18Immunological and functional features of decellularized xenogeneic heart valves after transplantation into GGTA1-KO pigs显示文摘Decellularization of xenogeneic heart valves might lead to excellent regenerative implants,from which many patients could benefit.However,this material carries various xenogeneic epitopes and thus bears a considerable inherent immunological risk.Here,we investigated the regenerative and immunogenic potential of xenogeneic decellularized heart valve implants using pigs deficient for the galactosyltransferase gene(GGTA1-KO)as novel large animal model.Decellularized aortic and pulmonary heart valves obtained from sheep,wild-type pigs or GGTA1-KO pigs were implanted into GGTA1-KO pigs for 3,or 6 months,respectively.Explants were analyzed histologically,immunhistologically(CD3,CD21 and CD172a)and anti-aGal antibody serum titers were determined by ELISA.Xenogeneic sheep derived implants exhibited a strong immune reaction upon implantation into GGTA1-KO pigs,characterized by massive inflammatory cells infiltrates,presence of foreign body giant cells,a dramatic increase of anti-aGal antibody titers and ultimately destruction of the graft,whereas wild-type porcine grafts induced only a mild reaction in GGTA1-KO pigs.Allogeneic implants,wild-type/wild-type and GGTA1-KO/GGTA1-KO valves did not induce a measurable immune reaction.Thus,GGTA1-KO pigs developed a‘human-like’immune response toward decellularized xenogeneic implants showing that immunogenicity of xenogeneic implants is not sufficiently reduced by decellularization,which detracts from their regenerative potential.Robert Ramm Tobias Goecke Peter Kohler Igor Tudorache Serghei Cebotari Anatol Ciubotaru Samir Sarikouch Klaus Hoffler Friederike Bothe Bjorn Petersen Axel Haverich Heiner Niemann Andres Hilfiker 2021Regenerative Biomaterials2021,8,5:0
19Chemically induced hypoxia by dimethyloxalylglycine(DMOG)-loaded nanoporous silica nanoparticles supports endothelial tube formation by sustained VEGF release from adipose tissue-derived stem cells显示文摘Inadequate vascularization leading to insufficient oxygen and nutrient supply in deeper layers of bioartificial tissues remains a limitation in current tissue engineering approaches to which prevascularization offers a promising solution.Hypoxia triggering pre-vascularization by enhanced vascular endothelial growth factor(VEGF)expression can be induced chemically by dimethyloxalylglycine(DMOG).Nanoporous silica nanoparticles(NPSNPs,or mesoporous silica nanoparticles,MSNs)enable sustained delivery of molecules and potentially release DMOG allowing a durable capillarization of a construct.Here we evaluated the effects of soluble DMOG and DMOG-loaded NPSNPs on VEGF secretion of adipose tissue-derived stem cells(ASC)and on tube formation by human umbilical vein endothelial cells(HUVEC)-ASC co-cultures.Repeated doses of 100 mM and 500 mM soluble DMOG on ASC resulted in 3-to 7-fold increased VEGF levels on day 9(P<0.0001).Same doses of DMOG-NPSNPs enhanced VEGF secretion 7.7-fold(P<0.0001)which could be maintained until day 12 with 500 mM DMOG-NPSNPs.In fibrin-based tube formation assays,100 mM DMOG-NPSNPs had inhibitory effects whereas 50 mM significantly increased tube length,area and number of junctions transiently for 4 days.Thus,DMOG-NPSNPs supported endothelial tube formation by upregulated VEGF secretion from ASC and thus display a promising tool for prevascularization of tissue-engineered constructs.Further studies will evaluate their effect in hydrogels under perfusion.Sarah Zippusch Karen FWBesecke Florian Helms Melanie Klingenberg Anne Lyons Peter Behrens Axel Haverich Mathias Wilhelmi Nina Ehlert Ulrike Boer 2021Regenerative Biomaterials2021,8,5:0
20Non-intubated minimally-invasive lung surgery in Germany-current status and outlook显示文摘Lung surgery under spontaneous ventilation(non-intubated video-assisted thoracoscopic surgery,niVATS)picked up a lot of momentum during the past few years.Surgeons and anesthetists alike want to forgo known risks of orotracheal intubation and mechanical ventilation,ranging from local complications,like mucosal injury or even deleterious tracheal laceration and rupture,to increased release of pro-inflammatory cytokines and diaphragmatic dysfunction secondary to muscle relaxation.Despite initial adoption of VATS in Germany having been slow due to a general reticence towards it in smaller units,major thoracic centers nowadays have well-established minimally invasive programs and even perform minor non-intubated procedures on a regular basis.Experience with major niVATS cases still is sparse across Western Europe,and limited to specialized and highly trained centers,where exceptionally complex and often multimorbid patients can safely undergo lung surgery under spontaneous ventilation.Our group pioneered niVATS lobectomy in Germany back in 2018,and since then we tried to spread the technique by means of multidisciplinary Masterclass-courses held at our center,in which the participant’s initial reticence to perform niVATS often changed to enthusiasm for the procedure.This in turn led several visiting units to start with their own programs,and some of them even reported their initial experience.This article offers a short historic overview of VATS in general,gives an insight into how niVATS got pioneered in Germany and presents the current state of affairs.Patrick Zardo Norman Zinne Alaa Selman Hayan Merhej Tobias Goecke Henning Starke Daniel Jaeger Jan Karsten Axel Haverich 2021中华胸部外科电子杂志2021,8,4:0
