1Feasibility and safety of autologous bone marrow mononuclear cell transplantation in patients with advanced chronic liver disease显示文摘AIM: To evaluate the safety and feasibility of bone marrow cell (BMC) transplantation in patients with chronic liver disease on the waiting list for liver transplantation. METHODS: Ten patients (eight males) with chronic liver disease were enrolled to receive infusion of autologous bone marrow-derived cells. Seven patients were classified as Child-Pugh B and three as Child-Pugh C. Baseline assessment included complete clinical and laboratory evaluation and abdominal MRI. Approximately 50 ml of bone marrow aspirate was prepared by centrifugation in a ficoll-hypaque gradient. At least of 100 millions of mononuclear-enriched BMCs were infused into the hepatic artery using the routine technique for arterial chemoembolization for liver tumors. Patients were followed up for adverse events up to 4 mo. RESULTS: The median age of the patients was 52 years (range 24-70 years). All patients were discharged 48 h after BMC infusion. Two patients complained ofmild pain at the bone marrow needle puncture site. No other complications or specific side effects related to the procedure were observed. Bilirubin levels were lower at 1 (2.19 ± 0.9) and 4 mo (2.10 ± 1.0) after cell transplantation that baseline levels (2.78 ± 1.2). Albumin levels 4 mo after BMC infusion (3.73 ± 0.5) were higher than baseline levels (3.47 ± 0.5). International normalized ratio (INR) decreased from 1.48 (SD = 0.23) to 1.43 (SD = 0.23) one month after cell transplantation. CONCLUSION: BMC infusion into hepatic artery of patients with advanced chronic liver disease is safe and feasible. In addition, a decrease in mean serum bilirubin and INR levels and an increase in albumin levels are observed. 2Current knowledge on diabetic retinopathy from human donor tissues显示文摘According to the American Diabetes Association,diabetes was the seventh leading cause of death,and diabetic retinopathy the leading cause of blindness in working age adults in the United States in 2010.Diabetes is characterized by hyperglycemia associated with either hypoinsulinemia or insulin resistance,and over time,this chronic metabolic condition may lead to various complications including kidney failure,heart attacks,and retinal degeneration.In order to better understand the molecular basis of this disease and its complications,animal models have been the primary approach used to investigate the effects of diabetes on various tissues or cell types of the body,including the retina.However,inherent to these animal models are critical limitations that make the insight gained from these models challenging to apply to the human pathology.These difficulties in translating the knowledge obtained from animal studies have led a growing number of research groups to explore the diabetes complications,especially diabetic retinopathy,on tissues from human donors.This review summarizes the data collected from diabetic patients at various stages of diabetic retinopathy and classifies the data based upon their relevance to the main aspects of diabetic retinopathy:retinal vasculature dysfunction,inflammation,and neurodegeneration.This review discusses the importance of those studies to discriminate and establish the relevance of the findings obtained from animal models but also the limitations of such approaches.Jessica H Eisma Jennifer E Dulle Patrice E Fort 2015World Journal of Diabetes2015,6,2:19
3Role of soil rhizobacteria in phytoremediation of heavy metal contaminated soils显示文摘Heavy metal pollution of soil is a significant environmental problem and has its negative impact on human health and agriculture. Rhizosphere, as an important interface of soil and plant, plays a significant role in phytoremediation of contaminated soil by heavy metals, in which, microbial populations are known to affect heavy metal mobility and availability to the plant through release of chelating agents, acidification, phosphate solubilization and redox changes, and therefore, have potential to enhance phytoremediation processes. Phytoremediation strategies with appropriate heavy metal-adapted rhizobacteria have re-ceived more and more attention. This article paper reviews some recent advances in effect and significance of rhizobacteria in phytoremediation of heavy metal contaminated soils. 