1A phase Ⅱ study of gemcitabine and carboplatin combination chemotherapy in gallbladder carcinoma显示文摘BACKGROUND: Patients with carcinoma of the gallbladder have advanced, unresectable tumor at the time of presentation and face a dismal prognosis in the absence of a standard palliative chemotherapy regimen. This study was undertaken to evaluate the efficacy and safety of combined chemotherapy of gemcitabine and carboplatin in 20 patients with advanced gallbladder carcinoma. METHODS: The criteria of eligibility included chemonaive patients with unresectable gallbladder cancer, bidimensionaIly measurable disease, Zubrod’s performance status≤2, and adequate major organ function. The patients received gemcitabine (1000 mg/m^2) on days 1 and 8, and carboplatin (target AUC of 5.0 mg/ml) on day 1, in a 21-day cycle. CT was used for response assessment. RESULTS: In this group of 20 patients with advanced gallbladder carcinoma 6 were men and 14 women, with a median age of 55 years. The stage of the tumor at presentation was IVB in 14 patients (70%), IVA in 3 (15%) and Ⅲ in 3 (15%). Four patients (21%) achieved a complete response, and 3 (15.7%), a partial response; an overall response rate was 36.7%. The median time to progression of the tumor was 33.8 weeks, and 1-year survival rate of the patients was 43.3%. Anemia of WHO grade Ⅲ or Ⅳ was seen in 2 patients (10%) and 1 patient (5%), respectively. Grade Ⅲ neutropenia and thrombocytopenia were observed in 2 patients (10%) and 1 patient (5%), respectively. 2Tuning the particle size and morphology of high energetic material nanocrystals显示文摘Morphology controlled synthesis of nanoparticles of powerful high energetic compounds(HECs) such as l,3,5-trinitro-l,3,5-triazinane(RDX) and 1,3,5,7-tetranitro-l,3,5,7-tetrazocane(HMX) were achieved by a simple solvent—antisolvent interaction(SAI) method at 70 ℃.The effects of different solvents on particle size and morphology of the prepared nano-HECs were studied systematically.Particle size and morphology of the nano-HECs was characterized using field emission scanning electron microscopy(FE-SEM) imaging.X-ray diffraction(XRD) and Fourier transform infrared(FTIR) spectroscopy studies revealed that RDX and HMX were precipitated in their most stable polymorphic forms,i.e.a and P,respectively.Thermogravimetric analysis coupled with differential scanning calorimetry(TGA-DSC) studies showed that the thermal response of the nanoparticles was similar to the respective raw-HECs.HEC nanoparticles with spherical and rod shaped morphology were observed under different solvent conditions.The mean particle size also varied considerably with the use of different solvents.Raj KUMAR Prem Felix SIRIL Pramod SONI 2015Defence Technology(防务技术)2015,11,4:7
3Gastric conduit perforation显示文摘As patients with carcinoma of the esophagus live longer, complications associated with the use of a gastric conduit are increasing. Ulcers form in the gastric conduit in 6.6% to 19.4% of patients. There are a few reports of perforation of a gastric conduit in the English literature. Almost all of these were associated with serious complications. We report a patient who developed a tension pneumothorax consequent to spontaneous perforation of an ulcer in the gastric conduit 7 years after the index surgery in a patient with carcinoma of the gastroesophageal junction. He responded well to conservative management. Complications related to a gastric conduit can be because of multiple factors. Periodic endoscopic surveillance of gastric conduits should be considered as these are at a higher risk of ulcer formation than a normal stomach. Long term treatment with proton pump inhibitors may decrease complications. 4Management of splenic artery aneurysm associated with extrahepatic portal vein obstruction显示文摘BACKGROUND: Splenic artery aneurysms although rare are clinically significant in view of their propensity for spontaneous rupture and life-threatening bleeding. While portal hypertension is an important etiological factor, the majority of reported cases are secondary to cirrhosis of the liver. We report three cases of splenic artery aneurysms associated with extrahepatic portal vein obstruction and discuss their management. METHODS: The records of three patients of splenic artery aneurysm associated with extrahepatic portal vein obstruction managed from 2003 to 2010 were reviewed retrospectively. The clinical presentation, surgical treatment and outcome were analyzed. RESULTS: The aneurysm was >3 cm in all patients. The clinical symptoms were secondary to extrahepatic portal vein obstruction (hematemesis in two, portal biliopathy in two) while the aneurysm was asymptomatic. Doppler ultrasound demonstrated aneurysms in all patients. A proximal splenorenal shunt was performed in two patients with excision of the aneurysm in one patient and ligation of the aneurysm in another one. The third patient had the splenic vein replaced by collaterals and hence underwent splenectomy with aneurysmectomy. All patients had an uneventful post-operative course. CONCLUSIONS: Splenic artery aneurysms are associated with extrahepatic portal vein obstruction. Surgery is the mainstay of treatment. 