1Influence of gut microbiota on neuropsychiatric disorders显示文摘The last decade has witnessed a growing appreciation of the fundamental role played by an early assembly of a diverse and balanced gut microbiota and its subsequent maintenance for future health of the host. Gut microbiota is currently viewed as a key regulator of a fluent bidirectional dialogue between the gut and the brain(gut-brain axis). A number of preclinical studies have suggested that the microbiota and its genome(microbiome) may play a key role in neurodevelopmental and neurodegenerative disorders. Furthermore, alterations in the gut microbiota composition in humans have also been linked to a variety of neuropsychiatric conditions, including depression, autism and Parkinson's disease. However, it is not yet clear whether these changes in the microbiome are causally related to such diseases or are secondary effects thereof. In this respect, recent studies in animals have indicated that gut microbiota transplantation can transfer a behavioral phenotype, suggesting that the gut microbiota may be a modifiable factor modulating the development or pathogenesis of neuropsychiatric conditions. Further studies are warranted to establish whether or not the findings of preclinical animal experiments can be generalized to humans. Moreover, although different communication routes between the microbiota and brain have been identified, further studies must elucidate all the underlying mechanisms involved. Such research is expected to contribute to the design of strategies to modulate the gut microbiota and its functions with a view to improving mental health, and thus provide opportunities to improve the management of psychiatric diseases. Here, we review the evidence supporting a role of the gut microbiota in neuropsychiatric disorders and the state of the art regarding the mechanisms underlying its contribution to mental illness and health. 4Gut microbiota-related complications in cirrhosis显示文摘Gut microbiota plays an important role in cirrhosis. The liver is constantly challenged with commensal bacteria and their products arriving through the portal vein in the so-called gut-liver axis. Bacterial translocation from the intestinal lumen through the intestinal wall and to mesenteric lymph nodes is facilitated by intestinal bacterial overgrowth, impairment in the permeability of the intestinal mucosal barrier, and deficiencies in local host immune defences. Deranged clearance of endogenous bacteria from portal and systemic circulation turns the gut into the major source of bacterialrelated complications. Liver function may therefore be affected by alterations in the composition of the intestinal microbiota and a role for commensal flora has been evidenced in the pathogenesis of several complications arising in end-stage liver disease such as hepatic encephalopathy, splanchnic arterial vasodilatation and spontaneous bacterial peritonitis. 5Serum leptin levels and insulin resistance are associated with gallstone disease in overweight subjects显示文摘AIM: To establish an association between the serum leptin levels and the development of gallstone disease (GD).METHODS: We carried out a non-matched case-controlled study in a university hospital in Mexico City. Two hundred and eighty-seven subjects were included: 97 cases with gallstones and 190 controls. Body mass index (BMI), fasting plasma leptin, insulin, serum lipid, and lipoprotein levels were measured. Insulin resistance was calculated by homeostasis model assessment (HOMA-IR). Unconditional logistic regression analysis (univariate and multivariate)stratified by BMI was used to calculate the risk of GD.RESULTS: The multivariate conditional regression analysis revealed a model for those patients with BMI <30. The selected variables in the model were HOMA-IR index with OR = 1.31, P= 0.02 and leptin higher than median with OR = 2.11, P= 0.05. 6Hepatitis B virus prevalence and transmission risk factors in inflammatory bowel disease patients at Clementino Fraga Filho university hospital显示文摘AIM: To evaluate the prevalence of hepatitis B virus (HBV) infection in inflammatory bowel disease (IBD) patients that followed up in our hospital and try to identify the possible risk factors involved in this infection transmission. METHODS: This was a cross-sectional study for which 176 patients were selected according to their arrival for the medical interview. All these patients had already IBD diagnosis. The patient was interviewed and a questionnaire was filled out. RESULTS: In the group of 176 patients whom we examined, we found that 17% (30) were anti-HBc positive. Out of 30 patients with positive anti-HBc, 2.3% (4) had positive HBsAg and negative HBV-DNA. In an attempt to identify the possible HBV infection transmission risk factors in IBD patients, it was observed that 117 patients had been submitted to some kind of surgical procedure, but only 24 patients had positive anti-HBc (P = 0.085). It was also observed that surgery to treat IBD complications was not a risk factor for HBV infection transmission, since we did not get a statically significant P value. However, IBDpatients that have been submitted to surgery to treat IBD complications received more blood transfusions then patients submitted to other surgical interventions (P = 0.015). 