4Silymarin in non alcoholic fatty liver disease显示文摘AIM: This study was undertaken to evaluate the hepatic effects of silybum marianum on non alcoholic fatty liver disease (NAFLD). METHODS: In 72 patients affected by NAFLD, main metabolic, hepatic and anti-inflammatory parameters were assayed after 3 mo of a restricted diet and before silymarin treatment (twice a day orally). The brightness of liver echography texture (hepatorenal ratio brightness) was also defined at same time. These evaluations were repeated after 6 mo of treatment. RESULTS: Serum levels of some metabolic and anti-inflammatory data nonsignificantly lowered after 6 mo of silymarin. On the contrary, Steato test, alanine aminotransferase (ALT), aspartate aminotransferase (AST) and gamma-glutamyl transpeptidase were significantly (P < 0.001) reduced. Instead, the AST/ALT ratio unchanged. Finally, the hepatorenal brightness ratio, as an index of hepatic steatosis, significantly (P < 0.05) dropped. CONCLUSION: The obtained results indicate that silymarin appears to be effective to reduce the biochemical, inflammatory and ultrasonic indices of hepatic steatosis. 6Root and leaf functional trait relations in Poaceae species: implications of differing resource- acquisition strategies显示文摘Aims root systems play an essential role in grassland functioning in both acquisition and storage of resources.Nevertheless,root functional traits have not received as much attention as those measured on above-ground organs,and little is known about their relations.our objec-tives were to test whether morphological and root system traits allowed identification of grass species’functional strategies and to determine whether a relation exists between above-and below-ground traits.Methods Functional traits of root tissues(specific root length,diameter,tissue density and nitrogen concentration),whole root systems(root mass,root length density,root mass percentage below a depth of 20 cm and fine root%)and two major leaf traits(specific leaf area and leaf dry matter content)were determined under field conditions and their rela-tions were analysed in eleven perennial temperate Poaceae species.Important Findings Canonical correspondence analysis along axis 1 revealed a gra-dient of species,from those with deep,dense and coarse root systems with a large root mass to those with shallow root sys-tems,thin roots and high specific root length;this suggests strong correlations among root traits.Correlations between specific root length and specific leaf area reveal two groups of species,which probably indicates different drought-tolerance capacities.root trait syndromes enable ranking grasses along a gradient from conservative-strategy species(from stressful habitats),which display a deep and coarse root system,to acquisitive species(from rich and moist meadows),which display a shallow and thin root system.although both types display similar above-ground strategies,drought-tolerant species have lower specific root lengths than drought-sensitive species,revealing more conservative root strategies.Florian Fort Claire Jouany Pablo Cruz 2013Journal of Plant Ecology2013,6,3:11
7Targeting the insulin-like growth factor pathway in hepatocellular carcinoma显示文摘Hepatocellular carcinoma(HCC) is the third leading cause of cancer-related deaths worldwide. Only 30%-40% of the patients with HCC are eligible for curative treatments, which include surgical resection as the first option, liver transplantation and percutaneous ablation. Unfortunately, there is a high frequency of tumor recurrence after surgical resection and most HCC seem resistant to conventional chemotherapy and radiotherapy. Sorafenib, a multi-tyrosine kinase inhibitor, is the only chemotherapeutic option for patients with advanced hepatocellular carcinoma. Patients treated with Sorafenib have a significant increase in overall survival of about three months. Therefore, there is an urgent need to develop alternative treatments. Due to its role in cell growth and development, the insulin-like growth factor system is commonly deregulated in many cancers. Indeed, the insulin-like growth factor(IGF) axis has recently emerged as a potential target for hepatocellular carcinoma treatment. To this aim, several inhibitors of the pathway