5Prospects to the formation and control of potential dimer impurity E of pantoprazole sodium sesquihydrate显示文摘Pantoprazole sodium, a substituted benzimidazole derivative, is an irreversible proton pump inhibitor which is primarily used for the treatment of duodenal ulcers, gastric ulcers, and gastroesophageal reflux disease (GERD). The monographs of European Pharmacopoeia (Ph. Eur.) and United States Pharmacopoeia (USP) specify six impurities, viz.;impurities A, B, C, D, E and F, respectively for its active pharmaceutical ingredient (API). The identification and synthesis of all impurities except impurity E are well described in the literature;however, there is no report related to impurity E. The prospects to the formation and controlling of impurity E up to -0.03% in the synthesis of pantoprazole sodium sesquihydrate (PAN) were discussed in detail for the first time. The present work described the journey towards the successful development of an optimal preparation procedure of dimer impurity E. 6Effect of Low Level Subchronic Microwave Radiation on Rat Brain显示文摘Objective The present study was designed to investigate the effects of subchronic low level microwave radiation(MWR) on cognitive function,heat shock protein 70(HSP70) level and DNA damage in brain of Fischer rats.Methods Experiments were performed on male Fischer rats exposed to microwave radiation for 90 days at three different frequencies: 900,1800,and 2450 MHz.Animals were divided into 4 groups: Group I: Sham exposed,Group II: animals exposed to microwave radiation at 900 MHz and specific absorption rate(SAR) 5.953 × 10-4 W/kg,Group III: animals exposed to 1800 MHz at SAR 5.835 × 10-4 W/kg and Group IV: animals exposed to 2450 MHz at SAR 6.672 × 10-4 W/kg.All the animals were tested for cognitive function using elevated plus maze and Morris water maze at the end of the exposure period and subsequently sacrificed to collect brain tissues.HSP70 levels were estimated by ELISA and DNA damage was assessed using alkaline comet assay.Results Microwave exposure at 900-2450 MHz with SAR values as mentioned above lead to decline in cognitive function,increase in HSP70 level and DNA damage in brain.Conclusion The results of the present study suggest that low level microwave exposure at frequencies 900,1800,and 2450 MHz may lead to hazardous effects on brain.Pravin Suryakantrao Deshmukh Kanu Megha Namita Nasare Basu Dev Banerjee Rafat Sultana Ahmed Mahesh Pandurang Abegaonkar Ashok Kumar Tripathi Pramod Kumari Mediratta 2016Biomedical and Environmental Sciences2016,29,12:4

9Effect of Semecarpus anacardium fruits on reproductive function of male albino rats显示文摘Aim: To evaluate the effect of an ethanolic extract of Semecarpus anacardium fruits on spermatogenesis in albino rats. Methods: Male albino rats were fed with a 50 % ethanolic extract of Semecarpus anacardium fruit at 100 mg·kg^-l·day^-1, 200 mg·kg^-l·day^-1 and 300 mg·kg^-1·day^-1 for 60 days. Fertility test was performed after 60 days of treatment. Sperm motility and density were observed in the cauda epididymis. Biochemical and histological analyses of the blood and reproductive organs were done. Recovery of fertility was followed to evaluate the reversibility of drug action. Results: S. anacardium fruit extract administration resulted in spermatogenic arrest in albino rats. Thes perm motility and density was reduced significantly. The RBC and WBC counts, haemoglobin, haematocrit, blood sugar and urea were found to be within the normal range in the whole blood. The protein, cholesterol and glycogen in the testes and the fructose in the seminal vesicle were significantly decreased after the treatment. The fruit extract feeding caused marked reduction in the number of primary spermatocytes, secondary spermatocytes and spermatids.The number of mature Leydig cells was also decreased and degenerating cells increased proportionately. 