7Diagnosis and treatment of pancreas divisum: A literature review显示文摘Background:Pancreas divisum is a congenital embryological disease caused by a lack of fusion between the ventral and dorsal pancreatic ducts in the early stages of embryogenesis.Recurrent acute pancreatitis,chronic pancreatitis or chronic abdominal pain are the main clinical syndromes at presentation and occur in only 5%of the patients with pancreas divisum.This review aimed to discuss diagnosis and treatment strategies in patients with symptomatic pancreas divisum.Data sources:We report a literature review from 1990 up to January 2018 to explore the various diagnostic modalities and surgical techniques and results reported in the surgical treatment of pancreas divisum.Results:There are limited reports available on this topic in the literature.We analyzed and described the main indications in the treatment of pancreas divisum,focusing on surgical treatment and a discussion of the different approaches.Furthermore,we report the results from our experience in two cases of pancreas divisum treated by pancreatic head resection with segmental duodenectomy(the Nakao procedure).Conclusions:Pancreas divisum is a common pancreatic malformation in which only a few patients develop a symptomatic disease.Surgical treatment is needed in case of endoscopic drainage failure and in cases complicated with chronic pancreatitis and local complications.Many techniques,of greater or lesser complexity,have been proposed.Valentina Ferri Emilio Vicente Yolanda Quijano Benedetto Ielpo Hipolito Duran Eduardo Diaz Isabel Fabra Riccardo Caruso 2019Hepatobiliary & Pancreatic Diseases International2019,18,4:9
8Cardiovascular assessment in liver transplant for nonalcoholic steatohepatitis patients:What we do,what we should do显示文摘Non-alcoholic fatty liver disease(NAFLD) is increasing considerably due to the current lifestyle,which means that it is becoming one of the main indications for liver transplantation.On the other hand,there is a strong association between NAFLD and cardiovascular disease.This has been evidenced in many studies revealing a higher presence of carotid plaques or carotid intima-media thickness,leading to cardiovascular events and,ultimately,mortality.According to the liver transplant guidelines,screening for heart disease in transplant candidates should be performed by electrocardiogram and transthoracic echocardiography while a stress echocardiogram should be reserved for those with more than two cardiovascular risk factors or greater than 50 years old.However,there are no specific recommendations in NAFLD patients requiring a liver transplantation,despite its well-known cardiovascular risk association.Many studies have shown that these patients probably require a more exhaustive assessment and a global approach including other specialists such as cardiologists or nutritionists.Also,the incidence of cardiovascular disease is also increased in NAFLD patients in the post-transplantation period in comparison with other etiologies,because of the pre-existent risk factors together with the immunosuppressive therapy.Therefore,an early intervention on the lifestyle and the individualized selection of the immunosuppressive regimen could lead to a modification of the cardiovascular risk factors in NAFLD patients requiring a liver transplantation.Yolanda Sanchez-Torrijos Javier Ampuero Manuel Romero-Gómez 2017World Journal of Hepatology2017,9,15:7
9Release of interleukin-10 and neurotrophic factors in the choroid plexus:possible inductors of neurogenesis following copolymer-1 immunization after cerebral ischemia显示文摘Copolymer-1(Cop-1) is a peptide with immunomodulatory properties, approved by the Food and Drug Administration of United States in the treatment of multiple sclerosis. Cop-1 has been shown to exert neuroprotective effects and induce neurogenesis in cerebral ischemia models. Nevertheless, the mechanism involved in the neurogenic action of this compound remains unknown. The choroid plexus(CP) is a network of cells that constitute the interphase between the immune and central nervous systems, with the ability to mediate neurogenesis through the release of cytokines and growth factors. Therefore, the CP could play a role in Cop-1-induced neurogenesis. In order to determine the participation of the CP in the induction of neurogenesis after