1Eosinophilic gastroenteritis:An unusual type of gastroenteritis显示文摘Eosinophilic gastroenteritis(EGE) is a rare disorder characterized by eosinophilic infiltration of the bowel wall with various gastrointestinal manifestations.Till date only 280 cases have been described in the literature.A high index of suspicion,by excluding other causes of peripheral eosinophilia,is a pre requisite for accurate diagnosis.EGE is an uncommon gastrointestinal disease affecting both children and adults.It was first described by Kaijser in 1937.Presentation may vary depending on location as well as depth and extent of bowel wall involvement and usually runs a chronic relapsing course.This condition can respond to low dose steroid therapy,thereby preventing grave complications like ascites and intestinal obstruction that might need surgical intervention.The natural history of EGE has not been well documented.Eosinophilic gastroenteritis is a chronic,waxing and waning condition.Mild and sporadic symptoms can be managed with reassurance and observation,whereas disabling gastrointestinal(GI) symptom flare-ups can often be controlled with oral corticosteroids.When the disease manifests in infancy and specific food sensitization can be identified,the likelihood of disease remission by late childhood is high.GI obstruction is the most common complication.Fatal outcomes are rare.Sachin B Ingle Chitra R Hinge(Ingle) 2013World Journal of Gastroenterology2013,19,31:47
2Primary intestinal lymphangiectasia: Minireview显示文摘Primary idiopathic intestinal lymphangiectasia is an unusual disease featured by the presence of dilated lymphatic channels which are located in the mucosa, submucosa or subserosa leading to protein loosing enteropathy.Most often affected were children and generally diagnosed before third year of life but may be rarely seen in adults too. Bilateral pitting oedema of lower limb is the main clinical manifestation mimicking the systemic disease and posing a real diagnostic dilemma to the clinicians to differentiate it from other common systemic diseases like Congestive cardiac failure, Nephrotic Syndrome, Protein Energy Malnutrition, etc. Diagnosis can be made on capsule endoscopy which can localise the lesion but unable to take biopsy samples. Thus, recently double-balloon enteroscopy and biopsy in combination can be used as an effective diagnostic tool to hit the correct diagnosis. Patients respond dramatically to diet constituting low long chain triglycerides and high protein content with supplements of medium chain triglyceride. 3Epithelial cysts of the spleen:A minireview显示文摘Primary splenic epithelial cyst is an unusual event in everyday surgical practice with about 800 cases reported until date in the English literature. Splenic cysts may be parasitic or non-parasitic in origin. Nonparasitic cysts are either primary or secondary. Primary cysts are also called true,congenital,epidermoid or epithelial cysts. Primary splenic cysts account for 10% of all benign non-parasitic splenic cysts and are the most frequent type of splenic cysts in children. Usually,splenic cysts are asymptomatic and can be found incidentally during imaging techniques or on laprotomy. The symptoms are related to the size of cysts. When they assume large sizes,they may present with fullness in the left abdomen,local or referred pain,symptoms due to compression of adjacent structures(like nausea,vomiting,flatulence,diarrhoea) or rarely thrombocytopenia,and occasionally complications such as infection,rupture and/or haemorrhage. The preoperative diagnosis of primary splenic cysts can be ascertained by ultrasonography(USG),computed tomography or magnetic resonance imaging,although the wide use of USG today has led to an increase in the incidence of splenic cysts by 1%. However,careful histopathological evaluation along with immunostaining for presence of epithelial lining is mandatory to arrive at the diagnosis. 4Synergistic anticancer effect of green synthesized nickel nanoparticles and quercetin extracted from Ocimum sanctum leaf extract显示文摘Leaf extract of medicinally important plant Ocimum sanctum(O. sanctum) has been used for the synthesis of nickel nanoparticles(Ni Gs) and extraction of quercetin(Qu). Qu has been conjugated with Ni Gs for enhanced anticancer effect on human breast cancer MCF–7 cells. Extracted Qu was conjugated with polyethylene glycol(PEG) coated Ni Gs(Qu–PEG–Ni Gs) which was used as carriers for breast cancer treatment. Anticancer activity of Qu–PEG–Ni Gs was evaluated by assessing cell viability, reactive oxygen species(ROS) production, caspase activity, mitochondrial membrane potential(MMP) and changes in nuclear morphology(staining methods). 