10Bipolar radiofrequency ablation of tibialchondroblastomas: A report of three cases显示文摘Chondroblastoma is a rare benign cartilaginous neoplasm of bone. The recurrence rate is high and complications are frequent following open curettage with bone grafting which is the standard treatment forchondroblastomas. We performed radiofrequency ablation in three cases of tibialchondroblastoma using the bipolar system. One patient experienced residual pain for which repeat ablation was performed. No other complications were observed during follow-up. Radiofrequency ablation may offer an effective alternative for the treatment of selected cases of chondroblastoma. 11Predicting suitability of different scale-dependent dispersivities for reactive solute transport through stratified porous media显示文摘In this paper,the behavior of breakthrough curves(BTCs) for reactive solute transport through stratified porous media is investigated.A physical laboratory model for layered porous media was constructed,in which thin layer of gravel was sandwiched in between two thick layers of natural soil.Gravel layer and natural soil layers were hydraulically connected as single porous continuum.A constant source of tracer was connected through gravel layer and elucidated at different sampling points in the direction of flow.Flexible multiprocess non-equilibrium(MPNE) transport equation with scale-dependent dispersivity function was used to simulate experimental BTCs of reactive solute transport through layered porous media.The values of equilibrium sorption coefficient and other input parameters were obtained experimentally.The simulation of BTC was performed using MPNE model with scale-dependent dispersivity.The simulation of different scale-dependent dispersivities was then compared and it was found that for field scale of estimation of dispersivity,asymptotic and exponential dispersivity functions performed better.In continuation to the comparison of simulated BTCs obtained using different models,spatial moment analysis of each aforesaid scale-dependent dispersivity model was also done.Spatial moment analysis provides the information related to mean solute mass,rate of mass travel,and mean plume dispersion.Linear and constant dispersivities showed higher variance as compared to asymptotic and exponential dispersion functions.This supports the field applicability of asymptotic and exponential dispersivity functions.The BTCs were also found to elucidate a nonzero concentration with time,which was clearly affected by physical non-equilibrium.In natural condition,such information is required in effective aquifer remediation process.Deepak Swami Abhimanyu Sharma Pramod Kumar Sharma Dericks P.Shukla 2016Journal of Rock Mechanics and Geotechnical Engineering2016,8,6:2

A parity prediction circuit based onthe code generator polynomial that leads lower space overhead has been designed. 13Climate change and finger millet: Perception, trend and impact on yield in different ecological regions in Central Nepal显示文摘The views of local people on climate change along different ecological regions are relatively unexplored in Nepal. This study was conducted in 13 villages in central Nepal at different altitudes to document the views of small holder farmers and compare their perception with trends of climatic variables, finger millet yield, natural disasters, plant phenology(flowering and fruiting), status of forest and wild life, as well as the spread of diseases and pests. Analysis on the climatic data of stations for 36-41 years between 1975 and 2016 showed significant increases in the minimum temperature in lower tropical climatic region(<500 m), upper tropical to subtropical climatic region(500-2000 m) and temperate climatic region(2000-3000 m) by 0.01, 0.026 and 0.054℃/year, respectively, and an increase of maximum temperature by 0.008, 0.018, and 0.019℃/year, respectively. Rainfall showed a strongly significant decreasing trend in all elevation regions. This result matches with the views of respondents except 38% respondent from temperate climatic region. People from the temperate climatic region also mentioned that current onset of snowfall is delayed but amount of snowfall remained the same. From the documented records, except events of wild fire, frequency of natural disasters events have increased in the recent years, which was in harmony with the views of local people. Multi-linear regression analysis showed that contribution of climatic variables on finger millet yield in lower tropical climatic region and upper tropical to subtropical regions was 23% and 57.3%, respectively, which was supported by increasing trend on average growing degree day(GDD) temperature at the rate of 0.01℃ in upper tropical to subtropical region and 0.007℃ in lower tropical climatic region yearly. Finger millet yield has been increasing at the rate of 7.39 and 36.9 kg/ha yearly in lower tropical climatic region and upper tropical to subtropical climatic region, respectively. 