have been developed suchas monoclonal antibodies, small molecules, antisense oligonucleotides or small interfering RNAs. However recent studies suggest that, unlike most tumors, HCC development requires increased signaling through insulin growth factor Ⅱ rather than insulin growth factor Ⅰ. This may have great implications in the future treatment of HCC. 8Early plasmapheresis and rituximab for acute humoral rejection after ABO-compatible liver transplantation显示文摘Acute humoral rejection (AHR) is uncommon after ABO-compatible liver transplantation. Herein, we report two cases of AHR treated with plasmapheresis and rituximab in two ABO-compatible liver-transplant patients with preformed anti-human leukocyte antigen donor-specif ic antibodies. Patient 1 experienced a biopsy-proven AHR at day 10 post-transplant. She was treated by steroid pulses, and OKT3. Because of persisting signs of biopsy-proven AHR at day 26, she was treated by plasmapheresis and rituximab. Liver enzyme levels did not improve, and she died on day 41. Patient 2 experienced a biopsy-proven AHR on day 10 post-transplant. She was treated by steroid pulses, plasmapheresis, and rituximab. Liver enzymes returned to within normal range 18 d after diagnosis. Liver biopsies, at 3 and 9 mo post-transplant, showed complete resolution of AHR. We conclude that plasmapheresis should be started as soon as AHR is diagnosed, and be associated with a B-cell depleting agent. 9通过手术导致的重量损失由胃绕过在疾病的肥胖的病人改进非含酒精的脂肪肝疾病显示文摘 AIM:To evaluate the effects of surgical weight loss(Roux-en-Y gastric bypass with a modified Fobi-Capella technique) on non alcoholic fatty liver disease in obese patients.METHODS:A group of 26 morbidly obese patients aged 45 ± 2 years and with a body mass index > 40 kg/m 2 who underwent open surgical weight loss operations had paired liver biopsies,the first at surgery and the second after 16 ± 3 mo of weight loss.Biopsies were evaluated and compared in a blinded fashion.The presence of metabolic syndrome,anthropometric and biochemical variables were also assessed at baseline and at the time of the second biopsy.RESULTS:Percentage of excess weight loss was 72.1% ± 6.6%.There was a reduction in prevalence of metabolic syndrome from 57.7%(15 patients) to 7.7%(2 patients)(P < 0.001).Any significance difference was observed in aspartate aminotransferase or alanine aminotransferase between pre and postsurgery.There were improvements in steatosis(P < 0.001),lobular(P < 0.001) and portal(P < 0.05) inflammation and fibrosis(P < 0.001) at the second biopsy.There were 25(96.1%) patients with non alcoholic steatohepatitis(NASH) in their index biopsy and only four(15.3%) of the repeat biopsies fulfilled the criteria for NASH.The persistence of fibrosis(F > 1) was present in five patients at second biopsy.Steatosis and fibrosis at surgery were predictors of significant fibrosis postsurgery.CONCLUSION:Restrictive mildly malabsorptive surgery provides significant weight loss,resolution of metabolic syndrome and associated abnormal liver histological features in most obese patients.Víctor Vargas Helena Allende Albert Lecube Maria Teresa Salcedo Juan A Baena-Fustegueras José M Fort Joaquín Rivero Roser Ferrer Roberto Catalán Eva Pardina Santiago Ramón y Cajal Jaime Guardia Julia Peinado-Onsurbe 2012World Journal of Hepatology2012,4,12:6

11保守疗法治疗先天性气管狭窄的效果:一项循证长期随访研究显示文摘Background/Purpose:Surgery has been the management of choice for severe congenital tracheal stenosis (CTS). The role of conservative management of CTS however is not clear. The aim of this study is to characterize the natural history of CTS,review the radiologic evidence of tracheal growth,and evaluate the clinical outcome and selection criteria of conservative management of CTS. Methods:A retrospective study was carried out on 22 consecutive children with symptomatic CTS admitted into a single institution between 1982 and 2001. The patients were categorized into operation (n = 11) and observation(n = 11) groups. Six patients of the observation group were followed up with serial computed tomography scan. Their tracheal growth was compared with that of healthy children of the same age. Results:The mortality rates of observation and operation groups were 9%and 27%,respectively,although the latter group consisted of more severely affected patients. The