Cop-1 immunization, we evaluated the gene expression of various growth factors(brain-derived neurotrophic factor, insulin-like growth factor 1, neurotrophin-3) and cytokines(tumor necrosis factor alpha, interferon-gamma, interleukin-4(IL-4), IL-10 and IL-17), in the CP at 14 days after ischemia. Furthermore, we analyzed the correlation between the expression of these genes and neurogenesis. Our results showed that Cop-1 was capable of stimulating an upregulation in the expression of the genes encoding for brain-derived neurotrophic factor, insulin-like growth factor 1, neurotrophin-3 and IL-10 in the CP, which correlated with an increase in neurogenesis in the subventricular and subgranular zone. As well, we observed a downregulation of IL-17 gene expression. 10Effect of probiotic Lactobacillus plantarum Dad-13 powder consumption on the gut microbiota and intestinal health of overweight adults显示文摘BACKGROUND Shifting on lifestyle,diet,and physical activity contributed on increasing number of obese people around the world.Multiple factors influence the development of obesity.Some research suggested that gut microbiota(GM)plays an important role in nutrient absorption and energy regulation of individuals,thus affecting their nutritional status.Report of Indonesia Basic Health Research showed that the prevalence of obesity in every province tended to increase.Although the root cause of obesity is excessive calorie intake compared with expenditure,the differences in gut microbial ecology between healthy and obese humans may affect energy homeostasis.GM affect body weight,especially obesity.Probiotics that are consumed while alive and able to colonize in the intestine are expected to increase the population of good bacteria,especially Bifidobacteria and Lactobacilli,and suppress pathogens such as Enterobacteriaceae and Staphylococcus.The strain of L.plantarum Dad-13 has been demonstrated to survive and colonize in the gastrointestinal tract of healthy Indonesian adults who consume fermented milk containing L.plantarum Dad-13.The consumption of probiotic L.plantarum Dad-13 powder decreased E.coli and non-E.coli coliform bacteria in school-aged children in Indonesia.L.plantarum is a dominant bacterium in the average Indonesian’s GM.For this reason,this bacterium is probably a more suitable probiotic for Indonesians.AIM To determine the effect of the consumption of indigenous probiotic Lactobacillus plantarum Dad-13 powder in overweight adults in Yogyakarta(Indonesia).METHODS Sixty overweight volunteers with a body mass index(BMI)equal to or greater than 25 consume indigenous probiotic powder L.plantarum Dad-13(2×109 CFU/gram/sachet)for 90 d.The study was a randomized,double-blind,placebocontrolled study.The volunteers filled in a diary on a daily basis,which consisted of questions on study product intake(only during ingestion period),other food intake,number of bowel movements,fecal quality(consistency and color),any medications received,and any symptom of discomfort,such as diarrhea,constipation,vomiting,gassing,sensation of illness,etc.Fecal samples and the subjects’diaries were collected on the morning of day 10+1,which was marked as the end of the baseline period and the start of the ingestion period.During the ingestion period(from day 11 to day 101),several parameters to measure and analyze the results included body weight and height(once a month),the lipid profile,GM analysis using MiSeq,short-chain fatty acid(SCFA)analysis using gas chromatography,and the measurement of fecal pH using a pH meter.RESULTS The consumption of indigenous probiotic powder L.plantarum Dad-13 caused the average body weight and BMI of the probiotic group to decrease from 84.54±17.64 kg to 83.14±14.71 kg and 33.10±6.15 kg/m2 to 32.57±5.01 kg/m2,respectively.No significant reduction of body weight and BMI in the placebo group was observed.An analysis of the microbiota showed that the number of Bacteroidetes,specifically Prevotella,increased significantly,while that of Firmicutes significantly decreased.No significant change in lipid profile in both groups was found.Also,no significant change in SCFAs(e.g.,butyrate,propionate,acetic acid)and pH level was found after the consumption of the probiotic.CONCLUSION No significant differences in pH before and after ingestion were observed in both the probiotic and placebo groups as well as in the lipid profile of both cholesterol and triglyceride,high-density lipoprotein(HDL),low-density lipoprotein(LDL),and the LDL/HDL ratio.In addition,no significant changes in the concentration of SCFAs(e.g.,acetic acid,propionate,and butyrate)were found after consumption.Interestingly,a significant decrease in body weight and BMI(P<0.05)was determined in the treatment group.An analysis of GM shows that L.plantarum Dad-13 caused the Firmicutes population to decrease and the Bacteroidetes population(especially Prevotella)to increase.Endang Sutriswati Rahayu Mariyatun Mariyatun Nancy Eka Putri Manurung Pratama Nur Hasan Phatthanaphong Therdtatha Riko Mishima Husnita Komalasari Nurul Ain Mahfuzah Fathyah Hanum Pamungkaningtyas Wahyu Krisna Yoga Dina Aulia Nurfiana Stefanie Yolanda Liwan Mohammad Juffrie Agung Endro Nugroho Tyas Utami 2021World Journal of Gastroenterology2021,27,1:6