14Habitat Range of two Alpine Medicinal Plants in a Trans-Himalayan Dry Valley,Central Nepal显示文摘Understanding of the habitat range of threatened Himalayan medicinal plants which are declining in their abundance due to high anthropogenic disturbances is essential for developing conservation strategies and agro-technologies for cultivation.In this communication, we have discussed the habitat range of two alpine medicinal plants, Aconitum naviculare(Brühl) Stapf and Neopicrorhiza scrophulariiflora(Pennel) Hong in a trans-Himalayan dry valley of central Nepal, Manang district.They are the most prioritized medicinal plants of the study area in terms of ethnomedicinal uses.A.naviculare occurs on warm and dry south facing slopes between 4090~4650 m asl along with sclerophyllous and thorny alpine scrubs, while N.scrophulariiflora is exclusively found on cool and moist north facing slope between 4000 and 4400 m asl where adequate water is available from snow melt to create a suitable habitat for this wetland dependent species.The soil in rooting zone of the two plants differs significantly in organic carbon(OC), organic matter(OM), total nitrogen(N) and carbon to nitrogen(C/N) ratio.Due to cool and moist condition of N.scrophulariiflora habitat, accumulation of soil OC is higher, but soil N content is lower probably due to slow release from litter, higher leaching loss and greater retention in perennial live biomass of the plant.The C/N ratio of soil is more suitable in A.navuculare habitat than that of N scrophulariiflora for N supply.Warm and sunny site with N rich soil can be suitable for cultivation of A.naviculare, while moist and cool site with organic soil for N.scrophulariiflora.The populations of both the plants are fragmented and small.Due to collection by human and tramplingdamage by livestock, the population of A.naviculare was found absent in open areas in five of the six sampling sites and it was confined only within the bushes of alpine scrubs.For N.scrophulariiflora, high probability of complete receding of small glaciers may be a new threat in future to its habitat.The information about habitat conditions, together with the information from other areas, can be useful to identify potential habitats and plan for cultivation or domestication of the two medicinal plants.Bharat Babu SHRESTHA Pramod Kumar JHA 2009Journal of Mountain Science2009,6,1:2

17On-current modeling of short-channel double-gate(DG) MOSFETs with a vertical Gaussian-like doping profile显示文摘An analytic drain current model is presented for doped short-channel double-gate MOSFETs with a Gaussian-like doping profile in the vertical direction of the channel.The present model is valid in linear and saturation regions of device operation.The drain current variation with various device parameters has been demonstrated. 18Synthesis,isolation,identification and characterization of new process-related impurity in isoproterenol hydrochloride by HPLC,LC/ESI-MS and NMR显示文摘One unknown impurity(Imp-Ⅱ) during the analysis of laboratory batches of isoproterenol hydrochloride was detected in the level ranging from 0.04% to 0.12% by high performance liquid chromatography with UV detection. The unknown impurity structure was proposed as 4-[2-(propan-2-ylamino)ethyl]benzene-1,2-diol(Imp-Ⅱ) using the liquid chromatography–mass spectrophotometry(LC–MS) analysis. Imp-Ⅱ was isolated by semi-preparative liquid chromatography from the impurity-enriched reaction crude sample. Its proposed structure was confirmed by nuclear magnetic spectroscopy such as ~1H,^(13)C, DEPT(1D NMR), HSQC(2D NMR)and infrared spectroscopy(IR), and retention time and purity with HPLC followed by the chemical synthesis.Due to less removable nature of Imp-Ⅱ during the purification, the synthetic process was optimized proficiently to control the formation of Imp-Ⅱ below to the limit < 0.12% in the course of reaction. 19Primary pediatric mid-brain lymphoma:Report of a rare pediatric tumor in a rare location显示文摘Primary central nervous system lymphoma(PCNSL) is a rare disease in pediatric age group. A thirteen-yearold male child presented with complaints of headache for six months, vomiting and diplopia for three days. Magnetic resonance imaging of the brain showed a single lesion of 1.7 cm × 1.6 cm × 1.6 cm in the mid brain and tectum. He underwent a gross total resection of the tumor. The histopathological evaluation revealed B cell high grade non Hodgkin lymphoma. The patient was treated with High dose methotrexate and cranio spinal radiation. The patient was alive without disease 12 mo after completion of treatment. This case highlights importance of keeping PCNSL as differential in brain stem lesions of pediatric patients also.