0.85 mg of quercetin was extracted from 1 g of leaves with retention time(R_t) of 2.914 min. Loading and encapsulation efficiency of quercetin onto PEG–Ni Gs was 15.04% and 82% respectively and Qu–PEG–Ni Gs has shown a sustained release of Qu of about 84% after 48 h. Qu and Qu–PEG–Ni Gs showed dose dependent(1.56–50 μg/m L) anticancer effect against MCF–7 cells with IC50 values of 50 and 6.25 μg/m L respectively which was mediated by oxidative stress due to ROS over-production that induced loss of mitochondrial membrane potential, capsase-9,-7 activities leading to apoptosis. 5Crohn's disease with gastroduodenal involvement: Diagnostic approach显示文摘Crohn's disease(CD) is a chronic idiopathic inflammatory disease of gastrointestinal tract characterizedby segmental and transmural involvement of gastrointestinal tract. Ileocolonic and colonic/anorectal is a most common and account for 40% of cases and involvement of small intestine is about 30%. Isolated involvement of stomach is an extremely unusual presentation of the disease accounting for less than 0.07% of all gastrointestinal CD. To date there are only a few documented case reports of adults with isolated gastric CD and no reports in the pediatric population. The diagnosis is difficult to establish in such cases with atypical presentation. In the absence of any other source of disease and in the presence of nonspecific upper gastrointestinal endoscopy and histological findings, serological testing can play a vital role in the diagnosis of atypical CD. Recent studies have suggested that perinuclear anti-neutrophil cytoplasmic antibody and anti-Saccharomycescervisia antibody may be used as additional diagnostic tools. 8Splenic lymphoma with massive splenomegaly: Case report with review of literature显示文摘As per strict criteria of Das Gupta et al, primary splenic lymphoma is very rare. Herein, we are reporting an unusual case of primary large cell splenic lymphoma of B lineage in a middle aged female presenting with massive splenomegaly(3.8 kg) and hypersplenism. After performing therapeutic splenectomy for hypersplenism, a precise diagnosis of diffuse large B cell lymphoma was made on histopathology and confirmed by immunohistochemistry. The patient responded well to standard(Cyclophosphamide, Hydroxydaunorubicin, Oncovin(vincristine), Prednisone or prednisolone) regimen last year and is now in full remission. The splenectomy thereby has prevented the potential grave complications related to hypersplenism and splenic rupture. In view of vague clinical presentation, it is difficult to arrive at the diagnosis. In such circumstances, histopathological diagnosis is very important. A precise diagnosis can only be made on histopathology and confirmed on immunohistochemistry.Emergency splenectomy is preferred as an effective therapeutic and diagnostic tool in cases with giant splenomegaly. Core biopsy is usually not advised due to a high risk of post-core biopsy complications in view of its high vascularity and fragility. Aim behind highlighting the topic is to specify that core biopsy/ fine needle aspiration cytology can be used as an effective diagnostic tool to arrive at correct diagnosis to prevent untoward complications related to disease and treatment. 11Microscopic colitis: Common cause of unexplained nonbloody diarrhea显示文摘Microscopic colitis(MC) is characterized by chronic, watery, secretory diarrhea, with a normal or near normal gross appearance of the colonic mucosa. Biopsy is diagnostic and usually reveals either lymphocytic colitis or collagenous colitis. The symptoms of collagenous colitis appear most commonly in the sixth decade. Patients report watery, nonbloody diarrhea of a chronic, intermittent or chronic recurrent course. With collagenous colitis, the major microscopic characteristic is a thickened collagen layer beneath the colonic mucosa, and with lymphocytic colitis, an increased number of intraepithelial lymphocytes. Histological workup can confirm a diagnosis of MC and distinguish the two distinct histological forms, namely, collagenous and lymphocytic colitis. Presently, both forms are diagnosed and treated in the same way; thus, the description of the two forms is not of clinical value although this may change in the future. Since microscopic colitis was first described in 1976 and only recently recognized as a common cause of diarrhea, many practicing physicians may not be aware of this entity. 