pathologic categorization of the CTS influenced the survival rates (P = 0.046,χ2),with the long segment type having the worst prognosis (67%). Serial computed tomography scans of 6 conservatively managed patients revealed that all stenotic tracheas continued to grow (P = .039,2-tailed paired Student’s t test). Of the 6 stenotic tracheas,5 grew at a faster-than normalrate,and the stenotic tracheal diameters approached those of normal diameters by the age of 9 years. Conclusions:The management of patients with symptomatic CTS should be individualized. A selected group of patients with CTS can be safely managed nonoperatively.Manson D.E. Forte V. P.C.W. 14Dietary supplementation of Rosmarinus officinalis L.leaves in sheep affects the abundance of rumen methanogens and other microbial populations显示文摘Background: Rumen microbiome has a great influence on ruminant health and productivity. Different plant extracts have been tested for their ability to modulate the rumen microbiome to improve feed digestion and fermentation. Among the evaluated plant extracts, essential oils, tannins, and saponins appeared to have positive effects on rumen protein metabolism, volatile fatty acids production, and methane and ammonia production.Methods: The objective of this study was to evaluate the effect of rosemary(Rosmarinus officinalis L.) leaves and essential oils on rumen microbial populations. Four ruminally cannulated sheep were used in a 4×4 Latin square design fed(21 d/period): 1) a control diet composed of alfalfa hay and concentrate pellet(CTR), 2) CTR supplemented with 7 g/d/sheep of rosemary essential oil adsorbed on an inert support(EO), 3) CTR with 10 g/d/sheep of dried and ground rosemary leaves(RL), and 4) CTR with 10 g/d of dried and ground rosemary leaves pelleted into concentrate(RL pellet). Abundance of total bacteria, archaea, protozoa, and some select bacterial species or groups was quantified using q PCR, while the community of bacteria and archaea was profiled using denaturing gradient gel electrophoresis.Results: No difference in abundance was noted for total bacteria, protozoa, or Ruminococcus flavefaciens between the control and the treatments, but the rosemary leaves, either in loose form or in pellet, decreased the abundance of archaea and the genus Prevotella(P < 0.001). The rosemary leaves in loose form also decreased(P < 0.001) the abundance of Ruminococcus albus and Clostridium aminophilum, while the EO increased(P < 0.001) the abundance of Fibrobacter succinogenes. 16Clinical epidemiology of chronic viral hepatitis B:A Tuscany real-world large-scale cohort study显示文摘AIM To build a regional database of chronic patients to define the clinical epidemiology of hepatitis B virus(HBV)-infected patients in the Tuscan public health care system.METHODS This study used a cross-sectional cohort design. We evaluated chronic viral hepatitis patients with HBV referred to the outpatient services of 16 hospital units. Information in the case report forms included main demographic data, blood chemistry data, viral hepatitis markers, instrumental evaluations, and eligibility for treatment or ongoing therapy and liver transplantation. RESULTS Of 4015 chronic viral hepatitis patients, 1096(27.3%) were HBV infected. The case report form was correctly completed for only 833 patients(64% males, 36% females; mean age 50.1 ± 15.4). Of these HBV-infected patients, 73% were Caucasian, 21% Asian, 4% Central African, 1% North African and 1% American. Stratifying patients by age and nationality, we found that 21.7% of HBV-infected patients were aged < 34 years(only 2.8% were Italian). The most represented routes of transmission were nosocomial/dental procedures(23%), mother-to-child(17%) and sexual transmission(12%). The most represented HBV genotypes were D(72%) and A(14%). Of the patients, 24.7% of patients were HBe Ag positive, and 75.3% were HBe Ag negative. Of the HBV patients 7% were anti-HDV positive. In the whole cohort, 26.9% were cirrhotic(35.8% aged < 45 years), and 47% were eligible for or currently undergoing treatment, of whom 41.9 % were cirrhotic. CONCLUSION Only 27.3% of chronic viral hepatitis patients were HBV infected. Our results provide evidence of HBV infection in people aged < 34 years, especially in the foreign population not protected by vaccination.
