METHODS: The investigation was performed on 29patients(14 men), aged 18-80 years(51.14 ± 18.34),who were referred to our Neurocardiology Laboratory at the Clinical and Hospital Center 'Bezanijska Kosa'with a diagnosis of GERD. One hundred sixteen healthy volunteers matched in age and sex with the examinees served as the control group. The study protocol included the evaluation of autonomic function and hemodynamic status, short-term heart rate variability(HRV) analysis, 24 h ambulatory ECG monitoring with long-term HRV analysis and 24 h ambulatory blood pressure monitoring.RESULTS: Pathologic results of cardiovascular reflex test were more common among patients with reflux compared to the control group. Severe autonomic dysfunction was detected in 44.4% of patients and in7.9% of controls(P < 0.001). Parameters of short-term analysis of RR variability, which are the indicators ofvagal activity, had lower values in patients with GERD than in the control group. Long-term HRV analysis of time-domain parameters indicated lower values in patients with reflux disease when compared to the control group. | 7 | Carcinosarcoma of the stomach:A case report and review of the literature显示文摘Carcinosarcomas are rare,malignant,biphasic tumors. We report the case of a 62-year-old man with gastric carcinosarcoma,along with its clinical,macroscopic and histopathological features. Macroscopically,a specimen of deformed stomach was obtained that measured 200 mm×150 mm×100 mm. A 150 mm×100 mm× 50 mm exophytic tumoral mass (Borrmann typeⅠ) was found,which involved the posterior wall from the cardia to the antrum. Histopathologically,a mixed type of malignancy was revealed: an adenocarcinoma with intestinal metaplasia,with interposed fascicles of fusiform atypical cells and numerous large,rounded and oval cells. The tumor showed positive histochemistry for cytokeratin 18,epithelial membrane antigen,carcinoembryonic antigen,chromogranin A and vimentin. Liver metastases were diagnosed 8 mo postoperatively,and the patient died 4 mo later. | 10 | Mitotic crossover- an evolutionary rudiment which promotes carcinogenesis of colorectal carcinoma显示文摘Mitotic crossover is a natural mechanism that is a main source of the genetic variability of primitive organisms.In complex organisms such as mammals,it represents an evolutionary rudiment which persisted as one of the numerous DNA repair mechanisms,and results in the production of homozygous allele combinations in all heterozygous genes located on the chromosome arm distal to the crossover.This event is familiar as loss of heterozygosity,which is one of the key mechanisms responsible for the development and progression of almost all cancers.We propose the hypothesis in which mitotic crossover is a principal source of the increased loss of heterozygosity that leads to the initiation and progression of colorectal carcinoma.The hypothesis could be tested by in vitro inhibition of Rad51 protein,orthotopic grafting of human colon cancer tissue into the gut of mice,and treatment with potential inhibitors.After these procedures,the frequency of mitotic crossover would be estimated.The development of selective inhibitors of mitotic crossover could stop further carcinogenesis of colorectal carcinoma,as well as many other neoplastic events.Loss of heterozygosity is an event responsible for carcinogenesis,its reduction by selective inhibitors of mitotic crossover could have a positive effect on cancer chemoprevention,as well as on growth reduction and a cessation in the progression of earlier developed tumors. | Branislav Rovcanin Ivan Ivanovski Olivera Djuric Dimitrije Nikolic Jelena Petrovic Petar Ivanovski | 2014 | World Journal of Gastroenterology2014，20，35： | 2 |

| 12 | Methyl acetate-methanol mixture separation by extractive distillation:Economic aspects显示文摘Methyl acetate is considered low toxicity volatile solvent produced either as a by-product during methanol carbonylation or via acetic acid esterification with methanol.In both cases,pure methyl acetate has to be isolated from the reaction mixture.Simulation of methyl acetate separation from its mixture with methanol by extraction distillation was carried out in ASPEN +soft-ware.In total three case studies were assumed using two different extraction solvents and two solvent regeneration strategies.In case A,novel extraction solvent 1-ethyl-3-methylimidazolium trifluoromethanesulfonate ionic liquid, was considered.Raw material separation was achieved in an extraction distillation column while the solvent regeneration was accomplished in a second distillation column in this case.In case study B,the same extraction solvent was used;however,its regeneration was carried out in a sing/e-effect evaporator.Dimethyl sulfoxide was the second extraction solvent selected.Its use in methyl acetate-methanol separation is presented in case study C. As high purity of dimethyl sulfoxide was required for the methyl acetate-methanol azeotrope breaking,its regeneration was carried out in the second distillation column only. To simulate the ternary methyl acetate--methanol-extraction solvent mixtures separation,vapor-liquid equilibrium was predicted based on the NRTL equation.Further, unknown properties of the considered ionic liquid and variation of these properties with temperature were predicted and introduced into the ASPEN +components properties database.Based on these data,optimum operation parameters of the respective separation equip- ment were established.In all case studies,the same condition had to be fulfilled,namely minimum methyl acetate content in the distillate from the extraction distillation column of 99.5mo1-%.Results of simulations using the respective optimum operation parameters were employed in the economic evaluation of the three separation unit designs studied.It was found that the least energy-demanding design corresponds to the case study B in terms of both capital as well as operation expenses. | Elena Graczova Branislav Sulgan Samuel Barabas Pavoi Steltenpohl | 2018 | Frontiers of Chemical Science and Engineering2018，12，4： | 2 |