12Exogenous Peroxidase Mitigates Cadmium Toxicity, Enhances Rhizobial Population and Lowers Root Knot Formation in Rice Seedlings显示文摘Soil cadmium (Cd) causes toxicity and oxidative stress, alters biochemical processes and rootknot formation in rice. Irrigation of exogenous peroxidase (POX) together with its co-substrate H2O2(POXRice + H2O2), is likely to have protective effect upon the biochemical and nodular changes in ricegrown in Cd-rich soil. Exposure to Cd concentration of 1.00 mg/L increased oxidative stress, loss of cellviability, electrolyte leakage and root knot formation, whereas it significantly lowered the chlorophyll leveland rhizobium growth in rice. Irrigation of exogenous POXRice + H2O2 to Cd-stressed rice seedlingsreversed the Cd-induced alterations in rice to levels similar in control (non-stressed) seedlings. Resultsprovided strong evidence of exogenous POXRice + H2O2-mediated reversal and restoration of physiologicaland biochemical processes as well as increased resistance of rice seedlings to root knot formation.Irrigation with POXRice + H2O2 appeared to contribute towards bringing normoxic conditions in the otherwisehypoxic soil environment by enhancing the O2 in pot-experiments due to reduced Cd uptake, enhancedmineral homeostasis of essential elements viz. P, Fe, Mo, Mg and Mn for maintenance of root architecturedamaged by lipid peroxidation and reduction in oxidative stress by reducing Cd-induced reactive oxygenspecies generation. 16Loss of mdig expression enhances DNA and histone methylation and metastasis of aggressive breast cancer显示文摘We previously reported that expression of an environmentally induced gene,mineral dust-induced gene(mdig),predicts overall survival in breast cancer patients.In the present report,we further demonstrate the differential roles of mdig between earlier-and later-stage breast cancers.In noncancerous breast,mdig is a proliferation factor for cell growth and cell motility.In breast cancer,however,higher levels of mdig negatively regulate the migration and invasion of cancer cells.Assessment of global DNA methylation,chromatin accessibility and H3K9me3 heterochromatin signature suggests that silencing mdig enhances DNA and histone methylation.Through immunostaining and data mining,we found that mdig is significantly upregulated in noninvasive and/or earlier-stage breast cancers.In contrast,in triple-negative and other invasive breast cancers,diminished mdig expression was noted,indicating that the loss of mdig expression could be an important feature of aggressive breast cancers.Taken together,our data suggest that mdig is a new biomarker that likely promotes tumor growth in the early stages of breast cancer while acting as a tumor suppressor to inhibit invasion and metastasis in later-stage tumors.Chitra Thakur Bailing Chen Lingzhi Li Qian Zhang Zeng-Quan Yang Fei Chen 2018Signal Transduction and Targeted Therapy2018,3,1:2

19Experience with hemoadsorption (CytoSorb~?) in the management of septic shock patients显示文摘BACKGROUND Cytokines and inflammatory mediators are the hallmarks of sepsis.Extracorporeal cytokine hemoadsorption devices are the newer clinical support system to overcome the cytokine storm during sepsis.AIM To retrospectively evaluate the clinical outcomes of patients admitted in intensive care unit with septic shock with different etiologies.METHODS The laboratory parameters including biomarkers such as procalcitonin,serum lactate and C-reactive protein;and the hemodynamic parameters;mean arterial pressure,vasopressor doses,sepsis scores,cytokine levels and other vital parameters were evaluated.We evaluated these outcomes among survivors and non-survivors.RESULTS Of 100 patients evaluated,40 patients survived.Post treatment,the vasopressors dosage remarkably decreased though it was not statistically different;34.15%(P=0.0816)for epinephrine,20.5%for norepinephrine(P=0.3099)and 51%(P=0.0678)for vasopressin.In the survivor group,a remarkable reduction of biomarkers levels;procalcitonin(65%,P=0.5859),C-reactive protein(27%,P=0.659),serum lactate(27%,P=0.0159)and bilirubin(43.11%;P=0.0565)were observed from baseline after CytoSorb^® therapy.A significant reduction in inflammatory markers;interleukin 6 and interleukin 10;(87%and 92%,P<0.0001)and in tumour necrosis factor(24%,P=0.0003)was also seen.Overall,28(28%)patients who were given CytoSorb^® therapy less than 48 h after onset of septic shock survived and the maximum duration of stay for 70%of these patients in intensive care unit was less than 15 d.CONCLUSION CytoSorb^® is a safe and well tolerated rescue therapy option in patients with septic shock.However,early(preferably within<48 h after onset of septic shock)initiation could result in better clinical outcomes.Further randomized trials are needed to define the potential benefits of this new treatment modality.Yatin Mehta Chitra Mehta Ashish Kumar Joby Varghese George Aditi Gupta Saurabh Nanda Gourav Kochhar Arun Raizada 2020World Journal of Critical Care Medicine2020,9,1:2