11Ammonium Ion Adsorption and Settleability Improvement Achieved in a Synthetic Zeolite-Amended Activated Sludge显示文摘Municipal wastewater treatment plants typically exhibit two classic problems: high ammonium concentration in water after conventional biological treatment and,in some cases,poor activated sludge sediment ability.Potential solutions to these problems were investigated by adding a synthetic zeolite obtained from coal fly ash to different steps of activated sludge treatment.The experimental results for ammonium removal fit well with the theoretical adsorption isotherms of the Freundlich model with a maximum adsorption capacity of 13.72 mg·g 1.Utilization of this kind of zeolite to improve activated sludge sediment ability is studied for the first time in this work.It is found that the addition of the zeolite(1 g·L 1) to an activated sludge with settling problems significantly enhances its sediment ability and compact ability.This is confirmed by the sludge volume index(SVI),which was reduced from 163 ml·g 1to 70 ml·g 1,the V60value,which was reduced from 894 ml·L 1to 427 ml·L 1,and the zeta potential(ζ),which was reduced from 19.81 mV to 14.29 mV.The results indicate that the addition of this synthetic zeolite to activated sludge,as an additional waste management practice,has a positive impact on both ammonium removal and sludge settleability.Emilia Otal Luls F. 12CD271 as a marker to identify mesenchymal stem cells from diverse sources before culture显示文摘Mesenchymal stem cells, due to their characteristics are ideal candidates for cellular therapy. Currently, in culture these cells are defined by their adherence to plastic, specific surface antigen expression and multipotent differentiation potential. However, the in vivo identification of mesenchymal stem cells, before culture, is not so well established. Pre-culture identification markers would ensure higher purity than that obtained with selection based on adherence to plastic. Up until now, CD271 has been described as the most specific marker for the characterization andpurification of human bone marrow mesenchymal stem cells. This marker has been shown to be specifically expressed by these cells. Thus, CD271 has been proposed as a versatile marker to selectively isolated and expand multipotent mesenchymal stem cells with both immunosuppressive and lymphohematopoietic engraftment-promoting properties. This review focuses on this marker, specifically on identification of mesenchymal stem cells from different tissues. Literature revision suggests that CD271 should not be defined as a universal marker to identify mesenchymal stem cells before culture from different sources. 15Angiopoietin-2/angiopoietin-1 as non-invasive biomarker of cirrhosis in chronic hepatitis C显示文摘AIM To evaluate the efficacy of peripheral blood concentrations of angiopoietins(Ang) as cirrhosis biomarkers of chronic hepatitis C(CHC).METHODS Ang1 and Ang2 serum levels were measured by enzyme-linked immunosorbent assays(ELISA) in samples from 179 cirrhotic and non-cirrhotic CHC patients, classified according to the METAVIR system.Groups were compared by non-parametric MannWhitney U test. Subsequently, the association of peripheral concentrations of angiopoietins with the stage of fibrosis was analyzed using Spearman correlation test. Finally, the accuracy, sensitivity and specificity of circulating angiopoietins for cirrhosis diagnosis were determined by the study of the respective area under the curve of receiver operator characteristics(AUC-ROC).RESULTS Peripheral blood concentrations of Ang1 and Ang2 in CHC patients were significantly related to fibrosis. While Ang1 was decreased in cirrhotic subjects compared to non-cirrhotic(P < 0.0001), Ang2 was significantly increased as CHC progressed to the end stage of liver disease(P < 0.0001). Consequently, Ang2/Ang1 ratio was notably amplified and significantly correlated with fibrosis(P < 0.0001). Interestingly, the individual performance of each angiopoietin for the diagnosis of cirrhosis reached notable AUC-ROC values(above 0.7, both), but the Ang2/Ang1 ratio was much better(AUC-ROC = 0.810) and displayed outstanding values of sensitivity(71%), specificity(84%) and accuracy(82.1%) at the optimal cut-off(10.33). 17Signalization options to improve capacity and delay at roundabouts through microsimulation approach:A case study on arterial roadways in Ghana显示文摘Roundabouts are very useful when flows are balanced but can have operational challenges when flows increase.Most roundabouts on arterial roadways in Ghana experience recurring congestion during peak traffic.The four roundabouts between Ejisu Municipality and Asokwa Municipality on the N6 highway are typical of the situation found on arterials countrywide.They experience recurring long queues and delays during the peak hours and over-representation of truck rollover accidents.In Ghana,historically,most roundabouts with capacity problems have been developed into interchanges at very high cost without consideration of other effective at-grade configurations.When roundabouts are signalized,various configurations can be explored to enhance capacity while preserving the safety benefits.Using simulation techniques,various configurations and design options were tested for their effectiveness at two of the roundabouts,located at KNUST police station and Ejisu,with similar demand and high left turning volumes.The objectives of this study were to a)evaluate the performance of the selected roundabouts on the corridor;b)explore alternative intersection configurations to improve capacity and delay.Using video data collection techniques,2-h time stamped footages of AM and PM peak traffic flows were captured for two days.Geometric design parameters of the existing roundabouts,queue lengths,travel times,and approach flows were also captured.VISSIM models of the selected roundabouts were developed and calibrated for the baseline conditions.Signalized unconventional roundabout configuration,with a phase for major road through traffic were simulated and assessed for capacity,delay and queue length.The results showed that signalized roundabout intersections effectively increased capacity by as much as 50%in some cases,reduced delay and queue length significantly,and maintained the benefits of roundabouts including safety.The performance could be further enhanced by introducing separate left turn phases to reduce conflicts or through a right turn U-turn(RTUT)concept.Kwame Kwakwa Osei Charles Anum Adams Williams Ackaah Yolanda Oliver-Commey 2021Journal of Traffic and Transportation Engineering(English Edition)2021,8,1:3
18Specific and cross-reactive immune response against Mycobacterium tuberculosis antigens in mice immunized with proteoliposomes from Mycobacterium bovis BCG显示文摘Objective: To characterize the immunogenicity and the induction of cross-reactive responses against Mycobacterium tuberculosis(M. tuberculosis) of a proteoliposome(PL)from Mycobacterium bovis Bacillus Calmette–Guerin(BCG) with and without alum hydroxide(AL) as adjuvant(PLBCG-AL and PLBCG, respectively) in BALB/c mice.Methods: BALB/c mice were inoculated with phosphate buffer solution, BCG, PLBCG and PLBCG-AL. The humoral immunogenicity was determined by ELISA [immunoglobulin G(Ig G), Ig G1 and Ig G2a] and the cellular immunogenicity was evaluated in vivo by delayed type hypersensitivity. The humoral cross-reactive response against M. tuberculosis was determined by Western blot.Results: Sera from animals immunized with PLBCG-AL and PLBCG showed significant increase in specific total Ig G and Ig G1 antibodies and the presence of cross-reactive antibodies against M. tuberculosis antigens, which were more intense with the use of alum as adjuvant. 20Approach to early-onset colorectal cancer:Clinicopathological,familial,molecular and immunohistochemical characteristics显示文摘AIM:To characterize clinicopathological and familial features of early-onset colorectal cancer(CRC) and compare features of tumors with and without microsatellite instability(MSI).METHODS:Forty-five patients with CRC aged 45 or younger were included in the study.Clinical information,a three-generation family history,and tumor samples were obtained.MSI status was analyzed and mismatch repair genes were examined in the MSI families.Tumors were included in a tissue microarray and an immunohistochemical study was carried out with a panel of selected antibodies.RESULTS:Early onset CRC is characterized by advanced stage at diagnosis,right colon location,low-grade of differentiation,mucin production,and presence of polyps.Hereditary forms represent at least 21% of cases.Eighty-one percent of patients who died during followup showed a lack of expression of cyclin E,which could be a marker of poor prognosis.β-catenin expression was normal in a high percentage of tumors.CONCLUSION:Early-onset CRC has an important familial component,with a high proportion of tumors showing microsatellite stable.Cyclin E might be a poor prognosis factor.Jose Perea Edurne Alvaro Yolanda Rodríguez Cristina Gravalos Eva Sánchez-Tomé Barbara Rivera Francisco Colina Pablo Carbonell Rogelio González-Sarmiento Manuel Hidalgo Miguel Urioste 2010World Journal of Gastroenterology2010,16,29:3
